1.An Experimental Investigation for Biologic Reactions of Carbon Fiber Implants

Ligament and tendon prostheses have all suffered because of failure of the particular material that has used in the past. Various materials, including, silk, nylon, Dacron, and combinstions of other synthetic substances, have all enjoyed temporary vogues and have vogues and have all passed out of habitual use because of collapse of the material employed. In an attempt to find an alternative material for use as a tendon or ligament prosthesis, a fundamentally new approach to the problem has been used, namely, that of tendon ligament induction. Carbon fiber has an attraction as a biologic implant because of its inherently inert nature when used in the pure form. Within recent years, it has been possible to manufacture filamentous carbon fiber of high purity and constant filament size, and this material has been examined as a tendon and ligament prosthesis. Both achilles tendons of 14 adult Korean rabbits were replsced with carbon fiber reinforced epoxies(CFRE, it was developed in department of chemical engineering, Chungnam national university, Daejeon, Korea) after artificially rupture, and serial histologic examination of neotendon formations and biologic responces in vivo were done during 8 weeks. The results obtained were as followings ;1. Hemorrhage and inflammatory reactions were observed at 2–3 weeks after operations. 2. Foreign body reactions were developed at 4 weeks after operation, also observed proliferations of foreign body giant cells and histiocytes. 3. Fibroblasts were increased at 4 weeks after operation, and most severe poliferations at 5–6 weeks. 4. Foreign body reactions and fibroblasts were markedly decreased at 7–8 weeks after operation, and examination of the collagen itself showed it to be closely similar to that found on the normsl side.5. Thus, it appeared that the carbon fiber initially acted as a tendon prosthesis.

2.Cystic Diseases of the Kidney in Chidren.

A case of metastatic adenoid cystic carcinoma of the lung, originated from the trachea, was diagnosed by fine needle aspiration. Although the cytologic features of adenoid cystic carcinoma have been well described, it is easy to confuse adenoid cystic carcinoma with more common primary small cell neoplasms of the lung, i.e., small cell carcinoma, well differentiated adenocarcinoma, and carcinoid tumor of the lung. The features distinguishing adenoid cystic carcinoma from these neoplasms include 1) tight, globular, honeycomb pattern of cells, 2) acelluair basement membrane material in the lumen, and 3) cells lacking true nuclear molding and having bland chromatin pattern. The mcrphologic feature of metastatic adenoid cystic carcinoma in this case was so distinctive as to permit a definite diagnosis by aspiration cytology.

3.Comparison of the Corneal Astigmatism Between Suture Methods After 7mm Scleral Pocket Incision Catartact Surgery.

Most of cataract surgeons have made a great effort to minimize or nullify corneal 'astigmatism resulted from the incision and closure, but a completely ideal wound system still eludes us. The scleral pocket incision and continuous single knotted shoelace suture of it has been known as one of the technique for reducing postoperative astigmatism. Recently, horizontal suture closure of scleral pocket incisior has been introduced because it doesn't cause suture induced wound compression. We implanted standard PMMA intraocular lenses in the bag of 400 patients through 7mm scleral pocket incision following CCC (continuous circular capsulorhexis) and bimanual phacoemulsification. The incision was closed with either the shoelace or horizontal suture. The keratometric measurement was maded at postoperative 1 day, 1 week, 1 month, 2 month, 3 month and 6 month. In shoelace suture group, the preoperative corneal astigmatism appeared -0.14 +/- 0.15D of with the rule astigmatism (WTR); at one day postoperatively -2.75 +/- 1.61 of WTR, at 3 month 0.18 +/- 1.14D of against the rule astigmatism (ATR), 6 month 0.3 +/- 1.22D of ATR. In horizontal suture group, the preoperative corneal astigmatism appeared -0.22 +/- 1.22D of WTR; at one day postoperatively -1.05 +/- 1.28 of WTR, at 2 month 0.57 +/- 1.0 of ATR, at 6 month 0.72 +/- 1.91 of ATR. The difference between two suture methods was statistically significant (p<0.01 until 3 month, p<0.05 until 3-6 month).

4.Bilateral Agenesis of the Internal Carotid Artery: Case Report .

5.The Role of Platelet-derived Growth Factor in Regeneration of Pasteurized Osteochondral Graft in Rabbits.

Platelet-derived growth factor(PDGF) is known to accelerate soft tissue fracture and periodontal bone healing, but little information is available for characterizing the healing of articualr cartilage. This study was designed to demonstrate the regeneration potential of pasteurized autogeneous osteochondral graft when PDFG was applied locally in vivo. Eighteen rabbits in two groups were used in the experiment. The free osteochondral fragment(1x1x1 cm) were taken from distal femur. The fragment were pasteurized in 65degrees C for 5 minutes. In experimental group(group II), the fragment were soaked in 0.25 mg/ml of recombinant human PDGF mixed 5 ml normal saline and reimplanted to the resected site, and followed with local administration of 0.25 mg/ml PDGF to knee joint. In control group(group I), the pasteurized fragment were reimplanted in the resected site without any treatments. The histologic characteristics of the transplanted autografts for three rabbits in each group were observed at 3, 5 and 7 weeks postoperatively. The results were as follows; Group II revealed the more favorable regeneration of articular cartilage, less arthritic changes and more mature arrangement of chondrocytes than group I. Even thought the cartilage is avascular and the regeneration capacity of pasteurized cartilage is severely limited, the regeneration of grafted articular cartilage is thought to be accellerated by increased newly formed blood vessels and soft tissues due to the diffusion of PDGF on grafted cartilage, and probably also by differentiation of mesenchymal cells in the initial stage of experiment. The results of this study suggest that local administration of PDGF could be used for the treatment of posttraumatic or degenerative arthritis and various cartilage damages.

6.Clinical Results of Silicone Foldable IOL Implanted Through Small Incision.

Small incision cataract surgery has many advantages such as less induced corneal astigmatism, fast visual recovery, and fast rehabilitation. We had implatned the newly developed Staar AA-4203 foldable silicone IOL in the ag of 100 cataractous eye and followed for at least 3 months (3-0 months, mean 6 months). Silicone IOLs were implanted using a specific injector through 4mm scleral pocket incision after continuous circular capsulorhexis and bimanual phacoemulsification. The lenses were centered well in 99% of eyes. Tear of anterior capsule occurred in one eye (1%), and tear of both anterior and posterior cassule occurred in another one eye (1%) during IOL implatation. The IOL was displaced into the vitreous in the latter eye, which was reoperated. The preoperative and postoperative cornedal astigmatism at 1 day, 1 month, 2 month and 3 month was -O.14 +/- 0.83 D of with the rule astigmatism (WTR), -0.55 +/- 1.06 D of WTR, -0.28 +/- 0.91 D of WTR, -0.07 +/- 0.81 D of WTR, and O.01 +/- 0.84 D of against the rule astigmatism respectively. Uncorrected VA of 1.0 was 35% at postoperative 1 day, 52% at 1 month, 48% at 2 month. Corrected VA of 1.0 was 65% at postoperative 1 day, 87% at 1 month, 88% at 2 month. Inflammatory reaction in the anterior chamber and vitreous had subsided within one or two months without any complications in 99% of eyes. Fibrinous membrane on the suface of IOL occurred in 2 eyes (2%), one of which caused grave sequelae. The results, although it showed early postoperative results, reflected that the above silicone foldable IOL fitted to the small incision cataract surgery guaranteed us the safety and fast visual recovery.

8.An Investigation of Articular Cartilage Degeneration Induced by Compression-Immobilization and Condylar resection of Knee Joint in Rabbits

Articular cartilage have dual functions of shock absorber and bearing surface in a moving joint. Articular cartilage is avascular, so the chondrocytes in mature adult must receive their nourishment solely from synovial-fluid perfusion, and it is known that alteration of synovial-fluid perfusion causes degeneration of articular cartilage. Now, in orthopedic fields, the corrective cast immobilization and traction is in popular use. In order to study the cartilage changes induced by this long-term cast immobilization and traction, compression-immobilization and condylar resection of knee joint in rabbits were performed, and weekly histoiogic examinations of articular cartilage up to 5 weeks were followed. The observations were as follows: 1. Degenerative changes of articular cartilage were observed respectively with compression-immobilization and condylar resection, and the severity of the histologic changes appeared to be proportional to the duration of compression-immobilization and condylar resection. 2. Earlier degenerative changes of articular cartilage in condylar resection group than in compression-immobilization group were noted, and this earlier degenerative changes in condylar resection group suggests that hemarthrosis may play a role in cartilage degeneration. 3. Histologically unchanged calcific cartilage zone and mildly hypertrophied subchondral bone were noted in compression-immobilization group, but vascular invasion to calcific cartilage zone and subchondral osteoporosis were noted in condylar resection group.

Osteopoikilosis is an asymptomatic osteosclerotic dysplasia, initially described by Albers-Schönberg and Ledoux-Lebard and associates. This disorder is described as extremely rare. Inherited and sporadic cases of osteopopikilosis have been reported. A Case of familial osteopoikilosis is presented with a brief review of literatures.

10.Lymphangiomatosis of Bone and Soft Tissue: A Case Report

A case of Lymphangiomatosis of the skeleton and soft tissue in a 23 years old woman is presented. Lymphangiomatosis are benign tumors formed of lymphatic vessel and like hemangiomatosis are probably best thought of as harmatomatous malformation. As term imply, the endothelial space in these lesions are lymph-rather than blood containing. Bone involvement are very rare condition that affect mainly the ilium and features a largely lytic pattern, consisting of tiny elliptical radiolucents slots. Treatment and Prognosis depend on the site and extent of involvement and progression. Although irradiation may be the treatment of choice, the lesions have not been usually successfully treated.
