2.Spontaneous Testicular Hemorrhagic Necrosis Masquerading as a Testis Tumor .

Spontaneous testicular hemorrhagic necrosis is a rare disease usually associated with testicular torsion. Partially involved and suspicious testis tumor cases are also defined from orchiectomy specimens. Herein, a spontaneous hemorrhagic necrosis, without any testicular torsion, but with involvement of the whole testicle and epididymis, is reported. A 21 year old patient, who presented with a painless left testicular enlargement of several days duration was believed, based on physical examination, ultrasonography and elevation of serum LDH, to have a testicular tumor. Diagnosis was made only after radical orchiectomy and histopathological examination.

3.Small Cell Carcinoma of the Prostate: A Case Report.

Primary small cell carcinoma of the prostate is a rare malignancy of unknown origin and with poor prognosis. We report a case occurring in a 74-year-old man. Radiologic findings showed that the prostate was markedly enlarged, with a lobulated contour and unclearly defined internal architecture. It was therefore impossible to distinguish it from prostatic adenocarcinoma, but the huge mass and normal range of serum tumor markers (prostate-specific antigen and prostate acid phosphatase) suggested small cell carcinoma of the prostate.

5.Analysis of Trabecular Bone Strength using Finite Element Analysis

OBJECTIVES: The purpose of this study is to develop a method of evaluation based on finite element analysis (FEA) using micro-CT images for the measurement of trabecular bone strength. METHODS: The primary compressive trabeculae were obtained from the human femoral head of three cadavers (21 year old male (M/21), 51 year old male (M/51), 51 year old female (F/51). All bone specimens were scanned using micro-CT at 24.9microm of spatial resolution under 70 kV's voltage and current of 141microA. The percent bone volume was calculated from the CTAn (SKYSCAN, Belgium) software, it's represented the bone mineral density (BMD). After scanning, the finite element model was reconstructed based on micro-CT images. All models were applied to be linear elastic, isotropic, and uniform with a tissue modulus of 5.17 GPa and a tissue Poisson's ratio of 0.3. RESULTS: The percent bone volume(%) were 31.819 (+/-0.648), 21.513 (+/-2.489), 20.280 (+/-1.891) and Bone strength (MPa) were 187.741 (+/-13.006), 61.585 (+/-11.094), 61.266 (+/-16.744) in M/20, M/51 and F/51. The trabecular bone strength of the primary compressive trabeculae in M/20 was 3 times more than the trabecular bone strength in M/51 and F/51. The percent bone volume in M/20 was 148% and 157% higher than the percent bone volume in M/51 and F/51. CONCLUSIONS: The finite element analysis is more sensitive than the percent bone volume in reflecting the morphometry index of primary compressive trabeculae. The high resolution FEA reconstructed from high resolution MRI or high resolution CT may improve the evaluation of trabecular bone strength in the medical field.

6.The Effect of an Abdominal Drawing-In Maneuver Combined with Low·High Frequency Neuromuscular Electrical Stimulation on Trunk Muscle Activity, Muscle Fatigue, and Balance in Stroke Patients

Purpose:
This study investigated the effects of an intervention that combined the abdominal drawing-in maneuver and frequency-specific neuromuscular electrical stimulation on changes in trunk muscle activity, muscle fatigue, and balance in stroke patients.
Methods:
Thirty stroke patients were randomly assigned to two groups. Fifteen subjects were assigned to group I which performed the abdominal drawing-in maneuver combined with low-frequency neuromuscular electrical stimulation and the other 15 subjects to group II where the abdominal drawing-in maneuver was combined with high-frequency neuromuscular electrical stimulation. Muscle activity and fatigue were measured using surface electromyography before the intervention. Balance was measured using the Trunk Impairment Scale and re-measured after six weeks of intervention for comparative analysis.
Results:
Both groups showed a significant increase in muscle activity and balance (p < 0.05), and there was no significant difference between the groups (p > 0.05). In the changes in muscle fatigue, only the experimental group II showed a significant increase in muscle fatigue (p < 0.05). The difference between the groups was statistically significant (p < 0.05).
Conclusion
It was confirmed that among stroke patients, the combination of the abdominal drawing-in maneuver and low-frequency neuromuscular electrical stimulation was more effective in changing the muscle activity and balance of the trunk by minimizing the occurrence of muscle fatigue compared to the combination of the abdominal drawing-in maneuver and high-frequency stimulation. These results can be used as basic data for clinical trunk stabilization training.
7.Effect of the separated airway on the Intubating condition in fiberoptic bronchoscope-guided intubation fiberoptic intubation using separated airway.

BACKGROUND: Fiberoptic bronchoscope guided intubation is an important method of difficult airway management. The use of specific airways has been devised to assist the fiberoptic intubation. The authours compared effectiveness of separated airway with fiberoptic bronchoscope guided intubation and the hemodynamic responses. METHODS: 104 adult patients of American Society of Anesthesiologists grading (ASA) I-II who scheduled for surgery under general anesthesia were randomly divided into the Laryngoscope group (L group, n = 30) or the Fiberoptic bronchoscope group (F group, n = 36) or the Fiberoptic bronchoscope with separated airway (MF group, n = 38). A Fiberoptic bronchoscope guided intubation and a fiberoptic bronchoscope with separated airway and a direct laryngoscope was performed after inducing anesthesia. Intubation time, Jaw thrust incidence, mean blood pressure and heart rate after anaesthesia induction, at intubation and every two minute for a further 7 min were recorded. RESULTS: The intubation time was significantly shorter in the MF group (58.3 +/- 13.7 sec) than F group (71.9 +/- 22.1 sec). Jaw thrust incidence was lower in the MF group (60.5%) than F group (100%). The changes of MAPs and HRs during the observation were not significantly different in three group. CONCLUSIONS: Fiberoptic intubation using separated airway reduced intubation time and the incidence of jaw thrust.

8.Intravascular ultrasound catheter for transesophageal echocardiography in congenital heart surgery: A case report.

Transesophageal echocardiography (TEE) has an important role during congenital heart surgery. TEE in small infants is associated with complications, including an inability to pass the TEE probe, esophageal trauma, airway compression, aortic compression, and interference with ventilation. Recently, a monoplane intravascular ultrasound catheter (IVUC) has been developed for intracardiac echocardiography. The efficacy of IVUC for transesophageal use has been shown in numerous animal studies and several human studies, but there have been few reports involving small infants using an IVUC probe. We examined 15 pediatric patients undergoing congenital cardiac surgery using an 8-Fr AcuNav IVUC probe. We checked the cardiac anatomy, cardiac function pre-operatively and de-aeration before weaning from CPB; the surgical repairs were evaluated post-operatively. Although the IVUC probe has limitations associated with the monoplane, we found the IVUC probe to be useful in small infants and safer than the TEE.

9.Visualization of Ostium Secundum Atrial Septal Defect by Transesophageal Echocardiography.

Atrial Septal Defect(ASD) is usually suspected clinically but requires a confirmative diagnostic procedure before surgical repair. Conventional transthoracic echocardiography has relatively high sensitivity and specificity for ASD, but difficulty in visualizing the ASD occasionally. Transesophageal echocardiography has special advantages for investigating the posteriorly located cardiac structures, including the atrial septum, which is imaged perpendicularly at a relatively short distance. We describe a case in which ostium secundum ASD was not visualized by conventional transthoracic echocardiography, but was diagnosed confidently by transesophageal echocardiography. It is concluded that transesophageal echocardiography appears to be a promising diagnostic tools for the evaluation of ASD on the basis of its ability to provide excellent imaging of the entire atrial septum and related posterior cardiac structures.

10.Antifungal effect of electrolyzed hydrogen water on Candida albicans biofilm.

PURPOSE: Candida albicans can cause mucosal disease in many vulnerable patients. Also they are associated with denture-related stomatitis. Electrolyzed water is generated by electric current passed via water using various metal electrodes and has antimicrobial activity. The aim of this study was to investigate antifungal activity of electrolyzed water on C. albicans biofilm. MATERIALS AND METHODS: C. albicans was cultured by sabouraud dextrose broth and F-12 nutrient medium in aerobic and 5% CO2 condition to form blastoconidia (yeast) and hyphae type, respectively. For formation of C. albicans biofilm, C. albicans was cultivated on rough surface 6-well plate by using F-12 nutrient medium in CO2 incubator for 48 hr. After electrolyzing tap water using various metal electrodes, the blastoconidia and hyphal type of C. albicans were treated with electrolyzed water. C. albicans formed blastoconidia and hyphae type when they were cultured by sabouraud dextrose broth and F-12 nutrient medium, respectively. RESULTS: The electrolyzed water using palladium electrode (EWP) exhibited antifungal effect on blastoconidia of C. albicans. Also, the EWP significantly has antifungal activity against C. albicans biofilm and hyphae. In the electrolyzed water using various metal electrodes, only the EWP have antifungal activity. CONCLUSION: The EWP may use a gargle solution and a soaking solution for prevention of oral candidiasis and denture-related stomatitis due to antifungal activity. (
