1.A Case of Atypical Sessile Dermatofibroma.

We report a case of a morphologic variant of dermatofibroma. A coin sized, brownish, shinny, somewhat fibrotic pedunculated tumor in a 39-year-old male was diagnosed as a sclerosing hemangioma, a type of dermatofibroma histopathologically. Its dome shape morphology was unusual in comparison with the typical morphology of dermatofibroma that we know.

Menetrier's disease is a rare, characterized by the presence of large rugal folds involving part or all of the stomach. Patients with hypertrophic gastropathy often have distressing abdominal symptoms, weight loss, and edema due to gastric protem loss. A 64 year old male patient was admitted with the chief complaint of epigastric discomfort. Endoscopic, radiologic and histologic examination, revealed characteristic findings of Menetrier's disease. A case of Menetrier's disease is reported with a brief review of literature.

4.Clinical Observation on 6 Cases of Juvenile Diabetes Mellitus.

Six cases of juvenile diabetes mellitus who were admitted Busan National University Hospital from Jan., 1976 to Dec., 1979 were observed. The result were summarized as follows; 1) The mean age of onset was 11 & male to female ratio was 2 to 1 with male preponder ance. One half cases were occrred during spring. Two had family history of diabetes inclu ding one case of diabetic sibling. 2) Polyuria and polydipsia were noted in all cases as initial symptoms and symptoms of polyphagia(4), weight loss(4), weakness(3), enuresis(2) and altered consciousness(2) were noted in order of frequency. More rapid onset, more severe clinical features were observed. Associated illness were noted 4 cases of infection & 1 case of cataract. Mean blood glucose level was 430mg%, urine sugar above 3+and S.G. of urine was above 1.030. 3) Three cases of ketoacidosis & diabetic coma were occurred on admission. Consciousness cleared in 3 days, and ketonuria disappeared in 2 days after insulin & fluid therapy. 4) Mean duration of admission was 24 days. At discharge, dose of insulin needed was 10 to 50 units/day. Two cases were readmitted due to ketoacidosis and postoperative complication but all cases were not sufficiently followed up.

Extramammary Pagets disease(EMPD) of the vulva is an uncommon neoplasm which accounting for 2.5% of all vulvar malignancies and 0.02% of all female cancer. Several hundred cases have been reported worldwide since the first description by sir James Paget in 1874, It generally affects postrnenopausal age poup and presents high rate of recurrence but the precise incidence remines unclear, Although Paget's disease of the breast and the vulva are histologically similar, their histogenesis and biological behavior are considered to be significantly different and EMPD has gcnerally perceived rnore benign compared to that of the breast. Four histologic forms of vulvar EMPD have been recognized and treated vulvar EMPD in according to the 4 histologic classification. The high rate of recurtence disease remains a challenge for optimal management. Recently we experienced a case of Paget's disease of the vulva, diagnosed as an intraepithelial Pagets disease preoperatively and underwent radical vulvectomy and superiomedial thigh(SMT) flap, and present with a brief review of literature.

6.Doppler Tissue Image for Diagnosis of Myocardial Dyssynchronicity in Congestive Heart Failure: Comparison with EKG.

BACKGROUND AND OBJECTIVES: Electrical dyssynchronicity (Dsyn) appears to be prognostic of survival in congestive heart failure (CHF). Recent study has shown some discrepancy between the electrical Dsyn and the Doppler tissue image (DTI) assessed mechanical Dsyn. The aim of our study was to evaluate the relationship between the QRS duration and DTI assessed Dsyn. SUBJCETS AND METHODS: One hundred and forty patients, with CHF and left ventricular ejection fractions < or =40%, were enrolled. DTI was performed on 5-basal and 5-mid segments to assess the time from the R-wave to the peak systolic velocity (RS time). A QRS duration >130 msec, standard deviation (SD) of the RS time >40 msec, or a difference in the maximal and minimal RS times (RS time-diff) >100 msec were indicators of'Dsyn'. RESULTS: The prevalence of myocardial Dsyn, by QRS duration, SD of the RS time and the RS time-diff were 19, 43 and 47%, respectively. The SD of the RS time (49.8+/-23.6 vs. 36.6+/-20.7, p<0.01) and the RS time-diff (139.2+/-63.2 vs. 98.0+/-54.3, p<0.01) were prolonged in the wide (>130 msec) compared with the narrow QRS group. There was also a weak positive correlation between the QRS duration and the SD of the RS time (R=0.34, p<0.001) and the RS time-diff (R=0.38, p<0.001). However, from a cross-tabulation analysis, more than one third of patients had a discrepancy between QRS duration and DTI assessed mechanical Dsyn. From a multivariate analysis, a major determinant of the SD of the RS time was the QRS duration. CONCLUSION: Although a major determinant of the DTI assessed Dsyn was the QRS duration, more than one-third of patients had a discrepancy between electrical and mechanical Dsyn. Therefore, not only the QRS duration, but the DTI assessed Dsyn, should be measured when considering cardiac resynchronization therapy.

BACKGROUND AND OBJECTIVES: Several echocardiographic methods, such as ejection fraction, fractional shortening, and Doppler tissue imaging (DTI), have been developed to quantify systolic function but all had several important limitations. The purpose of this study was to quantify the regional wall motion abnormality, using strain, in an acute myocardial infarction, compared with a visual estimation. SUBJECTS AND MEHTODS: Forty-five patients, with acute anterior (n=28) and inferior myocardial (n=17) infarctions, who underwent color DTI, were examined using longitudinal strain and standard echocardiography, and the results were compared with 54 normal controls. A total of 594 segments [3 segments (apical, mid and basal portion) of septum and inferior wallx99 patients] were evaluated. RESULTS: In the normal control group, the strain was uniformly distributed in all segments (-20%~-23%). In the infarction groups, the strain was negatively related with the wall motion score. The strain of the apical segments and mid-septum was decreased in the anterior wall infarctions, and the strain of basal septum and mid to basal inferior wall was decreased in the inferior wall infarctions. The dyskinetic segments showed positive strain. CONCLUSION: This study validates strain as a superior method for the quantification of the regional wall motion abnormality in an acute myocardial infarction than visual estimation.

9.Quad-Sparing Minimally Invasive Total Knee Arthroplasty.

PURPOSE: The purpose of this study was to investigate and define the factors affecting the success of quad-sparing (QS) minimally invasive total knee arthroplasty (TKA). MATERIALS AND METHODS: A total of 120 patients(150 knees) were tried consecutively to undergo QS minimally invasive TKA from July 2004 to February 2005. Seventy knees (Group 1) could be performed with this technique, but 80 knees (Group 2) were failed and required further incision. Two groups were compared for clinical, radiological and postoperative data. These cases were also divided into three groups according to the time undergone surgery. For these three groups, we analyzed the clinical data and surgical results. RESULTS: Age, height, weight, body mass index and pre-operative knee score were not significantly related to the success of QS minimally invasive TKA. Greater preoperative range of motion, low patella height, and smaller size of the tibial implant were of a significant correlation with the success of surgery. As the number of cases were cumulated, the success rate of this method increased and the operating time was shortened. CONCLUSION: QS minimally invasive TKA could not be successfully applied to every patients. For the success of this surgery, adequate preoperative assessment of the patient and sufficient experience on this surgical technique would be necessary.

10.Comparison between the Results of Minimally Invasive Total Knee Arthroplasty Performed with Mini-Midvastus Technique and Quadriceps-Sparing Technique.

PURPOSE: To compare the short-term clinical results of mini-midvatus minimally invasive total knee arthroplasty(TKA) with quadriceps-sparing minimally invasive TKA. MATERIALS AND METHODS: Between August 2005 and February 2006, 23 bilateral (46 knees) minimally invasive total knee arthroplasties were performed simultaneously using quadriceps-sparing surgical technique at one side and mini- midvastus minimally invasive technique at the contralateral side. The same surgeon performed all the operations with the same type of prosthesis(Nexgen LPS-flex Total Knee System) using MIS quadriceps-sparing instrument. The pos- toperative clinical and radiological results were analyzed in each group. RESULTS: There were no significant differences between the two groups in the operating time, total blood loss, length of skin incision, radiological results and postoperative HSS scores at 8 weeks and 1 year follow-up(p>0.05). Also, there were no differences in the postoperative range of motion and pain score assessed by visual analog scale at 1 day, 3 days, 7 days, 14 days, 8 weeks and 1 year follow-up(p>0.05). Conclusions: Compared to quadriceps-sparing TKA, mini-midvastus minimally invasive TKA revealed no significant differences in the short-term clinical and radiological results. Therefore, it can be considered as an effective, alternative minimally invasive technique.
