4.A Morphologic Study on the Bile Duct Changes Induced by Common Bile Duct Ligation in Rats.

In an attempt to elucidate the pathological changes following common bile duct ligation, the present study was undertaken in male Sprague-Dawley rats. Morphologic studies of the livers were performed at 1, 2, 3, 5, 7, 9, 11, 13 and 15 weeks after common bile duct ligation. In an attempt to clarify the relationship between the process of bile duct formation and the nature of primitive cells observable around the primitive biliary structure, light microscopic, immunohistochemical and electron microscopic studies were performed. The results were noted as follows: 1) Light microscopically, proliferation of biliary cells began in the periphery of portal areas and expanded toward hepatic lobules. In severe cases of biliary structure proliferation, hepatocytic cords and classic hepatic lobules were inconspicuous. 2) Immunohistochemically, CK-19 expression was limited to biliary structures in protal areas and proliferated biliary epithelial cells. In the serial sections of paraffin block, proliferated intrahepatic biliary structures were associated with those of portal areas. Some oval cells in the ductular hepatocytes were stained for both CK-19 and MNF 116. 3) Ultrastructurally, the proliferated biliary epithelial cells divided into three patterns: absence of lumen, formation of incomplete lumen, and formation of complete lumen. Furthermore these patterns had spectral continuity of maturation in their structures. 4) In some biliary structures, individual biliary cells pushed the basement membrane toward neighboring tissue with accompanying destruction of basement membranes, patterns of budding. Sometimes these cells and hepatocytes comprised the same lumen. In summary, the results obtained by the present study indicate that proliferated biliary structures may be derived from the preexisting intralobular or portal biliary system.

5.Two cases of Gaucher's disease in brothers.

We experienced two cases of Gaucher's disease of adult type in brother aged four years and two years. The patients showed hepatosplenomegaly with anemia and thrombocytopenia. Typical Gaucher cells were found in bone marrow and biopsy specimens of liver and spleen through light and electron microscopic examination. Splenectomy was followed by improvement of anemia and thrombocytopenia. A brief review of literature was made.

7.EXCISION OF PARAFFINOMA BY DIRECT INCISION ON NASAL DORSUM.

The injection of paraffin for cosmetic purpose is an illegal method as it evokes late complications such as the development of grauuloma, migration of foreign body, inflammation, ulceration, embolic phenomenon, etc. However, paraffin has been used by some unauthorized people for the augmentation of the soft tissue, i.e. face, breasts, as they can easily reansform the body contour in a short time and paraffin is not expensive to do so. Paraffinoma has been reported frequently as a delayed or late complication of foreign body reaction in the field of plastic surgery. And various mehtods of its surgical treatment also has been a topic of plastic surgeon, as postoperative outcome is not satisfactory. As the nose is located at the center of face, the patients who are suffering from paraffinoma in nose could not conceal their deformed appearance by cosmetics. Therefore the only method of improvement is surgical removal of paraffinoma. Paraffinoma of nose has been removed by transcolumellar, infracartilaginous, intercartilaginous incision. However, complete removal of the foreign material does not seem to be possible, may leave complications such as hematoma and overlying skin necrosis, and the remaining foreign material does not permit symmetric appearance. We experienced excision of paraffinoma by direct incision on nasal dorsum. 3 patients with paraffinoma of nose were successfully treated without any complications by vertical elliptical excision on the dorsum of the nose, and scars on nasal dorsum were acceptable with satisfaction by patients. Therefore, the authors concluded that excision of paraffinoma by direct incision on nasal dorsum is a good method for symmetric resection of foreign material, low risk of complication.
