1.Occurrence of Alternaria cassiae in Seeds of Sickle Senna in Korea.

Alternaria cassiae Jurair & Khan was recorded on seeds of sickle senna(Cassia tora L.). Seed infection ranged from 25~45% in two samples. Developing roots and shoots turn brown and died in case of severe infection. Detailed descriptions were given on the habit characters of the fungus and on the morphology of conidiophores and condia. This is the first report of A. cassiae on seeds of Cassia.

3.Effects of Spontaneous Recanalization of Left Ventricular Function after Acute Myocardial Infarction.

We studied the incidence of spontaneous infarct related artery recanalization and it's effect on LV function in 120 patients with acute myocardial infarction by angiography within 1 month after onset of acute MI between Nov. 1983 to Sep. 1988. The total occlusion rate of the infarct related artery in 34 cases catheterized from 1st day to 7th day was 55.8%, that of 33 cases from 8th day to 14th day 51.5%, that of 32 cases from 15th day to 21th day 40.7% and that of 21 cases from 21th day to 30th day was 47.7%. These results suggest that spontaneous recanalization of infarct related artery has mainly occurred within 24 hours after acute MI. LV ejection fraction among those patients who showed spontaneous recanalization(n=59) was not significantly superior to those with persistent occlusion of the infarct related artery. Both anterior and inferior MI patients showed no difference in LV function as regard to whether they showed recanalization or persistent occlusion. Although early spontaneous reperfusion of the infarct related artery is not uncommon in acute MI, LV function was not influnced by the spontaneous recanalization at least until 1 month after MI.

4.Mitral Regurgitation after Percutaneous Balloon Mitral Valvuloplasty(PMV): Results and Relationships to Valve Morphology.

To evaluate the change in valvular morphology and occurence and severity of mitral regurgitation produced by PMV, 45 patients(33 women and 12 men,mean age 38+/-10 years) were studied using two-dimensional(2-D) and Doppler echocaediography before and 1-2 days after this procedure. Mitral valve area after PMV increased in all patients, from 0.9+/-0.2 to 1.8+/-0.4cm2(P<0.0001). In valve area estimation, the correlation between Gorlin`s method and 2-Dechocardiography was better(r=0.61, p<0.0001) than that between Gorlin`s method and Doppler pressure halftime(r=0.38, P<0.01) before valvuloplasty, but after the procedure Gorlin`s and 2-D image valve area correlated less well(r=0.33, P<0.05) than Gorlin`s-Doppler pressure halftime correlation(r=0.46, P<0.002). Before PMV, 37 patients had no mitral regurgutation, 7 had grade 1 and 1 had grade 2 mitral regurgutation. After PMV, new mitral regurgutation occurred in 14 patients, increased in severity in 5 patients and so mitral regurgutation newly developed or increased in severity in 19(42%) patients. There were no differences between the patients with and those without an increase in mitral regurgutation after PMV, in age, sex, caediac rhythm, initial mitral valve area, increase in mitral valve area and fluoroscopic calcification. However, morphologic characteristics especially mobility(P<0.01) and thickening(P<0.05) of mitral leaflets were better pressured, and EBDA/BSA(effective balloon dilating area/body surface area) was significantly smaller(P<0.02) in patients without an increase in mitral regurgutation. Thus, an increase in mitral regurgutation after PMV might be related to the features of valve morphology especially and thickand EBDA/BSA.

5.No Association Between the 5-HT 2A Receptor Gene Promoter Polymorphism(-1438A/G) and Schizophrenia in a Korean Population.

OBJECTIVES: This study was designed to investigate the association between 5-HT 2A receptor gene promoter -1438A/G polymorphism and schizophrenia in a Korean population. METHOD: 5-HT 2A receptor gene promoter -1438A/G polymorphism was typed with Polymerase Chain Reaction in 132 patients with schizophrenia and 138 healthy normal controls. RESULT: There was no difference in allelic frequency of -1438A/G polymorphism between patients with schizophrenia and controls(K 2=2.261, df=1, p=0.133). A difference was found in genotype distribution(K 2=6.157, df=2, p=0.046), but this difference was being given by the increased A/A in th controls and A/G in the patients. The genotype frequency, which is the sum of homozygosity and heterozygosity for the -1438 G allele, was significantly higher in the patients(K 2=5.880, df=1, p=0.015). However, there was no difference between the patients with schizophrenia and conrols in the frequency of homozygosity for the -1438 G allele. CONCLULSION: These results suggest that -1438A/G polymorphism of the 5-HT 2A receptor gene promoter is not causally related to the development of schizophrenia in a Korean population.

6.Norfloxacin Hydrogel Coated Urethral Catheters for Prevention of Catheter Associated Urinary Tract Infection in the Rabbit.

PURPOSE: We developed a norfloxacin hydrogel coated urethral catheters, and evaluated its efficacy in a rabbit model. Our goal was to create a catheter that would prevent the development of catheter associated nosocomial urinary tract infections. MATERIALS AND METHODS: We inserted either an untreated, a polyurethane hydrogel coated or a polyurethane hydrogel with norfloxacin coated 8Fr Foley, catheter into the bladders of New Zealand White rabbits. Urine cultures were examined daily for 7 days. Scanning electron microscopy of the catheter surface was performed to structurally analyze the biofilm. RESULTS: There was a significant improvement (p=0.002) in the average time to a positive urine culture, from 3.1 to 5.7 days, and the rate of bacteriuria of the norfloxacin coated catheters was 70% lower than that of the untreated catheters. Scanning electron microscopy showed the formation of a bacterial biofilm on the surface of the untreated catheters, but deterioration of the biofilm was visible throughout the norfloxacin hydrogel coated catheters. CONCLUSIONS: A significant benefit was realized by coating the catheter surface with a norfloxacin impregnated hydrogel. This in vivo study suggests that this procedure may provide a significant clinical advantage, and reduce health care costs, for nosocomial infections.

A 17-year-old male was presented withfpruritic grouped erythematous papules on the face, chest, upper back, both axillae, and genital area. The histologic picture of a benign papilloma and spongiosis around the acrosyringium were prominent findings. The term acrosyringeal epidermal nevus is proposed for this histological picture, and some explanations for its pathogenesis are suggested.

8.Epitheilioid Trophoblastic Tumor of the Lung: A Case Report.

Epithelioid trophoblastic tumor is a rare type of gestational trophoblastic disease that is distinct from placental site trophoblastic tumor and choriocarcinoma, and epithelioid trophoblastic tumor has features resembling a carcinoma. We report here on an epithelioid trophoblastic tumor that was discovered as a solitary pulmonary nodule in the lung of a 50-year-old woman. The patient had suffered from a hydatidiform mole 20 years previously. Wedge resection of the lung was done and this showed a 1.9x1.5 cm sized, relatively well defined mass composed of mononuclear tumor cells admixed with hyaline-like material and necrosis. The tumor cells were positive for EMA, Cam5.2, alpha-inhibin, PLAP and hCG. After consulting the gynecologic department, a 7.5x6.5 cm sized mass was discovered in the uterine fundus. Hysterectomy was then done. The tumor cells were same to those of the lung mass. The lung mass is considered to be metastasis from the epithelioid trophoblastic tumor of the uterus. She has been an uneventful clinical course for three years.

9.Endotracheal Intubation with Laryngeal Mask Airway and Fiberoptic Bronchoscope.

It is easy to view the laryngeal aperature with a flexible fiberscope through a laryngeal mask airway (LMA). This is a case report that the LMA could facilitate a fiberscope-aided tracheal intubation easily. Patient was a 59 year old female who had some limitation of mouth opening due to ankylosis of bilateral temporomandibular joints. After thiopental (200 mg) and succinylcholine chloride (50 mg) were given intravenously, a number 3 sized LMA was inserted and the lungs were ventilated via the LMA. The anesthesia cirele system is then disconnected from the LMA and a fiberscope, with the proximal end jacketed with a well-lubricated, cuffless, 6-mm-ID endotracheal tube (ETT); was inserted into the trachea through the lumen of the LMA. The 6-mm-ID ETT was threaded over the fiberscope into the trachea before fiberscope was withdrawn, and a traeheal tube exchanger was inserted through the ETT after enough ventilation for a while, followed withdrawal of the LMA and ETT. Finally, we can make the 7 mm-ID ETT with cuff insert into the trachea over a tracheal tube exchanger very easily without any difficulties.

10.A study on changes of the Vertebral Pedicles and Mechanical Strengths after Screw Insertion

Spinal fixation using pedicle screws has recently been the focus of increased attention, but the adequate size of pedicle screw and maximum percentage fill as related to the pedicle diameter and are not well known. The objects of this study were to determine the ideal ratio among pedicle, drill and screw diameter, and to determine the maximum percentage fill of the screw without significant decrease of pull-out strength. The materials used for the experiments were 376 thoracic pedicles obtained from the 38 young pigs, and the diameters of pedicles ranged from 3.0 to 8.5mm. After 40% to 100% drilling as compared to pedicle diameter, screws were inserted carefully, and measurements were taken of the outer pedicle changes and pull-out strengths, and adequate drill and screw sizes as related to the diameters of given pedicles were determined. It was found that pull-out strength was the strongest after 60% drill, and the larger the drill diameter, the smaller the holding power, and the larger the screw diameter, the greater the holding power. Maximum pull-out strength was seen at 80-90% fill with 60% drill. After sequentially drilling each pedicle with increasingly larger drill bits, larger screws could be inserted with pedicle changes such as expansion, cutout, split fracture, and comminuted fracture. after larger drilling up to 100%, pedicle screws with diameters smaller than 115% of measured pedicle diameters could be safly inserted without fracture and significant decrease of pull-out strength. It is concluded that effective percentages of drill and screw diameters to the pedicle diameter are 60% and 80-90% respectively, and pedicle screw up to 115% of measured pedicle diameter can be safely inserted into pedicle without significant decrease of pull-out strength. It is thought that fresh pedicle has elasticity and larger screw can be inserted to the pedicle with strong holding after larger drilling.
