1.Observation on corrosion reaction and disinfection effect of two kinds of chlorine disinfectants to surgical instruments

Objective To observe the corrosion reaction and disinfection effect of the two different chlorine disinfectants toward the surgical instruments, to prevent the corrosion of instruments and the professional infection risk of cleaning staffs during washing the infectious disease-contaminated medical instruments manually in the primary hospitals. Methods The surgical instruments were soaked in the diluted chlorine effervescent tablets and the diluted chlorine disinfection powder, respectively. The corrosion of the two disinfectants to the equipment at different times was observed. The bloody surgical instruments were soaked in the diluted chlorine effervescent tablets and the chlorine disinfection powder for 4 hrs, respectively. The instruments' surfaces status was observed,and the bacterial reproduction status after soaking the instruments in the diluted liquid for 48 hrs was monitored.Results The diluted liquid of effervescent chlorine tablets had corrosion reaction after only 5- 10 minutes,but the diluted liquid of chlorine disinfection powder had no corrosion reaction after 48 hours. The dilution liquid of effervescent tablets and the dilution liquid of disinfection powder had a significant statistic difference in the rate of instruments corrosion. The bloody instruments' surface had difference after immersed for 4 hrs,and the pass rates of bacteria monitor was 100% after 48 hrs of immersion. Conclusions For the primary hospitals without cleansing and disinfection equipments,using the chlorine disinfection powder to soak the medical instruments before cleaning the infectious disease-contaminated medical instruments manually not only has no equipment corrosion but also achieves the disinfection effect.
2.Efficacy of low-dose daytime ambulatory peritoneal dialysis in diabetic end-stage renal disease patients with better residual renal function

Objective To study the efficacy of low-dose daytime ambulatory peritoneal dialysis (DAPD) and low-dose CAPD in diabetic end-stage renal disease (ESRD) patients with better residual renal function (RRF). Methods Forty stable diabetic ESRD patients with better RRF (rGFR ≥ 5 ml/min and urine volume ≥ 750 ml/d) were enrolled. They were randomly divided into two groups: low-dose DAPD group (n=20) and low-dose CAPD group (n=20). DAPD group received three 1.5 L to 2 L daily exchanges with a nocturnal empty belly, dwelling for 3 to 4 hours. CAPD group received three 1.5 L to 2 L daily exchange or four 1.5 L daily exchange regimens and dwelled during the night. At the beginning of the study and 6 months later, total weekly Kt/V and Ccr (peritoneal+renal), rGFR were calculated. Meanwhile 24-hour urinary protein,serum albumin (Alb), hemoglobin (Hb), fasting plasma glucose, glycosylated hemoglobin and insulin dosage were measured. Nutritional status was assessed by SGA. Results Thirty-five patients fulfilled the study. There were no significant differences between two groups in age, gender, BMI,PD time, D/Pcr, etc. At the end of the 6th month, the insulin dose[(33.6±10.9) U/d] and 24-hour dialysate protein [(11.13t4.95) g] in CAPD group were significantly higher as compared to DAPD group [(20.6±6.2) U/d, P<0.05 and (5.66±2.88) g, P<0.01 respectively]. Alb in CAPD group [(29.7±4.2) g/L] was significantly lower than that in DAPD group [(36.5 ±3.9) g/L, P<0.05].While the net ultrafiltration [(554±187) ml vs (309±177) ml], 24-hour urine volume [(1090±361)ml vs (750±258) ml] and rGFR [(8.21±2.40) ml/min vs (4.88±2.11) ml/min] in DAPD group were all significantly higher than those in CAPD group (all P<0.05). Conclusion For the diabetic ESRD patients with better RRF, the low-dose DAPD regimen is more effective to control plasma glucose, improve nutritional status and protect RRF than the low-dose CAPD.
3.Construction of the Expression Vector of Viruslike Particles Containing FMDV IRES RNA

The Coat protein and Maturase gene of E.coli bacteriophage MS2 was amplified by PCR,then the gene was cloned into pET32a to construct the intermediate vector pET32aCP.The conservative sequence of FMDV internal ribosome entry site(IRES) was cloned into the downstream of pET32aCP bacteriophage gene to construct the prokaryotic expression vector pCPES.The recombinant plasmid pCPES transformed into E.coli strain BL21(DE3) was induced to express with 1mmol/L IPTG.The expression products were purified by sucrose density gradient centrifugation.The expression products observed by TEM were circular viruslike particles,and the diameter of these particles was about 26nm.The stability of viruslike particles was detected,and the viruslike particles was identified by RTPCR.The results showed that the viruslike particles contain the FMDV IRES RNA and have good stability.The viruslike particles have great prospect as the standard and quality control in the area of RNA virus detection.
5.Pharmacokinetic and distribution of arctiin in rats

OBJECTIVE To study the pharmacokinetic and distribution of arctiin in rats. METHODS Each rat was given a single dose at random by oral administration. The arctiin in serum and organs were determined by use of RP-HPLC. All pharmacokinetic parameters were calculated with a 3P87 program. RESULTS After oral administration of arctiin at the dose of 300mg·kg-1, Arctiin plasma C-T curve conform to open two-compartment model. The Pharmacokinetic parameters were as follow: A=(37.374 5±8.964 7)μg·mL-1;B=(6.210 6±1.489 3)μg·mL-1;α=(0.004 3±0.000 9)min-1;β=(0.000 4±0.000 2)min-1;Kα=(0.420 2±0.167 5)min -1;t1/2α=(115.192 6±14.382 4)min ;t1/2β=(1 485.578 1±161.173 3)min;K10 =(0.001 0±0.000 4)min -1;K21=(0.001 4±0.000 6)min -1 ;K12=(0.002 3±0.001 3)min -1 ;Cmax=(41.786 3±7.521 7)μg·mL-1 ;Tmax=(9.891 9±4.341 4)min;AUC=(22 503.272 7±4 120.182 8)μg·min·mL-1. Liver had the highest concentration of arctiin after oral administration. CONCLUSION RP-HPLC method is rapid, sensitive and specific for the research of arctiin pharmacokinetic and its distribution in rats. Arctiin is distributed and eliminated quickly in rats.
6.Development of a hydrophilic anti-fogging agent for nasal endoscopy.

OBJECTIVETo develop a transparent, non-toxic, non-irritating anti-fogging agent with long-lasting effect for nasal endoscopy.
METHODSThe anti-fogging agent was prepared by mixing ethanol, propylene glycol, polyoxyethylene lauryl ether, sodium dodecyl sulfate, polyethylene glycol 400 and deionized water at different proportions based on an orthogonal test design. Twenty-seven test samples of the anti-fogging agents were obtained, which were colorless, transparent, and non-irritating, with a pH value of 7-8. Storz00 nasal endoscopy and its imaging system were used to test the anti-fogging time of the 27 samples, and each agent was tested for 3 times with medical Seoul iodine and 95% ethanol as control.
RESULTSThe optimal composition of the anti-fogging agent was 20% ethanol, 10% propylene glycol, 20% polyoxyethylene lauryl ether, 4% sodium dodecyl sulfate, 4% polyethylene glycol, 42% deionized water. The anti-fogging time of this agent reached 15 min, significantly longer than that of medical Seoul iodine (4 min) and 95% ethanol (18 s).
CONCLUSIONThis anti-fogging agent for nasal endoscopes is colorless and safe and has a long anti-fogging time by forming a homogenous transparent membrane over the endoscopic lens.

7.Influence factors and quality control of bloodstatned surgical instruments clena effect

Objective To compare the effect of two kinds of soaking methods on bloodstained surgical insturments,and analyze influence factors for instruments clean effect,control key link,improve quality and clean surgical instruments washing efficiency,and to reduce the consumption.Methods Points on three analysis phase:analysis of the cause and gory surgical instruments clean effect.Factors validation phase:on the basis of the moisturizing method is verified.Improve phase:for key factors,establishing countermeasure.Results Oxidation rate of instruments in water group was 19.60%,and 2.30% in disinfection group,the difference between two groups was significant ( x2=26.04,P <0.01 ).Re-washing rate of oval forceps was 23.7% and consumption rate was 15.0% before quality control and those were 0.7% and 0 after quality control,the difference was significant ( x2=74.25,48.69,respectively; P < 0.01 ).Conclusions Sterilization supply &distribution control two keys could improve the quality and washing efficiency of surgical instruments,and reduce the consumption.
8.siRNA suppresses the green fluorescent protein (GFP) gene expression in human umbilical vein endothelial cells

AIM: To establish human umbilical vein endothelial cells (HUVECs) to express green fluorescent protein (GFP), and to study the suppression of GFP by siRNA in HUVECs. METHODS: Using lipofectamine 2000 to transform plasmid pN_3-EGFP encoding GFP into HUVECs. The HUVEC containing pN_3-EGFP, named HUVEC-GFP, was screened and selected by antibiotic G418. Using in vitro transcription T7 kit, GFPsiRNA targeting GFP mRNA and control-siRNA used as control were synthesized. The siRNAs were transfected into HUVEC-GFP with oligofectamine. 48 h later, the expression levels of GFP protein and mRNA in HUVEC-GFP were determined. RESULTS: The HUVEC-GFP was screened to express GFP in the presence of G418. The agarose gel electrophoresis analysis showed that the siRNAs prepared were integrated. 48 h after transfection with siRNAs, compared to control group, the level of GFP fluorescence was obviously decreased in the HUVEC-GFP transfected with GFPsiRNA. The results of RT-PCR detection showed that GFP mRNA expression was obviously suppressed by GFPsiRNA at the rate of 40%, and no obvious suppression of GFP mRNA expression was found in the HUVEC-GFP transfected with control siRNA. CONCLUSION: The siRNA targeting GFP mRNA, synthesized in vitro, efficiently suppresses the GFP expression in HUVECs.
9.Influence of Marital Status on the Quality of Life of Chinese Adult Patients with Epilepsy

Background:Epilepsy is a chronic disorder characterized by recurrent seizures and has significant psychological and social consequence for everyday living.Epilepsy affects various aspects of ones' social life.The present study aimed to investigate the influence of marital status on the quality of life of adult Chinese patients with epilepsy.Methods:This study surveyed 805 Chinese adults who have been clinically diagnosed with epilepsy for longer than 1 year in 11 hospitals in Beijing.In this survey,532 (66.1%) participants were married.All of them completed the case report form with enquiries on demographic data,social factors,and illness.The marriage status of adult epileptic quality of life was the dependent variable,and demographic data and clinical data were independent variables,analyzed through the multiple linear regression analysis methods.The patients' quality of life was assessed using the Quality of Life in patients with Epilepsy-31 items (QOLIE-31) questionnaire,the Patient Health Questionnaire-9 items (PHQ-9),and the Generalized Anxiety Disorder-7 items (GAD-7).Results:The PHQ-9 and GAD-7 scores in the unmarried group (PHQ-9 =6.0 and GAD-7 =5.0) were significantly higher than that of the married group (PHQ-9 =4.0 and GAD-7 =3.0).The scores of married adult patients with epilepsy on QOLIE (61.8 ± 15.3) and social function (70.9 ± 22.7) were higher than the scores of the unmarried patients aged between 20 and 44 years.The scores of married adult epileptics on the QOLIE (58.4 ± 14.6) and the energy/fatigue (62.1 ± 20.4) were higher than the scores of the unmarried patients (QOLIE =58.4 ± 14.6 and the energy/fatigue =62.1 ± 20.4) aged between 45 and 59 years.For the adult epilepsy patients,depression,anxiety,seizures within the last year,disease course,medical expense category,and marriage* age are negatively correlated with the quality of life.Occupation,educational level,and average monthly income are closely related to the quality of life.Conclusions:Married adult epileptic patients have better quality of life than that of unmarried adult patients in young and middle-aged age groups.Unmarried adult patients with epilepsy are more anxious and depressed than married adult patients.
10.Effect of arctiin on hemorheology of experimental rats with blood stasis ayndrone

OBJECTIVE To study influence of arctiin from seeds of Arctium lappa on hemorheology of experimental rats with the blood stasis syndrone. METHODS The blood hemorheology parameters, Fib, aPTT and PT of experimental rats with the blood stasis syndrone were evaluated using semi-automatic biochemical analysis. RESULTS Arctiin obviously decreased their high shear, middle shear, low shear, the blood viscosity, red blood cell aggregation index, red blood cell rigidity index and reductive viscosity. It also significantly prolonged the time of aPTT and PT and lowed the Fib concentration. CONCLUSION Arctiin apparently ameliorated the blood rheology abnormality and enhanced anti-coagulation effect on experimental rats with the blood stasis.
