1.Characterization of multi-drug resistant Klebsiella pneumoniae isolates from urinary tract infected-women in Sylhet city, Bangladesh

Aims:
Klebsiella pneumoniae is considered to be one of the most frequent bacterial species associated with urinary tract infections (UTIs) and recurrent UTIs (RUTIs) worldwide. The present study aimed to comprehensively characterize K. pneumoniae isolates from women suffering from UTI and RUTIs.
Methodology and results:
A total of 15 clinical isolates, collected from different hospitals in Bangladesh, were tested for biochemical features, and amplified by PCR. Antibiogram assay was performed by disk-diffusion assay. Phylogenetic and functional features were analyzed using bioinformatics platform. XLSTAT was used for principal component analysis (PCA). PCR amplification using Klebsiella hemolysin gene (khe) confirmed the presence of K. pneumoniae in agarose gel with expected product size of 486 kb. Antibiogram assay revealed all K. pneumoniae isolates to be completely resistant to six out of ten relevant drugs namely ampicillin, cephradine, chloramphenicol, erythromycin, kanamycin and sulfamethoxazole used for treating UTIs in Bangladesh. Sequencing of 16S rRNA gene of clinically significant K. pneumoniae isolates showed a high level of sequence divergence among the isolates from UTI and RUTIs as well as functional features such as SNP variants and restriction sites.
Conclusion, significance and impact of study
We surmise that the results could be used as a pipeline for further research in the identification of K. pneumoniae associated with UTI and RUTIs, and treatment of infection.
2.Synchronous Carotid Body and Glomus Jugulare Tumors : A Case Report and Review of Literature

Paragangliomas are rare neuroendocrine tumors that are usually benign in nature. They may be either familial or sporadic in their occurrence. Numerous neuroendocrine tumors are collectively included under the umbrella of paragangliomas. Among them, carotid body tumors and glomus jugulare tumors are extremely rare. Thus, we present a rare case of 29-year-old male who was admitted with hearing difficulties and tinnitus in the left ear, with swelling on the left side of the neck. Based on clinical and radiological findings, a diagnosis of left-sided glomus jugulare with carotid body tumor was made. The patient underwent a twostage surgery with an interval of approximately 2 months. Histopathology revealed a paraganglioma. Herein, we present the clinical features, imaging findings, management, and a brief review of literature on the classification, evaluation, and management of carotid body and glomus jugulare tumors. Paraganglioma is a slow-growing tumor. The synchronous occurrence of carotid body and glomus jugulare tumors is infrequent. Microsurgical resection remains the primary treatment modality. Therefore, our patient underwent two-stage surgery. The rarity of occurrence and the proximity and adherence to vital neurovascular structures have resulted in the treatment of paragangliomas remaining a challenge.
3.Subjective Preference of New Prototypes Safety Helmets Device among Palm Oil Plantation Harvesters in Sandakan, Sabah

Introduction: Safety helmets are one of the personal protective equipment (PPE) that to decrease the impact of any falling object to the skull and to avoid head and brain injury by many industries, including palm oil plantation. Nevertheless, the level on the usage of the current safety helmet is very low due to a few factors that lead to the discomfort. Among the common issues for the non-compliance of safety helmets are their discomfort, ventilation, weight and safety. This study aims to determine subjective preference of the new prototypes’ safety helmets device among palm oil plantation harvesters. Methods: A cross-sectional study conducted among 124 harvesters in three palm oil plantations located in Sabah, Malaysia. A set of questionnaires used to collect data on their socio-demographic background, perceptions toward existing safety helmets and their subjective preference of new safety helmets prototypes. Apart from that, six harvesters were randomly choosing to attend an interview session for qualitative study. Results: The descriptive analysis indicate that among the emphasized issues regarding non-compliance of existing safety helmet were due to discomfort (66.1%), poor ventilation (97.6%), load of safety helmet (83.3%) and safety issues (68.5%). In terms of new safety helmets prototypes, 72.6% of the harvesters preferred Design C to be worn for work in the plantation. Conclusion: It can be suggested that the existing safety helmet is uncomfortable and was not design ergonomically namely loose size and discomfort. Design C was the most preferred to be worn for work in the palm oil plantation.
