Leiomyomas of vagina and vulva are rare solid tumors, but most common mesenchymal tumors in adult woman. Most of the reported cases have been benign. The tumors were firm, well-defined submucosal masses in a majority of patients. We experienced two cases of vulvar and vaginal leiomyoma, thus it is presented with a brief review of the cases and its literatures.

Patients with achondroplasia manifest various neurologic symptoms including megaencephaly, hydrocephalus, and progressive myelopathy or radiculopathy secondary to spinal stenosis. However, only anecdotal postmortem reports proved ruptured aneurysm or vascular malformation as a source of intracranial hemorrhage. We herein report a case of a 26-year-old woman with achondroplasia who underwent uneventful surgical treatment for the aneurysmal subarachnoid hemorrhage. In this literature, we review the pathophysiologic mechanism and emphasize the necessity of considering the possibility of sudden mental deterioration in achondroplastic patient.

Three-dimensional computed tomographic angiography (CTA), a representative noninvasive radiologic technique, is being widely used for detecting vascular lesions in specific intracranial bleeding under a certain circumstance (acute nontraumatic subarachnoid hemorrhage). We encountered a case of extravasation of the contrast medium on CTA images that appeared as ribbon-like high-attenuation lesion from an aneurysm at the distal anterior choroidal artery in a young adult moyamoya patient. As CTA is used more frequently, it is imperative to understand such unusual but, potentially lethal image findings to conduct a prompt intervention.

PURPOSE: To decide diagnostic titer in a single determination of antibody by indirect particle agglutination test and to characterize the occurrence of Mycoplasma pneumoniae during a 9 year-period in Seoul. METHODS: M. pneumoniae pneumonia diagnosed serologically at the Seoul National University Children's Hospital from 1986 to 1995 were reviewed, retrospectively. Antimycoplasma antibody was measured by indirect particle agglutination test. Diagnosis was based on 4 fold-rise or single high titer. Single diagnostic titer was set up by the value greater than the 90th percentile of the titer at one year of the children who initially had 4 fold-rise or > or =160. RESULTS: The geometric mean titer and the 90th percentile at 1 year of the 15 children who initially had antibody titer > or =160 or 4 fold-rise were 122 and 639, respectively, and single diagnostic titer was established as > or =640. A total of 126 childern were included by our diagnostic criteria. Male and female ratio was 1.17:1 and the mean age was 6 years 4 months. Pneumonia by M. pneumonuae occurred every years during the study period showing increased cases every 3 years. In the epidemic years, the cases occurred in late spring, summer and early fall. CONCLUSIONS: It is concluded that the occurrence of Mycoplasma pneumoniae pneumonia in Seoul from 1986 to 1995 was characterized by epidemics every 3 years as well as its endemicity.

Cryopreservation is able to store the surplus pre-embryos for freezing and furthermore thawing and transfer in a subsequent cycle. Cryopreserving cells which are maintaining their viability are the very complex process. This study has been carried out in order to find the effects of cryopreservation steps, freezing media and embryonic stages on the rates of viability and development of cryopreserved mouse embryos. Female ICR mice (6~8 weeks old) were induced to superovulate by sequential intraperitoneal injection of 5 IU PMSG and 5 IU hGC 48h apart. Mouse embryos were collected according to its developmental stage after the injection of hCG. Embryos were cryopreserved not only by cryoprotectant step (1 step~ 4 step) but also in a variety of media (HTF, IVF medium, D-PBS) and cell stage. The results were as follows: There is no clear advantage in these freezing media of rapid method, but 4 cell and 8 cell of slow method (2, 3, 4 step) have advantage in D-PBS. The development of embryos according to cell stage become greater in 8 cell stage. In the treatment steps of cryopreservation, the development of embryo to blastocyst was similar among rapid method, but the development of 4 cell and 8 cell embryos to blastocyst according to slow method was better than rapid method.

PURPOSE: To evaluate the effectiveness of core decompression with impaction bone graft in regard to the stage of the disease. MATERIALS AND METHODS: Forty-two cases in 32 patients were classified using ARCO (Association Research Circulation Osseous, 1992) classification and twenty-five cases were compared with postoperative follow-up MRI evaluation. There were 26 male and 6 female patients. Ten patients among 32 were bilatellally involved. RESULTS: There were 9 hips of stage 1 (A,O; B,1; C,8), 29 hips of stage 2 (A,1; B,6; C,22) and 4 hips of stage 3 (only C). Radiological progression was seen in 21 of 42 hips (50%); 22% of stage 1 (2/9), 59% of stage 2 (16/29) and 75% of stage 3 (3/4). Clinical failure with total hip arthroplasty conversion was seen in 11 of 42 hips (26%); 28% of stage 2 (8/29) and 75% of 3 (3/4). Forty-three percents of lateral lesions (11/26 cases) and all cases with femoral head involvement more than 30% showed clinical failure and radiologic progression. CONCLUSIONS: When the outcome of core decompression with impaction bone graft was evaluated according to ARCO classification, the progression of the stage or conversion to total hip arthroplasty was more common in the larger lesion and lateral location of necrosis. So, we could predict that core decompression with impaction bone graft was ineffective in preventing the progression of disease in the case of large extent and/or lateral location of necrosis.

Acute pancreatitis has two mortality peaks, which occurs within the first 2 weeks due to organ failure and then weeks or months later as result of multi-organ failure and local complications. Although there have been several clinical and multidisciplinary evaluation measures, imaging tests, and serological tests proposed to forecast severe acute pancreatitis, there is still no single test available to reliably predict the disease severity and time of death. Future large-scale studies are required to develop an assessment scale that can accurately predict the prognosis and mortality risk of severe acute pancreatitis.
