1.Myelographic differentiation of bulging disk from herniated disk

Among the many causes producing lower back pain herniated nucleus pulposus and bulging disk have occupiedlarge percentaages and uniformly showed defect on lateral aspect of contrast filled thecal sac. But it isessential to differentiate each conditions from the other because of their different treatment methods.differentiation at metrizamide myelography between a diffusely bullging disk(unlikely to cause nerve rootcompression) and a herniated disk is based on the curature, extent, and multipllicity of the extradural deformityof the anterolateral margin of the contrast filled sac and o hte presence of fusiform widening of the most distalpart of the affected nerve root. The deformities caused by a bullging disk are round, usually symmetrical(aothoughoccasionally more prominent on one side), do not extend above or below the disk space, and can show multiple levelinvolvement; the nerve root is uniform in caliber and normal in size(although some of severe bulging show fusiformwidening of the most distal part of the affected nerve root). The deformities caused by a herniated disk isangular and extends cephalad and/or caudal to the level of the disk space; the affected nerve root is usuallywidened in its most distal visible part. A consecutive series of 50 patients with low back pain and no pasthistory of back surgery who did metrizamide myelography underwent spine CT and /or laminectomy. Using the criterialisted above for differentiation of bulging from herniated disk on metrizamide myelography, the myelographicdiagnosis was correct in 32(22 patients) of 34(24 patients) (95%) surgically and/or computed tomographicallyconfirmed bulging disks and in all 26 (100%) surgically and/or computed tomographically confirmed herniated disks.

2.The accuracy of sonographic measurement

Sonographic measurements often give considerable diagnostic significance. However, large discrepancy betweenthe real anatomic sizes and sonographic measurements could cause an incorrect diagnosis. Authors carried out aseries of experiment using a phantom and two scanners and the results are as follows: 1. There is a littledifference in errors between two scanners, and the mesurement does not almost always correspond to the realdistance. 2. The rates of errors of the measurements are almost fixed regardless of the positions of thereflcetors. 3. There is no relationship between the transducer MHZ and error rate. 4. As the error is derived from the scanner electronics it is necessary to calibrate the scanner periodically, and if not calibrated one shouldconsider the error rate of the scanner in the anatomical measurement.

4.Inactivation of p73 Protein by Cervical Cancer-inducing HPV E6 Protein Novel p53-independent function of E6.

OBJECTIVE: Human papillomavirus (HPV) is strongly implicated as a causative agent in the etiology of cervical cancer. Of its gene products, E6 and E7 oncoproteins play major roles by inactivation of cellular p53 and pRb tumor suppressor proteins, respectively. However, it has been recently suggested that p53 and/or pRb-independent functions of E6 and E7 are involved in cervical carcinogenesis. The purpose of this study is to identify novel a cellular target, p73, of E6 and to determine how E6 inactivates p73 function, METHODS: The interaction between E6 and p73 were identified by the yeast two-hybrid assay in vivo and the GST pull-down assay in vitro. The function of the interaction was determined by transient transfections using p21 promoter-CAT reporter plasmid. The molecular mechanism underlying the functional significance of the interaction was further assessed by in vivo and in vitro protein degradation assays, and gel mobility shift assays. RESULTS: Yeast two-hybrid and GST pull-down assays indicate a physical interaction between p73 and either HPV-16 or HPV-11 E6 proteins in vivo and in vitro, respectively. Transactivation domain (amino acid residues 1-49) is found to be absolutely required for this interaction. Transient co-expression of E6 significantly inhibits the p73-mediated activation of p21WAF1 promoter in a p53-defective C33A cell line. Using Ga14-p73 fusion protein, we demonstrate that E6 inhibition of p73 transactivation function is independent of sequence-specific DNA binding, which is confirmed by direct electrophoretic mobility shift assay. Moreover, E6 inhibits p73 function by interfering with the activity of the amino-terminal activation domain. The protein degradation assays in vivo and in vitro indicate that p73, unlike p53, is not susceptible to E6-dependent proteolysis. CONCLUSION: Throughout this study, we identified p73 as a novel cellular target of HPV-E6 protein and found that E6 binds p73 through the amino-terminal transactivation domain, and inhibits its transactivation function independent of the protein degradation and DNA binding. These overall results, consequently, suggest that in addition to the inactivation of p53, the functional interference of p73 by HPV-E6 may, at least in part, contribute to E6-mediated cellular transformation.

5.A Case of Recurrent Endometriosis with Rectal Invasion.

Rectal invasion of endometriosis is unusal condition, since it represents an invasion of previously normal bowel by hormone-dependent nonmalignant cell from uterus of the same patient. It is estimated that the incidence of endometriosis is about 8-15% of reproductive women, of whom 3-34% show intestinal invasion of rectosigmoid colon, appendix, ileum, and cecum in order of decreasing frequency. We experienced a case of recurrent endometriosis with rectal invasion in 34-year-old female who had previously endometriosis. She had rectal bleeding with severe anemia and abdominal pain with palpable mass in cul-de-sac. We present the case with a brief review of literature.

6.Working conditions of interns/residents and patient safety: Painful training might not be authentic.

The current working conditions of medical interns and residents in South Korea pose dangers to patient safety as well as to staff health. A national survey in 2014 showed that interns/residents worked an average of 93 hours per week. The prevalence of physical and verbal violence experienced by medical residents in the preceding 12 months was reported as 13.1% and 61.5%, respectively. Furthermore, sleep deprivation due to long working hours was strongly associated with a high risk of committing 'near miss errors'. An act to govern the working environment of interns/residents was established in 2015 and stipulates a maximum working hours of 88 hours per week. This is an important step toward improving patient safety and intern/resident health in South Korea.

7.Production of Re-188.

188Re (beta=22 MeV; gamma=155 keV; T1/2=16.9 hours) is an attractive therapeutic radioisotope which is produced from decay of reactor-produced tungsten-188 parent (T1/2=69 days). 188W has been produced from the double neutron capture reaction of 186W. 188Re can be easily obtained by elution of saline on alumina based 186W/188Re generator, which is commercially available. Complexes labelled with 188Re have been developed for the radiotherapy treatment of diseases because of the desirable nuclear properties of the radioisotope and it's chemical properties similar to those of technetium, a well established diagnostic agent.

8.The Clinical Results of Conservative Treatment of Frozen Shoulder Using Continuous Passive Motion.

BACKGROUND: The purpose of this study is to administer conservative treatment in 30 patients diagnosed with idiopathic frozen shoulder, following the suggested frozen shoulder rehabilitation program and to assess the clinical outcome using a prospective study. METHODS: Thirty patients diagnosed with idiopathic frozen shoulder, treated with steroid hormone injection on the articular joint with an intra-articular steroid (triamcinolone 40 mg + lidocaine 4 ml) injection and started on stepwise shoulder extension exercise were chosen. The subjects were divided into two groups of 15 people each with one group undergoing rehabilitation with continuous passive motion (CPM) and the other group without it. Follow-ups were done before rehabilitation and at 4-week intervals with the 24th being the final week. At every follow-up, passive range of motion (ROM) was measured and surveys on pain and clinical score were administered. RESULTS: In the last follow-up, both groups showed statistically significant improvements in all evaluation criteria. However, no statistical difference in all values of the ROM and Constant score evaluation criteria was observed between the groups. Only in the last follow-up, group 1 had a visual analog scale (VAS) score of 2.4 +/- 2.1 points, which was lower, with statistical significance, than the VAS score of group 2, which was 4.4 +/- 3.1 points (p<0.001). CONCLUSIONS: Study using CPM in treatment of frozen shoulder has been inadequate, meaning that there is still room for improvement and need for more study on setting a more specific protocol and guidelines for this procedure.

9.The Outcomes of Proximal Humerus Fractures with Medial Metaphyseal Disruption Treated with Fibular Allograft Augmentation and Locking Plate.

BACKGROUND: Proximal humerus fracture is considered to be the third most common fracture for patients aged 65 years or older. Conservative treatment has been known to treat most of humerus fracture. However, fractures with severe displacement or dislocation may require surgical treatment. Intramedullary fibular allograft with a locking plate is frequently used in patients accompanying medial metaphyseal disruption. In this study, author intends to evaluate clinical and imaging results based on patients who underwent surgical treatment using fibular allograft with a locking plate. METHODS: This study is conducted prospectively at Wonju Severance Christian Hospital, targeting patients who previously underwent surgical treatment using open reduction and intramedullary fibular allograft with a locking plate between 2011 and 2015. A total of 26 patients were evaluated on the following: postoperational clinical assessment measuring Constant score, American Shoulder and Elbow Society (ASES) score, and the Disabilities of the Arm, Shoulder and Hand (DASH) score. Postoperational imaging assessments are evaluated via measuring the neck-shaft angle. The study subject were Neer classification type 3, 4 proximal humerus fracture cases with disrupted medial hinge and having cortical comminution in the region of the surgical neck. RESULTS: The average period of progression was 22.5 months, and the average age of patients was 72.6 years. At the final follow-up, the average Constant, average ASES, and average DASH scores were 80.1, 78.5, and 20.6 respectively. The average neck-shaft angle was 127.5°. CONCLUSIONS: In conclusion, fibular allograft augmentation with a locking plate showed satisfying results in both clinical and imaging studies.
