1.A Case of Torsion of Wandering Spleen.

The wandering spleen is a rare condition, in which the spleen is located in other than the left upper quadrant of the abdomen. The clinical manifestation is variable from asymptomatic to abdominal catastrophy due to torsion of the splenic pedicle. We experienced a case of torsion of wandering spleen in 8-year-old girl who admitted with fever, vomiting, abdominal pain, palpable left abdominal mass. She was diagnosed preoperatively with the aid of abdominal sonography and C.T. scanning. A splenectomy was performed and she made uneventful recovery. The case report illustrates some of the diagnostic and therapeutic considerations pertaining to wandering spleen with a brief review of related literature.

2.The study of pathogenesis of palmoplantar keratoderma.

We investigated the biochemiesl change of keratin by the methods of SDS-PAGE and Two-dimensional gel electrophoresis, and observed electron microscopic ultrestructural changes in five Unna-Thost palmoplantar keratoderma patients and two normal adults. The results are summarized as follows : 1. The increased bands of 51 kd and newly appearing 48 kd, 56 kd keratins were observed on the SDS-PAGE and compared to the normsl control. 2. The newly appearing 48 kd(acidic) paired with 56 kd(basic) keratins and 51 kd keratin and the disappearance of 59 kd(basic), 64 kd(basic) keratins were observed on the two-dimensional gel electrophoresis and compared to the normal control. 3. The variable sized, numerous, globular, irregularly beam-shaped and granular kerstohyaline granules were scattered in the granular cell and corneocyte. Numerous ribosomes were noted between the clumped tonofibrils and around the keratohyaline granules. The lipid droplets were seen in the corneocytes and granular cells on the electron microscope.

7.Overlap Syndrome: A case showing severe CNS involvement.

We describe a patient with overlap syndrome in whom systemic lupus erythematosus and scleroderma were combined. The patient expired due to severe central nervous system involvernent and pneumonia. Pertinent neuropsychiatric signs of the patient included generalied seizure, impairment of recent, memory, dysarthria, mental deterioration, Balints syndrome, right sided hemiparesis and right sided central type facial weakness. Hrain MRI and cerebral angiogram slowed cerebral vasculopathy. We suggest that a long-term follow-up is necessary for the confirmation of ciagnosis of a connective tissue disease, because the evolution, transition and overlapping features among the group of connective tissue diseases may commonly occur over time.

10.Clinical Study of Conservative Treatment in the Carpal Tunnel Syndrome

Twenty-four hands of eighteen patients of carpal tunnel syndrome were treated with two or times injection of 5 mg dexamethasone and three weeks splinting at the department of orthopedic surgery, Korea University Hospital From Jan. 1980 to Feb.1985. The results obtained were as follows; 1. Physical findings consisted of sensory disturbances in sixteen hands, positive Tinel sign in thirteen, positive Phalen test in eighteen and thenar atrophy in eight. 2. On electromyographic study, delay of motor conduction velocity of the median nerve was noted in twelve cases and sensory nerve velocity in fifteen. 3. Fifteen patients(eighteen cases) were followed up from 6 months to 30 months. Twelve cases(60%) were improved and three cases were recurred.
