1.Sacral Spinal Meningeal Cyst(Perineurial Cyst): A Case Report.

We experienced a rare case of sacral meningeal cyst so called sacral perineurial cyst(Tarlov cyst). A 15 years old girl was admitted to the hospital because of sudden severe low back pain and Lt. sciatica for about one week. We performed delayed myelography and spinal computeized tomographic myelography(C.T.M) for differential diagnosis. Delayed filling of contrast media in the cyst was significant in diagnosis of sacral meningeal cyst type II. Laminectomy of sacrum(S1) was performed for partial excision of cystic wall and plastic suture. Operative finding was typical sacral perineurial cyst(Tarlov cyst) which was recently clssified as sacral meningeal cyst type II. The classification of spinal meningeal cysts in the literature was indistinct and confused. We also agree with Nabors et. al, in current classificcation of spinal meningeal cysts, in which the spinal meningeal cysts, in which the spinal meningeal cysts were divided into 3 groups as extradural spinal Mcs Type I(Diverticulum), Type II(perineurial cyst), intradural spinal Mcs(arachnoid cyst).

2.Subjectivity on Childbearing in High School Students.

PURPOSE: This study was to explore subjectivity on childbearing in high school students. METHODS: A Q-methodology which provides a method of analyzing the subjectivity of each type was used. Forty-three high school students classified 40 selected Q-statements into 9 points standard. The obtained data were analyzed by using the pc-QUANL program. RESULTS: High school students' subjectivity on childbearing were analyzed into two types: Type 1 turned out to be 'FOLS (family oriented life style)' and Type 2 'CINK (couple important no kid)'. CONCLUSION: In order to resolve such problems as low birth rate and the advanced age of the population, effective youth and adult programs, policy and institution are required. The current demographic, economic and other factors such as personal values and policies may lower birth rate. In particular, youths' need for children and birth rate tend to be lower. There are positive and negative patterns in high school students' subjectivity on childbearing. Thus, national and social efforts are needed to change negative factors into positive ones. In order to maintain positive subjectivity on childbearing in high school students, it is necessary to apply family-centered educational programs and to implement birth-friendly and realistic programs for promoting child birth.

3.Radiologic Findings of Retroanastomotic Hernia after Gastrojejunostomy.

PURPOSE: To review the radiological findings of retroanastomotic hernia and to derive the useful US and CT criteria to assist in the diagnosis of the condition in patients who had previously undergone gastrojejunostomy. METHODS: During a recent period, 8 consecutive cases of retroanastomotic hernia were encountered. Of the patients involved, seven underwent US and CT imaging. The US and CT scans were reviewed retrospectively to determine the abnormal findings. Surgical confirmation was available in all cases. RESULTS: The efferent loop was herniated through the defect created behind the anastomosis in seven cases, and the afferent loop in one case. Retroanastomotic hernia was suggested prospectively in all cases. Among the seven cases of efferent loop herniation, the US and CT signs of retroanastomotic hernia included whirling of the mesenteric vessels, jejunal loops, and mesentery in the periumbilical abdomen (7/7); mural thickening of the herniated bowel loops (5/7); dilatation of the herniated bowel loops (2/7); and US showed decreased peristalsis of the herniated bowel loops (2/6). In one case, the US and CT signs of retroanastomotic hernia of the afferent loop included dilatation and whirling of a short length of the afferent loop behind the anastomosis. One out of the eight patients had reVersible bowel ischemia, and one had bowel necrosis. CONCLUSION: Retroanastomoic hernia is an important condition, and the US and CT findings might be used for its diagnosis.

4.Is Full-Length Intramedullary Nail Necessary for Atypical Subtrochanteric Femoral Fracture Associated with Bisphosphonate?

Background:
American Society for Bone and Mineral Research recommend the use of intramedullary reconstruction of full length-nail for atypical subtrochanteric femoral fracture (ASFF). However, there is no study on the incidence of the ipsilateral femoral fracture after index operation of ASFF, and full-length nail has disadvantage as iatrogenic fracture and leg length discrepancy (LLD). The aim of this study was to investigate the incidence of ipsilateral secondary fracture after using partial-length nail, and to compare the outcomes on surgery of ASFF between partial length-nails and full length-nails.
Methods:
Forty-five consecutive fractures with ASFFs which had undergone intramedullary fixation using cephalomedullary nail between 2011 and 2018 were enrolled. The 45 cases were grouped based on nail length into the partial-length nail group (n=26) and the full-length nail group (n=19). Ipsilateral secondary fracture, time to union, intra-operative iatrogenic fracture, metal failure, LLD, operative duration, and post-operative 24-hr blood loss were investigated.
Results:
There was no ipsilateral secondary fracture after index operation. There were no statistically significant differences between the partial-length nail and full-length nail groups in the time to union, LLD, and post-operative 24-hr blood loss (P=0.427, 0.478, and 0.228, respectively). Operative duration showed statistically significant difference between 2 groups (P=0.034). Metal failure were occurred in 1 (3%) case of the partial-length nail group and 2 (10%) cases of the full-length nail group. Iatrogenic fractures during nail insertion occurred in 2 (7%) cases of the partial-length-nail group and 3 cases (15%) of the full-length nail group.
Conclusions
Although large scale studies are required, our study indicate that full-length nails are not usually required for the treatment of ASFF.
5.Classification and Clinical Evaluation of Gastrointestinal Stromal Tumor: Immunohistochemical Expression of CD117, CD34, alpha- Smooth Muscle Actin, S-100.

PURPOSE: Gastrointestinal stromal tumors (GISTs) represent a distinct and the most important subset of, mesenchymal tumors of the GI tract. Stromal tumors of the gastrointestinal tract have long been a source of confusion and controversy, with regard to their classification, differentiation, criteria of malignancy and prognostic features. METHODS: The 26 case studies of patients treated for a Gastrointestinal mesenchymal tumor, including leiomyomas, leiomyosarcomas and GISTs, between 1994 and 2002 at Keimyung University Hospital, were evaluated retrospectively. The cases were confirmed as leiomyomas, schwannomas, or GISTs by pathological re-examination. 20 of the cases were diagnosed as GISTs, from the pathological examination, and were chosen for the evaluation of their clinicopathological and immunohistochemical characteristics, using CD34, CD117, alpha-SMA and S-100 done. RESULTS: The new diagnoses of the mesenchymal tumors were a leiomyoma in 3 cases, a schwannoma in 3 and gastric stromal tumors in all 20. The immunohistochemical studies were positive for CD117 and CD34 in 95 and 75% of the gastric stromal tumors, respectively. The histopathological findings showed 5 benign tumors, 3 borderline tumors, and 12 malignant tumors in the 20 patients. CONCLUSION: The immunohistochemical marker (CD117) for KIT is a specific marker for GISTs among the tumors occurring in the stomach, and can be used to distinguish GISTs from true leiomyomas and gastric schwannomas. We also found that severe cellularity, atypism, intratumoral hemorrhage and necrosis, large size and a high mitotic count correlate with malignant behaviour and a poor prognosis.

7.The effects of backward, upward, rightward pressure maneuver for intubation using the OptiscopeTM: a retrospective study

Background:
The OptiscopeTM and the backward, upward, rightward pressure (BURP) maneuver are widely used in clinical practice because the BURP maneuver facilitates intubation by improving visualization of the larynx. However, the effect of the BURP maneuver is unclear when using the OptiscopeTM. Therefore, we retrospectively investigated the effect of the BURP maneuver on intubation using the OptiscopeTM.
Methods:
Sixty-eight patients intubated with the OptiscopeTM were enrolled. We used the BURP maneuver in Group A (n = 33) and the conventional maneuver (which does not use the BURP maneuver) in Group B (n = 35). BURP application status was a binary variable representing whether the BURP maneuver was used during the intubation. A multiple linear regression analysis was performed to assess the effects of the BURP application status on intubation time controlling for body mass index, preoperative dental injury status, obstructive sleep apnea history, thyromental distance, sternomental distance (SMD), interincisor distance, history of neck rotation restriction, and Mallampati classification.
Results:
There was no difference in the intubation time between the two groups. According to the regression model (R2 = 0.308, P = 0.007), the BURP maneuver (Group A) decreased the intubation time by 6.089 seconds (95% confidence interval 1.303–10.875, P = 0.014) compared to Group B.
Conclusion
The BURP maneuver reduced intubation time when using the OptiscopeTM.
8.A study of job stress, suicidal ideation and suicide attempts in display manufacturing workers: a crosssectional study

Methods:
Data were collected from 836 workers in a display manufacturing company who participated in health screenings from May 22 to June 16, 2017. The data included general characteristics, night work, job tenure, previous physician-diagnosed chronic diseases, suicidal ideation/suicide attempts, and job stress. We investigated suicidal ideation/suicide attempts that covered the past year by using a self-reported questionnaire. Job stress was measured using the 43-item Korean Occupational Stress Scale. Multiple logistic regression analysis was used to investigate the association between job stress and suicidal ideation/ suicide attempts. The mediator effect of depression on suicidal ideation/suicide attempts was tested using a series of logistic regression by applying Baron and Kenny's mediation method.
Results:
In the model adjusting for variables (e.g., age, body mass index, smoking, alcohol consumption, regular exercise, shift work, job tenure, chronic disease and depression), physical environment (OR: 3.60, 95% CI: 1.08–12.02), lack of reward (OR: 5.31, 95% CI:1.54–18.34), and occupation climate (OR: 7.36, 95% CI: 2.28–23.72) were correlated with suicidal ideation/suicide attempts in women. However, all subscales of job stress were not significantly correlated with suicidal ideation/suicide attempts in men. In mediation analysis, job instability and occupational climate were correlated with suicidal ideation/suicide attempts and were mediated by depression in men workers.
Conclusions
In women workers, the experiences of suicidal ideation/suicide attempts were significantly correlated with the physical environment, lack of reward, and occupational climate that were subscales of job stress. In men workers, depression rather than job stress was correlated with experiences of suicidal ideation/suicide attempts.
