1.Rehabilitation Medicine in Chinese Medical Teams in Africa (Part Ⅱ)

By analyzing the experiences and lessons of expanding the patients' sources, promoting new technologies, developing equipments,enhancing the patients with special social status, disseminating Chinese culture and other aspects in medical foreign aid, the view that five combinations should be conducted was concluded to develop rehabilitation medicine effectively in Africa. Which were: the integration of the daily workload improving and the technical structure optimizing, the combination of the own advantages exerting and the actual local conditions considering, the linking of the medical knowledge disseminating and the core technologies leading, the unity of the fair share of things accomplishing and the social image shaping and the association of the masses of the people serving with close ties to senior.
2.Development of Chinese Software of Loewenstein Occupational Therapy Cognitive Assessment

ObjectiveTo develop the manipulation and scoring system of Loewenstein Occupational Therapy Cognitive Assessment (LOTCA) into a Chinese software.MethodsThe manual manipulation and scoring system of LOTCA was developed into a technological procedure and changed into primary product of software through conforming computer programs such as Basic, C, C++ and Flash Maker. The primary product was tested in clinic and feedback suggestions were collected. The questions found during assessment and items with reliability and validity not satisfied were optimized.ResultsThe Chinese software of LOTCA, including mandarin and Cantonese, composed manipulation system, scoring system, administer system and affiliated system. It could run on systems of Windows 2000 and Windows XP.ConclusionThe Chinese software of LOTCA is objective, standard and convenient for clinic.

Objective To investigate the application value of chronic kidney disease epidemiology collaboration (CKD-EPI) equations and amendatory equations for calculating glomerular filtration rate (GFR) in Chinese patients with diabetes,including CKD-EPI serum creatinine (Scr) equation (CKD-EPLcr),CKD-EPI serum cystatin C (ScysC) equation (CKD-EPLcysc),combination of CKD-EPLcr and CKD-EPIScrSCysC (CKD-EPISr SCysC),Asian modified CKD-EPI equation (CKD-EPIAsian),Chinese diabetes CKD-EPI equation (CKD-EPIDiabctes),refitted modification of diet in renal disease (MDRD) equation (rMDRD),Cockcroft-Gault equation (CG) and Simplified MDRD equation.Methods Serum creatinine (Scr) was detected using basic picric acid method,serum cystatin C (CysC) was measured by Latex enhanced turbidimetric,and the reference value of GFR was worked out by 99Tcm-DTPA renal dynamic imaging and standardization of body surface area.Relation of Scr,Scysc,GFR measured by every equations and rGFR were analyzed.The accuracy of 30% of each equation,the consistency and deviation of the evaluation of renal function by each equation were compared.The diagnostic efficacy in GFR<60 ml/(min · 1.73 m2) was compared using the receiver operating characteristic (ROC) curve.Results The rGFR had negative correlation with Scr、ScysC (both P<0.001).The rGFR had positive correlation with GFR measured by CKD-EPIScr,CKD-EPIScysC,CKD-EPIS-SCysC,CKD-EPIAsian,CKD-EPIDiabetes rMDRD,CG and Simplified MDRD (all P<0.001).The coincidence rate between 30% of rGFR and GFR measured byCKD-EPIDiabetes and simplified MDRD were higher.There was no significant difference of rGFR and GFR measured by CKD-EPIscr,CKD-EPIScysC,CKD-EPIScr-SCysC,CKD-EPIAsian,CKD-EPIDiabetes,rMDRD,CG and Simplified MDRD.GFR measured by CKD-EPIDiabetes has the best consistent degree with rGFR.ROC curve showed that the area under curve of GFR measured by CKD-EPIDiabctes was largest and the Youden index was the highest.Conclusion There are different degrees of deviation in eGFR predicted by the eight kinds of equations.The accuracy and consistency of CKD-EPIDiabetes in Chinese 2-type diabetes is the best.
4.Periplocin of cortex periplocae inhibits Stat3 signaling and induces apoptosis in human hepatocellular carcinoma cell line SMMC-7721

Objective To investigate the effect of periplocin of cortex periplocae (CPP) on Stat3 signaling and its probable molecular mechanism of inducing apoptosis and anti-tumor activity. Methods Cell proliferation was detected by MTT. Cell apoptosis and cell cycle were investigated by flow cytometry. Expression of Stat3 protein in SMMC-7721 cells was analyzed by Western blot. Mcl-1, Survivin and XIAP mRNA expressions were measured by RT-PCR. Results CPP inhibited the proliferation of SMMC-7721 cells significantly, induced their apoptosis and arrested their cell cycle at G2/M phase. Decreased expression of Stat3 protein in the cell nucleus was observed after CPP treatment, but no significant changes were found in cytoplasma. Mcl-1, Survivin and XIAP mRNA expression levels were decreased in a time-dependent manner. Conclusion CPP inhibits cell proliferation and induces apoptosis by inhibiting Stat3 signal transduction in human hepatocellular carcinoma cell line SMMC-7721.
5.siRNA targeting STAT5 induces apoptosis in human hepatocellular carcinoma cell line SMMC-7721

Objective To investigate the effects of inhibition of STAT5 gene expression by RNA interference technology on apoptosis of human hepatocellular carcinoma cell line SMMC-7721. Methods Three siRNA eukaryotic expression vectors against STAT5 were constructed and transfected with lipofectamineTM 2000 into SMMC-7721 cells. The changes in STAT5 expression were detected by semi-quantitative RT-PCR and Western blot. Cell apoptosis was assayed by flow cytometry (FCM). Results The sequence-specific siRNA could effectively and specifically inhibit STAT5 gene expression at both mRNA and protein levels. The inhibition rates of STAT5 mRNA expression were 70.43%, 43.02%, and 45.07%, respectively. The inhibition rates of STAT5 protein expression were 67.45%, 37.36%, and 41.86%, respectively. At 48 h after transfection, apoptosis rate was 25.61%. Conclusion siRNA against STAT5 can inhibit STAT5 gene expression in SMMC-7721 cells effectively and specifically and induce apoptosis of SMMC-7721 cells. siRNA targeting STAT5 has a great potential value in gene therapy of hepatocellular carcinoma.
6.Effects of platelet-rich plasma on proliferative and osteogenetic activity of skeletal muscle stem cells in vitro

BACKGROUND:Studies has confirmed that platelet-rich plasma(PRP) can affect the proliferation and differentiation of bone marrow mesenchymal stem cells and adipose derived stem cells. OBJECTIVE:To observe the effects of PRP on the proliferative and osteogenetic activity of skeletal muscle stem cells(SMSCs) in vitro,in addition,to elucidate the potential mechanism by which PRP affects SMSCs. DESIGN,TIME AND SETTING:A randomized controlled animal experiments was completed in the Center Laboratory of the Sixth People's Hospital Affiliated to Shanghai Jiao Tong University from June to October in 2008. MATERIALS:Nine New Zealand,ordinary grade rabbits,weight of 2.5-3.0 kg,1 year old,irrespective of gender. METHODS:The right soleus was dissected and cultured for SMSCs. PRP was prepared with central artery of rabbits’ ears. The cells were randomly divided into the experimental group and the control group. In the experimental group,the SMSCs were interfused with PRP(special culture media including 12.5% PRP) . There was no intervention in the control group. MAIN OUTCOME MEASURES:①Morphological observation. ②The proliferative ability of the cells was detected with MTT. ③ The osteogenetic ability was measured with alkaline phosphatase(AKP) staining,alizarin red staining and osteocalcin immunofluorescence assay. 8.Clinical application value of dual-source dual-energy CT scanning technology for tumor of thorax

Objective To investigate the clinical applications of dual-source dual-energy CT and dual-energy virtual unenhanced iodine distribution technology in thoracic tumor scans.Methods CT and clinical data of 58 cases of patients with thorax tumor were analyzed.The quality score,the noise,the average lesion CT values and radiation dose were compared between dual-energy group virtual unenhanced and conventional unenhanced image.The lesion benign and malignant differential diagnosis by strengthen the distribution of iodine value in dual-energy group and strengthen the value of manual measurement in conventional CT group were compared.Results The quality scores in virtual unenhanced and conventional unenhanced image were (4.62±0.52) and (4.71±0.46) points (t =2.319,P > 0.05).The noise in virtual unenhanced images and conventional unenhanced image were (8.10±1.16) and (7.30±1.06) HU (t =5.246,P< 0.05).The effective dose in virtual unenhanced and in conventional plain were (5.18±0.96) and (5.55±1.18) mSv (t =4.005,P < 0.05).CT value distribution of iodine was correlated with enhanced CT values.Conclusions The virtual unenhanced images can show well lung disease,which meet the diagnostic requirements and may be instead of the conventional plain chest potential value.Dual-energy iodine distribution of lesions can observe blood supply more directly and more sensitively,can quantityze expression of the extent of the lesions enhanced to provide richer information for clinical diagnosis of tumor lesions.
9.Study on relationship between cellular immunity and TCM typing in patients with minimal residual leukemia.

OBJECTIVETo analyse the correlation between the cellular immunity and TCM Syndrome typing of patients with minimal residual leukemia (MRI).
METHODSThe analysis was performed by detecting cellular immunological parameters in 30 MRI patients, 55 healthy persons and 36 patients with acute leukemia (AL) using three fluoresceine-conjugated monoclonal antibodies and flow cytometer.
RESULTSAs compared with those in the healthy persons, universal reductions of various parameters were shown in the MRI patients, including WBC count, absolute value of total T-lymphocytes, T-helper lymphocyte (P < 0.05) and total lymphocytes; the percentage and absolute value of NK cells (P < 0.01); and the percentages of total T-lymphocytes, CD4+ CD29+, T-suppressor cells and T-memory cells (P < 0.05 or P < 0.01), but without any rising of absolute value. As compared with those in the patients with AL, parameters were similar in the two groups with insignificant difference. The disturbances, including the lowering on ratio of T-helper/T-suppressor lymphocytes, in MRI patients of Qi-blood insufficiency type was the severest, that in the patients of Qi-Yin deficiency type was the mildest, and that in patients of Yin-deficiency with excess Fire type located between them.
CONCLUSIONThe immune function of MRI patients is low, belonging to the TCM Syndrome of vital energy deficiency with evil-lingering. Since the degree of cellular immune disturbance is different in various TCM Syndrome types, therefore, they should be treated with different dosages of different drugs.

10.Postoperative position change of fibular bone after reconstruction of maxillary defect using free fibular flap.

OBJECTIVE:
To investigate the position change of the fibular bone after maxillary reconstruction by free fibular flap and to analyze the factors affecting the position change.
METHODS:
Patients who underwent maxillary reconstruction by free fibular flap in the Department of Oral and Maxillofacial Surgery, Peking University School and Hospital of Stomatology from November 2012 to November 2016 were enrolled in this study. CT scans 1 week and 1 year postoperatively were collected and stored in DICOM format. The ProPlan CMF software was used to reconstruct the CT scans and separate the maxilla and each segment of the fibular flap. The Geomagic Control software was used to measure the long axis direction vector of each fibular segment. And the position change direction was recorded. The patients were divided into groups according to the use of the fibula or titanium plate to reconstruct the zygomaticmaxillary buttress.
RESULTS:
A total of 32 patients were enrolled. Among them, 21 were in the titanium plate group and 11 in the fibula bone group. The angle between the long axis of the fibular segment and the X axis in the X-Y plane was 95.65°±53.49° and 95.53°±52.77°, 1 week and 1 year postoperatively, and there was no statistical difference (P>0.05). The angle between the long axis of the fibular segment and the X axis in the X-Z plane was 96.88°±69.76° and 95.33°±67.42°, respectively, with statistical difference (P=0.0497). The angular changes of the long axis of the fibular segment in the titanium plate group and the fibular bone group were 3.23°±3.93° and 1.94°±1.78°, respectively, and the angular changes in the X-Z plane were 6.02°±9.89° and 3.27°±2.31°, respectively. There was no significant difference between the groups (P>0.05). The long axis changes of the fibular segment in the X-Y plane for reconstruction of the anterior alveolar, posterior alveolar, and buttress were 3.13°±3.78°, 2.56°±3.17°, and 5.51°±4.39°, respectively. There was a statistical difference (P = 0.023) between the posterior and buttress. In the X-Z plane, theses were 4.94°±4.75°, 5.26°±10.25°, 6.69°±6.52°, respectively. There was no statistical difference among the three groups (P>0.05). The main positional deviation directions of the titanium plate group and the fibular bone group were interior and superior sides, and there was no statistical difference between the two groups (P>0.05).
CONCLUSION
One year postoperatively, the position of the free fibular flap was changed compared with 1 week postoperatively. The position of the free fibular flap was mainly changed to the interior and superior sides.
