2.Experience of the Clinical Application of the Wagner's Apparatus in the Lower Extremity

Diaphyseal fractures of the long bone associated with massive soft tissue injury or severe comminution is difficult to treat. Fracture healing is often delayed due to the damaged soft tissue, deep seated wound infection, fracture instability and poor local circulation. The delayed fracture healing necessitates prolonged immobilization of the limb which results in joint stiffness. Various type of fracture treatment was suggested for this type of fractures to ease the treatment. The Wagner's apparatus was one of the device to treat it, which was originally introduced as a device for controlled leg lengthening. Its use also has been expanded to serve as a rigid unilateral external fixation frame for fracture. In order to obtain satisfactory fracture union without adjacent joint contracture, the Wagner's apparatus has been employed by us in three cases who were hospitalized in Kangnam St. May's Hospital. The experience of this technique was very encouraging; therefore we recommend its wide application in fracture treatment in cases of the patient who can not receive the bilateral external fixation frame.

3.Three Cases of Secondary Membranous Lipodystrophy.

Membranous lipodystrophy is a peculiar type of fat necrosis, present in patients with various types of skin diseases. It is charaeterized by the presence of microcysts and macrocysts lined by amorphous eosinophilic material with a crenelated arabesque appearance and microgranules in the subcutis with massive fat necrosis. The eosinophilic lining and microgranules stain positively with periodic acid-Schiff, are resistant to diastase, and also stain with Sudan black B. We report three cases of subcutanous membranous lipodystrophy in patients with erythema induratum, posttraumatic panniculitis and morphea with typical clinical and histopathologic findings.

4.The Effectiveness for Vagal Stimulation by Ice Water Application to Face during Controlled Respiration and Mechanism of Individual Variability of Responsiveness to Vagal Stimulation (Power Spectral Analysis of Heart Rate Variability).

BACKGROUND: The effectiveness for vagal stimulation by ice water aplication to face during metronome-controlled respiration of 15 breaths/minute was examed. The importance of basal vagal tone and sympathovagal interaction in the individual variability of responsiveness to the vagal stimulation was investigated. METHOD: Fifty three 12~13 year old healthy volunteers were included. Vagal tone and sympathovagal interaction before and after application of ice water to the face were assessed by power spectral analysis of RR interval(heart rate) variability. RESULT: Basal heart rate, high frequency power, and low to high frequency power ratio were 81+/-13(58~110 beats/min), 791+/-1061(56~4161 msec2) and 1.08+/-1.22(0.04~4.85) during controlled respiration. After ice water application twenty three children developed 5 more nodal escape beats due to severe bradycardia. Minimum heart rate, high frequency power and low to high power ratio changed to 42+/-12(19~72/min), 1890+/-1882(221~7258msec2) and 0.64+/-0.43(0.12~1.46). The increased ratio of high frequency power, maximum heart rate decrement and its percent after stimulation were 5.44+/-5.62(0.63~24.26), 39+/-14(10~81/min) and 47+/-15(16~81%) respectively. The increased ratio of high frequency power was correlated with basal logarithmic high frequency power(r=-0.60, p=0.0004). Maximum heart rate decrement was correlated with basal logarithmic high frequency power(r=-0.41, p=0.0018) and low to high frequency power ratio(r=0.27, p=0.04). CONCLUSION: Application of ice water to the face during controlled respiration produces powerful vagal stimulation and bradycardia, however, there is a wide individual variability of responsiveness to it. The absolute basal vagal tone contribute to this individual variability.

7.A Comparison of Growth Development and Nutrient Intakes between Double Income Families' and Full-Time Housewives' Preschool Children.

The purpose of this study was to examine the comparison of growth development, bone mineral density and nutrient intakes between double income families' preschool children(DIFPC) and full-time housewives' preschool children(FHPC). Subjects were 111 preschool children. Anthropometric characteristics and bone mineral density in right forearm were measured. The questionnaire was composed of health status, life style, dietary behaviors, and dietary intakes and was completed by the children's mothers. The average age of the DIFPC(n=60) was 53.02 months and that of the FHPC(n=51) was 54.80 months. The birth height and weight of the subjects were 50.47cm and 3.27Kg for DIFPC and 50.85cm and 3.36Kg for FHPC, respectively. The average height, weight, % body fat, and obesity index were 108.50cm, 18.35Kg, 15.35%, 96.71% in DIFPC and 111.46cm, 19.64Kg, 16.80%, 97.31% in FHPC, respectively. The bone mineral density in forearm of two groups were 0.24g/cm2 in all. The infant feeding method was significantly different between DIFPC and FHPC; 58.9% of DIFPC was fed formula, while 44.4% of FHPC was fed breast milk(p<0.05). Proportions of children for their regular meal were 59.4%, 89.6%, and 61.0% for breakfast, lunch, and dinner, respectively. The major reasons for irregular meal were lack of time and poor appetite for breakfast and snacks for lunch and dinner. Most of the children answered they have snack over once a day, and 60.0% have unbalanced diet. The intakes of energy, calcium, iron, zinc, vitamin A, vitamin B1, niacin, and vitamin C did not meet the Korean RDAs. The intakes of K and vitamin A for DIFPC were significantly higher than those of FHPC(p<0.05, p<0.05). In conclusion, double income families' preschool children more have a low frequency of breast feeding and low intakes of micro nutrients, such as K and vitamin A than full-time housewives' ones.

Eosinophilic panniculitis is characterized by a prominent infiltration of numerous eosinophils in subcutaneous fat, and has been identified in patients with a variety of associated clinical conditions. A case of eosinophilic panniculitis in a 20-year-old woman with a history of atopic dermatitis is reported. She later developed superior sagittal sinus thrombosis, and we stress the importance of systemic evaluations in patients with eosinophilic panniculitis.
