1.Influence of Intravitreal Bevacizumab Injection on Formation of Collateral Vessels in Branch Retinal Vein Occlusion.

PURPOSE: To evaluate the influence of retinal collateral vessels in eyes with macular edema secondary to branch retinal vein occlusion (BRVO) treated with intravitreal bevacizumab (IVB). METHODS: We reviewed the medical records of patients with BRVO who were followed up for 12 months. To compare formations of collateral vessels, the patients were divided into 2 groups. The treatment group included 20 patients (20 eyes) treated with IVB, and the control group included 41 patients (41 eyes) without treatment. RESULTS: In the treatment group, the mean age was 58.4 ± 9.5 years. The average number of IVB injections performed during the 12 months was 4.5 ± 2.5 (range 2 to 8). After 12 months from diagnosis, 13 eyes (65%) presented with collateral vessels. In the control group, the mean age was 60.6 ± 9.3 years and 28 eyes (68.3%) presented with collateral vessel after 12 months. There was no difference in incidence of collateral vessel formation between the treatment group and the control group (p = 0.574). CONCLUSIONS: IVB does not influence the formation of retinal collateralization in eyes treated for macular edema secondary to BRVO.

4.The Neuroprotective Effect of Intravitreal Melatonin Injection in Pressure-induced Retinal Ischemia.

PURPOSE: The authors sought to determine the neuroprotective effect of melatonin in a model of ischemic injury in rabbit retina. METHODS: Ischemia was induced by high intraocualr pressure. A dose of 100 microgram of melatonin or dimethyl sulfoxide(DMSO) alone was injected intravitreally just after the induction of ischemia. After 7 and 14 days, the neuroprotective effect of melatonin on ischemic retina was examined with light microscope and transmission electron microscope. RESULTS: The authors found reduction of cytoplasm of retinal ganglion cell(RGC), vacuole formation, chromatin condensation and rupture of nuclear membrane in ischemia-injured eyes treated with DMSO alone. But in melatonin treated eyes, we found that RGC layer's thickness and number of RGC reduced and destruction of cytoplasmic organells and nuclear damage were minimal. The partial recovery of wave is noted in melatonin-treated eyes after ischemia induction. CONCLUSIONS: The melatonin(100 microgram) protected the rabbit retina from high intraocular pressure-induced ischemic injury when administered intravitreally. Melatonin may be useful to decrease neuronal damage in the retina as a result of ischemic injury. But further investigations are neccesary to decide effective concentration, route and time of administration.

One hundred seven isolates of Staphylococcus aureus from bovine mastitis were investigated for colony morphology in serum-soft agar (SSA), autoagglutination in salt, and capsular serotype. Capsular polysaccharide (CP) was purified and quantified from the extracts of clinical isolates. Overall, 89 isolates (83.2%) were diffuse in the SSA, without any difference in the proportion of diffuse colony between type 5 and type 8 strains. Some strains exhibited compact colonies in the SSA and expressed CP as determined by an enzyme-linked immunosorbent assay, indicating that compact morphology does not exclude encapsulation. The majority of the strains (11/12) showed autoagglutination in the salt aggregation test. The serotype 336 accounted for 46.7% of the isolates followed by serotype 5 (12.1%) and serotype 8 (12.1%). Particularly, twenty-six (24.3%) isolates reacted with two serotypes; 7 for type 8/336 and 19 for type 5/336. Five isolates (4.7%) were nontypeable with monoclonal antibodies specific for CP serotype 5, 8, or 336. The CP concentration in culture supernatants varied with the serotypes, and the total amount of CP produced by cells grown in a liquid medium was much less than that produced by cells grown on a solid medium. The association of varicose vein, soft tissue and bony hypertrophy and cutaneous hemangi-oma of the port-wine variety confined to one extremity was first reported in 1900 by Klippel and Trenaunay, and then Weber. A case of Klippel-Trenaunay-Weber Syndrome with macrodactylia in a 2 months old male infant(dizygotic) is presented who was asmitted to B.N.U.H. with chief complaints of nevus fla-mmeus, swelling of the left lower extremity.

7.Factors Affecting Health-Related Quality of Life in Persons with Stroke Using SF-6D.

BACKGROUND: The SF-6D, derived from the SF-36, is a preference-based instrument measuring health-related quality of life (HRQOL). Using this tool, we measured the HRQOL of persons disabled by stroke dwelling in Gyeongju city and identified factors affecting the HRQOL. METHODS: As of March 2008, 991 persons who had had a stroke and were aged 50 and over were identified on the Gyeongju city disabled registry. Interviewers specifically trained for this face-to-face survey gathered pertinent information including general and clinical characteristics from June to October 2008. In addition, the health status was measured using the Modified Rankin Scale, the Barthel ADL index, and the SF-36. The SF-6D index was derived from the SF-36. We analyzed the known-group construct validity and convergent validity. Finally, we performed multiple regression analyses to identify factors affecting the HRQOL. RESULTS: Of the 991 potential subjects, 498 persons participated in the interviews (response rate: 50.3%). The SF-6D indices for females and those aged 70 and over were lower than indices for males and those aged 60 and less. Correlation coefficients between the Modified Rankin Scale, the Barthel index, and the SF-6D index were relatively high (-0.533 and 0.555, respectively) and statistically significant (P<0.001). Multiple regression analyses showed that sex, age, smoking, alcohol use, comorbidity, the Modified Rankin Scale, and the Barthel index had an effect on the SF-6D index of persons with stroke, and the Barthel index was the most important factor. CONCLUSIONS: Our results indicate that the SF-6D can be a valid tool for evaluating health states of persons disabled by stroke, and functional limitation was the main factor affecting the HRQOL.

8.A Case of Multiple Complications in Herpes Zoster Ophthalmicus.

PURPOSE: We report a case of stromal keratitis, corneal infiltration, anterior uveitis, central retinal artery occlusion and optic neuropathy in a patient with herpes zoster ophthalmicus. CASE SUMMARY: A 73-year-old man who was hospitalized for pain and vesicles on his left face was referred to our clinic with sudden onset visual disturbance in his left eye. His best corrected visual acuity in the right eye was 0.8 and light-perception in his left eye. Relative afferent pupillary defect was found in his left eye. Slit-lamp examination showed anterior uveitis secondary to herpes zoster ophthalmicus presented with stromal keratitis. Fundus examination showed retinal hemorrhage, vitreous opacity, cherry-red spot in the fovea and optic disc swelling. Delayed arterial filling and arteriovenous transit time were observed on fluorescence angiography. He was treated with topical antiviral and steroid eye drops for stromal keratitis and anterior uveitis. He was also treated systemically with an intravenous antiviral agent and oral steroid, but visual acuity did not improve. CONCLUSIONS: Stromal keratitis, corneal opacity, anterior uveitis, central retinal artery occlusion and optic neuropathy can be complications of herpes zoster ophthalmicus.

9.Maxillary sinus floor augmentation with anorganic bovine bone: Histologic evaluation in humans.

PURPOSE: The aim of this report is to investigate the efficacy of anorganic bovine bone xenograft(Bio-Oss(R) ) at maxillary sinus floor augmentation. MATERIALS AND METHODS: Two male patients who missed maxillary posterior teeth were included. They were performed maxillary sinus floor augmentation using anorganic bovine bone xenograft(Bio-Oss(R) ). After 10 or 13 months, the regenerated tissues were harvested using trephine drills with 2 or 4mm diameter and non-decalcified specimens were made. The specimens were examined histologically and histomorphometrically to investigate graft resorption and new bone formation. RESULTS: Newly formed bone was in contact with Bio-Oss(R) particles directly without any gap between the bone and the particles. The proportions of newly formed bone were 23.4~25.3% in patient 1(Pt.1) and 28.8% in patient 2(Pt.2). And the proportions of remained Bio-Oss(R) were 29.7~30.2% in Pt.1 and 29.2% in Pt.2. The fixtures installed at augmented area showed good stability and the augmented bone height was maintained well. CONCLUSION: Anorganic bovine bone xenograft(Bio-Oss(R) ) has high osteoconductivity and helps new bone formation, so that it can be used in maxillary sinus floor augmentation.
