2.A Comparative Retrospective Study of Mucosal and Submucosal Cancer in Early Gastric Cancer.

BACKGROUND: The 5-year survival rate for early gastric cancer (EGC) now exceeds 90% and EGC is now recognized as a curable malignancy. The less extensive treatment has attracted interest. It is of vital importance to select a surgical method appropriate to each individual case. A retrospective study was undertaken to compare clinicopathologic features between mucosal (M) and submucosal (SM) cancers and to evaluate treatment modality for EGC. METHODS: We analyzed clinicopathologic data on 345 surgically treated patients with early gastric cancer between September 1983 and December 1996 at the Department of Surgery, Korea University College of Medicine: 183 patients with mucosal cancer and 162 patients with submucosal cancer. RESULTS: The rate of detection of EGC has been increasing as high as 25% since 1990. There were significant differences between M and SM cancers with regard to the location of the tumor, lymph node metastasis and histologic type. The SM cancer had lower 1/3 in location, more lymph node metastasis and more differentiated than M cancer. Nodal involvement was evident in 49 patients (14.2%): 6.6% (12/183) with M cancer and 22.8% (37/162) with SM lesions. The 5-year survival rate of all patients was 94.7%: 95.3% in M cancer and 94.0% in submucosal SM cancer. The 5-year survival rates for SM cancer were 96.1% in node-negative cases and 86.3% in node-positive cases with significance and M cancer had no survival difference. CONCLUSIONS: As early gastric cancer can be cured by surgery, the submucosal carcinomas in this series had no indications for less extensive treatment, such as endoscopic or laparoscopic limited surgery. These carcinomas should be subject to standard surgery with gastrectomy and combined dissection of lymph nodes.

3.Primary Cementless Total Hip Arthroplasty with a Sandwich Type in Ceramic-Ceramic Articulation (Minimum 10-Year Follow up Results).

PURPOSE: To evaluate the minimum 10-year follow up results of primary total hip arthroplasty (THA) performed using a sandwich-type (alumina-polyethylene-titanium) ceramic bearing. MATERIALS AND METHODS: Thirty four patients (40 hips) who underwent a THA with sandwich typed liners from November 1998 to December 2000 were analyzed. Among the 34 patients, 25 were men and nine were women. Mean follow-up was 134 months (range, 120~145 months) and mean patient age at the time of THA was 47.1 years (range, 24~65 years). The clinical results were evaluated using the Harris hip score and level of thigh pain with a limping gait. The radiographic evaluation was done in terms of the endosteal new bone formation, radiolucent line, subsidence of the stem, migration of the acetabular cup, and proof of loosening. RESULTS: The mean preoperative Harris hip score of 50.2 points (range, 31~87 points) was improved to 90.9 points (range, 75~99 points) at the final follow-up and thigh pain with limping gait in one case. All cases had fixation by bony ingrowth. No radiographically detectable loosening was observed in any hip. Fracture of ceramic liner in one case required change of the polyethylene liner. CONCLUSION: At the minimum 10-year follow up, survival rate as the end-point was favorable. However, fracture of the ceramic is still a major problem. Satisfactory results can be obtained by more precise surgical technique for acerabular inclincation and anteversion, and by improving the quality of the ceramic.

4.Retroperitoneal Tumors Preoperatively Misdiagnosed as Adrenal Tumor.

PURPOSE: Despite of recent advances in the resolution of computed tomography (CT) and magnetic resonance imaging (MRI), there are different kinds of peri-adrenal retroperitoneal tumors that can hardly be differentiated from adrenal tumor preoperatively. By reviewing certain tumors and their characteristics, we may increase the preoperative diagnostic accuracy and so may perform the proper operation when facing this situation. METHODS: We retrospectively reviewed fourteen cases of non-adrenal origin retroperitoneal tumors that were preoperatively diagnosed as adrenal tumors during a seven-year period at Seoul National Universiry Hospital. RESULTS: The fourteen retroperitoneal tumors preoperatively misdiagnosed as adrenal tumors were extra-adrenal ganglioneuroma (6 cases), schwannoma (2 cases), retroperitoneal paraganglioma (1 case), bronchogenic cyst (2 cases), Castleman's disease (1 case), idiopathic retroperitoneal fibrosis (1 case) and leiomyosarcoma (1 case). All of them were large (> 6 cm), and most were solid. CONCLUSION: For the correct preoperative diagnosis and appropriate operation of peri-adrenal retroperitoneal tumors, and especially when the tumors are large and solid, the disease entities mentioned above should be excluded, and the application of additional diagnostic measures such as preoperative fine needle aspiration cytologic examination or biopsy, endoscopic ultrasound (EUS), intraoperative frozen biopsy, laparoscopic ultrasound (LUS), as well as preoperative CT and MRI, should be considered when the diagnosis is not conclusive.

5.Hemodynamic and Intrapulmonary Shunt Effects of Dobutamine / Adenosine Triphosphate and Dobutamine / Sodium Nitroprusside Infusion.

The purpose of this study was to evaluate the efficacy of adenosine triphosephate (ATP) in comparison to sodium nitroprusside (SNP) in reducing left ventricular afterload in 20 patients, ASA physical status I, during ethrane-N2O anesthesia. Hemodynamic effects of intravenous ATP (30~250 ug/kg/min) were compared with those of SNP (0.3~2.5 ug/kg/min) in group 1 (n=10). In group 2 (n=10), hemodynamic and intrapulmonary shunt effects of dobutamine (1 ug/kg/min) alone and in combination with ATP or SNP, required to maintain mean arterial pressure around 70 mmHg, were compared. The results were as follows. 1) Both ATP and SNP reduced arterial pressure rapidly resulting from a marked decrease in systemic vascular resistance in a dose-related manner. 2) Cardiac index increased from 3.31+/-0.201/min/m2 to 4.04+/-0.281/min/m2 (p<0.01) following dobutamine alone, and increased further to 5.71+/-0.38 1/min/m2 (p<0.001) and decreased to 3.77+/-0.28 1/min/m (NS) in combination with ATP and SNP, respectively. 3) At equivalent decrease in mean arterial pressure, ATP increased heart rate significantly less than SNP. 4) Hypotensive response was more stable during ATP infusion than during SNP infusion. 5) Arterial oxygen tension was significantly higher during dobutamine/ATP infusion than during dobutamine/SNP infusion (268+/-6 vs 256+/-9 mmHg, p<0.05). 6) Intrapulmonary shunt fraction increased from 4.49+/-0.65% to 5.51+/-0.71% (p<0.05) following dobutamine alone, and increased further to 9.92+/-1.13 (p<0.001) and to 7.21+/-0.77% (p<0.01) in combination with ATP and SNP, respectively. These results suggest, although ATP increases intrapulmonary shunt fraction more than does SNP, ATP has significant advantage over SNP, either alone or in combination with dobutamine, in improving cardiac performance in patients with low output states due to high peripheral vascular resistance.

6.Significant considerations of mechanical strength of BLT implant (Roxolid): Correlation between material and product strength under static and fatigue loads

The purpose of this study is to correlate the material properties of TiZr alloy material (Roxolid) and the mechanical strength of the implant fixture (BLT) through the maximum compressive load test and the fatigue test. Implant samples were purchased with BLT (Roxolid, Straumann, Switzerland) and Octa1 (cold-worked, Megagen Implant, Korea) fixtures made of two materials (TiZr and cold-worked Ti Grade 4). After the maximum compressive load test, the test specimens were analyzed with a optical microscope to confirm the fracture pattern. After the fatigue test, the samples that passed 5 million cycles were analyzed for the precision fit using a scanning electron microscope. Data were analyzed using Student's t-test (α=0.05). In the compressive load test, the small diameter (3.3 mm) implants with the same length and the common diameter (4.1 mm) implants had fracture or bending regardless of the material in both the test and control groups. The fatigue load showed the same results without significance and the gap between the fixture and abutment interface was within 10 µm in both the test and control groups in the precision fit test. There was no statistically significant difference in the maximum compressive load and fatigue test for the comparison of mechanical strength between two systems (BLT vs. Octa1) with same diameter and length, similar shapes and connections.

7.Detection of BRAF Mutation on Fine Needle Aspiration Biopsy Specimens Using the Colorimetric Mutation Detection Method.

PURPOSE: The detection of papillary thyroid carcinoma (PTC) is increasing these days; however, there is currently no satisfactory definitive preoperative diagnostic method. Fine-needle aspiration cytology (FNAC) is now the most accurate method to diagnose PTC preoperatively. It is known that the specificity of BRAF mutation is high in papillary thyroid carcinoma. Therefore, detection of BRAF mutation using a FNAC slide can be helpful to diagnose PTC preoperatively. METHODS: 13 patients with benign disease, 36 patients with PTC and 23 patients with an indeterminate diagnosis as determined histologically on the FNAC slide were evaluated to detect BRAF mutation with using FNAC slides and intraoperative fresh tissue. RESULTS: Mutation was detected using direct sequencing and the colorimetric method. The frequency of BRAF mutation was 86.3% for all the PTC cases. The concordance between the colorimetric method and direct sequencing was 57.1%. During DNA extraction from the FNAC slide, the DNA damage is so severe that direct sequencing is succeeded in only one case. CONCLUSION: We have to take measures to overcome and prevention DNA damage during extraction. The colorimetric method is not reliable.

8.Wall shear stress on vascular smooth muscle cells exerts angiogenic effects on extracranial arteriovenous malformations

Background:
In addition to vascular endothelial cells, vascular smooth muscle cells (VSMCs) are subject to continuous shear stress because of blood circulation. The angiogenic properties of VSMCs in extracranial arteriovenous malformations (AVMs) may exceed those of normal blood vessels if the body responds more sensitively to mechanical stimuli. This study was performed to investigate the hypothesis that rapid angiogenesis may be achieved by mechanical shear stress.
Methods:
VSMCs were obtained from six patients who had AVMs and six normal controls. The target genes were set to angiopoietin-2 (AGP2), aquaporin-1 (AQP1), and transforming growth factor-beta receptor 1 (TGFBR1). Reverse-transcriptase polymerase chain reaction (RT-PCR) and real-time PCR were implemented to identify the expression levels for target genes. Immunofluorescence was also conducted.
Results:
Under the shear stress condition, mean relative quantity values of AGP2, AQP1, and TGFBR1 in AVM tissues were 1.927±0.528, 1.291±0.031, and 2.284±1.461 when compared with neutral conditions. The expression levels of all three genes in AVMs were higher than those in normal tissue except for AQP1 under shear stress conditions. Immunofluorescence also revealed increased staining of shear stress-induced genes in the normal tissue and in AVM tissue.
Conclusions
Shear stress made the VSMCs of AVMs more sensitive. Although the pathogenesis of AVMs remains unclear, our study showed that biomechanical stimulation imposed by shear stress may aggravate angiogenesis in AVMs.

PURPOSE: This study sought to determine the 1-year clinical effectiveness and safety of a biodegradable, polymer-containing Biolimus A9™-eluting stent (BES) in Korean patients with acute coronary syndrome (ACS). MATERIALS AND METHODS: A total of 1000 ACS patients with 1251 lesions who underwent implantation of BESs at 22 centers in Korea were enrolled between May 2011 and July 2013. We assessed major adverse cardiac events (MACE) defined as the composite of cardiac death, non-fatal myocardial infarction (MI), and clinical-driven target vessel revascularization at 12 months. RESULTS: Patient mean age was 62.6±11.4 years. 72.8% of the patients were male, 28.5% had diabetes, 32.8% had multi-vessel disease (MVD), and 47.9% presented with acute MI (AMI). The mean global registry of acute coronary events risk score of all patients was 103.0±27.6. The number of stents per patient was 1.3±0.6. The incidences of MACE and definite stent thrombosis at 12 months were 3.9% and 0.2%, respectively. On multivariate Cox-regression analysis, age ≥65 years was identified as an independent predictors of 1-year MACE (hazard ratio=2.474; 95% confidence interval=1.202−5.091). Subgroup analyses revealed no significant differences in the incidence of MACE between patients with and without diabetes (4.3% vs. 3.7%, p=0.667), between those who presented with and without AMI (4.4% vs. 3.4%, p=0.403), and between those with and without MVD (4.6% vs. 3.5%, p=0.387). CONCLUSION: Our study demonstrated excellent 1-year clinical outcomes of BES implantation in patients at low-risk for ACS.

10.Transcriptional profiling of dental sensory and proprioceptive trigeminal neurons using single-cell RNA sequencing.

Dental primary afferent (DPA) neurons and proprioceptive mesencephalic trigeminal nucleus (MTN) neurons, located in the trigeminal ganglion and the brainstem, respectively, are essential for controlling masticatory functions. Despite extensive transcriptomic studies on various somatosensory neurons, there is still a lack of knowledge about the molecular identities of these populations due to technical challenges in their circuit-validated isolation. Here, we employed high-depth single-cell RNA sequencing (scRNA-seq) in combination with retrograde tracing in mice to identify intrinsic transcriptional features of DPA and MTN neurons. Our transcriptome analysis revealed five major types of DPA neurons with cell type-specific gene enrichment, some of which exhibit unique mechano-nociceptive properties capable of transmitting nociception in response to innocuous mechanical stimuli in the teeth. Furthermore, we discovered cellular heterogeneity within MTN neurons that potentially contribute to their responsiveness to mechanical stretch in the masseter muscle spindles. Additionally, DPA and MTN neurons represented sensory compartments with distinct molecular profiles characterized by various ion channels, receptors, neuropeptides, and mechanoreceptors. Together, our study provides new biological insights regarding the highly specialized mechanosensory functions of DPA and MTN neurons in pain and proprioception.
