1.Treatment of osteochondritis lesion in the knee with reduction and fixation under arthroscopy.

OBJECTIVETo study clinical results of reduction and fixation under arthroscopy for the treatment of osteochondrotical lesion in the knee.
METHODSFrom December 2012 to December 2013, 4 patients with the knee joint osteochondritis lesion and free bone fragments were treated with arthroscopy to detect the stripped cartilage, and then the cartilages were reduced with small incision and fixed by cartilage nail. There were 3 males and 1 female, with age of 15, 15, 20 and 27 years old. The durations of the disease were 1 d, 5 d, 1 month and 1 year. All the patients had swelling and effusion in the knee joint, and the floating patellar test was positive. CT examination showed bone defects and loose bodies. The Lysholm, VAS, and Tegner scale were used to evaluate the knee joint functions before and after operation.
RESULTSAll the patients underwent successful surgery. Postoperative CT showed good location of the cartilage without loss of reduced bone and cartilage. All the patients were followed up, and the duration ranged from 10 to 13 months, with a mean of 12 months. In one patient, arthroscopy examination was conducted for a second time to examine the connection of the fracture part to the surrounding cartilage, and it showed that the internal fixator was not absorbed. The Lysholm, VAS and Tegner scale of all patients were better than those before operation.
CONCLUSIONTreatment of osteochondritis lesion with the fixation of absorbable cartilage nails may reconstruct the integrity of articular surface and recover the stability of joints, and it is an effective treatment method.

3. Clinical effects of sublingual immunotherapy with dermatophagoides farinae drops on mite-allergic cough variant asthma in children

ObjectiveFew studies are reported on the sublingual immunotherapy with dermatophagoides farinae drops (DFD) for mite-allergic cough variant asthma (CVA) in children. This study aimed to investigate the clinical effects of DFD sublingual immunotherapy on mite-allergic CVA in children.MethodsWe retrospectively analyzed the clinical data on 225 children with mite-allergic CVA treated in Wuhan Children′s Hospital from January 2016 to January 2017. The patients were divided into a control (n=123) and an experimental group (n=102) to receive symptomatic treatment and symptomatic treatment combined with DFD sublingual immunotherapy, respectively, both for 10 months. We recorded the relevant indicators, including the symptom scores, lung function parameters, levels of dust mite IgE and inflammatory factors (IL-4, TGF-β and INF-γ), and related risk factors, and compared the data obtained between the two groups of patients before and after treatment.ResultsThere were no statistically significant differences between the two groups of patients in the baseline daily and nocturnal cough symptom scores, lung function parameters, or levels of serum IgE, IL-4, TGF-β and INF-γ (P>0.05). After treatment, the children in the experimental group showed markedly lower daily and nocturnal cough symptom scores than the control (0.87±0.06 and 0.91±0.11 vs 2.54±0.32 and 2.38±0.42, P<0.05), better pulmonary function parameters (FEV1/FVC: \[1.98±0.56\]% / \[79.67±9.05\]% vs \[1.65±0.37\] / \[75.87±11.56\]%, P<0.05), and a higher effectiveness rate (73.53% vs 54.47%, P<0.05). The single- and multi-factor analyses showed that age, disease course and treatment methods were independent risk factors influencing the therapeutic effect on mite-allergic CVA in children (P<0.05).ConclusionSublingual immunotherapy with DFD can effectively improve the clinical symptoms and lung function in children with mite-allergic CVA, which may be associated with the decreased levels of serum IgE, IL-4, TGF-β and INF-γ. Age, disease course and treatment methods are independent risk factors influencing the therapeutic effect.
4.Study on quality control of the next-generation sequencing data

Objective To investigate the quality control of the next-generation sequencing data by analyzing some im-portant problems with quality control in order to provide high-quality data for the downstream data analysis .Method The software Fastq_clean was developed to clean the data and to statistically analyze the quality of the data.Result Using a Chrysanthemum transcriptome dataset , the function of the Fastq_clean software was illustrated .Conclusion Based on the Bioconducter package , the Fastq_clean software can accurately remove low quality nucleotides and Ns , adapter contamina-tion, possible rRNA and virus contamination .Moreover, it can keep the largest possible amount of high-quality data reads.
5.Investigation on the Humanistic Quality of Nursing Undergraduate Interns

Objective:To investigate the status of humanistic quality in nursing undergraduates interns , to pro-vide a reference for optimization settings humanities courses and improving humanistic quality in nursing students . Methods:A self -designed questionnaire with convenience sampling method was used to investigate 90 nursing students in M hospital .The questionnaires included requirements for clinical teachers , needs for humanities cour-ses, cognitive knowledge on humanities curriculum .Results:The nursing intern students agreed with teachers're-spect for students , profession ability , attention to communication .They wanted to increase the humanities course of interpersonal and communication , nursing aesthetics and rituals , traditional Chinese culture courses .They had a humanistic understanding of the concept of learning is to help develop good mental state and healthy personality , enhance the sense of community , strengthen exchanges and cooperation with the person's ability .Conclusion:The nursing intern students have recognized the importance of humanities courses .Increasing humanities curriculum and using a combination of diverse teaching and assessment methods were proposed to promote their humanities quality of ability.
6.Optimum Selection of the Extracting Procedure of Rhizoma Corydalis

Objective To choose the bestway of extracting procedure of Rhizoma corydalis. Methods The optimal extraction process is selected with the orthogonal design. The content of corydalis B and the yield of extracts were used to evaluate the factor levels. Results The ideal extraction process is:extracted with 75% alcohol for 2 times, first 6 times amount, soaked 0.5 h, refluxed 1.5 h, then 4 times amount, refluxed 1.5 h. Conclusions This extractive condition is the best way.
7.Analysis on the Application of Narcotic Analgesics in Our Hospital During 2003~2007

OBJECTIVE: To investigate the clinical application and developing trend of narcotic analgesics in our hospital.METHODS: Analgesics used in our hospital during 2003~2007 were analyzed statistically.RESULTS: The consumption(amount of money and quantity) of narcotic analgesics especially that of morphine preparation witnessed an year-on-year increase while that of pethidine injection decreased year by year.CONCLUSION: The use of narcotic analgesics in our hospital was reasonable on the whole,but the dosage form and variety should be increased further.
8.Anesthesia Effect of Dezocine and Pentazocine in Painless Artificial Abortion

Objective To observe the anesthesia effect of dezocine and pentazocine in painless artificial abortion, in order to provide a basis for safe use of anesthesia drug in clinical practice. Methods Totally,300 patients undergoing painless artificial abortion were included in this study.The patients were randomly divided into four groups: group A (simple propofo), group B ( propofol combined with fentanyl) ,group C ( propofol combined with dezocine) and group D ( propofol combined with pentazocine)(n=75 each).Four groups of patients were intravenously injected with propofol (1-1.5 mg?kg-1) according to the situation in operation when necessary to maintain anesthesia effect.Heart rate (HR),mean arterial pressure (MAP),arterial oxygen saturation ( SpO2 ) and respiratory frequency were observed and recorded before induction,after induction,at the beginning of the operation,during operation and anesthesia recovery. The onset time of anesthesia, the recovery time, the recovery time of orientation,the postoperative pain score and the incidence of adverse reaction of the four groups were recorded. Results All of the patients achieved good effects of anesthesia in operations. There was no significant difference in the onset time of anesthesia (P>0.05).Awakening time and time of orientation recovery in groups B,C and D were significantly shorter than those in group A (P<0.05).The dosages of propofol in group C and D were significantly less than those in group A and B (P<0.05).The changes of HR,MAP,SpO2 and respiratory frequency after induction were not significantly different in group B,C and D (P>0.05),but the changes were relatively stable as compared with group A (P<0.05).The rates of adverse reactions in group B,C and D were significantly lower than that in group A (P<0.05).The incidence rates of respiratory depression,nausea and vomiting in group C and D were significantly lower than those in group A and B (P<0.05).The postoperative pain scores of VAS in group B,C and D were significantly lower than that of group A (P<0.05). Conclusion The analgesia effects of dezocine and pentazocine are similar.Application of them can reduce the dosage of propofol and shorten the anesthesia awakening time in painless artificial abortion,at the same time they have less adverse reactions,and they are safe and effective to be used in clinical anesthesia.
9.Retrovirus mediated IL-23 gene expression in mouse high-metastasis mammary cancer cells MA-891

Objective:The mouse mammary cancer cell line IL-23/MA-891 expressing IL-23 protein was set up and the effect of IL-23 on growth and other biological character of the cells was studied.Methods:The IL-23 gene was sequencely transfected into two packing cell lines (ecotropic ?2 and amphotropic PA317) by a retrovirus vector (LXSN),and the positive cellular clones were screened by G418.After infection to MA-891 cells with the culture supernatant of IL-23/PA317 cells and selected with G418,the IL-23/MA-891 cells to express IL-23 mRNA and protein was detected with RT-PCR,ELISA and immunocytochemical stairing,respectively.The expression of H-2Kb(MHCⅠ),I-Ab(MHC Ⅱ),CD80,CD86 and FAS was examined with flow cytometry.The ability to secrete IFN-? by mouse splenocytes stimulated with the culture supernatant of IL-23/MA-891 cells was detected with ELISA.The proliferation of MA-891,LXSN/MA-891 and IL-23/MA-891 cells was detected by MTT colorimetry and flow cytometry in vitro.Results:IL-23/MA-891 cells express IL-23 stably in mRNA and protein level.The expressing of H-2Kb(MHCⅠ),I-Ab( MHCⅡ),CD80,CD86 and FAS protein by flow cytometry were showing no difference in three kinds of cells.The proliferation rate of IL-23/MA-891 cells were showing little decresed,but statistically showing no difference with parental cells.The culture supernatant of IL-23/MA-891 cells induced secretion of IFN-? by mouse splenocytes were increased compared to parental cells.Conclusion:The cellular clone IL-23/MA-891 with high level expression of IL-23 was produced.
10.Differences of clinical characteristics and severity between RSV bronchiolitis and non-RSV bronchiolitis

Objective To investigate the differences of clinical characteristics and disease severity between respiratory syncytial virus (RSV) and non-RSV bronchiolitis.Methods Ninety-six bronchiolitis inpatients of Shengjing Hospital from November 2012 to March 2014 were enrolled.RSV detection was performed at enrollment by direct immunofluorescence assay and the patients were divided into RSV group and non-RSV group.Clinical characteristics were collected and compared, including patients' demographic data, allergy, etc.Clinical scores were assessed by modified-Tal score system, and the associations between clinical scores and duration of onset or length of stay were analyzed to assess disease severity.Results In all 96 patients, forty (46.17%) were determined to be infected with RSV.Patients with RSV bronchiolitis were younger at hospitalization (t =-2.706,P =0.008) and the weight was lower (t =-2.070, P =0.041).There was no significant differences in sex, birth way, birth weight, height, ete (P > 0.05).There was no significant differences in eczema, history of family allergy and total serum IgE (P > 0.05).For patients with RSV bronchiolitis, the clinical scores were higher (t =-0.405 ,P <0.001) and the length of stay trended to be longer (t =0.480 ,P =0.081).The clinical score had a positive relation with length of stay (r =0.305, P =0.003), but not with the duration of onset (P > 0.05).Conclusion Compared with non-RSV bronchiolitis, the clinical characteristics of patients with RSV bronchiolitis are younger age and lower weight, and have higher clinical scores, which indicates the clinical conditions may be more severe.For bronchiolitis, there is a positive correlation between clinical score and length of stay.The combination of etiological detection and modified-Tal score system may help to recognize severe cases, which should be observed carefully and treated immediately.
