1.Extracellular Matrix and Astrocytic Response during Regeneration following Cryogenic Injury in Adult Rat Cerebral Cortex.

Astrocytes are the most numerous cellular elements in the cerebrum, and they normally have a very slow turnover rate. But during regeneration after injury, they proliferate markedly resulting in astrogliosis. The extracellular matrix in the central nervous system is present in the vessel walls and in the external glia limitans as a basal lamina. The presence of an intact extracellular matrix framework is important in regeneration after injury. Understanding the properties of astrocytic proliferation will be helpful to find out new treatment for functional recovery in the central nervous system. In this study, after cryogenic injury was performed on the cerebral cortex in rats, changes in astrocytes and the extracellular matrix were observed using light microscopy, immunohistochemical stain for glial fibrillary acidic protein(GFAP), proliferating cell nuclear antigen(PCNA), fibronectin, laminin, and type IV collagen, autoradiography and electron microscopy. The results were as follows; 1) The coagulative necrosis, which followed cryogenic injury on the cerebral cortex was healed, forming a new pia mater above the lesion. 2) Some of the PCNA positive cells were astrocytes and some of the GFAP positive cells showed a positive reaction to PCNA. 3) Proliferating astrocytes labelled by autoradiography or immunohistochemical stain for PCNA reached maximal numbers 3days after the injury and they were no longer found 2 weeks after injury. 4) In autoradiography with immunohistochemical stain for GFAP, about 1% of GFAP positive astrocytes were labelled by autoradiography and in double immunohistochemical stain for PCNA and GFAP, about 8-16% of GFAP positive astrocytes were also stained by PCNA. 5) In immunohistochemical stain for fibronectin, laminin and type IV collagen, laminin and type IV collagen were present in the newly formed blood vessel walls and fibronectin showed a diffuse positive reaction within the lesion. The new pia mater was formed within 2 weeks after the injury. 6) On electron microscopic examination, basal lamina material was found in the vessel wall 1 week after the injury and at 2 weeks, a nearly complete and continuous basal lamina was formed although the thickness was uneven. According to these findings, astrocytes in the cerebral cortex of adult rats proliferate very early in the regenerative period after cryogenic injury. At 2 weeks after the injury, this regeneration ceases and the damaged basal lamina of pia mater and vessel wall were reconstituted.

2.Association of Herpes Zoster and Lymphosarcoma: Report of one Case.

Herpes zoster is generally believed to be caused by the activation of varicella-zoster virus present in the body since an original infection with varicella. According to the thesis, the virus remairis in a latent state in the cells of the sensory ganglia until immunity has waned sufficiently to pezmit multiplication of the virus and clinical infection. There are a number of reports that in patient with malignant disease, especially Jymphoma, a frequenry af association of zoster is greater and severity of its symptom is increased to be marked contrast to the benign uneventful course of zoster seen in healthy person. The author observed one case of zoster seen in patient with malignant disorder, and frequency of association of zoster in malignant disease and its etiological factors are reviewed in the literature.

3.Democidiosis in Rosacea Type.

The authors present seven cases of Democidiosis in rosacea type and the subject of human demodetic infestation has been reviewed. According to our experience, demodex folliculorum, although a normal inhabitant of the sebaceous glands in small numbers, when encouraged to multiply by the patients excessive use of facial cream and avoidance of soap, can produce democidiosis. The diagnosis was easily confirmed by characteristic clinical appearance and simple microscopy. The characteristie clinical appearances are indurated non-comedous papules and pustules with unique cherry pinkish red erythema. The response to treatment, consisting of daily soap and water cleansing of the face, followed by the local application of a suitable antiparasitic remedies is simple and gratifying. But the erythema was not respond to antiparasitic preparation.

4.Differences of Spectral EEG Analysis and Prognosis Following Single Hemispheric Infarction and Hemorrhage in Striatocapsular Area.

BACKGROUND: Vascular dementia is common cause of dementia, second to the dementia of Alzheimer desease. However in Asia and many developing countries, the incidence of vascular dementia exceeds that of Alzheimer's disease. Though many stroke-related factors related the nature of vascular injury, e.g. infarction and hemorrhage, have not assessed yet. Clarifying the difference of electroencephalograpy and clinical prognosis between infarction and hemorrhage, the aim of this study was to elucidate the role of nature of vascular injury. METHODS: to reduce confounding factors, the study population was restricted to the patients of single hemispheric striatocapsular infarction and hemorrhage saving cortex. On admission, we checked the KMMSE and FIM scores and using quantified EEG, we analyzed occipital peak frequency and the relative background alpha, theta and delta spectra power taken from 16 derivations by averaging twenty-2 -sec epoch in infarction, hemorrhage patients and elderly controls. After 6 months follow up, we compare the MMSE, FIM score between infarction and hemorrhage group. RESULTS: 1) Compared with infarction group, hemorrhage groups had a significantly bilateral lower occipital peak freqauency and background bilateral alpha spectra power. 2) In hemorrhage group, there is lower tendency in K-MMSE after 6 month follow up compared to infarction group. CONCLUSION: This study suggests that hemorrhage show more bilateral electrophysiological dysfunction than infarction group and possible grave prognosis for vascular dementia compared to infarction group.

5.The Value of Immunohistochemistry on Paraffin Embedded Tissue Sections in the Differentiation of Subgroups of Low Grade B-Cell Lymphomas.

There had been a continuous evolution of lymphoma classification and recently a Revised European-American Lymphoma Classification was proposed by the International Lymphoma Study Group. This new classification often requires information on immunophenotypic and molecular biologic markers in addition to the usual histologic findings. Recent advances in the production of commercially available monoclonal antibodies reactive on formalin-fixed paraffin-embedded tissues provide us a great help to classify the non-Hodgkin's lymphoma. We have analyzed 31 low grade B-cell lymphomas by the schemes proposed by the International Lymphoma Study Group using antibodies to CD3, CD5, CD20, CD23, CD43, cyclin D1, and bcl-2 protein, and have analyzed the immunophenotypic features. Among 31 low grade B-cell lymphomas, 8 small lymphocytic lymphomas, 5 mantle cell lymphomas, 7 follicle center lymphomas (2 grade I, 3 grade II, and 2 grade III), and 11 marginal zone B-cell lymphomas (all of which were extranodal) were identified. Among 8 small lymphocytic lymphomas, 5 cases were positive for CD5; 6 cases were positive for CD23; 7 cases were positive for CD43; all 8 cases were negative for cyclin D1; and 7 cases were positive for bcl-2. Among 5 mantle cell lymphomas, 4 cases were positive for CD5 and CD43; all five cases were negative for CD23; 4 cases were positive for cyclin D1 and bcl-2. All 7 follicle center lymphomas were negative for CD5, CD43 and cyclin D1 and 2 cases were positive for CD23; and 6 cases were positive for bcl-2. All marginal zone B-cell lymphomas were negative for CD5, CD23 and cyclin D1; 3 cases were positive for CD43 and 9 cases were positive for bcl-2. Diagnostic utility for CD5 antigen detection on paraffin embedded tissue has a limitation due to weak antigen expression in tumor cells of B-cell lymphomas; however, still be useful in differentiating small lymphocytic lymphoma and mantle cell lymphoma from other B-cell lymphomas when applied in conjunction with CD43. CD23, CD43, and cyclin D1 appear to be of great help in differentiating subgroups of low grade B-cell lymphomas. Bcl-2, as known, is found to be useful to rule out reactive follicular hyperplasia.

7.Histologic and Immunopathologic Study of Central Nervous System Lymphoma.

Twelve cases of primary malignant lymphoma of the central nervous system experienced between 1980 and 1990 were investigated by histological and immunohistochemical findings correlated with clinical observation. Of the 12 patients, 6 were male and 6 were female. Their ages ranged from 31 to 58 years(mean, 45.8 years). All Tumors were supratentorial except 1 case which was found in the spinal cord. The fronto-parietal lobe was the most common site, which accounted for 66.7%. Histologically, all the tumors showed unfavorable histology. Diffuse large cell type was the most frequent(66.7%). Immunohistochemical studies using monoclonal antibodies revealed predominance of B-cell phenotype. Although most cases were treated with a combination of surgery and irradiation, the outcome was poor in all.

10.Comparision of the Effects of Transpupillary Thermotherapy(TTT) of Pigmented and Albino Rabbit Retina.

PURPOSE: We compared the clinicopathologic effects of TTT between pigmented and albino rabbits. METHODS: TTT was delivered using infrared diode laser at 810 nm(Iris Medical Instrument, Mountain Veiw, CA, USA) and applied with spot size of 3 mm, duration of 60 seconds. At 1 week and 4 weeks after TTT, fundus photographs and simultaneous FAG/ICG angiogram were taken with SLO(Scanning Laser Ophthalmolscopy, Rodenstock, Munish, Germany). Light and electron microscopic examination were performed. RESULTS: In pigmented rabbits, visible funduscopic change was visible even with minimal power setting(100 mW). Obliteration of choroidal vessels was observed on ICG angiogram. In microscopic examination, entire layers of neural retina, retinal pigment epithelial cells, and deep choroid were severely damaged at the center of treated fields. Whereas, in albino rabbits fundus changes were not observed at any power setting. However, focal thrombosis at margin of lesion was identified on ICG angiogram after power of 300 mW. In microscopic examination, tissue damage was developed up to 600 mW and the lesion extended into the superficial choroid posteriorly and outer neural retina anterioly. CONCLUSIONS: The effect of TTT was increased with fundus pigmentation. Clinically we should adjust TTT power setting according to the amount of melanin pigmentation in the fundus.
