2.Atypical Abdominal Pain.

Abdominal pain usually originates from the abdomen, but the lack of evidence of any abdominal disease suggests the pain originate from the thorax or have metabolic and neurogenic causes. Especially, neurogenic abdominal pain is recurrent and sustained causalgic pain, regardless of diet and without abdominal distension or rigidity, may be accompanied by signs of neurologic dysfunction. We have experienced a very rare case of abdominal pain arising from the intramedullary cavernous hemangioma of spinal cord. We report this case with a review of the literature.

3.Post Traumatic Radioulnar Cross Union: A Case Report

The purpose of this paper is to report the rare complicated radioulnar cross union following open reduction for fractures of the forearm bones. The case was 62 years old male who complained of swelling and pain on forearm by the car accident. Roentgenogram revealed forearm bones fracture. Atpost operative 4 months, roentgenogram noted new bone crossing between radius and ulna. Excision of new bane was performed with extraction of nail and inserted chips of muscle. Post operative course was satisfactory, showing 50 degree of range of motion (20 degree supination and 30 degree pronation) from fixed pronation position.

4.Pantalar Arthrodesis

There are many procedures for stabilizing the foot but all frail feet, all complete drop feet, and all calcaneocavus deformities due to paralysis are best treated by pantalar fusion. Hunt and Thompson advised one stage pantalar arthrodesis by astragalectomy and replacement of the denuded bone in paraiytic feet resulting from poliomyelitis. At the Presbyterian Medical Center, Jeonju, Korea, 12 one stage pantalar arthrodesis were performed between 1973 and 1978 and following observations were made. 1. Seven of the patient had pantalar fusions because of poliomyelitis. 2. Five feet were equinovarus and 4 had frail feet with varying amounts of varus or valgus of the forepart of the foot and of the heel. 3. Nine patients were considered to have a good result, two had a fair and one was failure 4. Length of immobilization was from 13 weeks to 37 weeks and the average length of immobilization was 17 weeks. 5. In one stage pantalar arthrodesis, there was no complication except one non-union and the period of hospitalization was decreased.

5.A Case of Polymorphic Pemphigoid.

Polymorphic pemphigoid is a polymorphic variant of bullous pemphigoid. Tbe authors had observed a case of polymorphic pemphigoid in a 54-year-ald woman, in whom generalized chronic blistering lesions and aevere pruritus were the main symptoms. The diagnosis of polymorphic pemphigoid was based on clinical, histopa.thological, immunopathological and therapeutic features.

6.Binding of B Cell - Derived Autocrine Growth Factor to Hemoglobin.

Normal human B cells produce autocrine growth factor in response to Staphylococcus aureus Cowan I strain (SAC). However, the functional role and molecular nature of the B cell derived-B cell growth factor (B-BCGF) are largely unknown. We have tried to investigate the nature of B-BCGF using mAb for several years. We report here that B- BCGF is capable of binding to hemoglobin (Hb). The concentrated culture supernatant from tonsillar B cells stimulated with SAC for 24 h was loaded into the fast protein liquid chromatography and ion-exchange chromatography. The peak with BCGF activity was shown to have a M.W. of 16-18 Kda in polyacrylamide gel electrophoresis followed by silver stain. Amino acid sequence of the fraction was found to identical to human hemoglobin (Hb) by more than 85%. However, Hb itself had no BCGF activity. The presence of Hb in culture supernatant was due to the contamination of SRBC during B cell purification. SRSC were completely removed from B cells by percoll-gradient centrifugation and B cells were stimulated with SAC and exogenous Hb was added to the cultures. The Hb fraction from FPLC again showed a BCGF activity. These data strongly suggested that BCGF binds to Hb. We confirmed this in dot blot as well as Western blot. The M.W of Hb-binding BCGF was 20 Kda. This information may provide a rapid progress in research of BCGF.

7.Composite Carcinoma-Carcinoid Tumor of Stomach: Report of a case.

The concurrence of typical carcinoid admixed with adenocarcinoma of the gastrointestinal tract is a well known but unusual phenomenom that has been reported in the esophagus, stomach, gall bladder, colon and appendix. The amine precursor uptake and decarboxylation (APUD) concept included the idea that neuroendocrine cells of the gastrointestinal tract were dervied from the neural crest by a process of migration during embryogenesis. However the intimate admixture of cells which have morphologic and immunocytochemical properties of both carcinoid tumors and adenocarcinoma suggests that these tumors are derived from a multipotential stem cell showing bidirectional differentiation rather than from unrelated cell lines. The term composite carcinoma-carcinoid tumor is therefore an appropriate designation for these neoplasms. We report a case of composite carcinoma-carcinoid tumor of the stomach in a 38 year-old female with brief review of literature.

10.Scalenus Anticus Syndrome: Report of a Case

The Scalenus anticus syndrome is a definite clinical entity and the result of pressure upon the component parts of the brachial plexus and the subclavian artery by the scalenus anticus, scalenus medius, scalenus minimus muscle and the first rib. This case, a fifty one years old male, complained of pain, tingling sensation in the right shoulder and arm. Roentgenograms showed no specific finding. Symptom was dramatically relieved by the scalenotomy. Six years later he had no complaints and no evidence of recurrence.
