1.Radiologic Morphology of Proximal Femur: Comparison between Normal and Diseased Hips.

Diseases involving hip may influence on the anatomy of the proximal femur, especially on the endosteal canal shape. The simple radiographic comparative study was performed between normal and diseased hips. For the diseasd hip group, 74 hips ol' femoral head AVN(avascular necrosis) (average age: 49.3 years old, patients were all male) who had limped for average l9 months were selected and 77 male hips (average age: 48.9 years old) who had no prior hip disease were selected for the normal hip group. The external shape of both group showed no significant differences except for the femoral head dimensions. Statistically significant differences were ohserved hetween two groups in the proximal endosteal canal shape and cortical thickness. The femur of the diseased hip group had wider canal from the level of distal to lesser trochanter to the level of isthmus, with a significant reduction in the metaphyseal index (the ratio hetween the canal width proximal to lesser trochanter and distal to lesser trochanter) and the canal flare index (p<0.001). Theoretically this study suggests that the optimal fit and fill to the proximal endostcal canals of normal and diseased hips cannot be obtained by oneshape cementless femoral stcms.

3.Bone Marrow Pressure Study in Ostoenecrosis of the Femoral Head

Authors measured the bone marrow pressure(B.M.P.) of 59 femoral heads in cases of either suspicious or diagnosed osteonecrosis and obtained the significant relationship between increased B.M.P. and the osteonecrosis. Increased bone marrow pressure provided both predictive and early diagnostic importance in even the preclinical stage of evolution of the disease, which subsequently had core biopsy proved osteonecrosis of femoral head. The results obstained were as follows; 1. Roentgenographically apperent osteonecrosis showed abnormal B.M.P. pattern, which were proved by the core biopsy. 2. In 21 cases of preclinical osteonecrosis, which did not show roentgenographic changes of osteonecrosis, measured B.M.P. parterns indicated as early stage of osteonecrosis in core biopsy at all. 3. The incidence of abnormal pressure pattern I was 51% and that of pattern II was 30%. 4. Enneking radiological staging did not correlate with the results of B.M.P. 5. The measurement of B.M.P. is relatively easy and simple method without any remarkable risk. The results of this study had great meaning in emphasizing the contribution of the bone marrow pressure measurement dianosing the preclinical stage of osteonecrosis, which had no roentgenographical changes and diagnostic symptoms.

4.Renal Anomalies in Neonates with Isolated Preauricular Tags.

PURPOSE: The aim of this study was to determine the role of renal ultrasonography in neonates with isolated preauricular tags. METHODS: We performed a retrospective study of 10,997 newborn infants delivered from January 1995 to June 2003 in Wonkwang University Hospital. Nineteen newborns born with isolated preauricular tags were assessed for renal anomalies by performing renal ultrasonography within one week after birth. The study group was compared with a control group of 25 healthy neonates without preauricular tags during the same period, also using renal ultrasonography. RESULTS: Preauricular tags were detected in 19 of 10,997(0.17%) neonates; 57.9% were found on the right side. Renal anomalies in neonates with isolated preauricular tags were detected in four infants (21.1%); this incidence was higher, but was not significant, compared with the control group. Types of anomalies were mild hydronephrosis of grade 1(3 cases) and 2(1 case) which were normalized within one year. CONCLUSION: Routine renal ultrasonography is not recommended for newborn infant with isolated preauricular tags.

5.Pseudomyxoma Peritonei A case of long term survival after operation.

Pseudomyxoma peritonei may result from implantation of benign or malignant tumor in peritoneal cavity and is filled with gelatinous material (termed "Jelly Belly") in abdominal cavity. Its origin is usually an appendiceal or ovarian mucinous adenoma or cystadenocarcinoma, but other primary origin such as uterus, intestine, pancreas and stomach umor have been reported. Generally, pseudomyxoma peritonei is slowly progressive and has low grade malignant potential. This report presents a unusual long term survival after evacuation of 15,000 cc of gelatinous material from abdominal cavity which was the low grade mucinous adenocarcinoma and a review of the current literature, management and new its concept. The origin of pseudomyxoma peritonei of this case was most likely from appendiceal mvcinous adenocarcinoma.

6.Clinical Study of Neonatal Group B beta-hemolytic Streptococcal Meningitis: 10 Years Experience.

OBJECTIVE: Most common organism of bacterial meningitis was E.coli in the past. However, it is changed to group B beta-hemolytic streptococcus (GBS) recently. Incidence of neonatal GBS meningitis is still not reported in Korea, but sporadic cases were reported continuously. So, we investigated the exact incidence of neonatal GBS meningitis for 10 years including other clinical manifestations for it's diagnosis and treatment. METHODS: We analyzed 9 cases with chart review retrospectively who had been admitted to the pediatric ward or NICU in Wonkwang University Hospital from July 1994 to June 2004. They had proven neonatal GBS meningitis with culture or latex agglutination test in CSF. RESULTS: Incidence of disease was 0.79 with 0.09 in early onset and 0.7 in late onset type per 1,000 live births. Sex ratio was not different, and nearly all 8 cases (88.9%) were fullterm and late onset type. The associated obstetric factors were noted in 5 cases (55.6%), and c-section of 3 cases (33.3%) was most common factor. In clinical features, fever was seen in all cases, and lethargy and poor feeding in nearly all cases (88.9%), and irritability (55.6%), bulging fontanelle and convulsion (44.4%) were seen. There were abnormal findings in 7 cases (77.7%) <1.0 in CBC differential ratio and 8 cases (88.9%) >CRP 5 mg/dL in peripheral blood exams on admission. There were abnormal CSF findings which were increased wbc of 5 cases (55.6%, > or =1,000/mm3), neutrophil of 8 cases (88.9%), protein of 8 cases (88.9%, >200 mg/dL) and CRP of 6 cases (66.7%, 3 mg/dL), and decreased sugar of 6 cases (66.7%, <45 mg/dL) and severely decreased of 4 cases (44.4%, <20 mg/dL). There were abnormal brain CT findings in 6 cases (66.7%) such as cerebral infarction, subdural effusion, hydrocephalus, and intracranial hemorrhage. Eight cases (88.9%) were alive, and mean duration of therapy was 26.1+/-9.9 days. CONCLUSION: Although the incidence of GBS is not high, it is reported to be an important organism in neonatal bacterial meningitis. So, we recommend early detection and active treatment for neonatal GBS meningitis to prevent severe complication and prolonged therapy.

7.Histological Evaluation of Bone Ingrowth around Titanium and Hydroxyapatite-coated Titanium Implant in Dogs.

Since the concept of osseointegration has been introduced, a lot of researches were performed to achieve stronger fixation of the implant. In this study, we tried to find the different pattern of bone ingrowth after implanting pure titanium implant and Hydroxyapatite coated titanium implant in dogs. Four typed of cylindrical titanium implants (diameter 6 mm, length 1 mm) were inserted in the distal femur of adult dogs. The histological examination was done at postoperative 2, 4, 8, and 12 weeks. There was no foreign body reaction at contact site and all implants kept direct contact with bone tissue. In the light microscopic finding, after 2 weeks, the cases with Hydroxyapatite-coated Implant (HAP) showed greater formation of immature woven bone in comparison to Titanium implant. After 4 weeks, Hydroxyapatite-coated Implant (HAP) showed more mature lamellar bone. After 8 and 12 weeks, most of the woven bone was transformed into mature lamellar bone. The fluorescence microscopic finding, after 2 weeks, revealed calcein green fluorescence around implants in all cases and it was more prominent in the cases with Hydroxyapatite-coated Implant (HAP). After 4 weeks, the center of trabecula gave out green fluorescent light, the corner of them showed red fluorescent light, and they were more prominent in the cases with Hydroxyapatite-coated Implant (HAP). After 8 and 12 weeks, green fluorescence disappeared gradually, red fluorescence appeared for the most part, and there was no difference in each cases, especially in 12 weeks. As a result, we support the concept that Hydroxyapatite-coated Implant (HAP) induces faster bone ingrowth in 4 weeks than Titanium implant, and that it may be more profitable for early bone ingrowth and implant stability.

8.Intraoperative Blood Salvage and Transfusion During Spinal Surgery .

25 Patients were received major spinal surgeries at main operating theater of Kangnam General Hospital from May 1990 to December 1990 by using the Haemonetics cell saver which was employed as an intraoperative blood salvage and autologous blood transfuser. The products of washed red blood cells were an average of 1490 cc packed RBC per case. Under the Light Microscopy, Washed RBC disclosed the morphologies of normal RBC size and shape. Fragmented RBCs or cell debris were not found and in one case agglutinated and denaturated RBCs were seen. While 20 patients were received spinal surgery prior to the utilization of cell saver, an average of 7.85 units of banked whole blood was transfused compared with 5.55 units in the cases of using the cell saver (p<0.05). Homologous banked blood utilization during spinal surgery declined more than 30% with the use of the cell saver. There was no evidence of infection, renal and/or hepatic failure related to the use of the cell saver. Haemonetics cell saver appeared to be safe, efficient and more over in some instanes life saving.

9.The Relationship between Coping Mechanisms and Psychological Symptoms in Vitiligo Patients.

BACKGROUND: Vitiligo is an acquired disorder which causes body disfiguring and may provoke emotional stress, functional impairment, and psychiatric symptoms. OBJECTIVE: (1) To assess whether the psychiatric symptoms are more frequent in vitiligo patients than control patients. (2) To investigate whether the vitiligo patients with more severe psychiatric symptoms use more negative and passive coping mechanisms than patients with less severe psychiatric symptoms. To carry out this study we evaluated psychiatric symptoms, problems in daily activities, and the relation-ship between coping mechanisms and psychiatric symptoms. METHODS: A clinical study was done on 150 vitiligo patients. Other dermatology patients were used as a control group. Both groups had visited the dermatology clinic of the Severance hospital. Questionaires regarding age, sex, severity of disease, duration of disease, SCL-90(symptom check list 90), the coping list of Weisman, and items for adaptability of daily activity were recorded. Results : Male vitiligo patients showed more psychological symptoms than female patients. Vitiligo patients had many more difficulties in daily activities than the control patients. The patients who had more severe psychiatric symptoms used more negative and passive coping mechanisms than less severe patients. Conclusion : Vitiligo provoked psychiatric symptoms and difficulties in daily activities. Psychiatrically more severe patients used negative and passive coping mechanisms. Comprehensive dermatologic and psychiatric treatment may be required in the treatment of vitiligo.

10.The Effect of Aging on the Pulmonary Function of the Healthy Adults.

Pulmonary function tests were conducted in 60 male adults and 60 female adults at 20~79 years of age. They showed noevidenee of pulmonary disease on clinical symptoms, X-ray finidngs and past history. We wanted to evaluate the deterioration of pulmonary functional reserve in the aged. The results were as follows: 1) In males, FVC decreased significantly (p <0.01) from 4440+/-271.38 ml in 20~29 year-old subjects to 3090+/-445.75 ml 70~79 year-old subjects. The correlation coefficient with age was-0.6846. 2) In females, FVC decreased significantly (p<0.01) from 2097+/-479.42ml in 20~29 year-old subjects to 2142+/-233.13ml in 70~79 year-old subjects The correlation coefficient woth age was 0.6454. 3) In males, FEV, decreased significantly (p<0.01) from 4005+/-268.26 ml in 20~29 year-old subjects to 2373+/-326.36ml in 10~79 year-old subjects. The correlation coefficient with age was-0.8229. 4) In females, FEV1 decreased significantly (p<0.01) from 2586+/-519.00 ml in 20~29 yrar-old subjects to 1645+/-330.36ml in 70~79 year old subjects The correlation coefficient with age was 0.7013. 5) In males, FEV1/FVC decreased significantly (p<0.01) from 90.17+/-4.90% in 20~29 year-old subjects to 76.72+/-8.13% in 70~79 year-old subjects. The correlation coefficient with age was-0.5595. 6) In females, FEV1/FVC decreased significantly (p<0.01) from 88.66+/-6.58% in 20-29 year-old subjects to 76.80+/-9.32% in 70~79 year-old subjects. The correlation coefficient with age wae-0.4489.
