3.Clinical observation of mechanical ventilation in treatment of acute severe left heart failure

Objective To explore the effects of mechanical ventilation on the treatment of acute severe left heart failure.Methods While the routine drug therapy was used,25 patients with severe acute left heart failure received mechanical ventilation treatment.The changes of their artery blood gas,vital signs and clinical symptom were observed before and after mechanical ventilation.Results The parameters of their blood gas and clinical symptom were dramatically improved after the mechanical ventilation therapy in 21 patients(P<0.01).19 patients were saved successfully and 6 patients died.Conclusion The mechanical ventilation could effectively improve the clinical symptoms and respiratory function of patients with acute severe left heart failure,which is an effective method in the adjuvant treatment of patients with acute severe left heart failure.
4.The Changes of Postprandial Plasma Lipoproteins and Platelets Activity in the Elderly Patients with Coronary Heart Disease

Objective To study the parameters of the postprandial plasma lipoproteins metabolism and the platelet activity change in the elderly patients with coronary heart disease(CHD). Methods 48 elderly patients with CHD (CHD group) and 30 healthy elderly subjects (control group) were enrolled in this study. The levels of plasma lipoproteins including TG, CH, LDL-C and HDL-C, plama PDGF bioactivity, and vWF plasma concentration were measured at 12 hours after fasting and 4 hours after a fat-rich meal. Results The plasma levels of pre-prandial and postprandial TG, CH, LDL-C, PDGF and vWF in the CHD group were significantly higher than those in the control group, while plasma HLD-C level in the CHD group was significantly lower than that in the control group. Compared with fasting state, the levels of plasma TG, CH and LDL-C in the postprandial state significantly increased, but plasma HDL-C level did not significantly decreased. The plasm PDGF and vWF values of CHD group in the postprandial state significantly elevated. Conclusion These data suggested that the plasma lipoproteins metabolism of the elderly patients with CHD was abnormal, and the platelets activation in the elderly patients with CDH was correlated with the changes of postprandial plasma lipoproteins metabolism.
5.The characteristics of spatial vertical direction in the nasal endoscope view

Objective To observe the characteristics of the spatial vertical direction in the view of 0? endoscope with the elevation angle(between the endoscope and nasal bottom)unchanged,and to provide the basis for operations.Methods The ideal model of nasal cavity was designed and observed with the 0? rod-lens telescope.Space of the model in the visual field of endoscope with 30? or 45? elevation angle produced was simulated by SketchUp software.Then ten lines(perpendicular to the horizontal planes,distributed in different plane and in the same side of the visual vertical line)were drawn randomly and named as Ln(n=1-10)respectively.A point was randomly selected and a vertical line was drawn and assigned as b line.The position of b line in the visual view of endoscope with a 30? elevation angle was named as A1B1.The relationship of positions of Ln with A1B1 were observed when the distances of endoscope entering the model were changed and the endoscope was moved parallelly.When the point A1 was on the b line in visual view of endoscope with 45? elevation angle,the position of b line was named for A1B2.The relationship of positions of Ln with A1B2 were observed when the distances of endoscope entering the model were changed and the endoscope was moved parallelly.The position of A1B1 was compared with A1B2.Results When the distances of endoscope entering the model were changed and the endoscope was moved parallelly,the superposition of Ln with A1B1 and A1B2 could be observed in visual view with 30? and 45? endoscopic elevation angle.The positions of A1B1 and A1B2 were not in superimposition.Conclusion The spatial vertical directions in the view of the endoscope are correlated with the elevation angle but not correlated with the distance of endoscope entering the model and parallel movements of the endoscope.This finding may be helpful for the endoscopic operations.

To study the clinical significance of the changes in PDGF and 6 keto PGF 1? in patients before and after cutting balloon angioplasty (CBA) and plain conventional balloon angioplasty (POBA). Objective To establish a HPLC method for the determination of daphnetin, 7-OH coumarin and syringing in Zushima. Methods The separation was performed on YMC-Pack ODS (4.6 mm?250 mm, 5 ?m) column with a mobile phase consisting of methanol-0.5% acetic acid-water (19︰8︰1) as gradient elution at the flow rate of 1.0 mL/min. The dual detective wavelength was set at 327 nm and 266 nm. The column temperature was 30 ℃. Results Baseline separation was achieved for the analysis of the three index components. The average recoveries were 98.4%~100.2%. Conclusion This method is convenient, rapid and accurate. It can be used for quality control in production of Zushima.

10.Effects of estrogen on the expression of phosphofructokinase muscle-specific isoform in genioglossus of chronic intermittent hypoxia rats.

OBJECTIVETo investigate the effects of estrogen on the expression of phosphofructokinase muscle-specific isoform (PFK-M) in genioglossus of chronic intermittent hypoxia (CIH) rats.
METHODSFifty male SD rats were randomly divided into five groups: the normal control group (NC), the chronic intermittent hypoxia group (CIH), and three doses of estrogen plus hypoxia groups (LE, ME, HE). Rats in the latter four groups were used to build CIH models (8 h/d, 5 weeks). In the mean time, rats in the latter three groups were injected with three dose levels of estrogen (0.1, 0.2, 0.3 mg/kg), and rats in NC and CIH groups were injected with sterile olive oil as control. At the end of the treatment, the genioglossus was isolated and quickly removed. The mRNA levels of PFK-M were determined by real-time RT-PCR and the protein content of PFK-M was detected by Western blotting analysis.
RESULTSPFK-M mRNA and protein in CIH group (2.144 ± 0.260, 0.875 ± 0.025) were both higher than those (1.000 ± 0.259, 0.413 ± 0.013) in NC group (P < 0.05). The expression of PFK-M mRNA in LE, ME and HE groups were 1.424 ± 0.193, 1.395 ± 0.251 and 1.310 ± 0.094, respectively. The expression of protein in LE, ME and HE groups were 0.638 ± 0.015, 0.576 ± 0.017 and 0.505 ± 0.021, respectively. Compared with CIH group, the expression of PFK-M mRNA and protein in LE, ME and HE groups were all inhibited significantly (P < 0.05). Among the three treatment groups, decreased protein content of PFK-M was observed only in HE group when compared with LE group (P < 0.05), but no significant difference was detected in the expression of PFK-M mRNA.
CONCLUSIONSCIH exposure could increase the expression of PFK-M mRNA and protein in rat genioglossus, while estrogen administration could dose dependently inhibit the overexpression.
