1.Effects of Multi-Component Exercise and Retraining after Detraining on Functional Fitness and Cognitive Function in Elder People with Mild Dementia.

BACKGROUND: With rapid growth in the number of older Koreans and prevalence of dementia, exercise has been emphasized as first, second and third prevention of dementia. A few study, however, investigate effects of detraining and retraining in demented elders. Therefore, the purpose of this study was to investigate the effects of 24 week exercise followed by 12 week detraining and retraining on physical and cognitive function in demented seniors. METHODS: Thirty-five demented older adults were assigned into exercise and control group. Exercise group performed multi-component exercise, 2 times a week for 24 weeks. After 12 week detraining, subjects were retraining for 12 weeks. Subjects were tested four times on physiological variables, physical function and cognitive function. Ten subjects in exercise group and 6 subjects in control group were remained at final 48 week, and data from them were analyzed. RESULTS: There were no significant effects on physiological variable in both groups. However, 24 week exercise improved upper and lower body strength, flexibility, dynamic mobility and physical function. These effects, however, were significantly decreased in 12 week detraining period and increased again after 12 week retraining. There were significance group differences. Cognitive function was improved after 24 week exercise but decreased in detraining and retraining period and no significant difference between groups was shown. CONCLUSIONS: The present study suggests that positive effects after exercise tend to be reduced once one stops exercise, and be improved again after retraining. Therefore, strategies to encourage demented elders to exercise are needed.

2.A Case of Romberg Syndrome.

Romberg syndrome is a rare disease and characterized by unilaeril atrophy of the skin, subcutaneous tissue and the underlying structure of half the face. The lesion does not usually cross the midline of the scalp. Roriberg syndrome is sometimes mildsnosed as linear scleroderma, although they differ widely in their clinical and histological apperance. A 13-year-old boy was present with a one year history of idefined atrophic patch on the left side of face and neck. We involved skin was not bound down to the underlying structure. The history revealed no prcvious trauma or disease, He had no abnormal neurologic finding. Computerized tomography demonstrated a diminished subcutaneouat volume and also decreased thickness of back muscle is the left side face from cheek to neck.

4.Effect of Vasomotor Drugs on the Choroidal Blood Flow in Rabbits.

The effect of intravenously administered vasoconstrictor (nor-epinephrine, ephedrine and posterior pituitary hormone) and vasodilator (papaverine, nicotinic acid and theophylline) drugs on the choroidal blood flow of rabbits was studied by the apparatus based on the principle of Grayson's internal calorimetry. Thermistors, as the sensing elements, were introduced into the suprachoroidal space of rabbits eye, and continuous determinations were performed up to 60 minutes after the administration. Papaverine(2mg/kg), theophylline (5mg/kg and 50mg/kg) and nicotinic acid (25mg/kg and 50mg/kg) produced significant increase in the blood flow, papaverine and theophylline being the most powerful. Among the vasoconstrictor drugs, posterior pituitary hormone produced profound and prolonged decrease in the blood flow, whereas nor-epinephrine and ephedrine increase the blood flow following transient decreases in the blood flow. The relationships between blood pressure, intraocular pressure and choroidal blood flow, and the suggestions in the therapeutic applications to human eye were discussed.

Pseudo-Meigs' syndrome consists of pleural effusion, ascites, and benign tumors of the ovary other than fibromas. These benign tumors include those of the fallopian tube or uterus and mature teratomas, struma ovarii, and ovarian leiomyomas. Although uterine leiomyomas are frequent gynecological finding, they are rarely cause pseudo-Meigs' syndrome with elevated serum CA 125 levels. A case of pseudo-Meigs' syndrome associated with subserosal leiomyoma is presented with a brief review of literatures.

One hundred seven isolates of Staphylococcus aureus from bovine mastitis were investigated for colony morphology in serum-soft agar (SSA), autoagglutination in salt, and capsular serotype. Capsular polysaccharide (CP) was purified and quantified from the extracts of clinical isolates. Overall, 89 isolates (83.2%) were diffuse in the SSA, without any difference in the proportion of diffuse colony between type 5 and type 8 strains. Some strains exhibited compact colonies in the SSA and expressed CP as determined by an enzyme-linked immunosorbent assay, indicating that compact morphology does not exclude encapsulation. The majority of the strains (11/12) showed autoagglutination in the salt aggregation test. The serotype 336 accounted for 46.7% of the isolates followed by serotype 5 (12.1%) and serotype 8 (12.1%). Particularly, twenty-six (24.3%) isolates reacted with two serotypes; 7 for type 8/336 and 19 for type 5/336. Five isolates (4.7%) were nontypeable with monoclonal antibodies specific for CP serotype 5, 8, or 336. The CP concentration in culture supernatants varied with the serotypes, and the total amount of CP produced by cells grown in a liquid medium was much less than that produced by cells grown on a solid medium. 8.Suggestions for Increasing Physical Activity Levels of Korean Older Adults: A Public Health Perspective.

2010 is the year when the Korean baby boomer generation faces retirement. With the need for affective strategies to keep this population healthy, the government has placed much effort to increase the regular physical activity (PA) level of older adults. Yet the prevalence of active seniors is still relatively low. This paper reviews the recent government strategies and efforts to enhance the physical activity levels of older adults and the PA programs provided in the community settings by major organizations and institutions. In addition, the problems of the current approaches are shown, and based on a review, the author provides five suggestions for promoting the PA level of older adults as follows. First, specific physical activity goals and objectives for older adults should be included in the National Health Plan 2020. Second, the recommendations for PA for seniors, which include a proper intensity, duration and frequency of exercise, should be provided. Third, the role of organizations and institutions should be rearranged for the proper dissemination of government provided community-based exercise programs. Fourth, a standard quality-control system should be provided, and finally, an evidence-based exercise program, which should be specifically created for the older Korean population, should be developed. Physical activity offers opportunities for people to extend the years of their active independent life and reduce functional limitations. To maximize the effects of government efforts to promote PA for seniors, we feel the comments provided in this paper should be closely examined.

9.A Case of Giant Keratoacanthoma.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

10.Sacral Spinal Meningeal Cyst(Perineurial Cyst): A Case Report.

We experienced a rare case of sacral meningeal cyst so called sacral perineurial cyst(Tarlov cyst). A 15 years old girl was admitted to the hospital because of sudden severe low back pain and Lt. sciatica for about one week. We performed delayed myelography and spinal computeized tomographic myelography(C.T.M) for differential diagnosis. Delayed filling of contrast media in the cyst was significant in diagnosis of sacral meningeal cyst type II. Laminectomy of sacrum(S1) was performed for partial excision of cystic wall and plastic suture. Operative finding was typical sacral perineurial cyst(Tarlov cyst) which was recently clssified as sacral meningeal cyst type II. The classification of spinal meningeal cysts in the literature was indistinct and confused. We also agree with Nabors et. al, in current classificcation of spinal meningeal cysts, in which the spinal meningeal cysts, in which the spinal meningeal cysts were divided into 3 groups as extradural spinal Mcs Type I(Diverticulum), Type II(perineurial cyst), intradural spinal Mcs(arachnoid cyst).
