1.Pulmonary Cavernous Hemangioma: A case report.

The pulmonary cavernous hemangioma is usually from birth and there may be without symptoms until adulthood. Larger or multiple pulmonary angiomata with considerable pulmonary arteriovenous shunts may cause cyanosis, finger clubbing, dyspnea and frequently accompanyingbruit. Recently, we experienced a case of cavernous hemangioma of the lung. A 34-year-old woman was admitted to our hospital for surgical evaluation of a 4 cm solitary, round nodule in the right upper lobe on the chest X-ray and CT scan. She had no symptoms. Laboratory findings are within normal limits except for elevated glucose levels. At surgery, the mass was well encapsulated and easily excised from the peripheral portion of the posterior segment of the right upper lobe. Grossly, it consisted of a 4 cm in diameter, round, soft, sponge-like, hemorrhagic, slightly lobulated mass with a smooth external surface. Microscopically, the mass was composed of vessels, which were thin walled, dilated and filled with blood. The wall of the abnormal vessels was thin and composed of endothelium and fibrous connective tissue with only a little smooth muscle. Immunohistochemically, the wall of the dilated abnormal vessesls showed negative reaction for cytokeratin(low and high) and epithelial membrane antigen but weakly positive reaction for UEA-1 in focal areas.

2.THR with Hydroxyapatite - coated Hip Prosthesis.

Synthetic hydroxyapatite is a safe, nontoxic, biocompatible, and osteoconductive material. Hydroxyapatite-coated implants have the property of achieving a very strong bond with living bone in a relatively short period. From March 1992 to June 1994, 140 hydroxyapatite-coated primary total hip prostheses were implanted. This study included 60 cases that could follow up minimum 3 years. At the time of mean follow up, 45 months after operation, the mean Harris hip score was 92 points. Only two patients complained of thigh pain at last follow up. On radiographic evaluation, radiolucent lines were seen around the uncoated distal part of the stem in 46 percents of the implants. No radiolucent line was observed around the coated area of the prosthesis. Cancellous condensation was seen in 95 per cents of the implants at the Gruen zone 2 and 6 area. Cortical hypertrophy was present in 32 percents. Fourteen cases had the gaps at the bone to acetabular cup interface in the immediate postoperative period. The gaps disappeared in 12 months after operation. Five implants were revised because of polyethylene wear, cup migration and periprosthetic fracture. The clinical outcome was excellent. But this result was obtained in the relatively short-term follow up. Longer-term data will be the only true test of the value of hydroxyapatite coated implant.

3.Sparganosis in Subcutaneous Tissue of Thigh: A Case Report.

A case of rare subcutaneous sparganosis in thigh treated by surgical excision is reported. In this 49year-old male with a palpable mass on the anteromedial aspect of mid-thigh (5x7x5cm sized) which was misdiagnosed with a soft tissue tumor initially, a sparganosis was suspected by a plain x-rays, bone scan and his past history which he frequently had raw snakes, frogs and raw fishes before but confirmed by MRI and surgical excision. This represents tor warning to some Koreans who have frequently comsumed raw fishes, snakes or frogs etc., and to some doctors because it is easily confused with a soft tissue tumor.

5.Expression of Maspin Protein in Ductal Hyperplasia, Intraductal Carcinoma and Invasive Ductal Carcinoma of the Breast.

Maspin is a recently described gene with tumor suppressor activity. The gene product is a 42 kD protein with homology to the serpin family of protease inhibitors and may play a role as an inhibitor of tumor cell invasion. The prior observation that invasive breast cancers and their metastases showed decreased maspin protein expression by immunostaining supports this speculation. However, the role of maspin in breast cancer progression has not been studied in detail. We, therefore, studied maspin protein expression in a series of hyperplasia, atypical ductal hyperplasia, intraductal carcinoma and invasive carcinomas. Immunohistochemical staining (IHC) for maspin was performed on paraffin sections of 136 breast specimens using a commercially available monoclonal antibody. Among the 106 cases studied were 36 moderate/florid ductal hyperplasia, 11 atypical ductal hyperplasia (ADH), 29 intraductal carcinoma (IDC) (4 low grade, 13 intermediate grade, 12 high grade) and 30 invasive ductal carcinomas. Thirty cases of normal breast were also studied as control group. IHC stains were scored using a semiquantitative scoring system. The mean IHC scores for maspin for normal, moderate/florid hyperplasia, atypical ductal hyperplasia, intraductal carcinoma, and invasive carcinoma were 5.51 1.30, 7.36 0.72, 3.82 1.60, 4.48 2.69, 3.97 3.30, respectively. These scores for each category were statistically significant (p<0.05), except between ADH and IDC. Maspin protein expression was increased in most cases of moderate/florid hyperplasia, while maspin expression was more heterogeneous in ADH and IDC. In high grade IDC, maspin protein expression was stronger than low and intermediate grade IDC, and this suggests the possibility of a compensatory cellular response against the forces driving further tumor progression. Two thirds of invasive ductal carcinomas expressed maspin protein weakly and focally. All metastatic carcinomas of lymph nodes were negative for maspin. It is possible that high grade IDC with strong maspin expression may represent a subset less likely to progress to invasive cancer. This speculation merits investigation in clinical outcome studies.

6.A Case of Plasmodium falciparum Gametocytemia Successfully Treated with Primaquine.

We experienced a case of Plasmodium falciparum gametocytemia successfully treated with primaquine in a twenty seven-years old woman. The patient had been admitted due to general malaise after diagosis and treatment of P. falciparum at Tanzania one month ago. On microscopic examination, P. falciparum gametocytemia was seen and treated with mefloquine for one week but gametocytemia was not disappeared. After primaquine treatment for two weeks, she was successfully treated.

7.Current Trends and Future Tasks of Cohort Study for Disaster Victims.

An analysis of domestic and foreign cohort studies of disaster victims can suggest directions for domestic cohort studies of disaster. Research papers on disaster cohorts were found with search engines such as PubMed and RISS. The key words used were disaster, trauma, cohort, and posttraumatic stress disorder (PTSD). Foreign research papers on human-made disaster (n=16), technological disaster (n=11), natural disaster (n=12), and domestic research papers on disaster (n=6) were reviewed. Analysis revealed that domestic cohort research on disaster victims is lacking compared to foreign countries. This results in the current limitations in planning for active intervention and support for disaster victims. To establish appropriate crisis intervention and management plans for disaster situations, a cohort study of disaster victims should be actively implemented through epidemiologic survey and assessment of risk factors on mental health, PTSD, depression, anxiety and other deleterious outcomes.

8.A Short-Term and Long-Term Follow-up Study on Change of Hearing and Tympanogram after Tympanoplasty Type 1.

BACKGROUND AND OBJECTIVES: The objective of this study was to evaluate improvement in hearing and changes in the tympanogram according to the types of mastoid pneumatization and the mucosal condition of the Eustachian tube orifice after performing tympanoplasty type 1. MATERIALS AND METHODS: Among the patients with chronic otitis media who had received tympanoplasty type 1 at Asan Medical Center from June 1990 to March 1997, sixty five patients were evaluated with pure tone audiometry and tympanometry for a short-term (<6 months) and long-term (>1 year) follow-up period. RESULTS: Greater improvement in hearing was shown in patients with tympanogram type A as compared to the patients with type B and C in both short-term and long-term follow-ups. Patients who showed normal mastoid pneumatization were found to have type A tympanogram more frequently than patients who showed abnormal mastoid pneumatization. However, the degree of postoperative hearing improvement did not depend on the types of mastoid pneumatization. Mucosal condition of the Eustachian tube orifice had some influence on the outcome of postoperative tympanogram and hearing. Greater improvement in the hearing levels were observed more frequently in the normal mucosa. CONCLUSION: Postoperative improvement in hearing was closely related to the postoperative change in the tympanogram. Postoperative hearing results and tympanograms were mainly influenced by the mucosal condition of the Eustachian tube orifice. Relatively greater improvement in hearing was observed in the long-term follow-up compared to the short-term follow-up. However, in order to reach any conclusion on this matter, a longer follow-up period and closer observation would be required.

9.Normal Humeral Head Retroversion Angle in Korean Measured with Semil - axial View.

A reduced retroversion angle of humeral head may predispose to recurrent anterior shoulder dislocation and may also be a factor in persistent instability after soft tissue procedures. Rotation osteotomy of proximal humerus is one of many surgical modalities proposed for recurrent anterior shoulder dislocation. To support such an operation, fundamental knowledge of shoulder anatomy is essential. The semi-axial view by Soderlund have been regarded as simple and reliable method for measuring humeral head retroversion angle. The purpose of this study was to evaluate validity of the semi-axial view and to assess reference values for humeral head retroversion angle in Korean. Humeral head retroversion angle was determined from 80 healthy subjects, 48 men and 32 women. Radiographs which shown less than 10degrees between humeral shaft axis and epicondylar axis were defined as acceptable radiographs by Soderlund. Acceptable radiographs were selected and two orthopedic surgeons measured retroversion angle, separately. Acceptable radiographs were obtained in only 70 shoulders (43.8%). The mean angle was 35.2+/-8.24degrees for dominant hand and 32+/-6.27degrees for nondominant in Korean. The mean angle was 35.3+/- 7.78degrees for right side and 31.9+/-6.8degrees for left. The interobserver difference was 2.9degrees. The semi-axial view by Soderlund was not reproducible solely. But if correct arm position is considered, the method presented is easy to use daily.

10.The effects of fluoride releasing orthodontic sealants on the prevention and the progressive inhibition of enamel demineralization in vitro.

The purpose of this study was to identifu the preventive and the preventive and the progressive inhibitory effects of enamel demineralization with fluoride releasing light-and self-cured orthodontic sealants(FluoroBond), in vitro, under the polarizing light microscope and the scanning electon microscope. The polarizing light microscopic group was subdivided into seven groups(Group A-Group G). The scanning electron microscopic group was also subdivided into seven groups(Group A'-Group G'). For polarzing light microscopic evaluation, longitudinal sections were made longitudinally by Maruto cutter(Maruto Co., Japan) and Maruto grinding machine(Maruto Co., Japan). Sections were examined and photographed by the polarizing light microscope(Olympus Optical Co., Japan) using crossed polars and with the enamel rod longitudinal axis oriented at 45degrees to the extinction position. For scanning electron microscopic evaluation, the specimens were coated with a highly conducting layer of gold palladium in a model Hus-4 high0vacuum evaporator* and examined in an ISI-100B scanning electron microcpe** operated at 20kV. The results of this study were as follows : 1. The mean deoths of artificial carious lesions under a polarized light microscope were Group A(5.08microgram), Group B(47.82microgram), Group C(8.42microgram), Group D(7.20microgram), Group E(85.41microgram), Group F(60.38microgram), Group G(60.13microgram). 2. There were statistically significant differences in Group B compared with Group A, C, and D(p<0.05), and also, in Group E compared with Group F and Group G(p<0.05). 3. Light-and self-cured orthodontic sealants had the preventive effects of enamel demineralization. 4. Light-and self-cured orthodontic sealants had the progressive inhibitory effects enamel demineralization. 5. The time progress of demineralizing agent no ingluecse on the samples of light-and self-cured orthodontic sealants under the scanning electron microscope. 6.
