Harrington Instrumentation is very effective method for treating the unstable thoraco-lumbar spine fracture and fracture-dislocations. Authors performed Harrington Instrumentations in 8 cases of unstable thoraco-lumbar spine fracture and fracture-dlslocations. The results obtained were as follows; 1. Spinal fracture and fracture-dislocations are most common in thoraco-lumbar junction. 2. Laminectomy increases the instability of the spine. 3. Pain was more common in the patient who has severe deformity of the spine. 4. Neurological recovery; a) In case of complete paralysis... returns of useless motor power in 2 of 6 cases. b) In case of incomplete paralysis... complete returns in all 2 patients. 5. Harrington Instrumentation is very effective for restoring the spine stability in unstable fracture and fracture-dislocations. 6. Early mobilization was possible and hospitalization period was shortened by Harrington Instrumentation.

2.A Case of Central Diabetes Insipidus Associated with Brachycephaly.

Brachycephaly is a kind of craniosynostosis. Because of premature closure of the coronal suture, the skull is shorter in the anteroposterior diameter but is widened with a high vault and the occiput and forehead are flattened. Diabetes insipidus had been reported in oxycephaly. We have experienced a case of central diabetes insipidus associated with brachycephaly. A brief review of related literatures is included in this report.

4.Chondrodystrophia Calcificans Congenita

Chondrodystrophia calcificans congenita is a rare disorder of bone, occuring in infants, which is characterized by radiographic manifestation of premature deposition of punctate calcific density in those epiphyseal areas, preformed in cartilage. Associated with this basic disturbance, following additional features have been reported: Asymetries in the lengths of the extremities, micromelia, rhizomelia, joint contractures, calcifications of the hyoid bone, thyroid and tracheal cartilage, saddle nose, cleft palate, congenital dysplasia of the hips, catarract, and dystrophic skin changes. A case of chondrodystrophia calcificans congenita is presented with a brief review of literatures.

6.Clinical observation of aseptic meningitis associated with mucocutaneous lymph node syndrome.

Clinical observation was carried out for 147 patients with mucocutaneous lymph node syndrome (MCLS) who were admitted to the Department of Pediatrics, Wonju Christian Hospital during the period from June, 1983 to June, 1992. A spinal tap was performed on 114 of these patients and the following results were obtained. 1) The incidence of aseptic meningitis in patients with MCLS was 52.6%. (60 cases of the 114 cases who had spinal taps (52.6%).) 2) The predominent age group for aseptic meningitis was 6 months to 1 year of age (35%). 3) For the majority, the value of glucose and protein in the CSF were within normal limit or only mildly elevated. 4) The neurologic manifestations associated with aseptic meningitis in patients with MCLS were irritability (78.3%), vomiting (25.0%), nuchal rigidity (11.7%), convulsion (5.0%) and facial nerve palsy (1.7%) in that order.

8.2 cases of acute disseminated encephalomyelitis.

Acute disseminated encephalomyelitis (ADEM) is an acute inflammatory demyelinating disease of the central nervous system. It is thought to be an autoimmune mediated disorder which occurs usually after viral or bacterial infections, or after immunization. The clinical manifestations reflect sudden onset of the diffuse involvement of the brain, spinal cord and the meninges. We experienced two cases of acute disseminated encephalomyelitis. The diagnoses were made by history, neurologic examination, CSF analysis and brain MRI. The symptoms were improved with steroid treatment. A brief reveiw of literatures also was made.

9.Treatment of Cytomegalov irus-associated IgA Nephropathy by Deflazacort and Intrav enous Immunoglobulin.

It has been suspected that various infections, including cytomegalovirus(CMV) infection, are associated with IgA nephropathy. In case of CMV infection, ganciclovir is known to be a treatment of choice for severe CMV infection in general. But ganciclovir has a lot of severe toxicity, so children with normal immunity are seldom treated by ganciclovir when CMV infection is suspected. On the other hand, intravenous immunoglobulin can also be used to treat CMV infection.

10.Effective and Practical Complete Blood Count Delta Check Method and Criteria for the Quality Control of Automated Hematology Analyzers

Background:
Delta checks increase patient safety by identifying automated hematology analyzer errors. International standards and guidelines for the complete blood count (CBC) delta check method have not been established. We established an effective, practical CBC delta check method and criteria.
Methods:
We assessed five delta check methods for nine CBC items (Hb, mean corpuscular volume, platelet count, white blood cell [WBC] count, and five-part WBC differential counts) using 219,804 blood samples from outpatients and inpatients collected over nine months. We adopted the best method and criteria and evaluated them using 42,652 CBC samples collected over two weeks with a new workflow algorithm for identifying test errors and corrections for Hb and platelet count.
Results:
The median delta check time interval was 1 and 21 days for inpatients and outpatients (range, 1–20 and 1–222 days), respectively. We used delta values at 99.5% as delta check criteria; the criteria varied among the five methods and between outpatients and inpatients. The delta percent change (DPC)/reference range (RR) rate performed best as the delta check for CBC items. Using the new DPC/RR rate method, 1.7% of total test results exceeded the delta check criteria; the retesting and resampling rates were 0.5% and 0.001%, respectively.
Conclusions
We developed an effective, practical delta check method, including RRs and delta check time intervals, and delta check criteria for nine CBC items. The criteria differ between outpatients and inpatients. Using the new workflow algorithm, we can identify the causes of criterion exceedance and report correct test results.
