2.Sexual dysfunction in patients with chronic renal failure.

Sexual dysfunction is a highly prevalent problem among patients with chronic renal failure, which affects patients in the quality of life. However, it has not received enough attention. The genesis of sexual dysfunction is multifactorial, including physiological, psychological and organic factors. This review summarized the incidence, main manifestation, evaluation, risk factors and treatments associated with sexual dysfunction in patient of the chronic renal failure.

3.Sexual function and mental state in patients with redundant prepuce or phimosis.

OBJECTIVETo investigate the correlation of sexual dysfunction with psychological abnormalities by analyzing the sexual function and mental state of the patients with redundant prepuce or phimosis.
METHODSThis study included 216 randomly selected patients with redundant prepuce or phimosis and 85 normal male controls. We conducted investigations among the subjects using a questionnaire on the general data of the patients, Chinese Index of Sexual Function for Premature Ejaculation (CIPE), International Index of Erectile Function (IIEF), and Symptom Checklist 90 (SCL-90). Then we assessed the status of premature ejaculation (PE) and erectile dysfunction (ED), calculated the incidence of psychological abnormalities, such as depression and anxiety, and analyzed the correlation of PE and ED with the mental state of the patients.
RESULTSThe PE and ED patients scored significantly higher than normal controls on SCL-90, somatization, compulsion, depression, anxiety and other factors (P < 0.05). CIPE scores were correlated with the scores on SCL-90, somatization, compulsion, interpersonal sensitivity, depression, anxiety and other factors, while ED-related scores showed no correlation with the scores on SCL-90 and other factors.
CONCLUSIONPatients with redundant prepuce or phimosis have poor mental health, and there is an interaction between PE and the mental state of the patient.

4.Sexual dysfunction in diabetic women.

Diabetes mellitus is a common problem, and female sexual dysfunction is one of its complications in diabetic women. Recent studies show that the major risk factors of sexual dysfunction in diabetic women are diabetes-induced vascular disease, neuropathy, endocrine abnormalities and psychological problems and so on. 5.Male sexual dysfunction and Parkinson's disease: a preliminary investigation.

OBJECTIVETo investigate the prevalence of male sexual dysfunction in males with Parkinson's disease and the pathogenesis and related factors of the problem.
METHODSWe evaluated the sexual function of 140 men with Parkinson's disease using Mini-mental State Examination (MMSE), Unified Parkinson's Disease Rating Scale (Part III) (UPDRS III), Hoenhn-Yahr Staging (HYS), Hamilton Depression Scale (HAMD) and Sexual Dysfunction Standard of ICD-10. We calculated the Levodopa equivalent doses (LED) for all the patients.
RESULTSSexual dysfunction was found in 58 (41.43%) of the patients with Parkinson's disease. There were no significant differences in age, education, age of onset, course of disease and scores on UPDRS III, HYS and LED between the sexual dysfunction and normal sexual function groups. The HAMD score was 14.95 +/- 9.12 in the sexual dysfunction group, significantly higher than 10.96 +/- 9.82 in the normal sexual function group (P < 0.05), and it was positively correlated with the inci- dence of male sexual dysfunction (P < 0.05).
CONCLUSIONSexual dysfunction is a common symptom in males with Parkinson's disease, and is correlated with the high HAMD score of Parkinson's disease patients.

6.Clinical research of needle-pricking therapy for functional retrograde ejaculatioin.

OBJECTIVETo observe the clinical effects of needle-pricking therapy, a newly medical and minimally invasive technique, for functional retrograde ejaculation and to explore its mechanism. Methods Thirty-six patients with functional retrograde ejaculation were randomly divided into an observation group(19 cases) and a control group(17 cases) In the observation group,needle-pricking therapy was used at Guanyuan(CV 4) and bilateral sacral plexus nerve,lumbar 1 nerve and greater occipital nerve stimulating points,once a week. In the control group, midodrine tablets were prescribed orally,three times a day. All the treatment was given for 9 weeks. The clinical effects of the two groups were observed, and the levels of luteinizing hormone(LH), testosterone(Tes) and estra4 diol(E2) were compared between the two groups.
RESULTSThe total effective rate of the observation group was, 89. 47%(17/19), which was better than 47.06% (8/17) of the control group(P<0. 05). The LH and Tes were obviously increased and E2 was decreased compared with those before treatment in the observation group(all P< 0. 01). Tes was raised(P<0. 05) and E2 was apparently declined in the control group(P<0. 01). After treatment, the differences of serum LH and Tes were statistically significant between the two groups(both P<0. 01).
CONCLUSIONNeedle-pricking therapy has advantages for functional retrograde ejaculation probably in that stimulating lumbosacral nerves can strengthen the function of pelvic floor muscles and urethral expansion muscle and regulate sexual gland axis.

7.Animal models in sexual dysfunction.

Animal models in sexual dysfunction were reviewed to further improve the modeling methods and to promote the effectiveness of drug evaluation translation from animal models to humans. A MEDLINE search was performed to retrieve articles relating to animal models in sexual dysfunction. Researches on a variety of animal models in sexual dysfunction, with their own merits, has to a certain extent contributed to the understanding of sexual function. However, no models could give a fully accurate assessment of sexual function. The existing sexual function studies on animal models of interpretive function, the development mechanisms, the effects of drugs on sexual function and the clinical translation still have some deficiencies, but with their basic principles and ideas for the improvement of the models and the preservation of the valuable data of drugs and clinical trials.

8.Investigation of sexual function in 623 patients with chronic prostatitis.

OBJECTIVETo investigate the influence of chronic prostatitis on premature ejaculation (PE) and erectile dysfunction (ED) and the correlation of chronic prostatitis with PE and ED.
METHODSWe adopted NIH-CPSI, CISFPE and IIEF-5, interviewed 623 patients with chronic prostatitis, and recorded their scores on the above systems.
RESULTSThe incidence of PE was 39.0% and the rates of the mild, moderate and severe types were 26.2%, 12.0% and 0.8%, respectively. It developed mostly among the younger patients, accounting for 42.9% (103/240), 37.0% (95/257) and 35.7% (45/126) in the 18-30, 30-40 and 40-57 yr age groups, respectively. The incidence of ED was 16.9% and the rates of the mild, moderate and severe types were 14.9%, 1.0% and 1.0%, respectively. It occurred mostly among the older patients, accounting for 13.8% (33/240), 11.3% (29/257) and 34.1% (43/126) in the 18-30, 30-40 and 40-57 yr age groups, respectively. Analyses revealed no significant correlation among the scores on NIH-CPSI, CISFPE and IIEF-5.
CONCLUSIONThere is a high incidence of PE and ED among patients with chronic prostatitis, but without significant correlation between the severity of chronic prostatitis and that of PE and ED.

9.Selective resection of the branches of the two dorsal penile nerves for primary premature ejaculation.

OBJECTIVETo evaluate the safety and effect of selective resection of the branches of the two dorsal penile nerves in the treatment of primary premature ejaculation (PPE).
METHODSFrom September 2003 to December 2006, 483 PPE patients aged 21-71 years (mean 32) underwent selective resection of the branches of the two dorsal penile nerves, with only 2 of the branches reserved, 3 resected in 89 cases, 4 in 183, 5 in 125, 6 in 38, 7 in 32, 8 in 12, 9 in 3 and 10 in 1. The patients could have sexual intercourse 4 weeks after the operation and were followed up for 3-36 months.
RESULTSNo infection, hemorrhage and erectile dysfunction were observed. Decreased penile sensibility was noted in all the patients, obviously prolonged ejaculation latency in 352, improvement in 93 and failure in 38, with a total effectiveness rate of 92.13%.
CONCLUSIONSelective resection of the branches of the two dorsal penile nerves, which can definitely reduce the sensivity of the penis, is a safe and effective surgical option for the treatment of PPE.

10.Semen biochemical markers and their significance in the patients with premature ejaculation.

OBJECTIVETo detect the changes of biochemical markers in the semen of premature ejaculation patients and investigate the correlation of the markers with premature ejaculation.
METHODSFifty-six premature ejaculation patients and 60 males with normal sexual behavior were enrolled in this experiment. Acid phosphatase, alpha- glucosidase and fructose were assayed by the methods of glucose oxidase, disodium phenyl phosphate and disodium phenyl phosphate respectively.
RESULTSThe contents of acid phosphatase, alpha-glucosidase and fructose were (36.37 +/- 31.33) U/ml, (39.97 +/- 22. 09) U/ml and (3.40 +/- 1.92) mg/ml in the premature ejaculation patients and (54. 27 +/- 20. 96) U/ml, (55.71 +/- 16.19) U/ml and (2.55 +/- 1.12) mg/ml in the normal control, respectively, with significant differences in the former two markers between the two groups. The rate of the abnormal content of both acid phosphatase and alpha- glucosidase was 31% and 13% (P < 0.05) , while that of the normal content of the three markers was 10% and 33% in premature ejaculation group and the control, respectively (P < 0. 05 ).
CONCLUSIONThe abnormality of both acid phosphatase and alpha-glucosidase is one of the causes of premature ejaculation. Because acid phosphatase and alpha- glucosidase reflect the functions of the prostate and epididymis, we should pay attention to the status of these two organs in the treatment of premature ejaculation.
