2.Self-care and Physical Symptom by Gender in Korean Type2 Diabetic Patients.

PURPOSE: The purpose of this study was to investigate the self-care and the physical symptom and to analyse the correlation between self-care and physical symptom in type2 diabetic patients. METHOD: 201 Participants were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. The data were collected by self report self-care and physical symptom questionnaire. RESULTS: The alcohol drinking frequency was lower in female than in male patients. The frequency of nausea or vomiting, morning headaches, lightheadedness, weakness, pain in the heart, pain in the back, stiffness in any join or muscle, and getting very tired in a short time were higher in female than in male diabetic patients. The exercise frequency was negatively correlated with physical symptom in female patients. CONCLUSION: The alcohol drinking frequency was lower in female than in male patients. The female patients had more physical symptoms than the male patients. The exercise frequency was negatively correlated with physical symptom in female patients.

3.Comparison of the Effects of Internet Diabetes Education by Gender in Type 2 Diabetic Patients.

PURPOSE: The purpose of this study was to evaluate whether the effect of Intenet diabetes education varied by gender. METHOD: An experimental group assessed pre- and post intervention was used to assess the effectiveness of diabetes education by nurses. Forty patients separated into two groups by gender partook in the study. The goal of the intervention was to keep blood glucose concentrations close to normal range. The intervention was applied weekly for 3 months. Participants were requested to input their blood glucose level, diet, and exercise diary everyday at http: //www.biodang.com by cellular phone or wire Internet. The researcher sent optimal recommendations to each patient using the short message service of cellular phones and wire Internet. All medication adjustments were communicated to the subjects' doctors. The plasma glucose levels, serum lipids, and care satisfaction were measured before and after the intervention. RESULT: Glycosylated hemoglobin (HbA1c) decreased 1.6% and 0.8% in male and female patients respectively after 3 months of education. Total cholesterol decreased 37.2mg/dl in male patients but increased 80.5mg/dl in female patients. CONCLUSION: These findings indicated that Internet diabetes education could improve HbA1c and total cholesterol in male patients.

4.Detection of Human Papillomavirus DNA by In Situ Hybridization using Biotinylated DNA Probes in Cervical Intraepithelial Neoplasias and Squamous Cell Carcinomas.

The authors examined 9 condylomas, 26 cervical intraepithelial neoplasms(CIN) and 22 invasive squamous cell carcinomas for the presence of human papillomavirus(HPV) DNA sequences by DNA-DNA in situ hybridization. In situ hybridization revealed target HPV DNA sequences mostly in the nuclei of the superficial cells from epithelium which contained either maturation or koilocytotic atypias. With the use of biotinylated HPV DNA probes 6/11, 16/18 and 31/33/35, 42 of the 57(73.7%) were positive with HPV-6/11, 23 with HPV-16/18, 32 with HPV-31/33/35 and 18 with two or more mixed probes. HPV-31/33/35 was wht most prevalent in CIN and invasive squamous cell carcinomas, follwed by HPV-16/18. The incidence of HPV DNA increased from 66.7% to 86.4% with increasing severity of the lesions from condylomas to invasive squamous cell carcinomas. Flat condyloma was most freuently accompanied by CIN.

5.Depression and Blood Glucose Testing in Women Type2 Diabetic Patients.

PURPOSE: The purpose of this study was to investigate the depression and the frequency of blood glucose testing in women type2 diabetic patients. METHOD: 114 Participants were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. Depression was measured by visual analogue scale. Blood glucose testing was measured the frequency during past 1 week. RESULT: Depression was higher in hyperglycemia patient (fasting blood glucose> or =110mg/dl) than in normoglycemia patient(fasting blood glucose <110mg/dl). The blood glucose testing frequency as lower in 50-59 years old than in less than 39 years old. And it was lower in middle school graduate than in college graduate. The blood glucose testing was negatively correlated with patient's age. CONCLUSION: The depression program should be developed for hyperglycemia diabetic patients. And the blood glucose testing education program should be developed for aged and low educational level patients.

6.Non-Conscious Perception of Emotions in Psychiatric Disorders: The Unsolved Puzzle of Psychopathology.

Psychophysiological and functional neuroimaging studies have frequently and consistently shown that emotional information can be processed outside of the conscious awareness. Non-conscious processing comprises automatic, uncontrolled, and fast processing that occurs without subjective awareness. However, how such non-conscious emotional processing occurs in patients with various psychiatric disorders requires further examination. In this article, we reviewed and discussed previous studies on the non-conscious emotional processing in patients diagnosed with anxiety disorder, schizophrenia, bipolar disorder, and depression, to further understand how non-conscious emotional processing varies across these psychiatric disorders. Although the symptom profile of each disorder does not often overlap with one another, these patients commonly show abnormal emotional processing based on the pathology of their mood and cognitive function. This indicates that the observed abnormalities of emotional processing in certain social interactions may derive from a biased mood or cognition process that precedes consciously controlled and voluntary processes. Since preconscious forms of emotional processing appear to have a major effect on behaviour and cognition in patients with these disorders, further investigation is required to understand these processes and their impact on patient pathology.

7.Direct Care Stress and Job Satisfaction in Home Care Nurses*.

PURPOSE: This study was aimed to investigate direct care stress and job satisfaction in home care nurses. METHOD: Subjects were 139 home care nurses of hospitals in urban cities. Data was collected by a self report questionnaire. Direct care stress was measured by Community Health Nurses Perceptions of Work-Related Stressors Questionnaire. Job satisfaction was measured by a visual analogue scale. RESULTS: The average score of direct care stress was 60.4. There were significantly high direct care stress in those who had less than that 3 years experience than in their counterparts. The average score of job satisfaction was 79.5. There were significant high job satisfaction in those who willingly chose to work. Direct care stress negatively correlated with the length of office experience of the home care nurse. CONCLUSION: A direct care educational program should be developed for those who have less than 3 years experience to decrease their stress level.

8.Contact dermatitis among male workers exposed to metalworking fluids.

In an epidemiological study of metal workers exposed to metalworking fluids(MWF), the prevalence time of evoultion, seasonal occurrence and clinical type of contact dermatitis were investigated. Composinal analyses of MWF with HPLC, dermatological examination and two consecutive questionnaire surveys were conducted. Study population was divided into two groups ; workers contact to cutting oil and workers contact to rust preventive oil. In the analysis of MWF, aliphatic hydrocarbons, having 12-20 carbons, was most common composition(49.04%) of cutting oil otherwise, major contents (90.99%) of the rust preventives oil were aliphatic hydrocarbons composed of 6-9 carbons. The frequency (point prevalence) of contact dermatitis(CD) was 7(12.7 per 100 subjects) in the dermatological examination of 55 workers. As the result of second survey for contact dermatitis, cumulative prevalence of oil working full-time and recent 1 year prevalence in two groups were 28.0, 16.7 and 15.1, 12.5 per 100 subjects. There were no difference in the prevalence of CD by oil, age, oil contact duration. Summer is the most common evolution season in workers exposed to cuttiogs, but not in workers exposed to rust preventive oil. Major clinical type of CD was erythematous papules in both groups. It presents the importance of preventive measures that 51.1% suffer from contact dermatitis had medical care at their own expense, and 47.1% of them felt serious about their contact dermatitis. From the fact that 68.6% think cotton gloves protective apparatus, we emphasize the need for health education.

Bacillus anthracis is a soil pathogen capable of causing anthrax in animals and humans. To establish a method for specifically detecting B. anthracis, we used nested polymerase chain reaction. Outer and inner sets of oligonucleotide primers were designed from the protective antigen (pag) gene and from the cya gene of the plasmid pXO1. Ainplification of 482 bp or 208 bp DNA fragment obtained from a nested PCR method provided the basis for rapid and reliable assay for the detection and identification of B. anthracis.

10.A Case of Torsade de Pointes after Combined Use of Terfenadine and Itraconazole.

Torsade de pointes is a life-threatening, polymorphic ventricular tachycardia associated with prolongation of the QTc interval. Although torsade de pointes is found in many clinical settings, it is mostly drug induced. Similar problems have been described with nonsedating H1-selective antihistamines like terfenadine and astemizole. The increased risks of both H1-antihistamines were associated with exposure to supratherapeutic doses or concomitant exposure to the cytochrome P-450 inhibitors, ketoconazole, erythromycin and cimetidine. We report a 51-year-old woman with torsade de pointes and a long QTc interval caused by the combined use of terfenadine and itraconazole. After discontinuation of these drugs and treatments with electrical cardioversion and magnesium sulfate, torsade de pointes and prolonged QTc interval were no longer observed and she was discharged in good condition with a normal ECG. In conclusion, physicians should be aware that terfenadine and astemizole can cause torsade de pointes in rare cases.
