1.Clinical Evaluation of 103 Cases of Adenocarcinoma of the Uterine Cervix.

OBJECTIVE: The purpose of this study was to investigate clinical charactoristics of adenocarcinoma of the uterine cervix. STUDY METHODS: This study is a retrospective analysis of 103 patients with adenocarcinoma of cervix treated at the Kosin Medical center forom 1984 to 1993. RESULTS: The distribution by stage of disease included FIGO stage I, 51(49.6%); stage II, 32(31%); Stage III, 17(16.5%);Stage IV, 3(2.9%). The 2-year survival results for patients with stage I was 93.5%; stage II was 72.4%;stage III was 46.2% and stage IV was none survived. And 4 year survival results,'83.3/o, 591% and 33.3/o in stage I, II and III, reqxetively. The 4-year survivel rate varied significantly according to tumor size and lymph nade metastasis status-below 2cm 84.2%, 2cm~4cm 75%, above 4cm 0%: (stage I);negative node 91.3%, positive node 40% (stage I). The results of resurrent rate according to clinical stage was 11.8%, 25.8%, 84.6% and 100 % in stage I, ll, III and lV, respectively. CONCLUSION: Treatment strategies should be based on the stage of disease, tumor size, lymph node metastasis status and recurrent.

2.Neuromuscular monitoring of a patient with Charcot-Marie-Tooth disease; which monitoring technique is adequate? - A case report and literature review -

Background:
Charcot-Marie-Tooth disease (CMTD) is a hereditary polyneuropathy associated with a life-threatening risk of pulmonary complications.Case: A 61-year-old male with CMTD for 40 years was admitted for the drainage of an abscess in his left ankle. Total intravenous anesthesia was administered, and an electromyography device was attached to the hand for neuromuscular monitoring; however, the response was not measured. Kinemyography and acceleromyography devices were attached to both hands, and responses were obtained. After neuromuscular blockade (NMB) with rocuronium 0.6 mg/kg, the train-of-four (TOF) response on kinemyography was normally measured, but the post-tetanic count on acceleromyography consistently showed 0 during anesthesia. Sugammadex 200 mg was injected to reverse the NMB. After 5 min, the TOF ratios for kinemyography and acceleromyography exceeded 90%. The patient recovered without any complications.
Conclusions
For CMTD patients, acceleromyography or kinemyography is superior to electromyography, and sugammadex can be used to reverse NMB successfully.
3.A case of Wiskott-Aldrich syndrome.

Wiskott-Aldrich syndrome is an X-linked combined immunodeficiency disorder characterized by severely decreased number of platelets which are small in size, eczema resembling atopic dermatitis and recurrent infection. The serum of the patient contains elevated concentrations of IgA and IgE, whereas the IgG level is usually normal and IgM level is decreased. The patients also shows skin test anergy and progressive T-lymphocytopenia. Bleedings and recurrent infections are the main causes of death and the patients usually die before age 10. Bone marrow transplantation is accepted to be the only radical therapy. We experienced a case compatible with Wiskott-Aldrich syndrome in a 5 year old male child who accompanied above clinical manifestations and laboratory findings.

5.Effects of Rocuronium on Smooth Muscle Tension of Isolated Rat Thoracic Aorta.

BACKGROUND: The cardiovascular effects of muscle relaxants may be produced by a muscarinic receptor block, ganglion block, increased noradrenaline release and blockage of it's reuptake or histamine liberation. However certain analogues of steroidal muscle relaxants directly cause relaxation of isolated vascular smooth muscles. Rocuronium is the 2-morpholino, 3-desacetyl, 16-N-allyl-pyrrolidino derivative of vecuronium, known to have a relative lack of ganglion blocking, sympathomimetic effects or histamine release. The aim of this study was to investigate the effect and action mechanism of rocuronium on isolated aortic smooth muscle in rats. METHODS: The ability of rocuronium to elicit a direct relaxant effect on vascular smooth muscle has been studied using isolated rat thoracic aortic rings contracted with phenylephrine (PE). Each ring of aortic arteries was suspended on wire supports in a 20 ml tissue bath under 2 g of resting tension. All tissues were bathed in Tris Tyrode solution at 37degrees C and 100% oxygen was supplied. RESULTS: Rocuronium 10(-4) M and 10(-2) M shifted the cumulative concentration-effect curves of PE to the right significantly (P<0.05). This relaxation effect of the aortic rings was not reversed with L-NAME or methylene blue pretreatment. However indomethacine shifted this curve to the right, and intracellular calcium release and extracellular calcium influx decreased significantly (P<0.05). CONCLUSIONS: From the results obtained with this experiment, it is concluded that the relaxation effects of rocuronium is not endothelium dependent and in part, is related with cyclooxygenase inhibition. Rocuronium inhibited intracelluar calcium release and extracelluar calcium influx.

6.Correlation of the Immunohistochemical Coexpression of p53 and HER-2/neu and the Prognosis of Breast Cancer.

PURPOSE: Among the many biological parameters of breast cancer, the p53 and HER-2/neu genes, which are located on the chromosome 17, and their gene products have been studied extensively by many researchers in regard their relation to the prognosis. The aim of this study was to evaluate the prognostic significance of the coexpression of p53 and HER-2/neu in breast cancer. METHODS: Formalin-fixed, paraffin-embedded tissues from 126 patients with invasive breast cancer were immunostained for p53 and HER-2/neu. Other prognostic parameters such as the patients' age, tumor size, lymph node metastasis, histologic grade, the presence of estrogen receptor (ER), and progesterone receptor (PgR) were evaluated. The patients' outcome was analyzed according to the expression of p53 and HER-2/neu as well as to the other prognostic factors. RESULTS: Among the 126 patients, p53 and HER-2/neu were positive in 46 (36.5%) and 33 (26.2%) patients. According to the expression of p53 and HER-2/neu, the patients were classified into three groups; group 1: negative expression for both factors (n = 63), and group 2: positive expression for either one of the factors (n = 47), and group 3: positive expression for both factors (n = 16). The median follow-up period was 50+/-11.1 months. On the Kaplan-Meier analysis, p53 and HER-2/neu were both related to the 5-year disease free survival rate (DFSR). Group 3 showed a significantly lower 5-year DFSR than did group 2 (66.2% vs. 85.2%, respectively, p = 0.05) and the group 1 (66.2% vs. 94.0%, respectively, p = 0.01). The multivariate analysis indicated that lymph node metastasis, ER, PgR, p53 and HER-2/neu expressions were significantly related to the patients'5-year DFSR. CONCLUSION: The expression of p53 and HER-2/neu was related to the 5-year DFSR of breast cancer, and the coexpression of p53 and HER-2/neu was found to have a more significant prognostic value than either one of these two parameters alone.

7.A case of unilateral megalencephaly.

The unilateral megalencephaly is a rare brain malformation characterized by cerebral asymmetry and cortical dysplasia caused by faulty migration of the subependymal neuroblasts. We experienced a case of unilateral megalencephaly in a two day-old male with the chief complaint of asymmetric head appearance. Large left hemisphere with agyria, pachygyria, dilatation of lateral ventricle, and the thick cortex of the ipsilateral hemisphere were showed in brain MRL, Clinical findings in this case were intractable seizure, hemiparesis, and psychomotor retardation. A review of literatures was also presented briefly.

8.The Relationships of Chewing Frequency, Eating Rates, and Food Consumption with the Body Mass Index and Gender of College Students.

In this study, we examined chewing frequency, eating rates, and food consumption volume according to the body mass index of male and female college students. Twenty-five male college students were divided into a normal weight male group (<23 body mass index, n=14) and an obese male group (> or =23 body mass index, n=11). Twenty-five female college students were divided into a normal weight female group (<23 body mass index, n=21) and an obese female group (> or =23 body mass index, n=4). Subjects were provided with abundant fried rice and allowed to consume it at their own pace. Chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite were measured. There were no significant differences in chewing frequency, total mealtime, rate of eating, consumption volume, calorie intake, and the amount of one bite between the normal and obese male groups. There were also no significant differences in chewing frequency, consumption volume, calorie intake, and the amount of one bite between the normal and obese female groups. However, the obese female group had a significantly shorter mealtime and significantly higher eating rate compared with the normal group. Thus, our study suggests that females who eat a meal for a longer time may reduce their food intake, consequently inducing body weight loss.

9.Delayed Vasospasm after Aneurysmal Subarachnoid Hemorrhage in Behcet Syndrome.

A man visited the emergency room with a headache. Brain computed tomography showed aneurysmal subarachnoid hemorrhage (SAH) and multiple aneurysms. After aneurysm clipping surgery, the patient was discharged. After 5 days, he was admitted to the hospital with skin ulceration and was diagnosed with Behcet syndrome. An angiogram taken 7 weeks after aneurysmal SAH showed intracranial vasospasm. Because inflammation in Behcet syndrome may aggravate intracranial vasospasm, intracranial vasospasm after aneurysmal SAH in Behcet syndrome should be monitored for longer compared to general aneurysmal SAH.
