Malignant fibrous histiocytoma is a rare malignant tumor of histiocytic origin, arising from either bone, or soft tissue. Six cases of malignant fibrous histiocytoma are presented with emphasis on radiographic features. Five cases are of primary bone origin and one of soft tissue. Ill-defined osteolytic bone destruction with no sclerosis and with no periosteal reaction is the principal radiologic feature of the malignant fibrous histiocytoma of bone. Therefore, malignant fibrous histiocytoma should be considered in differential diagnosis of primary intraosseous or extraosseous malignancies.

Primary carcinoma of the bile ducts is an uncommon lesion that is difficult to diagnosis in early stage. Correct diagnosis with determination of site and extent of the carcinoma is essential for proper management. Authors presented 35 cases of bile duct carcinoma, confirmed at Seoul National University Hospital from 1978 to 1981, and analyzed their clinical and radiological findings. The results were as follows; 1. The 6th decade was the predilection age, and the ratio of male to female was 4:3. 2. The locations of bile duct carcinomas were common bile duct in 31%, porta hepatis in 26%, junction in 17%, diffuse involvement in 17%, and common hepatic duct in 9% in order of frequency. 3. Ultrasonography was done in 31 cases. Dilatation of intrahepatic ducts was noticed in 28 cases. Among the 28 cases, correct diagnosis as bile duct carcinoma were made in 5 cases. Proximal obstructive patterns were observed in 14 cases and distal obstructive patterns in 9 cases. 4. ERCP was done in 15cases. Successful studies of ERCP were done in 8 cases of them. Among the cases, diagnosis as bile duct carcinoma were made in 4 cases. 5. PTC was done in 20 cases and successful in all of them. Correct diagnosis as bile duct carcinoma were made in 16 cases and the diagnostic accuracy was 80%.

Since calcification or ossification of the posterior longitudinal ligament of the spine was first described inJapan in 1960 by Tsukimoto, Terayama used the term of OPLL(Ossification of Posterior Longitudinal Ligament) in1964 and mainly reported in Japanese. But recently, the incidence of the OPLL has been reported increasing amongthe non-Japanese. Because of the OPLL may be associated wtih severe neurologic symptoms, which need to havesurgical decompression, exact diagnosis and analysis are necessary. The OPLL can be diagnosed by simple spine,conventional tomography, myelography and CT. Authors analysed radiologic findings of the OPLL in 8 patients, whowere diagnosed by simple spine. Conventional tomography, myelography and CT, and then performed spinal operation,at Hanyang University Hospital from March 1980 to June 1983. The results were as followings; 1. The age range wasbetween 45 and 63 years and most prevalent age was 6th decades(63%). 2. All of the patients were male. 3. All ofthe OPLL occurred in the cervical spine and predominant at the level of C3,4 (48%). 4. The most common length ofthe OPLL was 2 vertebral level. 5. Except one, which was discontinuous at the level of disc space, all of the OPLLwere continuous in length. 6. 7 of 8 OPLL were located in the midline, but one was in right. 7. The OPLL seemed tocorrelate with spondylotic changes. 8. Of all diagnostic procedure, CT provided better and exact visualization ofthe lesion with axial scan and sagittal reconstruction.

Twenty cases of hepatocelluar carcinoma were examined by selective celiac and superior mesentericarteriography. Obstruction of the main portal vein due to tumor thrombus was revealed in 7 cases and 3 of thesecases had carvenous transformation of the protal vein(CTPV). The authors intended in this study to evaluate CTPVgroup and non-CTPV group clinically and radiologically. The results obtained are as follows; 1. The duration ofillness was shorter in CTPV group than non-CTPV group. 2. There was no significant difference in tumor sizebetween two groups, and main portion of tumor was located in the right lobe in both groups. 3. Arterioportal shuntwas present in 2 of 4 cases in non-CTPV group, but was no present at all in CTPV group. 4. Hepatofugal collateralsof portal vein were developed in all but one in both groups. 5. There was no significant difference in bloodchemistry between two groups. 6. CTPV may play an important role maintain the hepatic blood flow.

Since early 1980's high resolution ultrasound has been world-widely used for detection of cranial lesions ininfants but not widely used in korea. Authors prospectively analysed ultrasonographic findings of 107 cases whichwere confirmed by CT, autopsy or follow-up studies as supplement. the distribution of 107 cases was intracranialhematoma 40 cases, hydrocephalus 36 cases, hypoxic-ischemic encephalopathy 10 cases, porencephalic cyst 5 cases,cephalhematoma 5 cases, agenesis of corpus callosum 4 cases, medulloblastoma 2 cases and each one case of A-Vmalformation, intraventricular cyst, Dandy Walker cyst, lipoma and hydranecephaly. We could conclude thatneurosonography of infants was very useful and effective method in detection of cranial lesions such asintracranial hematoma, especially germinal matrix hemorrhage or intraventricular hemorrhage in preterm infant,hydrocephalus, hypoxic-ischemic encephalopathy and congenital anomalies.

Tomography has played an important role in diagnosis of facial bone fracture. Nowadays, it still acts as ainevitable process in decision of adequate treatment in patient with facial bone fracture. At the Department ofRadiology, Hanyang University Hospital, 74 patients who have facial bone fracture, were observed and analyzed withsimple skull films, mainly skull A-P, lateral and Water's view, after comparison with tomographic findings. Theresults were as follows; 1. Male was in 90.5% incidence. Most frequent age distribution was 21 to 40 years andmarked 62.2%. 2. Most frequent cause of trauma was traffic accident as 41 cases and the next one was fall down as13 cases. Other cause were blunt trauma such as first, stone, heavy metal etc. and explosion. 3. Clinical symptomsand physical signs were painful swelling, abrasion, ecchymosis and subconjuctival hemorrhage in almost allpatient. 4. Fracture distribution was 22 cases in simple fracure and 52 cases in complex fractures which were 26cases in Tripod fractures, 14 cases in Le Fort fractures and 12 cases in combined complex fractures. 5. Theradiologic findings on tomography were opacity of orbit of P.N.S. 87.8%, regional soft tissue swelling 85.1%,displacement of bony fragment 56.8%, abnormal linear density in orbit or P.N.S. 48.6%, bony fragment in orbit orP.N.S. 47.3% change of size of orbit of P.N.S. 40.5%, foreign body in orbit or P.N.S. 16.2%, and others 27.0%.These radiologic findings of simple fracture were less than those of complex fracture. 6. Radiologic findings offacial bone fracture on simple films were analyzed after comparison with tomograhs. Detectable possibility ofobvious fracture lines such as cortical disruption or separation was 67.6% on simple filsm. Detectable possibilityof other radiologic findings on simple films were displacement of bony fragment 86.7%, abnormal linear density inorbit or P.N.S. 88.6%, bony fragment in orbit or P.N.S. 71.4%, opacity of orbit or P.N.S. 90.8%, change of size ororbit or P.N.S. 96.4%, regional soft tissue swelling 96.2%, foreign boy in orbit or P.N.S.41.7%. Others such asabsence or deformity of infraorbital foramen, subcutaneous emphysema or intraorbital air 40.0%. Thereforetomography is inevitably taken to correct diagnosis of facial bone fracture.

In this report, a case of a 70-year-old man with a large ganglion cyst formed after anterior transposition of the left ulnar nerve is presented. Three months after the index surgery, the patient presented with a painless superficial ovoid, soft mass measuring 5x4x2 cm in size located at the posteromedial aspect of the left elbow, the previously operated site. Magnetic resonance imaging showed a well demarcated cystic mass with a stalk connecting to the elbow joint. Excisional biopsy was performed and pathologic findings showed that the cystic wall had no definite lining cells with myxoid degeneration compatible with findings of ganglion cyst.

In parentage testing DNA profiles are used to link the alleged father with paternity by matching their patterns. The probative value of a match is often calculated by multiplying together the estimated frequencies with which each particular VNTR or STR pattern occurs in a reference population. When this calculating method applies to the motherless case of paternity disputes, a calculation must usually be based on types determined for the child and the alleged father. In such case, the first consideration is to exclude a man from paternity of a child when the man did not have the child's paternal allele at some loci, or if the paternal allele cannot be determined, when the man had neither of the child s alleles. The second is to evaluate the DNA evidence when a man is not excluded by the paternal allele. This work is to provide theories of paternity analysis with three approach methods for the motherless case, and to evaluate their efficiency compared to the trio case when the man tested is not excluded. Consequently, the motherless case offers lower probability exclusion and questionable cumulative paternity index than those of the trio case as being typed with 14 STR loci(CSF1PO, TH01, TPDX, vWA, D5S818, D13S317, D7S820, D16S539, FGA, D21S11, FES/FPS, F13A1, D18S80, D17S5). Since the motherless case in paternity disputes is less efficient for paternity exclusion of the child, the use of genetic maker systems with the higher value of mean exclusion chance(MEC) and exact levels of the relative probability of paternity must be of importance considered in the analysis of such deficiency cases.

Intracranial tuberculomas have been reported occasionally, especially in Asia, though much decreased in recentyears. Those lesions can be diagnosed more easily and acurately using CT than conventional method, including angiography. Authors analysed CT findings of 21 cases, confirmed as tuberculoma, at Hanyang University Hospital from May 1979 to June 1983. The resuslts were as follows; 1. Of all 21 cases, multiple lesions were seen in 14 cases (67%) and single in 7(33%). 2. Of all 21 cases, lesions located only at supratentorial were in 19 cases(90%) and remained 2(10%) had lesions at both supra and infratentorial area. And temporal and parietal lobes were common location (65%) of all lesions. 3. In precontrast scan, density of tuberculoma showed largely isodense (68%)and others were slight high (29%) and low(3%). 4. All lesions were enhanced showed as homogeneous nodular (68%),ring-shaped(29%) and target shaped(3%). 5. All rings were continuous and thickness was largely uniform(67%), anddensity of center of the ring was mainly low(67%). 6. Edema was seen in 58% of all lesions: comparing with thesize of tuberculoma, edema size was smaller in 50%, lager in 33% and almost the same in 17%.

BACKGROUND AND OBJECTIVE: Despite many multicenter trials on percutaneous coronary intervention (PCI), versus coronary artery bypass surgery (CABG), in multivessel coronary artery disease (MVCAD), the most appropriate treatment remains a matter of debate. Moreover, studies comparing the 2 strategies in eastern society are rare. The aim of this study was to assess the relative merits of PCI and CABG in MVCAD in the post-stent era. SUBJECTS AND METHODS: Patients, with MVCAD, indicated for revascularization were enrolled from 9 centers in Korea. Out of the 3,279 patients in the registry, 2102 (CABG 609 patients, PCI 1,493 patients) were selected for a comparison of their outcomes, after a statistical adjustments for the disparity for 6 independent risk factors, for the prognosis between the two groups. RESULTS: There was no significantly different in the 3 year survival rates between PCI and CABG groups. In the diabetic patients, the 3-year mortality rate in PCI group was 1.6-fold higher than in the CABG group, although it was not statistically significant (PCI 19.8%, CABG 12.5%, p=0.24). The incidence of cerebrovascular events (CVE) was higher in the CABG group. The thirty-day death rate, myocardial infarction or CVE were higher in the CABG group (PCI 1.3%, CABG 4.2%, p<0.001). Both the long and short-term revascularization rates were higher in PCI group compared to CABG group. CONCLUSION: Our Korean registry demonstrated a comparable survival rate between the PCI and CABG groups. A PCI was associated with a lower early morbidity, but with a greater need for repeated revascularization compared to a CABG.
