1.Expression.purification and antigenicity of a recombinant respiratory syncyttal virus fusion gene encoded protein(amino acids 168-289)in the insect cells

Objective To explore the antigenicity of the recombinant respiration syncytial virus (RSV)fusion protein (amino acids 168-289) encoded by 546-881 bases of the fusion gene expressed in insect baculoviruses expression system.Methods The fragment F' of fusion gene 546-881 bases was amplified from viral RNA( Long strain ) by the reverse transcription-polymerase chain reaction(RT-PCR).F' was inserted into transfer vector pBacPAK9 and the recombinant plasmid pBacRSV F' was constructed.Sf9 insect cells were then co-transfeeted with a mixture of recombinant plasmids pBacRSV F' and linearized BacPAK6 viral DNA( Bsu36 Ⅰ -digested).The recombinant baculoviruses BacPAK F' was constructed and was able to express the recombinant protein in Sf9 insect cells.The recombinant protein was purified by Ni2+ NTA chromatography and its antigenicity was identified by Western blot(WB) analysis.Specimens including the nasopharyngeal aspirates(NPAs) and sera were collected from 33 infants and young children with acute lower respiration tract infection. Indirect immunofluorecenee assay (IFA) and WB were used to detect the RSV antigen in NPAs and the anti-RSV antibody in sera respectively.Results The recombinant protein (molecular weight 13 000) was expressed in Sf9 insect cells.WB analysis demonstrated that the purified recombinant protein had a specific RSV antibody-binding activity. The recombinant protein could be recognized by positive serum infected by RSV.The positive rate was 27.3% and 33.3% respectively.There was no significant difference between them(X2 = 0.29 ,P > 0.05).Conclusion The recombinant respiratory syncytial virus fusion gene (546-881 bp) encoding protein is expressed in Sf9 insect cells and it has strong antigenicity and could have clinical application value for detection of RSV infection.

5.Comparing clinical effects of titanic elastic nail and locking compression pine fixation in treating subtrochanteric fractures in older children.

OBJECTIVETo explore optimal choice of surgical treatment for subtrochanteric fractures in older children.
METHODSA retrospective study of 36 older children with subtrochanteric fractures was performed between January 2010 and January 2012. Among them, 18 patients (11 males and 7 females) aged from 7 to 13 years old with an average of 9.4 were treated with titanic elastic nail (TEN) fixation, 4 cases were Type II A, 3 cases were II B, 2 cases were II C, 4 cases were III A, 3 cases were III B according to Seinsheimer classification. Eighteen patients (10 males and 8 females) aged was from 8 to 13 years with an average of 9.6 were treated with locking compression pine (LCP) fixation, and 3 cases were Type II A, 4 cases were II B, 3 cases were II C, 4 cases were IIIA, 2 cases were III B. Fracture healing time, postoperative complications (including wound infection, failure and breakage of internal fixtion, deformities of angular on the sagittal view, deformities of coxa vara) and recovery of hip joint function were observed and recorded.
RESULTSAll children were followed up from 15 to 36 months with an average of 21. Fracture were all healed, the time ranged from 7 to 16 weeks (mean 9.5). Three cases in TEN group occurred mild deformities of angular on the sagittal view, 3 cases occurred deformities of coxa vara and 2 cases occurred limb shortening; while 1 case occurred mild deformities of angular on the sagittal view, and no deformities of coxa vara and limb shortening occurred in LCP group. No early close of epiphyseal injury, avascular necrosis of femoral head occurred. Clinical efficacy were evaluated by Sanders standard, 14 cases got excellent results, 3 cases were moderate in LCP group, while 9 cases in excellent, 4 in moderate in TEN group. There were no significant differences between two group in recovery of hip joint function and complications.
CONCLUSIONFor the treatment of subtrochanteric fractures in older children,the efficacy of LCP fixation is better than that of TFN fixation, which has advantages of reliable fixation, and less complications.

6.Improvement of the oocyte quality with electroacupuncture in infertility patients of kidney deficiency pattern.

OBJECTIVETo explore the impact and effect mechanism of electroacupuncture (EA) on oocyte quali ty in the patients with infertility of kidney deficiency pattern.
METHODSSixty-six cases differentiated as kidney de ficiency and with in vitro fertilization-embryo transplantation (IVF-ET), aged fromnt 35 to 42 years were rando- mized into an observation group and a control group, 33 cases in each one. The IVF-ET therapy of the long proto- col with gonadotrophin releasing hormone agonist was adopted in the two groups. In the observation group. on the 5th day of menstruation in IVF cycle, EA was applied to Sanyinjiao (SP 6). Zigong (EX CA 1), Zhongji (CV 3) and Guanyuan (CV 4). In the control group, the sham-acupuncture was applied to the same acupoints. The treatment was given once every two days till the date of egg collection and the needles were retained for 30 min each time. The change in the score of kidney deficiency syndrome, the high-quality oocyte rate, the high-quality embryo rate and clinical pregnant rate were observed in the two groups. The levels of insulin-like growth factor-i (IGF-1) and IGF-2 in follicular fluid and the serum β-endorphin β-EP) on the date of egg collection and the correlation with oocyte quality were compared bIetween the two groups.
RESULTS1) In the observation group, the kidney deficiency syndrome score after treatment was reduced apparently as compared with that before treatment (P<0. 05), the score after treatment in the observation group was reduced much more apparently as compared with the control group (P<0.05). 2) The high-quality egg rate and the high-quality embryo rate in the observation group were both higher than those in the control group [81.3% (161/198) vs 57.6% (98/170), 59.8% (58/97) vs 37.7% (26/69), both P<0.05]. 3) Compared with the control group. the levels of IGF-1 and IGF-2 in follicular fluid and serum β-EP on the day of egg collection were all increased obviously in the observation group (all P<0. 05). 4) The levels of IGF-1 and IGF-2 in follicular fluid and serum β-EP presented the linear positive correlation with the high-quality egg rate.
CONCLUSIONEA effectively improves the expressions of IGF in follicular fluid and serum β-EP, increases the high-quality egg rate and high-quality embryo rate and relieves the symptoms of kidney deficiency.

7. Risk factors for bleeding esophageal varices in patients with hepatocirrhosis: A meta-analysis

Objective: To analyze the related risk factors for esophageal variceal bleeding (EVB) in patients with hepatocirrhosis and portal hypertension, so as to provide clinical evidences for establishing preventive measures for EVB. Methods: Using "* esophag *", "varice *", "bleeding", "hemorrhage", and "risk factor *" as the key words, we searched the clinical studies (1986-2006) about the risk factors of EVB in hepatocirrhosis patients in PubMed, Medline, Chinese Biomedical Database, Elsevier Database, OVID Database, etc. for Meta-analysis. The odds ratio(OR)of each risk factor was estimated and the 95% confidence interval[95% CI] was calculated. Results: Totally 19 papers met our criteria and were included in this Meta-analysis. The 19 papers involved 995 EVB patients and 1 854 controls. Meta-analysis revealed that a hepatic function of Child C, decreased prothrombin activity, hypoalbuminemia, severe esophageal varices, positive red-color sign, extended portal vein width and splenic vein width, thrombopenia, leucopenia and anemia were the risk factors of EVB; a hepatic function of Child A and mild esophageal varices were the protective factors of EVB. The gender, age, hepatic function of Child B, ascites, hepatic encephalopathy, hyperbilirubinemia and midrange esophageal varices were not significantly associated with EVB. Conclusion: Improvement of poor hepatic function, blood coagulation status, hypoalbuminemia and treatment & prevention of severe esophageal varices (by endoscopic variceal ligation, devascularization and shunt) can help to reduce the incidence of EVB.
8.Ultrasound-Guided Percutaneous Pneumatic Nephrolithotomy Combined with Ultrasonic Lithotripsy for Treating Complicated Renal Calculi: A Report of 59 Cases

Objective To evaluate the clinical efficacy of ultrasound-guided percutaneous pneumatic nephrolithotomy combined with ultrasonic lithotripsy in the treatment of renal calculi.Methods Fifty-nine patients with complicated renal calculi(ranged from 2.8 to 7.0 cm in diameter measured by KUB),including 36 patients with unilateral calculi and 23 patients with bilateral calculi,received percutaneous nephrolithotomy by using ultrasonic and pneumatic lithotripter(EMS Ⅲ),puncture hard wire,and mental dilator.A track from the skin surface into the kidney was established under the guidance of B ultrasonography.Results 82 tracks in the 59 patients were successfully established.The mean operation time was 90 minutes(45-150 minutes),and the management time of stones was 30-70 minutes.Residual stones were found in 15 kidneys and were removed by extracorporeal lithotripsy or two stage lithotripsy two weeks after operation.The clearance rate of stone in one session and two sessions was 82%(67/82) and 93%(14/15),respectively.Hospital stay was 5-15 days(mean,10 days).A follow-up of 1-6 months(mean,3.2 months) in 59 cases showed no severe bleeding,infections,and stone recurrence.Conclusions Ultrasound-guided percutaneous pneumatic nephrolithotomy combined with ultrasonic lithotripsy for complicated renal calculi is characterized by high efficiency,safety and minimal invasion.

Objective To study the effect of hydrogen peroxide on microstructure of mice kidney and discuss the toxic effect on mice kidney.Methods Thirty healthy male mice of Kunming Genus were divided into 3 groups at random:control group and two experimental groups. Running water was fed to control group for 10 days while 0.3,3 g/L hydrogen peroxide running water readily prepared was fed to the experimental groups for 10 days. On the 10th day,the kidneys were taken out,and fixed in the fixation solutions,conventionally produced and stained.Finally,they were studied under the optical microscope.Results Experimental groups:in the kidney tissue cytoplasm of proximal convoluted tubule showed hydropic degeneration and vacuolation which depend on dose of hydrogen peroxide.Conclusion Toxic effect on mice kidney can be caused by hydrogen peroxide.
