4.Observation of the curative effect of rigid gas permeable contact lens in the treatment of adult high and special type ametropia

?AlM:To explore the use of rigid gas permeable ( RGP ) contact lens, and its clinical effect and safety in the treatment of adult high and special type ametropia.
? METHODS: Totally 110 cases ( 185 eyes ) of Ophthalmology Center fitting of RGP from January 2010 to December 2012 were retrospective analyzed. According to the refractive error types, patients were divided into high myopia astigmatism group( 49 cases, 92 eyes), 36 cases (64 eyes) of keratoconus group, special ametropia group (25 cases, 29 eyes). Corrected visual acuity differences, adverse reactions of different temporal acuity and conventional frame in the three group were observed after wearing RGP.
?RESULTS:After wearred RGP, corrected visual acuity of high myopic astigmatism group was 4. 94 ± 0. 16, keratoconus group was 4. 98±0. 15, and special ametropia group was 4. 87±0. 19; they were significantly better than wearing frame mirror 4. 86±0. 23, 4. 79±0. 22, 4. 61±0. 27 and the differences were statistically significant (P<0. 05). After 3 and 6mo of wearing RGP, patients eyesight after RGP was better than that of wearing frame glasses ( P<0. 05 ) . As the astigmatism group, keratoconus group, special ametropia group wearing RGP, visual acuity after distribution were obviously better than the case when wearing frame glasses(P<0. 05).
?CONCLUSlON: Wearring RGP can correct adult high and special type ametropia than ordinary frame glasses, which avoid the occurence of dry eye, foreign body sensation, and easy fatigue. lt is worthy of clinical application.
5.Therapeutic observation on point-toward-point needling at points on the low back regions for residual back pain after percutaneous kyphoplasty

Objective:To observe the effect of point-toward-point needling at points on the low back regions on residual back pain after percutaneous kyphoplasty (PKP),thoracolumbar function and quality of life (QOL) in patients with osteoporotic vertebral compression fracture (OVCF).Methods:A total of 72 patients with OVCF and residual back pain after PKP were included and allocated into a treatment group (n=36) and a control group (n=36) by the random number table.Cases in the control group received salmon calcitonin injection (miacalcic),for 1 mL each time,once a day,and oral intake of calcium carbonate D3 pill,600 mg each time,once a day,whereas cases in the treatment group received point-toward-point needling at points on the low back regions and electroacupuncture (EA) for 30 min each time,once a day and 6 times a week on the basis of the treatment in the control group.Treatment in both groups lasted for 2 weeks.Therapeutic efficacy indicators including visual analog scale (VAS),Oswestry disability index (ODI) and Barthel index (BI) were evaluated before and after treatment and at follow-up visit (1 month after treatment).Results:The total effective rate was 94.4% and 88.9% in the treatment group after treatment and at the follow-up visit,respectively,higher than 77.8% and 69.4% in the control group,and the between-group differences were statistically significant (both P<0.05).After treatment and at the follow-up visit,the VAS and ODI scores decreased,and BI scores increased in both groups,and the intra-group differences were statistically significant (all P<0.05).After treatment and at the follow-up visit,between-group differences of VAS,ODI and BI scores were statistically significant (all P<0.05).Conclusion:On the basis of conventional medication treatment,point-toward-point needling at points on the low back regions has a good therapeutic effect in relieving residual back pain after PKP,improving thoracolumbar function and QOL,better than conventional medication treatment alone.
6.Surgical management of gastrointestinal stromal tumors.

Gastrointestinal stromal tumors (GISTs) are the most common mesenchymal tumor of the gastrointestinal tract. GISTs have characteristic by a specific histological and immunohistochemical pattern. Malignant biological behaviors should be paid more attention in treatment. At present, operation is still the most important treatment method for GISTs. For patients with primary, localized GISTs, surgery represents the only chance of cure. The principle of surgery is complete resection of visible and microscopic lesions. Special care needs to be taken to avoid capsule rupture and intra-abdominal spillage, which increase the risk of recurrence. Positive resection margins may result in a higher risk of local and peritoneal relapse. Every effort should be taken to achieve negative margins. Although wide margins have not been shown to be beneficial, an en bloc resection is still recommended, when GISTs are densely adherent to adjacent organs. As GISTs rarely metastasize to lymph nodes, lymphadenectomy is not routinely indicated. While laparoscopic resection of GISTs is technically possible, it should only be undertaken when it will not increase the chance of tumor rupture. The success of imatinib in the treatment of recurrent and metastatic GISTs have prompted investigation into the adjuvant treatment for the significant risk of recurrence of GISTs and neo-adjuvant treatment for unresectable cases. Combined complete surgical resection and molecular therapy are the new therapy model which will improve therapeutic effect.

7.The analysis of the changes of high density lipoprotein cholesterol and high sensitivity C reactive protein in coronary heart disease patients combined with type 2 diabetes mellitus

Objective To explore the significance of the changes of high density lipoprotein cholesterol (HDL-C) and high sensitivity C reactive protein (hs-CRP) in coronary heart disease patients combined with type 2 diabetes mellitus,and analyze the relationship between HDL-C and hs-CRP.Methods Two hundred and ten elder patients were selected.The patients were divided into simple coronary heart disease group (A group,70 cases),coronary heart disease combined with type 2 diabetes mellitus group (B group,71 cases),and simple type 2 diabetes mellitus group (C group,69 cases) according to the pathogenetic condition.Meanwhile 50 healthy controls were selected as control group (D group).The levels of HDL-C and hs-CRP were measured,and correlation analysis used Pearson linear correlation.Results The levels of HDL-C in A,B and C group were significantly lower than those in D group:(1.98 ± 0.47),(0.99 ± 0.31) and (1.27 ± 0.44) mmol/L vs.(2.18 ± 0.42) mmol/L,and there were statistical differences (P < 0.05 or <0.01);the level of HDL-C in B group was significantly lower than that in A and C group,and there were statistical differences (P < 0.05).The levels of hs-CRP in A,B,and C group were significantly higher than that in D group:(5.71 ± 1.81),(7.71 ± 2.32) and (4.68 ± 2.09) mg/L vs.(1.60 ± 0.80) mg/L,and there were statistical differences (P< 0.01);the level of hs-CRP in B group was significantly higher than that in A and C group,and there were statistical differences (P < 0.05).Correlation analysis results showed that hs-CRP and HDL-C had signficant negative correlation (r =-0.304,P < 0.01),and hs-CRP and LDL-C had signficant positive correlation (r =0.341,P < 0.01),hs-CRP and triacylglycerol (TG) had no correlation (r =0.006,P > 0.05);hs-CRP and fasting plasma glucose (FPG) had signficant positive correlation (r =0.415,P < 0.01),and hs-CRP and glycosylated hemoglobin (HbA1c) had signficant positive correlation (r =0.384,P < 0.01).Conclusions The levels of HDL-C in patients with elder patients with simple coronary heart disease and simple type 2 diabetes mellitus are significantly lower,and the levels of hs-CRP are significantly higher.The level of HDL-C in elder coronary heart disease patients combined with type 2 diabetes mellitus is significantly lower than that in elder patients with simple coronary heart disease and simple type 2 diabetes mellitus,and the level of hs-CRP is significantly higher than that in elder patients with simple coronary heart disease and simple type 2 diabetes mellitus.The level of hs-CRP is HDL-C have signficant negative correlation.
8.Clinical observation of repairing the perforation of pulp floor with MTA

The perforation of pulp floor is common in the treatment of pulpal diseases,but the prognosis is not good.Although light-cured calcium hydroxide was widely-used to treat the perforation of pulp floor in the clinical treatment,its clinical effects were not ideal as expected because the slight leakage along the edge of the filler was prone to cause the relapse of inflammation in the pulp floor area.Thus in recent years,MTA(mineral trioxide aggregate)was adopted in our hospital to repair the perforation of pulp floor.42 cases were selected and observed their clinical effects over one year,and the conclusion was that the success rate with MTA was apparently greater than that with usual repairing material.MTA is a nice repairing material to treat the perforation of pulp floor and is worth of clinical application,for it has better closure and biological compatibility.
9.Effect of orange juice on talinolol pharmacokinetcs

10.The Transformation of Medical Security Fund Management System and Evaluation

Medical security fund is the core of medical security system. The traditional system of medical security fund management has enabled various governmental departments to play positive roles, and has controlled the growth of expenditure. Since the Reform and opening-up, along with the reform of medical security system, a scientific and reasonable fund-raising mechanism has been established, the management of medical security fund has been societalized, and the supervision and regulation mechanisms of registered healthcare agencies have been initially established, all of which further promote the reform and development of healthcare system. But the existing management system is not perfect, and a medical security fund system which adapts to China's national conditions and the needs of the development of healthcare system must be further explored.
