1.Analysis of the results of creatine kinase reference method in ring trial program

ObjectiveTo provide a reference for the establishment and appliance of enzyme reference measurement of CK in China by comparing and analyzing the RELA results of CK in IFCC from 2006-2008. Methods The RELA samples of CK were measured according to the reference procedure for the measurement of catalytic activity concentration of CK (37 ℃) ,which had been published by IFCC. The EP5A2 protocol was used for evaluation of the imprecision and ERM was used for verification of the trueness. Results In RELA 2006, the result of sample A was (9. 896 ±0. 112) μkat/L, and the result of sample B was (4.953 ±0. 120) μkat/L. In RELA 2007, the result of sample A was (2.684 ±0.054)μkat/L, and the result of sample B was (8.798 μ0. 101) μkat/L. In RELA 2008, the result of sample A was (10. 523 ±0. 149) μkat/L,and the result of sample B was (10. 551 ±0. 141) μkat/L. The precision of the CK reference method in the year 2006 to 2008 was 0. 92%, 0. 86% and 0. 88% respectively, each of them is less than 1% and the results of ERMs were consistent with the certified value(1. 68 ± 0. 07)μkat/L,which verify the imprecision and accuracy of the reference method. Conclusions All of the results in the continuous three years were in the range of equivalence limits suggested by IFCC. The CK reference method suggested by IFCC has been established and it is getting better.
2.Clinical evaluation of Decitabine on the treatment of elderly patients with acute myeloid leukemia

Objective To evaluate the clinical effect of Decitabine as the basis of the chemotherapy regimen on the treatment of elderly patients with acute myeloid leukemia( AML)?Methods The clinical data of elderly patients with AML admitted to the Third People ’ s Hospital of Yancheng from January 2013 to November 2015 were retrospective analyzed?The patients were divided into Decitabine group ( cases ) and the traditional group(19 cases) according to whether or not useed Decitabine,then evaluated the efficacy?Results The overall response rate( ORR) was 70?0% in 10 patients received decitabine,47?4% in 9 patents of the traditional group, and the difference was significant( P=0?03)?At the same time,the median survival time of the two groups was 44?87 months and 13?40 months respectively,the difference was statistically significant between the two groups (P=0?04)?There was no significant difference in the incidence of adverse reactions between the two groups ( granulocyte reduction, PLT reduction, fatigue, cardiovascular events, and respiratory tract infections ) ( P>0?05)?Conclusion The effect of Decitabine as the basis of the chemotherapy regimen on the treatment of AML is better,can prolong the survival time.

4.Resistant Genes of Beta-lactam Antibiotics from Clinical Isolates of Pseudomonas aeruginosa

OBJECTIVE To investigate ?-lactamase coding genes and oprD2 gene in multidrug-resistant Pseudomonas aeruginosa isolated.METHODS Polymerase chain reaction(PCR) was used to detect various ?-lactamase coding genes including TEM,SHV,OXA,PER,GES,IMP,VIM,plasmid type AmpC ?-lactamase DHA,MIR and oprD2 in 20 strains of P.aeruginosa.RESULTS The detection rate of ?-lactamase coding genes plasmid type AmpC,VIM,TEM,SHV,OXA and IMP were 15%,5%,15%,10%,10% and 5%,respectively,the loss rate of oprD2 was 25%,but PER and GES genes were negative.CONCLUSIONS The study indicated that the P.aeruginosa is carrying genes of DHA,TEM,SHV,OXA,VIM,IMP and being lost oprD2 gene,the latter is the essential resistance mechanism of P.aeruginosa to beta-lactam antibiotics in local area.
5.Expression of Snail in bladder urothelial carcinoma and its relationship with E-cadherin and a subset of T cell Groups

ObjectiveTo investigate the expression of Snail in bladder urothelial carcinoma and evaluate its relationship with E-cadherin and a subset of T cell groups.MethodsImmunohistochemical method was used to detect the expression of Snail and E-cadherin proteins in tissue from 156 cases of bladder urothelial carcinoma and 80 cases of the para-cancerous mucosa. The proteins expression status was analyzed with clinico-pathological data. Meanwhile, correlations with Snail and CD4+, CD8+, CD4+/CD8+ were analyzed.ResultsThe positive rate of Snail in bladder urothelial carcinoma was 65.4% , which was significantly higher than that in para-cancerous mucosa (48.8%). The expression of Snail was significantly correlated with the clinical stage, pathological grade, tumor quantity, distant metastasis and tumor recurrence.There was negative correlation between the expression of Snail and E-cadherin in bladder urothelial carcinoma. Positive expression of Snail had a negative correlation with the number of CD4+ and CD4+/CD8+ , whereas it had no significant correlation with the number of CD8+.ConclusionsBy inhibition of the E-cadherin expression and inducing local immunosuppression, Snail might play an important role in the process of the invasion and metastasis of bladder urothelial carcinoma.
6.Application and effects of bone lengthening growth diary in self- management of patients after lower limb lengthening

Objective To explore the application and effects of bone lengthening growth diary in self management of patients after lower limb lengthening. Methods A total of 54 cases applying lower limb lengthening were divided into the experimental group (n=28) and the control group (n=26) with random digit table. The basic treatment was consistent, the control group was given routine guidance, the experi-mental group implemented self- management with bone lengthening growth diary. Self- recognition of disease was assessed and compared between the two groups 12 weeks after operation. Self- efficacy and self- manage ment were assessed and compared between the two groups 2 and 12 weeks after operation. Results The self- recognition of disease in the observation group was better than that of the control group, for patients with higher self- recognition degree (21 cases vs. 10 cases), χ2=8.64, P<0.05, the difference was significantly different. The total scores of self- efficacy and self- management in the observation group were higher than those of the control group,(48. 42±3.43) vs.(34.14±4.13),t=11.750, P<0.01;(55.35±2.69) vs.(42.15±3.67), t=15.130, P<0.01, the difference was significantly different. Conclusions Bone lengthening growth diary plays a beneficial role on self-management of patients with lower limb lengthening.
7.Clinical application of detection of procalcitonin and high sensitivity C-reactive protein in the early diagnosis of bloodstream infection

Objective To investigate the clinical value of detection of procalcitonin(PCT ) and high sensitivity C-reactive protein (hs-CRP) in early diagnosis of bloodstream infection(BSI) .Methods The serum levels of PCT and hs-CRP of 58 BSI patients(test group) and 58 non BSI patients(control group) were detected .The early diagnosis value of PCT and hs-CRP was evaluated by ROC curve .Results There were significant difference between the serum PCT and hs-CRP levels of test group and control group(P<0 .05) .The ROC AUC of PCT and hs-CRP were 0 .902(95% CI:0 .850-0 .955) and 0 .706(95% CI:0 .611-0 .801) ,respectively . With 2 .24 ng/mL being the diagnostic cut-off value ,the diagnostic sensitivity ,specificity ,positive predictive value ,and negative pre-dictive value of PCT were 77 .6% ,91 .4% ,90 .0% ,and 80 .3% ,respectively .With 64 .83 mg/L being the diagnostic cut-off value ,the di-agnostic sensitivity ,specificity ,positive predictive value ,and negative predictive value of hs-CRP were 74 .1% ,62 .1% ,54 .4% ,and 59 .5% , respectively .Conclusion Detection of serum PCT and hs-CRP has important clinical value in early diagnosis of BSI .
8.Activation of NF-?B in airway epithelial celland modulation mechanism of NAC

Aim The expression of NF-?B activation and the effect of antioxidant (N- acetylcysteine, NAC) on NF-?B activity in human airway epithelial cell was assessed. Methods Using the TNF-?,the airway epithelial cell strains of normal subject(16HBE) and tumor patient (H292) was activated and using Western-Blot and ELISA the expression of NF-?B and IL-8 were detected. Results It was found that the activity of NF-?B could be stimulated by the TNF-? and increase with the amount of TNF-? with the peak occurring at 2 to 4 hours after stimulation and then decreasing at six hours. At the same time, the level of IL-8 was elevated, but decreased with inhibition of NF-?B activity by NAC, that means the action of NAC has a dose-dependent effect. Conclusion NAC not only blocks the signal transmission activated by NF-?B, but also anticipates the transcription modulation of expression of many cell factors and inflammatory mediums. It suggests that NAC may play a role in the anti-inflammatory treatment of respiratory diseases.
9.Epidemiological features on 3 important viral diarrhea among children in Zhuhai during winter and spring.

OBJECTIVETo explore the epidemiological characteristics of viral diarrhea of norovirus (NV), sapovirus (SV) and astrovirus (AstV) among children in Zhuhai during winter and spring.
METHODSStool specimens were collected from children with viral diarrhea in Maternal and Child Health Hospital of Zhuhai from November 21, 2009 to April 3, 2010. Nucleic acid of NV, SV and AstV from negative specimens of rotavirus and adenovirus were detected by using Reverse transcription-polymerase chain reaction (RT-PCR), and the types of positive samples of NV were also classified at the same time.
RESULTSThe total detection rate of the three viruses is 21.49 percent, the highest detection rate is 29.05% in December 2009, the lowest detection rate is 12.20% in February 2010, 87.96% of positive specimens were from children patients aged from 0 to 30 months. The season detection rate of NV, SV and AstV are 14.70%, 2.75% and 4.04% respectively. There were significant differences of NV and SV detection rates in every month of the season, whereas the AstV detection rate was comparatively stable. The highest detection rate of NV is 34.09% in children patients aged from 12 to 18 months, the highest SV detection rate is 12.5% in children patients aged from 60 to 120 months, and the highest AstV detection rate is 16.67% in children patients aged from 24 to 30 months. All the NV were belong to G II genogroup.
CONCLUSIONSNV is one of the main pathogens causing viral diarrhea among children in Zhuhai during winter and spring, SV and AstV are also important pathogens. So we should strengthen the monitoring of viral diarrhea caused by NV, SV and AstV in infants and young children.

10.Clinical blood routine and bone marrow smear manifestations of disseminated penicilliosis marneffei.

OBJECTIVETo investigate the manifestation of clinical, blood routine and bone marrow smear of disseminated penicilliosis marneffei.
METHODSThere were 13 cases of penicilliosis marneffei whose peripheral blood had been drawn for routine tests, as well as blood and bone marrow aspiration for smears. Wright's-Giemsa stain, Gomori's methenamine-silver stain (GMS) and periodic acid Schiff's reaction (PAS) were performed for light microscopy in consultation with pathologic diagnosis and fungi culture for precise diagnosis.
RESULTSSeven cases of bone marrow and 2 peripheral blood smears were found positive for penicillium marneffei in the test group. The morphology of penicillium marneffei was extremely similar to that of histoplasma capsulatum. However, the observation of sausage cells and central cross wall, which are signs of dividing by fission and not by budding, aided in differential diagnosis.
CONCLUSIONBone marrow smear or occasional blood smear examination play an important role in the diagnosis of disseminated penicilliosis marneffei.
