1.Clinical study of bacillus coagulans for pruritus in uremia patients

Objective To investigate the clinical characterastics of uremia pruritus and the value of treatment by bacillus coagulans. Methods The clinical data of 312 patients with maintenance hemodialysis were collected. The incidence rate, clinical manifestation and correlation factors of uremia pruritus were analyzed. Then, 40 refractory pruritus patients were divides into 2 groups by random digits table, with 20 patients in each group. The patients in experiment group were treated by bacillus coagulans, and the patients in control group were given vitamin E. The relieve of pruritus in two groups was compared. Results In 312 patients, 158 patients didn′t have pruritus, 56 patients had mild pruritus, 68 patients had moderate pruritus , and 30 patients had severe pruritus. The levels of urea nitrogen, creatinine, calcium, ferroprotein in different degree of pruritus patients had no significant differences (P>0.05), but the levels of phosphorus, calcium-phosphorus product, immunoreactive parathyroid hormone (iPTH), C reactive protein (CRP), interleukin (IL)-1 and IL-6 increased with the severity of pruritus (P<0.05). Logistic regression analysis showed that the levels of CRP, IL-6, iPTH were risk factors for pruritus (P<0.01 or <0.05). The VAS scores of moderate and severe pruritus patients in experiment group after treatment were significantly decreased compared with that before treatment (P<0.05). The levels of CRP, IL-1 and IL-6 before and after treatment in control group had no significant differences (P>0.05). The levels of CRP, IL-1 and IL-6 after treatment in experiment group significantly decreased (P<0.05). The rate of pruritus was very high , and there was markedly positive correlation between pruritus and serium inorganic phosphorus、iPTH and CRP. Bacillus coagulans can alleviate pruritus. Conclusions On the basis of maintenance hemodialysis, bacillus coagulans can regulate the intestinal flora, alleviate inflammatory reaction, and help to release the symptom of pruritus.
2.Comparison of gentiopicroside content among gentiana macrophylla from different origins in Gansu province

Objective: To evaluate the quality of gentiana macrophylla from different origins in Gansu by determination the content of gentiopicroside.Methods: HPLC method was used.Results: The calibration curve of gentiopicroside was linear within 0.52-5.2?g(r=0.9991),the average recovery rate was 99.2%(RSD=1.23%,n=5).Conclusion: The content of gentiopicroside among gentiana macrophylla from different origins in Gansu are different,and all of them are higher than the standard of China Pharmacopoeia.Gentiopicroside content in planting gentiana macrophylla is higher than that of wild gentiana macrophylla.It provides a scientific basis of cultivating gentiana macrophylla and protecting wild gentiana macrophylla.
3.Investigation on parent's and preschool children's dental health behavior in pudong new area of shanghai

Objective To get informed of the parents' oral health behaviors,children's oral hygiene habits and the correlation between the two,to lay the foundation of building up joint educational pattern of the oral health between pre-school children and their parents.Methods The questionnaire between preschool children and their parents was applied.By cluster sampling,124 pre-school children from 5 kindergartens in the local community were selected to participate the survey.The oral health behaviors of parents and their kids were investigated and the results underwent analysis.Results Parents’ oral health behaviors were closely related to children's,which was embodied by three parts,such as the starting age of brushing teeth,the duration of brushing and the habits of gargling after meals.As to parents,the knowledge sources of oral health mainly came from TV and broadcast programs (82.3%),newspapers and periodicals,magazines and popular science readings (51.6%),relatives and friends (27.4%),but the anticipated knowledge sources were from medical personnel (90.3%),TV and broadcast programs (53.2%),community activities on health education (51.6%).Conclusions Pre-school children's oral health behaviors are currently good; more emphasis should be laid on the effect of parents' behaviors to their children's oral health; it's necessary to hold more community activities on health education.
4.THERAPEUTIC EFFECTS OF INTERLEUKIN-12 GENE in vivo VACCINE FOR MURINE T CELL LYMPHOMA (EL4) IN C57BL/6 MICE

C57BL/6 synergistical mice were divided into 4 groups (10 mice each). The first group was negative control without any interference. Each mouse in the other 3 groups was subcutaneously inoculated with 1 10 6 wide type (wt) EL4 tumor cells. Then each group was treated either with interleukin 12 (mIL 12) in vivo vaccine or NeoR control vaccine or PBS(positive control group) on day 1, 7, 14, 21, on the same place where wt EL4 tumor was inoculated. mIL 12 in vivo vaccine and NeoR control vaccine were package cells which can produce retrovirus (RV) with mIL 12 or NeoR gene, the vaccine was 60 Co irradiated and injected (1?10 7 cells/mouse). All mice in positive group and NeoR control group died of tumors in a month, while 5/10 mice in mIL 12 in vivo vaccine groups survived without tumors for more than 60 days. The 5 survived mice were rechallenged with 5?10 5 wt EL4 cells, 3/5 mice even survived without tumors for another 60 days. 5.Unqualified Medical Appliance Packs Checked Before Sterilization:Causes and Countermeasures

OBJECTIVE To analyze the causes of unqualified medical appliance packs before sterilization and to find countermeasures in order to assure their quality,and to avoid emergence of hospital infection and medical tangles. METHODS Totally 320 packs were checked before sterilization from supply room and clinical departments from Jun 1,2004 to Mar 30,2005. RESULTS The rate of unqualified packs checked before sterilization was 14.12% including 460 medical apparatuses unqualified(21.70%). CONCLUSIONS To avoid the emergence of hospital infection and medical tangles,we should strengthen the work of supply room to realize the centralization of sterilization and supply,and make much of personnel′s professional training,normalize job′s process and quality criterion,and intensify professional and systematizing management of medical appliance purged and packed.
6.Endoscopic examination and pathology of colonic polyps in children

Objective To retrospectively study the results of endoscopic manifestation pathology,and treatment of colonic polyps. Methods Clinical data of 121 patients diagnosed with colonic polyps and treated by endoscopy between November 2002 and September 2009 were studied. Complete colonic investigation was done by Olympus PCF240I electron endoscopy. Colonic polyp excision was done by high frequency electric coagulation. Results All patients had bloody stool with mucus for 1 week to 8 years. Ninty four(77.7%)patients had a single polyp,6(5.0%)had two polyps,21(17.3%)with multiple polyps,and 5 patients had familial polyposis coli. The size of the polyps ranged from 0.3 cm to 4 cm and the morphology of Yamada Ⅰ to Ⅳ. The pathological changes were juvenile polyps,inflammatory polyps,polypoid formation,and canalicular adenoma. Most common pathology types were Yamada Ⅳ and juvenile polyps,107 of the 121 patients(88.4%)had juvenile polyps,and one was found to have malignant transformation. 115 patients accepted high frequency electric coagulation excision under general anesthesia,without complication. Conclusions Colonic polyp is one of the common diseases in children. Children should be checked by total colonoscopy in time when they present with bloody stool,anemia,rectal polyps prolapse or intussusception. Polypectomy under endoscope is a most safe and reliable treatment method when the diagnosis is confirmed. For isolated juvenile polyp,regular recheck is not necessary after polypectomy except when bloody stool recurs. Regular follow up by colonoscope is necessary for juvenile polyp combined with adenoma and adenomatous polyps. Colonoscope is necessary at any time when bloody stool occurs. Familial polyposis coli is difficult to be treated by endoscopy,thus surgical procedure is suggested.

8.Preditive value of modified early warning score for emergency potential in critically ill patients

Objective To investigate the predictive value of mnodified early warning score (MEWS) in predicting emergency potential in critically ill patients and its feasibility.Methods A total of 298 patients in the emergency department and emergency room were evaluated using the MEWS.The distribution of transfer to cadiopulmanary resuscitation (CPR),intensive care unit (ICU) and high dependency unit (HDU),length of hospital stay ( more than 1 month or not) and transferred to advanced hospital,death were recorded,and all patients were followed up.Results Patients with different MEWS were treated significantly differently ( x2=258.697,P < 0.001 ),and their prognosis were also significantly different ( x2=115.938,P < 0.001 ).the condition of patiens were significantly correlated with MEWS ( r=0.848,P < 0.001 ).Conclusion The MEWS in the prediction of potential in critically ill patients in emergency department is feasible as a special emergency assessment system and is recommended for promotion.
9.Study of the Correlation of Testing Results by Two Different D-dimer Reagents with Fibrin Degradation Products Result Values

Objective To evaluate the clinical practicality of two different D-dimer assays by studying the correlation of the testing results by two different D-dimer reagents with fibrin degradation products (FDP)result values and analyzing the Clinical alignment.Methods A representative cross-sectional study of 762 clinical patients’plasma specimens were taken to measure the results of D-dimer and FDP by using Sysmex CA-1500 automated coagulation analyzer,D-DIMER detection rea-gent respectively Siemens Innovance D-DIMER and Dade Behring D-DIMER Plus reagents.Then rating frequency analysis and Spearman correlation test were applied to analyze the correlation of measurement results.Results Overall,to the same patient,the correlation of D-dimer tested by Siemens Innovance D-DIMER Reagent with FDP level was significant positive (r= 0.954,P<0.01),better than that tested by Siemens DD Plus Reagent (r=0.885,P<0.01);and this correlation had an increasing trend when D-dimer level raised.When D-dimer level > 5 mg/L FEU,the correlation was the best (P<0.01). And by calculating the amount of positive FDP (FDP>5μg/ml)and FDP positive rate in both different cases when D-dimer results was also positive,FDP positive rate in Innovance D-DIMER Reagent group (about 65.76%)was lower than DD Plus Reagent group,it indicated that D-dimer results tested by Innovance D-DIMER Reagent could raise before FDP reacted,so D-dimer had a higher sensitivity than FDP.Conclusion It can provide a more precise clinical diagnosis and treatment infor-mation when using Siemens Innovance D-DIMER Reagent to detect D-dimer values and combined FDP values at the same time.
10.Evaluation and comparison of early outcome and corneal biomechanical changes between SMILE-corneal collagen cross linking and SMILE for myopic eyes

Objective:To evaluate early corneal biomechanical change, safety and effectiveness after small-incision lenticule extraction combined with ultraviolet A corneal collagen cross-linking (SMILE-CXL) for myopic eyes and compare with SMILE.Methods:A non-randomized controlled clinical study was performed.Forty-four myopic eyes of 25 patients were included in Hankou Aier Eye Hospital from December 2017 to July 2018.SMILE-CXL or SMILE was carried out for 22 eyes of 12 patients or 22 eyes of 13 patients, respectively, based on the normal posterior surface manifestation of Belin by Pentacam tomography or not.The posterior surface manifestation of Belin was normal in the SMILE group, and the posterior surface of Belin was yellow or red with Corvis biomechanical index and tomographic biomechanical index<0.3 (except keratoconus) in the SMILE-CXL group.The refractive diopter of the eyes was detected before and 6 months after surgery, including spherical diopter, cylindrical diopter and spherical equivalent (SE) with a comprehensive refractometer.The corneal biomechanical parameters of the eyes were detected before and 6 months after surgery with a Scheimpflug-based dynamic tonometry (Corvis ST). The safety index, a ratio of postoperative mean best corrected visual acuity (BCVA) to preoperative mean BCVA, and efficacy index, a ratio of postoperative uncorrected visual acuity (UCVA) to preoperative BCVA, were compared between SMILE-CXL group and SMILE group.BCVA and UCVA were examined using standard chart and converted to logarithm of the minimal angle of resolution (LogMAR) units.This protocol complied with the Declaration of Helsinki and was approved by an Ethics Committee of Hankou Aier Eye Hospital (No.WHS2017052701). Written informed consent was obtained from each patient prior to entering into the cohort.Results:The postoperative UCVA of the eyes in both groups was more than 1.0.There were no significant differences in safety index, efficacy index and SE change between the SMILE-CXL group and SMILE group (all at P≥0.05). At 6 months after surgery, the values of second applanation time (A2T), time from the start until the highest concavity (HC-Time) and DA ratio 2 mm were significantly increased in comparison with before operation in both SMILE-CXL group and SMILE group (all at P<0.05). The changes of A2T, HC-Time and DA ratio 2 mm in the SMILE-CXL group were significantly lowered than those in the SMILE group at the sixth month after surgery ( P=0.001, 0.001, 0.036). The deformation amplitude, maximum corneal velocity during the first applanation (Vin), distance between both non-deformed peaks and integrated radius during the maximum depression were significantly increased, and the central curvature radius at highest concavity, cord length of first applanation, cord length of second applanation, Ambrósio's relational thickness horizontal, stiffness parameter applanation 1 and biomechanical intraocular pressure during the maximum depression were significantly lower at the sixth month after surgery than those before surgery (all at P<0.05). There were no significant differences in the parameters mentioned above between the two groups (all at P<0.05). Conclusions:Compared with SMILE alone, SMILE-CXL shows a comparable safety and efficacy, and better corneal biomechanical properties.
