2.A Study on the Perceived Burden and the Quality of Life of Family Caregivers of Hemodialysis Patients.

This study was designed to identify the relationship between the perceived burden that is felt by family caregivers of hemodialysis patients and the quality of life of these caregivers. The subjects for this study were 102 family caregivers of patients who received hemodialysis regularly in the hemodialysis units of 4 hospitals located in Chonbuk. The data was collected during the period from February 17 to March 28, 1997 through interviews and by distributing a structured questionnaire to family caregivers through hemodialysis patients, and by mail service. The instruments used for this study are as follows : The burden instrument was the Burden Questionnaire developed by Lee Sook-Ja by referring to the Burden Scale developed by Novak & Guest(1989) and Zarit et al.(1980). The quality of life instrument was the Andrews' Scale that was treanslated and proof-read by Jeong Chu-Ja and partially amended by the author of this study. The collected data was analyzed by descriptive statistics, ANOVA, the Scheff test, Pearson's correlation and Stepwise Multiple Regression using the SAS PC+ program. The results of this study are as follow : 1. Hypothesis 1 : "The higher the perceived burden, the lower the quality of life of family caregivers of hemodialysis patients" was supported(r=-0.463, p=.000). 2. Hypothesis 2 : "The burden of family caregivers of hemodialysis patients will differ depending on the demographic characteristics of hemodialysis patients and family caregivers" was partially supported-age of family caregiver(F=3.04, p=.020), educational level of family caregiver(F=3.81, p=.012), marital status of family caregiver(F=5.82, p=.004), relationship with hemodialysis patient(F=2.74, p=.016), sex of hemodialysis patient(F=6.79, p=.010). 3. Hypothesis 3 : "The quality of life family caregivers of hemodialysis patients will differ depending on the demographic characteristics of hemodialysis patients and family caregivers" was partially supported-age of family caregiver(F=2.82, p=.029), educational level of family caregiver(F=4.64, p=.004), marital status of family caregiver(F=6.62, p=.002), monthly total income of family(F=5.61, p=.001), age of hemodialysis patient (F=3.42, p=.011), occupation change of hemodialysis patient(F=3.54, p=.032). 4. Stepwise Multiple Regression Analysis indicated that the six variables-the perceived burden of family caregiver, monthly total income of family, religion of family caregiver, occupation of family caregiver, marital status of hemodialysis patient, education level of family caregiver-were significantly predictive of the quality of life of family caregivers of hemodialysis patients. The six variables explained 41.7% of the variance in quality of life. In conclusion, this study revealed that the perceived burden is an important factor related to the quality of life of family caregivers of hemodialysis patients. Therefore, nurses must recognize family caregivers as well as hemodialysis patients as important care receivers and incorporate an intervening plan that reduces the burden and promotes the quality of life, taking into account the demographic characteristics of hemodialysis patients and their family caregivers.

6.A Case Report of Myotonia Atrophica

The phenomenon of Myotonia consist in a failure of voluntary muscles to relax immediately when voluntary innervation ceases. The stiffness is accentuated by cold and relieved by exercise, while generalized muscle weakness and atrophy (or not commonly hypertrophy of muscle) is common. Myotonia is a feature of four principal clinical syndromes which have been classified myotonia congenita (Thomsens disease), myotonia atrophica (Steinerts disease), paramyotonia and myotonia and myotonia acquisita by Walton et al. We report here a forty six years old male of myotonia atrophica (Dystrophia myotonia, Steinerts disease) who presents bilateral cataracts, frontal baldness, gonadal atrophy, facial myopathy, sterno-cleidomastoid muscle atrophy and a progressive generalized myopathy of peripheral distribution in the limbs.

PURPOSE: This descriptive - comparison study was conducted to investigate the critical thinking disposition of senior nursing students in three types of nursing program. Method: The convenience sample was drawn from 472 nursing students. Instruments used in this study were made California Critical Thinking Disposition Inventory developed by Facione & Facione (1994). Data was analyzed by descriptive statistics, t-test, ANOVA, Scheffe test with SAS 6.12 program. RESULTS: 1. The total mean score of critical thinking disposition was 279.06. Inquisitiveness was the highest with an mean score 45.50, while systematicity was the lowest with mean score of 36.15. There was a statistically significant difference in critical thinking disposition(F=14.36, p=.00)according to senior nursing students in three types of nursing programs. 2. There was a statistically significant difference in critical thinking disposition(F=5.53, p=.00) according to age, marital status(t=-3.15, p=.00), religion(F=3.26, p=.02) and critical thinking course taken previously(t=3.10, p=.00). CONCLUSION: There was a statistically significant difference in critical thinking disposition(F=14.36, p=.00) according to senior nursing students in three types of nursing program. 9.The Endocrine Manifestations and Growth of the Patients with 22q11.2 Microdeletion Syndrome.

PURPOSE:Varying clinical phenotypes are associated with the chromosome 22q11.2 microdeletion syndrome. The endocrine manifestation are latent or overt hypoparathyroidism, thyroid dysfunction and short stature. This study was undertaken to investigate frequencies of endocrine abnormalities and short stature in patients with the chromosome 22q11.2 microdeletion syndrome. METHODS:Forty three unrelated patients were diagnosed having chromosome 22q11.2 microdeletion syndrome. Chromosomal microdeletion was confirmed by fluorescent in situ hybridation (FISH) with DNA probe (22q11.2 LSI TUPLE1 from Vysis). Serum total calcium and intact parathyroid hormone (PTH) were measured in all patients. Thyroid function tests including free thyroxine(T4), thyroid stimulating hormone (TSH) and thyroid autoantibodies were performed in all patients. Insulin-like growth factor-1 (IGF-1) was measured in 10 patients. Height, weight and body mass index were compared with chronological age in all patients. RESULTS:Seven patients (16%) had an overt hypoparathyroidism, presenting with hypocalcemic tetany. Thirteen patients (31%) showing hypocalcemia with normal PTH were regarded as having latent hypoparathyroidism since their PTH secretion response was blunted. Out of 2 patients with thyroid diseases, one patient had Graves disease and the other had Hashimoto thyroiditis. Five patients (12%) were below the 3rd percentile in height at evaluation. The BMI was below the 5th percentile in 23% of patients. CONCLUSION: Twenty patients (47%) presented with overt and latent hypoparathyroidism. Interestingly, autoimmune thyroid diseases such as Graves disease and Hashimoto thyroiditis were associated in patients with chromosome 22q11.2 microdeletion, indicating predisposition to autoimmune disorders. Therefore, a careful endocrine and growth evaluation is needed in these patients.
