1.A case of Limited Scleroderma Associated with Antiphospholipid Syndrome.

The antiphospholipid syndrome is characterized by arterial thrombosis, venous thrombosis, pregnancy wastage, and thrombocytopenia associated with a persis tently positive lupus anticoagulant and/or moderate to high positive anticardiolipin antibodies(IgG or IgM). The antiphospholipid antibodies have been detected in many medical conditions, but the antiphospholipid syndrome (APS) has mainly been restricted to the primary antiphospholipid syndrome and APS associated with systemic lupus erythematosus. Rarely, the APS has been reported in other autoimmune disorders in the literature. We describe a woman with a limited form of scleroderma and the APS manifested by complete occlusion of left axillary artery with probable thrombotic occlusive nature, thrombocytope nia, prolonged aPTT, and persistently positive lupus anticoagulant.

2.MR Findings of Knee Injuries in Skiing: Relation with the Mechanism of Injury.

PURPOSE: To evaluate the MR findings of knee injuries in skiing and to explain the mechanism of injury with MR findings. MATERIALS AND METHODS: We reviewed MR findings of 18 patients with history of knee injuries in skiing. The MR images were evaluated retrospectively to identify the ligament injuries, bone lesions and meniscal injuries. RESULTS: Ligament injuries were seen in 16 patients, bone contusions in 16 patients, meniscal lesions in two patients. The most common group of injury was anterior cruciate ligament (ACL) and roedial collateral ligament (MCL) injuries with bone contusion on posterior lip of the lateral tibial plateau (LTP). The second common group of injury was isolated ACL injury with bone contusions on the lateral fernoral condyle (LFC) and posterior lip of the LTP. We considered that the mechanism of injury of the former group may be correlated with the valgus torque with secondary anterior displacement of the tibia and the latter group may be correlated with the pivot shift phenomenon. CONCLUSION: MR may play an important role in the diagnosis of knee injuries in skiing and its findings may explain the mechanism of injury.

4.Assessment of Resistive Index in Acute Epididymitis on Doppler Sonography.

PURPOSE: This study was aimed to review findings of gray-scale ultrasonography and to assess the diagnostic value of the resistive index (RI) in patients with acute epididymitis by comparing with that in normal volunteers. MATERIALS & METHODS: Gray-scale ultrasonogram was reviewed in nine patients with acute epididymitis for echogenicity, size and reactive hydrocele. Normal values of resistive index (RI) were obtained in 20 epididymides from 10 volunteers (aged 20-28, mean 23.7). Nine patients of epididymitis (aged 18-67, mean 37.2) were examinated with color Doppler sonography and RI was measured at 11 sites on epididymal arteries. For the prediction of acute epididymitis, accuracy, sensitivity and specificity were calculated at the cut-off value of RI in 0.65 and 0.70. RESULTS: Gray-scale ultrasonography showed echogenicity that was variable among the 9 cases (hype- rechoic 2 cases, isoechoic 2, hypoechoic 5), enlargement of epididymis in 8, and reactive hydrocele in 8 cases. Normal volunteers displayed color Doppler signals in 14 out of 20. The range of RI in normal volunteers were between 0.64 and 1.00 (mean, 0.79 +/- SD 0.10). In patients with acute epididymitis, color Doppler signals were detected in all patients. The range of RI in epididymitis were 0.40-0.68 (mean, 0.56 +/- SD 0.10). At cut-off RI value of 0.65, accuracy was 88%, sensitivity 81.8%, and specificity 92.7%. At cut-off value of 0.70, accuracy was 92%, sensitivity 100%, and specificity 85.7%. CONCLUSION: In addition to the findings of gray-scale ultrasonography, resistive index and color flow changes are valuable in diagnosing acute epididymitis.

6.Transient Right-sided Heart Failure after Percutaneous Transluminal Angioplasty (PTA) of Membranous Obstruction of Inferior Vena Cava: A Case Report.

We experienced a case of transient right-sided heart failure after angioplasty of membranous obstruction of the inferior vena cava confirmed by sonography and an inferior vena cavogram. Angioplasty involved the use of a self-expandable metallic stent, but after successful recanalization of the obstruction , the patient became dyspneic. Chest radiography revealed mild cardiomegaly with pulmonary congestion, but this was resolved spontaneously. For the prevention of serious heart failure, we recommend preprocedural evaluation of cardiac function.

Leiomyomas most frequently occur in the genitourinary and gastrointestinal system. This report describes the clinical and imaging findings of a leiomyoma originating in the prevesical space, a rare location. The mass abutted the fundus of the uterus, but after the correct identification of feeding vessels from the right iliac artery, prevesical leiomyoma was diagnosed radiologically.

10.Benign Prostatic Hyperplasia: Its Feature and Impact on the Quality of Life in Rural Area of Kangwon Province.

PURPOSE: We studied 250 men who are over 50 years of age to evaluate the feature of benign prostatic hyperplasia and its impact on quality of life in rural areas of the Korea. MATERIALS AND METHODS: Two hundred fifty men participated in this study. Urinalysis, urine culture, uroflowmetry, digital rectal examination and serum PSA were examined on all the participants. Transrectal ultrasonography(TRUS) were performed in 234 men. RESULTS: Of these 250 men, symptom prevalence was 78.71%. The age distributions of 50-59, 60-69, 70-79 and > or =80 years of age were 91, 111, 43 and 5 men respectively(mean 62.4). The mean prostatic volumes for 50-59, 60-69, 70-79 and > or =80 years of age group were 27.33cm3, 29.95cm3, 28.89cm3 and 33.55cm3. Of these, its volume difference was not statistically significant. Of 250 men, mildly symptomatic group(IPSS< or =7) was 22.5%, moderately symptomatic group(8< or =IPSS< or =19) 43%, and severely symptomatic group(IPSS> or =20) 34.5%. Severely symptomatic group showed higher mean age(p=0.0478), lower maximal flow rate(MFR)(p=0.024) and average flow rate(AFR)(p=0.003). Mildly symptomatic group showed smaller prostatic volume(p=0.013). Mean quality of life score for mildly, moderately, and severely symptomatic group were 2.09, 3.50, and 4.51 points, the score of which showed statistical difference(p<0.001). On comparison between the group of MFR<15ml/sec and the group of MFR> or =15ml/sec, statistical difference for the mean age (p<0.001), voided volume (p<0.001), QOL (p=0.02) and symptom score (p=0.002) was found. But no difference was found in prostatic volume(p=0.261). MFR, AFR and voided volume did not show significant correlation with prostatic volume. But serum PSA level was strongly correlated with prostatic volume. The AFR was a greatest determinant to symptom score in multiple regression analysis. CONCLUSIONS: The symptom prevalence in rural areas of the Korea is not any less than that of western society. This study shows that it too has a great impact on the quality of life in rural Korea. The prostatic volume itself had no significance on symptom score nor uroflowmetric parameters. The uroflowmetric parameter such as MFR is more important in evaluating symptomatic patients than prostatic volume itself.
