1.Porokeratotic Eccrine Ostial and Dermal Duct Nevus.

A 6-year-old boy with porokeratotic eccrine ostial and dermal duct nevus on the left sole is reported. The patient was born with keratotic plugs, each measuring 1-2 mm in diameter, on the left sole. Light-microscopic examination revealed epidermal invagination and cornoid lamella, which was found to be connected with hyperplastic intraepidermal and dermal eccrine duct. The present case can be histologically differentiated from nevus comedonicus and punctate porokeratotis.

3.A Case of Angiosarcoma of the Face and Scalp.

We report a case of angiosarcoma of the face and scalp in a 76-year-old man. Immunochemical staining with factor 8-related antigen and ultrastructural find ings confirm that this tumor was origined from endothelium. The patient had a good initial response to treatment with radiotherapy.

4.A Case of Subacute Cutaneous Lupus Erythematosus.

Subacute cutaneous lupus erythematosus is a recently recognized distinct subset of lupus erythematosus, having characteristics of clinical, pathologic, serologic, irnrnunologic features. We present a case of subacute cutaneous lupus erythematosus in a 29-year-old woman with symmetric, superficial, nonscarring papulosquamous lesion on the face, scalp, and the extensor surface of the upper extremities. She has a mild systemic illness of musculoskeletal complaints. Serologic abnormalities of the patients are positive rheumatoid factor and anti-Ro antibody test, but fluorescent antinuclear antibody test is negative, which is done by the conventional methods uaing rodent tissue for the substrate. The histopathologic finding of the lesion show the sarne general pattern of the cutaneous lupus erythematosus, and lupus band test is positive.

8.The Characteristics of the Contralateral Inguinal Internal Ring and Hernia Sac by Transinguinal Laparoscopy in Boys with Unilateral Indirect Inguinal Hernia: Analysis of Risk Factors and Associated Features with Metachronous Development.

PURPOSE: We evaluated the contralateral patent processus vaginalis(CPPV) by transinguinal laparoscopy in boys with unilateral inguinal hernia, to determine its incidence and the association with various clinical parameters. We also investigated the factors responsible for the development of a metachronous inguinal hernia. MATERIALS AND METHODS: One hundred sixty seven boys with unilateral inguinal hernia were analyzed. The morphology of the ring was classified into four patterns(type 1, 2a, 2b, and 3). We analyzed the morphology of the internal ring and the variables related to a CPPV by age, laterality and the size of the ipsilateral and/or bilateral hernia sac. RESULTS: The incidence of a CPPV was 33.9%. The positive predictive value was 94.9%(56/59). There was no statistical difference in the incidence of a CPPV by laterality. The incidence of a CPPV was significantly higher in the group with a larger size(>or=1cm) compared to the smaller abnormalities(<1cm). The incidence of the type 3 pattern was higher than the type 2b pattern in the group with larger abnormalities. With increasing age, type 1 and 2a patterns increased in frequency, but type 2b decreased and the type 3 pattern was stable. The size of the contralateral hernia sac was significantly larger with the type 3 pattern than the type 2b. CONCLUSIONS: The results of our study showed that the size of the hernia sac and patient age were related to the presence of a CPPV. A type 3 pattern was associated with a more primitive morphology, a wider hernia sac and no change in the incidence with advancing age. Therefore, the findings of this study suggest that a metachronous inguinal hernia develops more commonly in patients with a type 3 pattern internal ring.

9.Pseudomyxoma Peritonei A case of long term survival after operation.

Pseudomyxoma peritonei may result from implantation of benign or malignant tumor in peritoneal cavity and is filled with gelatinous material (termed "Jelly Belly") in abdominal cavity. Its origin is usually an appendiceal or ovarian mucinous adenoma or cystadenocarcinoma, but other primary origin such as uterus, intestine, pancreas and stomach umor have been reported. Generally, pseudomyxoma peritonei is slowly progressive and has low grade malignant potential. This report presents a unusual long term survival after evacuation of 15,000 cc of gelatinous material from abdominal cavity which was the low grade mucinous adenocarcinoma and a review of the current literature, management and new its concept. The origin of pseudomyxoma peritonei of this case was most likely from appendiceal mvcinous adenocarcinoma.

10.Two cases of peritoneal tuberculosis mimicking ovarian carcinoma diagnosed with laparoscopy.

Peritoneal tuberculosis is a rare presentation of tuberculosis. The presenting signs and symptoms, imaging examinations and CA-125 status in peritoneal tuberculosis sometimes resemble that of ovarian cancer. Thus, the possibility of peritoneal tuberculosis should be considered in the differential diagnosis of ovarian carcinoma. We report two cases of peritoneal tuberculosis initially suspected as ovarian carcinoma but diagnosed as peritoneal tuberculosis with laparoscopy. Using diagnostic laparoscopy unnecessary laparotomy was avoided. We suggest that diagnostic laparoscopy can be helpful in differential diagnosis between peritoneal tuberculosis and ovarian carcinoma.
