OBJECTIVE:
To explore the clinical manifestations in pediatric laryngopharyngeal reflux and to provide more information on diagnosis and treatment.
METHOD:
Sixty-two cases with recurrent respiratory infections, hoarseness and chronic cough were examined with reflux symptom index (RSI), reflux finding score (RFS), and 24-hour pH monitoring. Those who had at least two positive test for reflux were given PPI for diagnostic therapy.
RESULT:
All patients completed reflux symptom index (RSI) questionnaire and underwent fiberoptic laryngoscopy, and reflux finding score (RFS) was evaluated. The positive rate of RSI and RFS was 91.94% and 79.03% differently. 24-hours pH monitoring and diagnostic therapy was about 30.76% and 85.48% differently. The symptoms show hoarseness 90.32%, postnasal drip 77.42%, difficulty swallowing 74.19%, abdominal pain and chest pain 72.58%, throat clearing 64.52%, chronic cough 56.45%, dysphagia 51.61%, throat abnormal feeling 48.39%. Laryngoscope examination shows inter-arytenoid erythema 100%, vocal mucosal oedema 75.81%, diffuse laryngeal edema 50.00%, posterior commissure hypertrophy 33.87%, subglottic edema 4.84%, no granuloma case. There was 16 cases showing positive in 24-hours pH monitoring test. The positive rate was 30.76%. All cases accepted diagnostic therapy. Fifty-three cases were effective. The positive rate was 85.48%.
CONCLUSION
There are no clinical presentations specific to pediatric laryngopharyngeal reflux. Patients often present with a wide range of atypical symptoms and signs. RSI questionnaire and RFS may provide diagnostic datas. Primary treatment includes lifestyle and medical therapy.

AIM:To analyze the relationship between epicardial adipose tissue (EAT) thickness and plasma N-terminal pro-B-type natriuretic peptide ( NT-proBNP ) level in the patients with stable coronary artery disease . METHODS:The patients with chest pain ( n=115) admitted to our hospital underwent coronary artery computer tomo-graphy and further underwent coronary angiography for confirming whether they had coronary artery disease .EAT thickness was evaluated at the right ventricular free wall imaged by coronary artery computer tomography .Plasma NT-proBNP level was examined by an automatic biochemistry analyzer .RESULTS:Eighty-one patients were confirmed to have stable coro-nary artery disease and thirty-four patients were excluded to have coronary artery disease .Left ventricular ejection fraction of these patients of 2 groups were all normal.The natural logarithm of plasma NT-proBNP level [ln(NT-proBNP)] of the patients with stable coronary artery disease was significantly higher than that of the patients without coronary artery disease (P<0.05).EAT thickness of the patients with stable coronary artery disease was also higher than that of the patients with -out coronary artery disease(P<0.05).EAT thickness was related to ln(NT-proBNP) positively (P<0.05).After adjust-ment of related impact factors , EAT thickness was still related to ln (NT-proBNP) positively (P<0.05).Multiple-factor regression analysis showed that EAT thickness was the independent influence factor on LnNT -proBNP (P<0.05).CON-CLUSION:EAT thickness and plasma NT-proBNP level are both increased significantly and is related to each other in the patients with stable coronary artery disease .
4.Investigation on incidence of anemia and serum ferritin level among3262 women during second and third trimester

ObjectiveTo understand the prevalence of maternal anemia and iron deficiency during the second and third trimesters and their relationship with adverse pregnant outcomes.MethodsData of 3 262 gravidas, who received prenatal care and delivered at Drum Tower Hospital from October 1, 2013 to October 31, 2014, were retrospectively collected and analyzed. Blood routine test was performed for all subjects both at the second (20-22 weeks) and third trimesters (38 weeks of gestation or before delivery) using automatic blood analyzer. Moreover, serum ferritin level was determined at the second trimester with microparticle chemiluminescence immunoassay. Treatment was offered to those diagnosed as iron deficiency anemia or severe iron deficiency, and the adverse pregnancy outcomes were analyzed.Chi-square test and TrendChi-square test were performed for statistics.Results(1) Out of the 3 262 women, 601 (18.42%) were diagnosed as anemia at second trimester, among which 444 (73.88%) recovered at the third trimester after proper treatment. However, there were 285 (8.74%) new-onset anemia pregnancies at last trimester. Thus, the total prevalence of anemia in pregnancy was 27.16% (886/3 262). (2) The mean serum ferritin level at the second trimester was (29.40± 30.12) ng/ml, and 1 565 (47.98%) were diagnosed as iron deficiency at the same period. (3) During the mid-term pregnancies, 345 iron deficiency anemic women were identified, which accounted for 57.40% of all anemic cases in this study. Two hundred and fifty out of the 345 women were cured at the third trimester. However, another 206 new-onset iron deficiency anemic pregnancies were identified at the third trimester. For iron deficiency anemia both at mid- and late-term pregnancy, the prevalence was 16.89% (551/3 262), which accounted for 62.19% (551/886) of all anemic patients. (4) There were more women with microcytic hypochromic anemia in the late-trimester than in the mid-trimester [19.93%(60/301) vs 4.93% (17/345),χ2=34.478,P<0.01]. (5) Among women with serum ferritin≥30,≥20- <30,≥10- <20- <10 ng/ml, the prevalence of mild anemia in mid-trimester was 13.33%(142/1 065), 14.40%(91/632), 14.71%(130/884) and 24.82%(169/681), the prevalence of new-onset anemia in the third trimester was 3.94%(42/1 065), 5.85%(37/632), 12.78%(113/884), and 13.66%(93/681) respectively. The lower the serum ferritin level, the higher the prevalence of anemia in the mid-trimester (χ2trend=30.697,P<0.01) and the new-onset anemia in the last trimester (χ2trend=69.871,P<0.01). (6) The incidence of postpartum hemorrhage in pregnancies with serum ferritin level at≥10 but <20 ng/ml was significantly higher than those normal one [20.39%(52/255) vs 11.92%(75/629),χ2=10.577,P<0.01]. Neither serum ferritin level nor anemia was associated with other adverse pregnancy outcomes.ConclusionsThe incidences of anemia and iron deficiency remain at a high level at the second and third trimesters. The lower the serum ferritin level at mid-trimester, the higher the incidence of anemia.
5.Effects of needling acupoints at different nerve segments on oxytocin neurons in rat's hypothalamic paraventricular nucleus and intragastric pressure

Objective:To compare and explore the effects of needling acupoints at different nerve segmentson the oxytocin (OT) neurons in the paraventricular nucleus of hypothalamus (PVN) and the intragastric pressure, and discuss the possible mechanisms. Methods: Thirty-two healthy adult Sprague-Dawley (SD) rats were numbered and divided into 4 groups according to the random number table, a Zusanli (ST 36) group, a Neiguan (PC 6) group, a Weishu (BL 21) group and a control group, with 8 rats in each group. Except the control group, rats in the other three groups received acupuncture at the corresponding acupoints. To observe the differences in double-labeled OT neurons and c-fos neurons of the hypothalamic PVN and the intragastric pressure after acupuncture among the three groups of needling acupoints at different nerve segments. Results:Compared with the control group, the numbers of double-labeled cells in the PVN of the Zusanli (ST 36) group and the Neiguan (PC 6) group decreased significantly, while the intragastric pressure increased significantly (allP<0.05), and the inter-group differences were statistically significant (P<0.05). The intragastric pressure in the Weishu (BL 21) group decreased significantly, and the inter-group difference was statistically significant (P<0.05). Compared with the Weishu (BL 21) group, the numbers of OT/c-fos double-labeled cells in PVN of the Zusanli (ST 36) group and the Neiguan (PC 6) group decreased significantly, and the intragastric pressure increased significantly, the inter-group differences were statistically significant (allP<0.01). Conclusion:Acupoints at different nerve segments have different regulation effects on intragastric pressure. The difference may be related to the different nerve conduction pathways by acupoints at different nerve segments in regulating the intragastric pressure. The PVN may be one common integration center for the regulation of gastric function in the three acupoints [Zusanli (ST 36), Neiguan (PC 6) and Weishu (BL 21)] at different nerve segments.
6.Low Concentrations of STI571 Enhances beta1 Integrin Mediated Inhibitory Effect on Proliferation of Myeloid Progenitors in Ph(+)Chronic Myeloid Leukemia

To investigate whether ABL specific tyrosine kinase specific inhibitor STI571 can restore beta1 integrin mediated negative effect on Ph(+) chronic myeloid leukemia(CML), the inhibitory effect of beta 1 integrin activator (beta1 integrin activating antibody 8A2, cytokines such as GM-CSF, G-CSF and SCF) and/or FN on the granulocyte-macrophage colony forming unit (CFU-GM) from 16 patients with Ph(+)CML and 13 normal individuals were examined; the bone marrow mononuclear cells (BMMNC) before and after ABL kinase specific inhibitor STI571 pretreatment (0.1 micro mol/L for 30-60 minutes) were target cells in this study. The roles which VLA4 and VLA5 played in this process were evaluated through blocking assay. The results showed: (1) beta1 integrin activator(s) or FN alone have no effect on CFU-GM from CML or normal bone marrow mononuclear cells before or after STI571 pretreatment, nor STI571 pretreatment itself. (2) The inhibitory effect of beta1 integrin activator(s) plus FN on CML CFU-GM are significantly lower than that on normal CFU-GM. (3) The inhibitory effect of beta1 integrin activator(s) plus FN on CML CFU-GM after STI571 pretreatment is comparable to that on normal CFU-GM. (4) Monoclonal antibody to VLA4 and VLA5 or to total beta1 integrins almost completely abrogate the above effect of STI571. The results suggested enhancing beta1 integrin mediated negative effect on myeloid progenitors in Ph(+)CML is one of the therapeutic mechanisms of STI571 on Ph(+)CML.
7.Risk factors for suicide attempt among college students at Central South University.

OBJECTIVETo understand the prevalence and risk factors for suicidal ideation among college students and to provide a scientific basis for promoting psychological health and suicide prevention.
METHODS623 college students at Central South University were selected using stratified cluster sampling and administered a suicide ideation questionnaire, a Symptom Check List (SCL-90), an Adolescent Self-Rating Life Events Check List (ASLEC), a Social Support Rating Scale (SSRS) and a questionnaire about background information. Multivariate logistic regression analysis was employed to identify risk factors for suicide ideation.
RESULTSOne year prior to our investigation, 14.6% of respondents had suicide ideation, 2.5% had made a specific suicide plan, and 1.8% had made a suicide attempt. The main risk factors for suicide ideation were dissatisfaction with the selected major of study, limited social support, recent negative life events and depressive tendency.
CONCLUSIONSThe prevalence of suicide ideation among these college students was high. Appropriate measures focusing on the risk factors identified in this study should be urgently developed to prevent suicides in college students.

8.Immunophenotypic characteristics of multiple myeloma cells.

To investigate the immunophenotypic characteristics of multiple myeloma (MM) cells, 20 bone marrow samples from patients with multiple myeloma were analyzed by flow cytometry with three-color direct immunofluorescence staining. Results showed that all of myeloma cells expressed bright CD38, dim or negative CD45 and negative CD19. Most of the cells were CD56(+) and a small portions were ckappa(+) or clambda(+), or CD20(+). The phenotypes of normal plasmocyte, CD19(+) and CD56(-), except CD56(-) in one-third samples, were not appeared in all detected samples. It was concluded that the surface marker analysis of myeloma cells is a useful tool for diagnosis and further evaluating prognosis of multiple myeloma.

9.Observation the clinical curative effect of children's laryngopharyngeal reflux and sleep apnea hypopnea syndrome.

OBJECTIVETo observe the clinical curative effect of anti-reflux treatment for obstructive sleep apnea hypopnea syndrome (OSAHS) in children.
METHODSTwenty children with sleep-disordered breathing were included in this study. There were 15 males and 5 females, aged 3-9 years old, median 6 years old. The electronic laryngoscope, polysomnography (PSG) monitoring, Reflux symptom index (RSI) questionnaire and Reflux finding score (RFS) were used to establish the initial diagnosis of OSAHS with LPRD, preclude adenoid hypertrophy and tonsil hypertrophy and nasal disorders. Oral Domperidone and Omeprazole were given for treatment. For children under 3-year-old, the dosage of Domperidone was 0.6 ml.kg⁻¹.day⁻¹.For children over 3-year-old, Domperidone combined with Omeprazole were given with the dosage of 0.3 mg.kg⁻¹.day⁻¹.
RESULTSAfter 4 weeks of treatment, 19 patients symptoms of OSAHS include disturbed sleep, dyspnoea and apneic attack improved. After 8 weeks of treatment, 20 cases with OSAHS symptoms improved than before treatment. Under the electronic laryngoscope, the decrease in pharyngeal lymphoid follicles, the epiglottis, aryepiglottic fold and scoop intergenic region erythema shallow, edema lessened. After treatment of 4 weeks and 8 weeks, there was statistically significant (P < 0.05). Before and after treatment, the difference of RFS was statistically significant (P < 0.05); PSG monitoring proved significant effect in 3 cases (15.0%), effective in 11 cases (55.0%) and 6 cases were ineffective (30.0%). Twenty patients with obstructive apnea index change was not obvious (P > 0.05), apnea hypopnea index and lowest artery oxygen saturation better, differences were statistically significant (Z of 2.819 and 2.733 respectively, P < 0.05).
CONCLUSIONThe treatment of LPRD can improve the symptoms of OSAHS, these two diseases may coexist in mutual relations.

10.Anxiety symptoms in patients with Mayer-Rokitansky-Küster-Hauser syndrome: a cross-sectional study

Background:As a congenital malformation that results in infertility and an inability to have vaginal intercourse,Mayer-RokitanskyKüster-Hauser (MRKH) syndrome places a considerable psychological burden on patients,which results in anxiety symptoms.However,only single case studies or a few small to medium-sized cross-sectional studies were identified to focus on anxiety symptoms in MRKH patients.Thus,the aim of this study was to explore the status of anxiety symptoms and the related factors in patients with MRKH syndrome.Methods:This cross-sectional study involving 141 patients with MRKH syndrome and 178 healthy women was conducted from January 2018 to December 2018.All participants were required to complete a demographic questionnaire and the Generalized Anxiety Disorder 7-item scale (GAD-7),Patient Health Questionnaire-9,Eysenck Personality Questionnaire-Revised,Short Scale for Chinese,and Chinese Version of the Female Sexual Function Index.The main outcome was the anxiety symptoms measured by the GAD-7.Main outcome was compared between the MRKH syndrome group and the healthy control group.Then,we explored the related factors by comparing patients with and without anxiety symptoms.Results:Of the respondents,24.1% experienced moderate to severe anxiety symptoms.Patients with MRKH syndrome manifested more severe anxiety symptoms than healthy women.Negative self-evaluation of femininity (odds ratio [OR] 2.706,95% confidence interval [CI] 1.010-7.247),neurotic personality traits (OR 1.100,95% CI 1.029-1.175),and coexisting depressive symptoms (OR 4.422,95% CI 1.498-13.049) were more prevalent in anxious patients.Conclusion:The findings stress the importance of anxiety symptom screening in MRKH patients and identify patients at risk of anxiety symptoms,providing a possible basis for future intervention.
