1.Influences of Deduhonghua -7 powder on matrix metalloproteinases and transforming growth factor beta expression in liver fibrosis

Objective To observe the therapeutic effect of Deduhonghua-7 powder on liver fibrosis and its correlation with matrix metalloproteinases ( MMPs) and transforming growth factor β1 ( TGF-β1 ) .Methods The rats were divided into 4 groups, blank group, model group, control group, experiment group, 8 rats each group.The CCl4 was used to induce rat liver fibrosis.The control group was given colchicine pill, CMC sodium cellulosate was used to prepare 2% of colchicine suspen-sion, once time per day, 0.2 mL · kg-1 each time.The experiment group was given Deduhonghua-7 powder, CMC sodium cellulosate was used to prepare 3% of Deduhonghua-7 powder suspension, once time per day, 0.2 mL· kg -1 each time; blank group and model group were given same volume of pure water.All rats were sacrificed after 40 days of medication.Western blot was acted to detect the matrix metalloproteinase-1 (MMP-1), matrix metalloproteinase-13 (MMP-13),tissue inhibitor of metalloproteinases-1 (TIMP-1) protein expression.The PCR was used to detect the changesoftype Ⅰ, Ⅲ collagen, TGF -β1 mRNA gene expression.Results The MMP-1, MMP -13 protein expression in rats of model group was down -regulated compared with blank group ( P<0.05 ).The TIMP-1 protein expression was up-regulated compared with blank group ( P<0.05 ) . The mRNA expressions of typeⅠprocollagen, typeⅢprocollagen and TGF-β1 were increased compared with blank group(P<0.05).The MMP-1, MMP-13 protein expression of control group and experiment group were significant-ly up-regulated compared with model group.The TIMP-1 protein expression significantly down-regulated compared with model group.The mRNA gene expression of type I procollagen, typeⅢprocollagen and TGF-β1 were down-regulated compared with model group ( P<0.05 ).Conclusion Deduhonghua-7 powder regulate and maintain the stabilization of MMP/TIMP to block the expression of TGF-βand inhibit the transcription of type I,Ⅲ collagen and TGF-β1 mRNA to reach the purpose of detention of occurrence and development of liver fibrosis.

Vancomycin (VCM) is the first-line therapy for invasive multi-resistant gram-positive bacterial infections involving Methicillinresistant Staphylococcus aureus.The therapeutic range of VCM is narrow,and its use is associated with nephrotoxicity.The pharmacokinetic characteristics of VCM vary greatly with renal function.Therapeutic drug monitoring and pharmacokinetic (PK) monitoring are required for maximizing its efficacy and minimizing adverse effect.Several studies focused on vancomycin individualized dosing have been published recently.The advances of vancomycin individualized dosing via PK tool are reviewed in this study.
3.Influences of deduhonghua -7 powder on transforming growth factor beta and smad protein pathways in liver fibrosis tissue

Objective To observe the therapeutic effect of deduhonghua-7 powder on liver fibrosis and explore its relevance with transforming growth factor beta ( TGF-β) and smad protein pathways.Methods The liver fibrosis rat model were randomly divided to A group, B group, C group, D group, 8 rats for each.A group was administered 0.6 g·kg-1 of deduhonghua -7 powder, B group was administered 0.4 mg·kg-1 of colchicines, C group and D group were administered dis-tilled water intragastrically 1 time per day for 40 days.The liver tissue was prepared by histopathology slides, than elastic and collagen fiber staining was acted to observe the degree of fibrosis.Enzyme-linked im-munosorbent assay ( ELISA ) was acted to detect the hyaluronic acid ( HA) , laminin ( LN) content of liver tissue homogenate.Immunohisto-chemistry detecting was acted to determine the changes of smad2, smad3, smad6, smad7, and TGF-β1 protein expressions.Results Compared with D group, the collagen fiber and contents of HA, LN of C group significantly increased (P<0.05).Compared with C group, the collagen fiber and the contents of HA, LA of A group and B group significantly decreased (P<0.05).The TGF-β1, smad2, smad3 protein expressions of C group were up-regulated compared with those of D group with significant differences ( P<0.05).The TGF-β1, smad2, smad3 protein expres-sions of A and B groups were down-regulated compared with those of C group, with significant differences ( P <0.05) .The smad6, smad7 protein expressions of C group were down-regulated compared with those of D group, with significant differences (P<0.05).The smad6, smad7 protein expressions of A and B groups were up-regulated com-pared with C group, with significant differences (P<0.05).Conclusion Deduhonghua-7 powder can regulate the TGF-β/smad protein patheways and block the TGF-βexpression, inhibit the synthesis of extracellular matrixc and degradation of extracellular matrixc to reach the purpose of blocking and deferring the development of liver fibrosis.
4.Whole brain irradiation for non-small-cell lung cancer with brain metastasis.

OBJECTIVETo investigate the time of whole brain irradiation and the prognostic factors for non-small lung cancer patients with brain metastasis.
METHODSFrom August 1996 to December 2003, 147 patients with brain metastasis from non-small cell lung cancer received whole brain irradiation. The patients were divided into two groups: with or without symptoms caused by brain metastasis, each group was then divided into two sub-groups, early whole brain irradiation group (the interval between the diagnosis of brain metastasis and the brain irradiation < or = one month) and late group ( the interval > one month ). Univariate and multivariate analysis (Cox regression) as well as Kaplan-Meier method in SPSS software package 11.5 was used to analyze the data of the 147 patients including 72 with brain metastasis symptom and 75 without.
RESULTSThe median survival time (MS) of patients with or without extracranial metastasis was 9.9 months and 11.3 months (P = 0.0002). Multivariate analysis indicated that extracranial metastasis was an independent prognostic factor (P = 0.0004). For 72 patients with brain metastasis symptom, the MS of the patients with and without extracranial metastasis was 9.3 months and 11.3 months (P = 0.0036). The MS of patients with early and late whole brain irradiation was 11.4 months and 9.2 months (P = 0.001). Multivariate analysis showed that extracranial metastasis, the interval between the diagnosis of brain metastasis and the whole brain irradiation were independent prognostic factors. However, for 75 patients without brain metastasis symptom, the MS difference of those with early or late whole brain irradiation was not statistically significant (P = 0.1643).
CONCLUSIONThe extracranial metastasis in non-small cell lung cancer patients with brain metastasis is an independent prognostic factors. Early whole brain irradiation may improve the survival for those with brain metastasis symptoms.

5.Cardiac risk stratification in patients with congestive heart failure: a catecholamines-beta-adrenoceptor-cAMP pathway.

OBJECTIVETo investigate the stratification risk of catecholamines-beta-adrenoceptor (beta-AR)-cAMP pathway for cardiogenic death events in patients with congestive heart failure (CHF).
METHODSA total of 83 identified CHF patients with a baseline and follow-up plasma levels of norepinephrine (NE) and epinephrine (E), lymphocytes beta-AR density (Bmax), and intralymphocyte cAMP content in peripheral blood were followed up. Major cardiogenic death events were registered.
RESULTSThe period between the initial entry and the last follow-up measurement were 51 +/- 16 months, the total duration of clinical follow-up after the last measurement were 14 +/- 8 months. During follow-up, 39 patients died of cardiogenic (sudden death 17 patients, worsening heart failure 22 patients). Persistence of high NE, E, and cAMP from baseline to follow-up were confirmed as risk predicting factors of cardiovascular events. Persistence NE above 4.0 nmol/L, E above 3.5 nmol/L, and the intralymphocyte cAMP content above 3.5 pmd x mg(-1) x pro(-1) from baseline to follow-up were significant adverse prognostic predictors. The major cardiogenic death events rates per 100 patients-years were 1.33 and 4.82 in patients with NE below and above 4.0 nmol/L (HR: 2.91; 95% CI: 1.08-7.33; P = 0.015); were 1.42 and 4.36 in the patients with E levels below and above 3.5 nmol/L (HR: 2.64; 95% CI: 1.02-6.41; P = 0.019); were 1.81 and 4.67 in the patients with the intralymphocyte cAMP content below and above 3.5 pmd x mg(-1) x pro(-1) (HR: 2.79; 95% CI: 1.04-6.83; P = 0.017), but difference was not significant between the beta-AR density below and above median.
CONCLUSIONSPersistent increase in circulating catecholamines and intralymphocyte cAMP content may increase the long-term mortality in CHF patients.

6.Establishment and evaluation of two common pressure ulcer rat models

Objective A comparison of two method of establishing pressure ulcer rat models to determine which is the most suitable for experimental use.Methods 18 male SD rats were randomly divided into control(n=6),model A(n=6)and model B(n=6)groups.In the control group,iodophor treatment was given after hair removal at the simulated modeling site.In model group A,longitudinal compression was performed by simple deep-tissue foreign body implantation.In model group B,transverse compression was performed via the magnet compression method.The times required to complete the process and for each stage of pressure ulcer model establishment in each group were recorded.The general condition of the rats was observed,and the modeling rate,mortality rate,and infection rate were compared.Results By naked eye,we observed that the model A and model B groups gradually developed redness and swelling,ulceration,bleeding,exudation,and necrosis.Comparison of the whole time to produce pressure uler between model A and model B groups:the difference between the two groups was statitically significant(P<0.05).Comparison of the time to produce pressure injury between Model A and Model B:The difference between the two groups at stage Ⅰ was not statistically significant(P>0.05);the difference between the two groups at stage Ⅱ was statistically significant(P<0 05);the difference between the two groups at stage Ⅲ was statistically significant(P<0 05);the difference between the two groups at stage Ⅳ was statistically significant(P<0 05).The mental and sports scores of the rats in the control group were significantly different from those in the model A and model B groups(P<0.05).The general state of rats in the model group A was significantly different from that in the model B group,and coat color was dimer and activity decreased in the model group A.The modelling rate of rats in both model A and model B groups was 100%.The mortality and infection rates of the model group A were higher than those of the model group B,which were 33.34%and 16.70%,respectively.Conclusions Successful preparation of a four-stage model of pressure ulers in both modalities.The two method have both commonalities and distinct characteristics.The magnet compression method required less time,the rats were generally in good condition,and the mortality and infection rates were low;thus it is suitable for short-term intervention research.The simple deep-tissue foreign body implantation method took longer,required rats to have a certain level of tolerance,had high infection and mortality rates,and is more suitable for use for long-term observations of pressure ulcers.
7.Associations of reproductive health indicators with lung function and COPD among female community residents aged 40 years and above in Songjiang District,Shanghai

Objective To investigate the associations of reproductive health indicators with lung function and chronic obstructive pulmonary disease(COPD)among women aged 40 years and above.Methods From Jul to Sep,2021,female subjects aged 40 years and above were randomly selected from the Shanghai Suburban Adult Cohort and Biobank for COPD screening.A questionnaire was used to obtain information on demographic characteristics and reproductive health indicators.Linear regression was used to analyze the effects of reproductive health indicators on forced vital capacity(FVC)and forced expiratory volume in the first second(FEV1).Logistic regression was also used to analyze the effects of reproductive health factors on FVC as a percentage of the predicted value(FVC%Pred)and FEV1%Pred as well as on COPD.Results A total of 1876 women aged 40 years and above were enrolled with mean age of(62.1±8.2)years old,among them,78.1%were menopausal,and 40.9%had been pregnant≥3 times.Multivariate analysis showed that FVC and FEV1 decreased in postmenopausal women,but menopause was not associated with a decrease in their percentage of predicted values.Pregnancies≥3 times was a risk factor for COPD(for 3 times,OR=4.92,95%CI:1.48-19.95,P<0.05;for≥4 times,OR=9.06,95%CI:2.32-41.57,P<0.01),while pregnancies of 2 times did not increase the risk of COPD.Conclusion In women aged 40 years and above,menopause is associated with poorer FVC and FEV1,and excessive pregnancy(≥3 times)is a risk factor for COPD.
8.Development of a droplet digital polymerase chain reaction assay for the sensitive detection of total and integrated HIV-1 DNA

Background::Total human immunodeficiency virus (HIV) DNA and integrated HIV DNA are widely used markers of HIV persistence. Droplet digital polymerase chain reaction (ddPCR) can be used for absolute quantification without needing a standard curve. Here, we developed duplex ddPCR assays to detect and quantify total HIV DNA and integrated HIV DNA.Methods::The limit of detection, dynamic ranges, sensitivity, and reproducibility were evaluated by plasmid constructs containing both the HIV long terminal repeat (LTR) and human CD3 gene (for total HIV DNA) and ACH-2 cells (for integrated HIV DNA). Forty-two cases on stable suppressive antiretroviral therapy (ART) were assayed in total HIV DNA and integrated HIV DNA. Correlation coefficient analysis was performed on the data related to DNA copies and cluster of differentiation 4 positive (CD4 +) T-cell counts, CD8 + T-cell counts and CD4/CD8 T-cell ratio, respectively. The assay linear dynamic range and lower limit of detection (LLOD) were also assessed. Results::The assay could detect the presence of HIV-1 copies 100% at concentrations of 6.3 copies/reaction, and the estimated LLOD of the ddPCR assay was 4.4 HIV DNA copies/reaction (95% confidence intervals [CI]: 3.6-6.5 copies/reaction) with linearity over a 5-log 10-unit range in total HIV DNA assay. For the integrated HIV DNA assay, the LLOD was 8.0 copies/reaction (95% CI: 5.8-16.6 copies/reaction) with linearity over a 3-log 10-unit range. Total HIV DNA in CD4 + T cells was positively associated with integrated HIV DNA ( r = 0.76, P <0.0001). Meanwhile, both total HIV DNA and integrated HIV DNA in CD4 + T cells were inversely correlated with the ratio of CD4/CD8 but positively correlated with the CD8 + T-cell counts. Conclusions::This ddPCR assay can quantify total HIV DNA and integrated HIV DNA efficiently with robustness and sensitivity. It can be readily adapted for measuring HIV DNA with non-B clades, and it could be beneficial for testing in clinical trials.
9.Multidimensional supportive psychological intervention in clinical practice teaching of andrological nursing

Objective:To examine the application of a novel pedagogical approach multidimensional supportive psychological intervention(MSPI)in the clinical practice teaching of andrological nursing care.Methods:Using the Hamilton Depression Scale(HAMD),we assessed the psychology of 100 nursing interns about to enter clinical practice in the Department of Andrology from De-cember 2021 to December 2022.We equally randomized the subjects into an experimental and a control group,the former receiving MSPI and the latter trained on the conventional teaching model without any psychological support intervention.Results:Compared with the baseline,the HAMD scores were significantly decreased in the experimental group after intervention(12.4±2.1 vs 8.9±2.4,P<0.01),but increased in the controls(13.1±1.8 vs 14.7±1.9,P<0.01);the skill scores dramatically increased in the experimental group(82.6±4.7 vs 91.2±2.4,P<0.01),but decreased in the control group after intervention(81.0±3.5 vs 80.4±2.7,P=0.28).Conclusion:MSPI can significantly enhance the learning enthusiasm of nursing students in a short period,re-duce their psychological stress and improve teaching outcomes.This approach,combining psychology with teaching,can also strength-en the mental resilience of nursing students and better confront them with future professional challenges.
10.Study on rapid quality analysis method of Shengxuebao Mixture.

A rapid analysis method based on ultraviolet-visual(UV-Vis) spectroscopy, near infrared(NIR) spectroscopy and multivariable data analysis was established for quality evaluation of Shengxuebao Mixture. The contents of eight active ingredients of Shengxuebao Mixture including albiflorin, paeoniflorin, 2, 3, 5, 4'-tetra-hydroxy-stilbene-2-O-β-D-glucopyranoside, specnuezhenide,ecliptasaponin D, emodin, calycosin-7-glucoside and astragaloside Ⅳ were simultaneously detected by using this method. HPLC-UV-MS was used as a reference method for determining the contents of these ingredients. Partial least squares(PLS) analysis was implemented as a linear method for multivariate models calibrated between UV spectrum/NIR spectrum and contents of 8 ingredients. Finally, the performance of the model was evaluated by 24 batches of test samples. The results showed that both UV-Vis and NIR models gave a good calibration ability with an R~2 value above 0.9, and the prediction ability was also satisfactory, with an R~2 value higher than 0.83 for UV-Vis model and higher than 0.79 for NIR model. The overall results demonstrate that the established method is accurate, robust and fast, therefore, it can be used for rapid quality evaluation of Shengxuebao Mixture.
