2.A Case of Fordyce's Disease with Wide Distribution.

Fordyce's disease is a condition known as ectopically located sebaceous glands on the vermilion borders of the lips and oral mucosa. Clinically, it is groups of minute, yellowish, globoid macules and papules. Histologically, it is characterized by sebaceous glands not associated with hair follicles. We report on a 40-year-old man with Fordyce's disease showing particularly wide distribution on the buccal mucosa and upper lip.

The retrospective evaluation of 50 cases of head trauma that were subjected to CT leads to the following conclusion; 1. CT survey, should be the first neuroradiological procedures to performe. 2. There is generally adirect relationship between the severity of clinical presentation and the CT findings of the abnormality; responsible for the clinical status, the number and intensity of tissue abnormalities on CT scans increase proportionately with the severity of the clinical signs and symptoms. 3. An accuracy is 100% in diagnosis of intra and extracerebral collections of blood. 4. The causes of acute head tauma were automobile, 64% and falling down,32%.

4.Total Number of Lymph Nodes Retrieved in Stage III Rectal Cancer Patient.

PURPOSE: Adequate lymph node analysis is critical for appropriate staging in colorectal cancer. The aim of this study is to determine whether 12 or more nodes recovered in stage III rectal cancer results in improved oncologic outcomes. METHODS: Two hundred and forty-eight patients with stage III rectal cancer from 1995 through 2004 were reviewed. They were categorized into 2 subgroups by the number of nodes retrieved (<12 and > or =12), and oncologic outcomes in terms of 5-year overall and disease-free survival were analyzed for all patients, patients with American Joint Committee on Cancer (AJCC) N1 disease (N=145), and those with AJCC N2 (N=103). RESULTS: Five-year overall and disease-free survival was 79.0% and 58.4%, respectively. There was no significant difference in clincopathologic features between <12 retrieval group and > or =12 group. Although there was significant difference in overall survival and disease-free survival between the number (<12 and > or =12) of lymph nodes removed in N2 disease (P=0.043; P=0.022) in univariate analysis, the total number of lymph nodes retrieved was not a prognostic factor affecting survival in multivariate analysis. The N2 stage and lateral margin involvement were prognostic factors affecting survival in multivariate analysis. CONCLUSION: This study showed that the total number of lymph nodes analyzed for stage III rectal cancer is not a prognostic factor on overall or disease-free survival in multivariate analysis.

5.A Case of Kikuchi's disease associated with Systemic Lupus Erythematosus.

Described originally in young Japanese women, Kikuchi's disease produces a histiocytic necrotizing lymphadenitis of predominant cervical lymph nodes. Histiocytic necrotizing lymphadenitis, Kikuchi's disease, is a condition rarely associated with systemic lupus erythematosus(SLE). We present a 34-year-old woman with tender multiple cervical lymphadenopathy of 1-year duration and SLE symptoms. Histologic examination of lymph node shows histiocytic necrotizing lymphadenitis. She showed high titer of antinuclear antibody, skin lesion with photosensitivity, thrombocytopenia and arthritis. We describe a case of Kikuchi's disease associated with SLE.

6.A Case of Eruptive Steatocystoma Multiplex on the Scalp.

A 46-year-old man had noticed an asymptomatic, small, yellowish, papular eruption on the scalp about one year ago. Since then, the lesions had slowly increased in size and number. There was no family history of similar lesions. An incision made into the cysts discharged oily yellow material. Biopsy of a lesion disclosed that the cysts had typical features of steatocystoma multiplex histologically. This case was a unique eruptive variety of steatocystoma multiplex on the scalp.

7.Retroperitoneal Desmoplastic Small Round Cell Tumor.

A desmoplastic small round cell tumor (DSRCT) is a rare and aggressive neoplasm with an extremely poor prognosis. It primarily affects children and young males. The most common sites are the abdomen areas, particularly the pelvic region. Usually, it is a large tumor mass accompanied by extensive peritoneal seeding. Microscopically, the typical appearance is a well-defined nests or clusters of small round tumor cells with an undifferentiated type, surrounded by a prominent desmoplastic stroma. Immunohistochemically, the tumor cells show a primitive uncertain histogenesis that can simultaneously express the epithelial, mesenchymal, and neural phenotype. There is no definitive treatment modality. Although a surgical resection, chemotherapy and radiotherapy are to the main treatments, the prognosis is poor. We experienced a case of a desmoplastic small round cell tumor at the retroperitoneal space. A 42-year-old woman was admitted for a palpable mass in the left upper abdomen for 1 months. Compoterized tomography revealed a retroperitoneal tumor. A complete excision of the tumor was performed. The histological examination confirmed the diagnosis of a desmoplastic small round cell tumor. The tumor recurred 3 months later. She received the first chemotherapy regimin with high-dose cyclophosphamied, adriamycin and vincristine. Computed tomography showed that the recurred tumor disappeared after 6 cycles of chemotherapy, but it relapsed 6 months later. She died of sepsis during the second chemotherapy regimen. The survival time was 21 months.

8.Dilutional Hyponatremia during Hysteroscopic Myomectomy: A Case Report.

Hysteroscopy is utilized for making the diagnosis and treating a series of uterine disease. It's advantages are more accurate removal of lesion, a short operating time, low morbidity and rapid postoperative recovery. However, serious complications can happen following hysteroscopic surgery. The complications can be divided into the procedure-related, media-related and postoperative events. The procedure-related complications include cervical laceration, uterine perforation, bowel and bladder injury, and hemorrhage. The media-related complications include hyponatremia, gas embolism and excessive fluid absorption. The postoperative events include endometritis and postoperative synechiae. We experienced hyponatermia with pulmonary edema due to excessive fuid absorption in a 52-year-old woman who underwent elective hysteroscopic myomectomy under general anesthesia. She was treated with oxygen therapy, normal saline and furosemide and she recovered without sequelae.

9.A Case of Left Coronary Artery Milking Treated by Direct Stenting During Percutaneous Coronary Intervention in A Patient with Unstable Angina.

Coronary artery milking is defined as a systolic compression of the coronary artery, usually resulting from myocardial bridging. Rarely, congenital heart disease, pulmonary hypertension and ventricular aneurysm can lead to coronary artery milking, and there has been a reported case of left anterior descending coronary artery milking after coronary stenting. Percutaneous coronary intervention (PCI) is an emerging strategy for treatment of stenosis in the left main stem. We report a case of symtomatic systolic milking at the left main stem treated by direct stenting during PCI in a patient with unstable angina pectoris.

10.Pilot Study for the Prediction of Response to Radiotherapy Using (18)FFluorothymidine PET in Nasopharyngeal Cancer: Comparison with (18)FFDG PET.

PURPOSE: This study was performed to know whether [(18)F]Fluorothymidine (FLT) positron emission tomography (PET) can be used to monitor early response to radiotherapy in comparison with [(18)F]Fluorodeoxyglucose (FDG) PET, and to establish the optimal imaging time for prediction of therapy response. MATERIALS AND METHODS: Two patients with nasopharyngeal cancer underwent serial FLT PET and FDG PET before and during radiotherapy. Three on-treatment FLT and FDG PET scans were performed on 1 week, 2 weeks and 3 weeks (at each time of 10 Gy, 20 Gy and 30 Gy delivered). The peak standardized uptake values (SUV(peak)) of primary tumors were measured on FLT and FDG PET. Then, percent changes of SUV(peak) after therapy were calculated. RESULTS: In two patients, baseline values of SUV(peak) on FDT PET were higher than those on FLT PET (FLT vs FDG; 3.7 vs 5.0, and 5.7 vs 15.0). In patient 1, FLT SUV(peak) showed 78%, 78% and 84% of decrease on 1 week, 2 and 3 weeks after treatment, whereas FDG SUV(peak) showed 18%, 52% and 66% of decrease, respectively. In patient 2, FLT SUV(peak) showed 75%, 75% and 68% of decrease, whereas FDG SUV(peak) showed 51%, 49% and 58% of decrease, respectively. Both patients reached to complete remission after radiotherapy. CONCLUSION: After radiotherapy, the decrease of FLT tumor uptake preceded the decrease of FDG tumor uptake in patients with nasopharyngeal cancer, and 1 week after therapy may be appropriate time for the assessment of early response. FLT PET might be more useful than FDG PET for monitoring early response to radiotherapy.
