1.Astrocytes and ischenic stroke
International Journal of Cerebrovascular Diseases 2011;19(9):703-707
As an important component of the “neurovascular unit”,astrocytes provide protective effect for nervous through intaking excessive excitatory amino acids, providing energy substances, maintaining extracellular K + and water balance, scavenging oxygen free radicals and secreting neurotrophic factor during ischemic stroke. This article elaborates the mechanisms of astrocytes participating in ischemic stroke in recent years.

Objective To summarize the effect of transurethral vaporization of the prostate (TUVP) in combination with transurethral resection of the prostate (TURP) for the treatment of benign prostatic hyperplasia (BPH). Methods A combined use of TUVP and TURP was carried out for treating 122 cases of BPH. There were 8 cases of Rous stage Ⅰ disease, 58 cases of stage Ⅱ, 32 cases of stage Ⅲ, and 24 cases of Ⅳ. Results The length of duration of the operation was 20~140 min (mean, 68 min). The intraoperative blood loss was 40~200 ml (mean, 80 ml). A blood transfusion of 200 ml was required in 2 cases after the operation. No transurethral resection syndrome (TURS) occurred. The mean postoperative catheterization time was 6 days. All 122 patients had been followed for 4~19 months (mean, 10 months). The international prostate symptom score (IPSS) decreased from 30.2?2.3 pre-)operation to 10.8?2.5 post-operation (t=10.84,P=0.000); the residual urine (RU) decreased from 252.6?65.3 ml pre-operation to 35.6?10.4 ml post-operation (t=23.52,P=0.000); the maximum urinary flow rate (Qmax) increased from 8.5?2.8 ml/s to 20.6?3.8 ml/s (t=6.67,P=0.000). Postoperatively, transient urinary incontinence was observed in 2 cases, and anterior urethral stricture occurred in 2 cases. Conclusions Combined use of transurethral electrovaporization and resection of the prostate for the treatment of BPH gives satisfactory effects.

Objective: To summarize the research status of the changes in local microenvironment of acupoints caused by acupuncture, provide theoretical guidance for the initiation mechanisms of local acupuncture effect at acupoints. Methods: Using acupuncture, acupoint as key words to search China National Knowledge Infrastructure (CNKI), Wanfang Academic Journal Full-text Database (Wanfang), Chongqing VIP Database (CQVIP), PubMed and other databases, the representative articles were selected for review. Results: Acupuncture could excite afferent nerves, activate cells, and promote the release of chemical substances like neuropeptide, hormone, cytokines, etc. in the local site of acupoints. Besides, it may cause mechanical deformation of connective tissues, and change chemical ions as well as ion channels. Conclusion: The microenvironment changes around acupoints are crucial to acupuncture effect; the concept of 'acupoints network' can be used to objectively describe the local changes around the acupoints after acupuncture.

Hair follicle (HF), one of the skin appendages, has received a lot of attention to be a new target and pathway for drug delivery. The development of hair follicle targeted drug delivery system (HFTDDS) through percutaneous permeation is particularly important for skin diseases derived from HF such as acne, hair loss, and folliculitis for their on-site action. This review describes the structure and physiological function of HF, the microenvironment of HF, and factors affecting HF permeation. Multiple nanoformulations used to improve the HF permeation and technologies to characterize the HF permeation were introduced. The latest advance of HFTDDS based on nanoformulations were systematically summarized and analyzed in the treatment of acne and hair loss. Finally, the challenges of formulating HFTDDS were discussed. The review is expected to provide some ideas and references for developing delivery systems for treating skin diseases derived from HF.

Objective To summary the experience of management for orthotopic homologous heart transplantation,heart-lung co-transplantation and to discuss the postoperative treatment of short-term complications. Methods Orthotopic homologous heart transplantation and heart-lung co- transplantation on 9 cases were performed from May 2004 to February 2006.The patients were traced closely and all complications were treated in time. Results During hospital stay, hypertension oc- curred in one case, right heart insufficiency in one case, infection of mycetes and cytomegalovirus in 2 cases after the heart transplantation, and in the patient undergoing heart-lung co-transplantation,renal inadequacy, bronchial anastomosis stenosis and fungus infection occurred. All patients obtained symp- tomatic relief before discharge. Conclusion Proper perioperative management, immunosuppressive therapy and management of postoperative complications could reduce mortality and obtain satisfactory therapeutic effect in the near future.
