ObjectiveFew studies are reported on the sublingual immunotherapy with dermatophagoides farinae drops (DFD) for mite-allergic cough variant asthma (CVA) in children. This study aimed to investigate the clinical effects of DFD sublingual immunotherapy on mite-allergic CVA in children.MethodsWe retrospectively analyzed the clinical data on 225 children with mite-allergic CVA treated in Wuhan Children′s Hospital from January 2016 to January 2017. The patients were divided into a control (n=123) and an experimental group (n=102) to receive symptomatic treatment and symptomatic treatment combined with DFD sublingual immunotherapy, respectively, both for 10 months. We recorded the relevant indicators, including the symptom scores, lung function parameters, levels of dust mite IgE and inflammatory factors (IL-4, TGF-β and INF-γ), and related risk factors, and compared the data obtained between the two groups of patients before and after treatment.ResultsThere were no statistically significant differences between the two groups of patients in the baseline daily and nocturnal cough symptom scores, lung function parameters, or levels of serum IgE, IL-4, TGF-β and INF-γ (P>0.05). After treatment, the children in the experimental group showed markedly lower daily and nocturnal cough symptom scores than the control (0.87±0.06 and 0.91±0.11 vs 2.54±0.32 and 2.38±0.42, P<0.05), better pulmonary function parameters (FEV1/FVC: \[1.98±0.56\]% / \[79.67±9.05\]% vs \[1.65±0.37\] / \[75.87±11.56\]%, P<0.05), and a higher effectiveness rate (73.53% vs 54.47%, P<0.05). The single- and multi-factor analyses showed that age, disease course and treatment methods were independent risk factors influencing the therapeutic effect on mite-allergic CVA in children (P<0.05).ConclusionSublingual immunotherapy with DFD can effectively improve the clinical symptoms and lung function in children with mite-allergic CVA, which may be associated with the decreased levels of serum IgE, IL-4, TGF-β and INF-γ. Age, disease course and treatment methods are independent risk factors influencing the therapeutic effect.

Objective To build emergency specialist nurse training evaluation index system model.Methods By Delphi method,21 emergency care specialists participated in the consultation,using the Analytic Hierarchy Process (AHP)to build a hierarchical model,establishing judgment matrix,through statistical calculations to determine the indicators and weight coefficients of the index system of emergency specialist nurse training.Results Four first-level indicators,11 second-level indicators,31 third-level indicators emergency specialist nurse training evaluation system model were established.Conclusions Establishment of emergency nurse specialist training evaluation index system provides an effective method and reference to improve the overall capacity of the emergency specialist nursing team to ensure the quality of training of emergency nurse specialists.

Objective To evaluate the effects of etomidate preconditioning on etomidate-induced toxicity to rat adrenal cortical cells in vitro.Methods After being primarily cultured for 7-9 days,the rat adrenal cortical cells at the exponential growth phase were seeded into 96-well culture plates (1 × 106 cells/ml) and cultured for 24 h.The cells were then randomly divided into 3 groups with 6 wells in each group:control group (group C),etomidate group (group E),and etomidate preconditioning group (group EP).In group E,the cells were incubated with 700 μmol/L etomidate for 24 h.In group EP,the cells were incubated with 1.25 μmol/L etomidate for 1 h,then washed out and incubated with 700 μmol/L eomidate for 24 h.The cell viability was determined by CCK-8 assay and the concentration of cortisol was determined by ELISA.Results Compared with group C,the cell viability and cortisol concentration were significantly decreased in E and EP groups.Compared with group E,the cell viability and cortisol concentration were significantly increased in group EP.Conclusion Etomidate preconditioning can reduce etomidate-induced toxicity to rat adrenal cortical cells in vitro.

ObjectiveTo study the anatomical characteristics of the renal pedicle and its clinical application during retroperitoneoscopic nephrectomy.MethodsThe imaging, surgical video and clinical data of 278 cases undergoing retroperitoneoscopic nephrectomy between July 2007 and September 2009 were retrospectively analyzed. The renal pedicle was found located at the psoas and the medial arcuate ligament of the diaphragm for anatomic landmark before the gap of the psoas. A statistical analysis of the mean operative time, the mean time to look for the renal pedicle, the mean intraoperative estimated blood loss, the mean time to ambulation, the mean time to resuming oral intake, the mean postoperative hospital stay day and the rate of complications was performed.ResultsAll the operations were performed successfully, with no conversion to open and blood transfusion. The mean time to find the renal pedicle was (3.5 ± 1.3 ) min, the mean operative time was (95.6 ±23.8) min, the mean estimated blood loss was (72.4 ±27.5) ml, the mean time to resuming oral intake was (2.1 ±0.7) d, the mean time to ambulation was (1.8 ±0.9) d, the mean postoperative hospital stay was (8.5 ± 2.7 ) d.ConclusionsDuring retroperitoneoscopic nephrectomy, the best anatomical space to look for the renal pedicle is the space before the psoas muscle. The exact location of the renal pedicle is under the diaphragm medial arcuate ligament about 2 - 4 cm, in the gap of the psoas muscle and the vena cava (abdominal aorta). Following this procedure could reduce the time to look for the renal pedicle, thus reducing operative time.

Stem cells are characterized by self-renewing, multipotent differentiation, and high proliferation and receiving more and more attention for their roles in the development and management of various diseases. There are epithelial stem cells and mesenchymal stem cells in the prostate. The markers of the epithelial stem cells include cytokeratin, stem cell antigen-1, and integrins alpha2beta1, CD49f, CD133, CD117, and CD44. The markers of the mesenchymal stem cells include CD30, CD44, CD133, neuron-specific enolase, and vascular endothelial growth factor receptor-1. Prostate stem cells are involved in the development and treatment of prostatic diseases. This review focuses on the latest progress in the studies of prostate stem cells.

Objective Study the prevention and early diagnosis of severe complications of thoracoscopy. Methods 400 patients,(18～81)years,ASA Ⅰ-Ⅲ,underwent thoracoscoy,including 320 pulmonary bubble resections and 80 thorax,mediastinum or lung tumor biopsy or resections.All patients were anesthetized with one-lung ventilation using a double-lumen tube and monitored continuously in circulatory and respiratory functions with a multi-runctional monitor. Results Severe intraoperative complications occurred in 8 patients,including 5 reexpansion pulmonary edema,1 complete occlusion of double-lumen tube due to massive pleural effusion,1 massive bleeding and 1 cardiac arrest due to bilateral pneumothorax.All complications were successfully treated and the patients were cured and discharged. Conclusions Thoracoscopy must be well prepared.Applying one lung anesthesia with double-lumen tube,careful observation during operation,early recognition and treatment of complications may contribute to assuring the safety of this procedure.

[Objective] To investigate professor SHAN Zhaowei's experience of diagnosing and treating bile reflux gastritis(BRG). [Method] The experience of Professor SHAN Zhaowei on recognition of etiology and pathogenesis, treatment principle, characteristics of prescription selection and administration of BRG is summarized and studied. Besides his academic ideas and clinical medication experience are shown with typical cases. [Result] In his view, emotional disorder is the most common etiology, and liver-stomach disharmony is the basic pathogenesis. He puts forward that we should pay attention to dispersing the stagnated liver Qi, regulating the stomach to lower the adverse flow of Qi when we treat it, and emphasizes the combination of disease differentiation and syndrome differentiation. Meanwhile, herbs of protecting gastric mucosa are compatibly used to improve the curative effect. And the case in this article has obtained good treatment results. [Conclusion] Pro. SHAN Zhaowei thinks that emotional regulation to patency for patients is very important. Objective To detect serum anti-Treponema pallidum specific antibody of 26 707 cases by Abbott I2000SR auto-matic chemiluminescent microparticle immunoassay analyzer,and treponema pallidum particle agglutination assay (TPPA) was regarded as a standard reference method which was used to detect anti-Treponema pallidum specific antibody.To analyze the false positive rate of Abbott I2000SR according to the TPPA.Methods Collected 26 707 serums from inpatients and outpatients of the hospital during September 1,2013 to March 5,2014.The subjects were asked to fasting conditions taking venous blood 3 ml,3 000 r/min centrifugal 10 min utes after the separation of serum,detected the Anti-TP by CMIA (Ab-bott I2000SR)and the TPPA testing,analyzed test results by statistical methods.Results There were 52 cases detected by I2000SR whose S/CO values of 26 707 cases of serum Treponema pallidum specific antibodies were 1 to 2,of which 9 cases were verified positive by TPPA,and the positive rate was 17.31%.There were 26 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 2 to 3,of which 9 cases were verified positive by TPPA,and the posi-tive rate was 34.62%.There were 26 cases detected by I2000SR whose S/CO values of Treponema pallidum specific anti-bodies were 3 to 5,of which 9 cases were verified positive by TPPA,and the positive rate was 34.62%.There were 25 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 5 to 7,of which 11 cases were veri-fied positive by TPPA,and the positive rate was 44%.There were 25 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 7 to 10,of which 17 cases were verified positive by TPPA,and the positive rate was 68%.There were 28 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 10to 13,of which 24 cases were verified positive by TPPA,and the positive rate was 85.71%.There were 23 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 13 to 17,of which 20 cases were verified posi-tive by TPPA,and the positive rate was 86.96%.There were 24 cases detected by I2000SR whose S/CO values of Trepone-ma pallidum specific antibodies were 17 to 21,of which 22 cases were verified positive by TPPA,and the positive rate was 91.67%.There were 29 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were 21 to 26,of which 28 cases were verified positive by TPPA,and the positive rate was 96.55%.There were 104 cases detected by I2000SR whose S/CO values of Treponema pallidum specific antibodies were above 26,of which 104 cases were verified posi-tive by TPPA,and the positive rate was 100%.The total number of positive cases were 364,of which 254 were positive ca-ses,the positive rate was 69.78%.False positive rate was 0.42% and positive predictive value was 69.78%.Conclusion Abbott I2000SR automated chemiluminescent microparticle immunoassay analyzer has the feature of automated detection, closed reagents,simple operation,speed,and more accurate results and so on.Although high sensitivity but its results have false positive,so cannot diagnose based on the results of Abbott I2000SR,and need use of the TPPA to test and corroborate.

Objective To investigate the relationship between the numbers of deep medullary veins (DMV) beside the ipsilateral lateral ventricle and the stenotic degree of large artery of ipsilateral internal carotid artery (ICA) system and cerebrovascular reactivity (CVR) in patients with acute cerebral infarction showed on susceptibility-weighted imaging (SWI) in middle cerebral artery (MCA) territory.Methods The patients with acute cerebral infarction in MCA territory who underwent SWI,cerebral angiography and the transcranlal Doppler carbon dioxide challenge test during the hospital stay were enrolled retrospectively.According to the numbers of DMV beside the lateral ventricle on SWI,they were classified into 3 grades:Grade 1:DMV ＜5; Grade 2:DMV 5-10; Grade 3:DMV ＞ 10.The correlations of the DMV grades and the degree of ipsilateral ICA/MCA and CVR were analyzed.Results A total of 44 patients with acute cerebral infarction in MCA territory were enrolled.Twenty-four patients were DMV 1,15 were DMV2,and 5 were DMV 3.The mean stenosis degree of the main arteries of the ipsilateral ICA system was 40.52％ ±30.62％.The mean CVR of MCA was 0.41 ± 0.16.The stenosis degrees in the DMA grade 1,2,and 3groups were 31.31％ ± 26.02％,44.93％ ± 31.03％,and 71.76％ ± 26.68％,respectively.There were significant differences among the 3 groups (F=4.589,P =0.018),and the stenosis degree in the DMV grade 3 group was significantly higher than that in the DMV grade 1 group (P=0.014).The CVRs in the grade 1,2,and 3 groups were 0.45 ± 0.12,0.41 ± 0.18,and 0.22 ± 0.12,respectively.There were significant differences among the 3 groups (F=6.179,P=0.005),and the CVR in the DMV grade 3 group was significantly lower than that in the DMV grade 1 group (P =0.006) and in the DMV grade 2 group (P =0.038).Spearman correlation analysis showed that DMV grade was significantly positively correlated with the stenosis degree (rs =0.381,P =0.013),and was significantly negatively correlated with CVR (rs =-0.375,P=0.012).Conclusions The DMV grade in patients with cerebral infarction was positively correlated with the stenosis degree of responsible large artery and was negatively correlated with the MCA CVR in the responsible side.

Objective To investigate the effects of enteral nutrition with galactooligosaccharides (GOS) on serum inflammatory cytokines in rats with severe acute pancreatitis (SAP). Methods SD rats were randomly divided into sham operation control group, SAP with enteral nutrition (EN) group and SAP with EN supplemented with GOS (GOS-EN) group, and each group was divided into 4 d and 7 d subgroups according to the time that animals were sacrificed (n=8 in each subgroup). Rat SAP models were established by injection of 38 g/L sodium taurocholate beneath the pancreatic capsule. The serum amylase, inflammatory cytokines TNF-α, IL-2 and IL-10 were detected. Results At each time point, the levels of serum amylase in all SAP groups were significantly higher than those in sham operation control group (P < 0.01), and the levels in GOS-EN group were significantly lower than those in EN group (P < 0.01). The levels of serum TNF-α and IL-10 in all SAP groups were significantly higher than those in sham operation control group (P < 0.01), while the levels of IL-2 in all SAP groups were significantly lower than those in sham operation control group. The levels of TNF-α in GOS-EN group were significantly lower than those in EN group (P <0.05), while the levels of IL-2 and IL-10 and the ratio of IL-10/TNF-α were significantly higher than those in GOS-EN group (P < 0.05). Conclusion Early EN supplemented with GOS could modulate the balance of pro- and anti-inflammatory response.
