4.Fracture of the Tivial Shaft

1. Two hundred and fifty cases of fractures of the tibia treated at the orthopedic department of the Catholic Medical College during period of January, 1966 to September, 1971 were subjected to analysis. 2. The evaluation was done according to the classification of Ellis. Among 250 cases of the tibial fractures 120 being mild grade, 63 moderate grade and 67 severe grade. 3. Of 6 groups, 1st group was treated with toe to groin cast, 2nd group was treated with functional below the knee cast, 3rd group was treated with pin & plaster, 4th group was treated with 1–2 screw fixation & wiring, 5th group was treated with plate fixation and 6th group was treated with intramedullary nailing. 4. The healing time is almost proportional to the severity of the fracture grade and also the better prognosis largely depends on the mild grade. 5. We experienced 9 cases of non-union (3.6%), 19 cases of delayed union (7.6%) in most of moderate and severe grade.

5.Vascular Injury of Lower Extremity

Blunt trauma severe enough to cause fracture of the long bones of the lower extremity may seriously damage adjacent arteries either by direct or indirect force. Diagnosis of disruption or thrombosis by this combination of injuries, is frequently delayed until the opportunity to salvage the extremity is lost. This error will result in an excessively high amputation rate. From September 1971 to August 1972, six Cases with or without fracture were treated at the Orthopedic Department of the Catholic Medical College. Of these six, three resulted in amputation by losing the chance to make the early diagnosis of injury.

6.Retrovirus-Mediated Herpes Simplex Virus Thymidine Kinase Gene Therapy for the Prevention of Stenosis in Rat Carotid Artery Injury Model.

BACKGROUND: Herpes simplex virus thymidine kinase (HSVtk) phosphorylates the prodrug ganciclovir to a nucleoside analog that inhibits DNA synthesis, causing cell death. Neighbouring nontransfected cells may be affected through a 'bystander effect', thereby amplifying the antiproliferative actions. This study was carried out to determine whether retrovirus-mediated HSVtk gene therapy could reduce intimal hyperplasia and prevent stenosis following balloon injury of the rat carotid artery. METHODS: A replication-defective recombinant retroviral vector containing HSVtk cDNA (LtkSN) was constructed. Cultured primary rat smooth muscle cells (SMCs) infected with this vector (SMC/LtkSN) were transplanted to the balloon injured rat right carotid artery. One week after transplantation, HSVtk gene therapy group was administered a 2-week treatment of ganciclovir (30 mg/kg/d). Three weeks after balloon injury and SMC/LtkSN transplantation, carotid arteriography was performed and carotid arteries were perfusion-fixed for histologic examination. RESULTS: Carotid arteriographic evaluation comparing with the uninjured left carotid artery showed that the mean luminal diameter of HSVtk gene therapy group (n=5, 85+/-3%) was significantly larger than that of balloon injury only group (n=5, 65+/-5%). The neointimal mass of HSVtk gene therapy group was less than that of balloon injury only group. SMC/LtkSN transplantation without ganciclovir treatment group (n=3) showed asymmetric intimal proliferation probably because of gravitational pooling of seeding. There were inflammatory cell infiltrations at the gravity dependent portion of HSVtk gene therapy group. CONCLUSION: Retrovirus-mediated HSVtk gene therapy following balloon injury of the rat carotid artery reduced neointimal expansion and arteriographic stenosis.

9.Clinico-Radiologic Findings of Entrapped Inferior Oblique Muscle in a Fracture of the Orbital Floor.

A 51-year old man presented with vertical and torsional diplopia after reduction of a blowout fracture at another hospital one year ago. He had no anormalies of head position and 14 prism diopters (PD) right hypertropia (RHT) in the primary position. In upgaze no vertical deviation was found, and hyperdeviation on downgaze was 35PD. Bielschowsky head tilt test showed a negative response. Distinct superior oblique (SO) and inferior rectus (IR) underaction of the right eye was noted but IO overaction was mild on the ocular version test. Double Maddox rod test (DMRT) revealed 10-degree extorsion, but fundus extorsion was minimal in the right eye.Thin-section coronal CT scan showed that there was no fracture line on the anterior orbital floor, but a fracture remained on the posterior orbital floor. Also, the anterior part of the right inferior oblique muscle was vertically reoriented and the medial portion of the inferior oblique muscle was not traced on the coronal CT scan. The patient underwent 14 mm right IO recession and 3 mm right IR resection. One month after the surgery, his vertical and torsional diplopia were eliminated in the primary position.
