1.A Morphologic Study on the Bile Duct Changes Induced by Common Bile Duct Ligation in Rats.

In an attempt to elucidate the pathological changes following common bile duct ligation, the present study was undertaken in male Sprague-Dawley rats. Morphologic studies of the livers were performed at 1, 2, 3, 5, 7, 9, 11, 13 and 15 weeks after common bile duct ligation. In an attempt to clarify the relationship between the process of bile duct formation and the nature of primitive cells observable around the primitive biliary structure, light microscopic, immunohistochemical and electron microscopic studies were performed. The results were noted as follows: 1) Light microscopically, proliferation of biliary cells began in the periphery of portal areas and expanded toward hepatic lobules. In severe cases of biliary structure proliferation, hepatocytic cords and classic hepatic lobules were inconspicuous. 2) Immunohistochemically, CK-19 expression was limited to biliary structures in protal areas and proliferated biliary epithelial cells. In the serial sections of paraffin block, proliferated intrahepatic biliary structures were associated with those of portal areas. Some oval cells in the ductular hepatocytes were stained for both CK-19 and MNF 116. 3) Ultrastructurally, the proliferated biliary epithelial cells divided into three patterns: absence of lumen, formation of incomplete lumen, and formation of complete lumen. Furthermore these patterns had spectral continuity of maturation in their structures. 4) In some biliary structures, individual biliary cells pushed the basement membrane toward neighboring tissue with accompanying destruction of basement membranes, patterns of budding. Sometimes these cells and hepatocytes comprised the same lumen. In summary, the results obtained by the present study indicate that proliferated biliary structures may be derived from the preexisting intralobular or portal biliary system.

2.The Fractured Carpal Scaphoid

The carpal scaphoid is the commonest of the carpal bones to be fractured, and this injury is the commonest to befall the wrist of working man. There is still some disaggrement as to the optimum treatment and as to how long immobilization should be maintained. The twenty seven cases of the carpal scaphoid were treated at department of orthopaedic surgery, Maryknoll Hospital. They were analyzed in regared to the clinical and radiological aspects. The results were summarized as follows. l. Of 27 cases 24 cases were in the age group 20 to 39, and all cases were male patients. 2. In 17 cases, the fracture was found in right side. 3. In 19 cases (70.4%), the fracture was found on the middle 1/3 of the carpal scaphoid and in 18 cases(66.8%), the fracture line was transverse. 4. The union rate of fracture was 100% in the distal 1/3 fracture, 89.5% in the middle 1/3 fracture and 66.3% in the proximal fracture. The average period of immobilization were 10.3, 14.9 and 15.3 weeks respectively. 5. By Maudsley's assessment, the end result was good in all cases of the distal 1/3 fracture, 84.2% in the middle 1/3 fracture and 66.7% in the proximal 1/3 fracture. 6. Traumatic arthritis developed in 3 cases, avascular ncerosis in 7 cases, nonunion in 4 cases.

4.A Treatment of Ipsilateral Shaft Fracture of the Femur and Tibia in Adult

Twenty-eight cases of fracture of the femur and tibia on the same leg were treated in Pusan Maryknoll Hospital during the period from April 1981 to Jun 1988. We studied all of these patients, divided by five groups according to the method of treatment, with analysis of treatment and end results. The following results were obtained. 1. The incidence of trauma was high in the young man, and most frequent in the third decade (42.9%). 2. The main cause of injuries was traffic accident; 22 patients (78.6%). 3. The common fracture site was middle one-third in femur and tibia respectively. 4. The most common associsted injury was head trauma (9 case). 5. The average healing time of fracture was 20.7 weeks in femur and 22.7 weeks in tibia of group 3, 4, 5. 6. The best results were obtained in cases both fractures stabilized surgically.

5.Treatment of the Scaphoid Fracture

Twenty-three cases of the scaphoid fracture which were treated at the Department of the Orthopaedic Surgery, Catholic University Medical College during the period of december 1978 to december 1987 were analysed. The main purpose of this study was to clarify the significant factors affecting the union rate of the scaphoid fracture. The results obtained were as follows ; 1. The great majority of 23 cases was male with mean age 31. 2. Ulnar deviated stress oblique view with msking a fist was the most reliable means for diagnosing the fracture, location of fracture and stability of wrist after scaphoid fracture. 3. Union rate was obtained from 88.8% in undisplaced fresh fracture, 85.7% in displaced fresh fracture and 71.4% in old neglected fracture. 4. The union rate and associated factors affecting the prognosis after scaphoid frscture seems to be depended on the location of fracture, the severity of initial displacement of fracture fragments, time elapsed of treatment after fracture and accompanied injuries around the ipsilateral wrist.

6.The Clinical analysis of Acetabular Fracture

If the treatment of acetabular fractures are not satisfactory, there will be serious complication such as traumatic arthritis or avascular necrosis of the femoral head. There is much continuing discussion as to where conservative or surgical treatment should be used. The authors experienced 41 cases of acetabular fractures, that wrere treated at the Department of Orthopaedic surgery, Maryknoll Hospital from January 1981 to September 1987. The results of 29 cases who were followed over a 1 year period are as follows:1. According to Letournel's classification, the most common type of elementray fracture was posterior wall(24.1%) and a T- shsped fracture was 10.3 % and both column fractures were 10.3%. 2. Complications were traumatic arthritis(14 cases), superficial infection(5 cases), ectopic ossification(5 cases), osteomyelitis(1 case), AVN of the femoral head(1 case), and iartrogenic femoral shaft fracture(1 case). 3. According to matta's assessment, satisfactory results of conservative treatment were 38% clinically and 38% radiologically, and by surgical treatment, 73% clinically and 64% radiologically. 4. Surgical treatment in acetabular fractures, such as an anatomical reduction and rigid fixation fixation followed by early joint motion were improved the result and decresed the rate of traumatic arthritis.

7.The Use of Ender nails in Distal One-Third Fractures of Tibia

The distal one-third fractures of tibia have much difficulty in treatment due to enumerous complications such as skin necrosis, angular deformity and delayed union. As for using Ender nails in distal tibial fractures, the stability of fixation can be strengthened with fanning of the nails in distal fragments and using more than 3 nails and 1cm longer nail. Twenty-two patients having distal tibial fractures, were carried out Ender nailing at Department of Orthopaedic Surgery, Maryknoll Hospital from May 1989 to September 1992. The results were as follows; 1. The distal end of fracture line was situated from 3cm to 9cm above plafond, and the mean duration of radiological bony union was 14.9 weeks except one case of nonunion. 2. Mild angulation developed in two cases and complications such as nonunion, superficial infection and backing out of nail were noted, but it did not affect the results.

8.Effect of Diet and Apoliprotein E (Apo E) Polymorphism on the Variation of Serum Lipid Profile in Korean Males.

BACKGROUNG AND OBJECTIVES: Diet is the basic and principal therapeutic modality for hyperlipidemia. However, diet therapy alone showed variable responses in lowering lipid levels in different studies. This research is to prove the effect of diet and Apo E polymorphism on the variation of serum lipid profile in Korean males. MATERIALS AND METHODS: To evaluate the gene-diet interation, serum total cholesterol (Chol), triglycerides (TG), HDL-cholesterol and LDL-cholesterol (HDLc, LDLc), lipoprotein (a) (Lp (a)), and fasting blood glucose (FBS) were measured with Apo E genotyping in vegeterians (group A, n=154) and in healthy Korean male adults (group B, n=150) of similar mean age (50.1 vs. 49.3). RESULTS: Lipid profiles showed significantly lower levels in group A compared to group B (Chol 168.3+/-30.5 mg/dL vs. 181.3+/-33.4 mg/dL, p<0.001;TG 131.0+/-62.9 mg/dL vs. 149.4+/-76.7 mg/dL, p=0.023;HDLc 56.0+/-11.0 mg/dL vs. 56.9+/-11.5 mg/dL, p=0.509;LDLc 92.5+/-28.1 mg/dL vs 100.6+/-29.9 mg/dL, p=0.016;Lp (a) 22.1+/-14.6 mg/dL vs. 26.9+/-13.8 mg/dL, p=0.004;FBS 85.1+/-14.1 mg/dL vs. 102.7+/-16.6 mg/dL, p<0.001). The Apo E genotyping showed Epsilon3/3, 64.1%; Epsilon3/4, 20.7%;Epsilon2/3, 11.8%;Epsilon2/2, 1.3%;Epsilon4/4, 0.6% in the combined groups. The distribution was similar in both groups. Chol and LDLc were significantly (p<0.05) higher in Epsilon3/4 allele group compared to other allele groups among non-vegetarians. On the other hand, Chol and LDLc were significantly (p<0.01) lower in vegetarians compared to non-vegetarians only in Epsilon3/4 allele group. CONCLUSION: Vegetarian diet significantly lowered Chol, TG, LDLc, Lp (a) and FBS levels. Significant lipid lowering effect of vegetarian diet was noted in Apo E allele Epsilon3/4 group which had significantly higher Chol and LDLc levels without diet intervention. These data suggest that the influence of diet on serum lipid profiles differ according to apo E genotypes.

9.Relationship between Pulmonary Function and Apnea-Hypopnea Index in Asthmatic Children: The Preliminary Study.

OBJECTIVE: It has been reported that the sleep apnea syndrome in the asthmatic patients is prevalent, however, the systematic study in this field using polysomnography has rarely been performed. The aim of this study is to investigate the relationship between the apnea-hypopnea index (AHI) and the pulmonary function in asthmatic children. METHODS: This study enrolled 19 male and 12 female asthmatic children aged 6-13 years (average 8.2+/-1.7 years old). Complete overnight polysomnography and pulmonary function test were performed for the participants. RESULTS: Of the 31 asthmatic children, 21 (67.7%) met the diagnostic criteria of the pediatric sleep apnea and the average AHI was 1.7+/-1.5/h. The children with higher AHI showed poorer pulmonary function (FEV1/FVC ratio : p=0.002, FEV1%pred : p=0.047). CONCLUSION: These results suggest that the prevalence of the pediatric sleep apnea could be very high among the asthmatic children and the severity of the sleep apnea correlates with the pulmonary function. However, the case-control study to compare the AHI between the asthma and control groups is absolutely necessary because few normative data are available for the children.

10.Visual Prognosis and the Development of After-cataract following Cataract Surgery in Atopic Dermatitis.

PURPOSE: To investigate the visual prognosis, the interval, and the rate of development of after-cataract following cataract surgery in atopic dermatitis. METHODS: Twenty eyes of 13 patients diagnosed as cataract associated with atopic dermatitis were operated between January, 1994 and January, 2000. Visual acuity, postoperative inflammatory reaction, the interval, and the rate of development of after-cataract were investigated and compared with the cataract cases without atopic dermatitis. RESULTS: The average preoperative best corrected visual acuity of 20 eyes was 0.23+/-0.22. The average postoperative best corrected visual acuity at 1, 2, 7, and 12 months after surgery were 0.78+/-0.23, 0.83+/-0.16, 0.78+/-0.23, 0.83+/-0.26, respectively. These values showed no significant difference compared to the postoperative visual acuity of the cataract cases without atopic dermatitis. Aqueous flares measured with laser flare meter were 15.09+/-9.20, 11.15+/-4.68, and 7.38+/-0.75 photon counts/millisecond at 10 days, 1, and 2 months after surgery, which did not seem to influence the final visual acuity and the development of after-cataract (p>0.05). Among the 20 eyes, 66.67% finally developed after-cataract and the average time interval of the development of after-cataract was 11.25+/-4.22 months. CONCLUSIONS: Visual prognosis following cataract surgery in atopic dermatitis is good, while after-cataract seems to develop more frequently and earlier compared to the cases without atopic dermatitis.
