2.The effectiveness of hyaluronidase for calcium gluconate-inducing extravasation necrosis : a dose and time response.

Extravasation necrosis due to intravenously administered fluids and drugs is an increasing problem in hospital practice. The incidence of extravasation is variable but skin necrosis is a potentially devastating complication of intravenous therapy. Local injection of hyaluronidase has been recommended for several types of infusion extravasations. The previous studies found hyaluronidase to be effective in the prevention of necrosis following intradermal nafcillin, 12% dextrose, sodium bicarbonate, aminophylline or vince alkaloids.The objectives of the study are to determine the dosage of hyaluronidase which is effective in reducing extravasation necrosis caused by 10% calcium-gluconate and to establish how soon after this extravasation it must be given to retain its effectiveness.Study I evaluated control versus only normal saline group and normal saline with hyaluronidase groups (dose:75,150,300,450 units; all in volume 2 ml, treatment delay:immediate, 15-minutes delay, half hour delay, one hour delay, three hour delay). Size and rate of eschar were compared between groups. Study II was undertaken to examine the evolution of calcium-gluconate induced soft tissue injury in the rabbit. The histologic findings of extravasation sites were compared between groups. A statistically significant protective effect was found in the treated group versus the nontreated group within 15 minutes to 30 minutes. The most effective protection was achieved by the immediate injection of 300 units dosage of hyaluronidase.In conclusion, in the 10% calcium gluconate-induced extravasation, the given data suggest that one can expect the most protective effect with a 300 units dosage of hyaluronidase and within half-hour delay in the treatment group.

3.A Case of Intavascular Papillary Endothelial Hyperplasia on Dorsum of Tongue.

Intravascular papillary endothelial hyperplasia is a benign solitary turnor that may be histologically misdiagnosed as angiosarcoma. We present herein a case of intravascular papillary endothelial hyperplasia occuring in 40-year-old female who hsd a slowly growing tumor on the tongue for 3 years. the histopathologic findings revealed. papillary endothelial hyperplasia in blood vessel accompanied with thrombi.

4.Pulmonary Edema Druing Hysteroscopic Myomectomy with Sorbitol-Mannitol Distention Medium.

A 45-Year-old para 1-0-1-1 had submucosal myoma diagnosed by ultrasonogram and hysteroscopic examination. During 60 minutes hysteroscopic myomectomy, 8,000 ml of sorbitol-mannitol distention medium was used and 6,500 ml collected, resulting in a deficit of 1,500 ml. The patient received Ringer`s lactate 1,000 ml intravenously. Her urine output was 400 ml. Blood loss was 40 ml. After operation, hyponatremia(115 mEq/L) was developed and pulmonary edema was also diagnosed clinically at that time and later confirmed by chest radiograph. The judicious administration of diuretics and normal saline fluid replacement to prevent overcorrection to hypernatremia resulted in return of serum electrolytes to normal level and resolution of the pulmonary edema by postoperative day 1. We have experienced a case of pulmonary edema during hysteroscopic myomectomy with sorbitol-mannitol distention medium, which is presented with a brief review of literatures.

5.Two Cases of Keratosis Palmaris et Plantaris.

We reviewed two cases of keratosis punctata palmaris et plantaris(KPP) and the scanning electron microscopic findings(SEM). The skin lesions of KPP are multiple, hyperkeratotic on the palms and soles with central pitting. There was no subjective symptoms. The histologic findigs revea,l marked hyperkeratosis, hypergranulosis, and acanthosis with depression of the underlying malpighian layers. The SEM findings reveal one hyperkeratoic plug on punctate lesion(case I ) and three hyperkeratotic plugs on depression of the malpighian layer(case ll ).

7.A Case of Eruptive Collagenoma.

The eruptive collagenoma is a connective tissue nevus without family history. We report a case of eruptive collagenoma in a 5 year old boy, who had multiple, variable sized orange colored nodules on his back since birth. The histopathologic finding showed increased number and size of collagen fiber and increased dermal thickness.

8.Flow Cytometric Analysis of BRCA1 Protein in Sporadic Breast Cancer.

PURPOSE: To study the subcellular localization with flow-cytometry and to evaluate their prognostic values. MATERIALS AND METHODS: The breast tissues were obtained from 28 patients with breast cancer and 6 patients with benign mass. The expression of BRCA1 protein was analyzed with the flow cytometry(Coulter Epics-XL, Coulter Corps, FL, USA) using the monoclonal antibody(BRCA1(Ab-1), Calbiochem, MA, USA) before and after nuclear and cytoplasmic permeabilization in association with DNA ploidy analysis. Several BRCA1 protein indices were derived including 95 percentile channel fluorescence(95% CF) and mean channel fluorescence(MCF) and percentage of BRCA 1 positive cell population arbitarily defined as those above 0.12 channel fluorescence. RESULTS: Cytoplasmic 95% CF were higher in breast cancer(n=28, 0.65+/-0.26) than in benign mass(n=6, 0.40+/-0.13, p=0.0211). Cytoplasmic BRCAl positive cell percentages were significantly higher in malignant tissues(24.0+/-10.3) than in benign mass(43.4+/-15.2, p=0.0059). Cytoplasmic BRCA1 positive cell percentages were significantly different according to the stages(stage I vs II, 32.6+/-9.8 vs 48.3+/-18.8, p=0.048, stage I vs stage III, 32.6+/-9.8 vs 47.0+/-10.9, p=0.010). The BRCA1 protein indices were not significantly correlated with histologic grades and DNA indices(aneuploidy, S phase and proliferation fractions). CONCLUSIONS: Flowcytometric assay offers an alternative approach to evaluating BRCA1 protein status of breast cancer tissue and detection of cytoplasmic BRCA1 protein by this method may help to understand the role of BRCA1 in breast cancer cell biology. The further study on cytoplasmic or nuclear BRCA1 protein in association with clinical therapeutic response or prognosis seems to be warranted.

9.Epidemiologic Findings of Behcet's Syndrome.

For the purpose of epidemiologic study, the patients who visited Severance Hospital Behcet's Syndrome Specialty Clinic were grouped by Shimizu classification into the complete, incomplete, suspected types and evaluated. The results of the evaluation with respect to personal characteristics, geographic distribution, socioeconomic characteristics, family history, and clinical characteristics are as follows. 1. The incidence among the subgroups was incomplete type(46.3%), suspected type(36.7%), complete type(17.0%) and the male to female ratio was 0.60: 1, and the average age of onset was 29.1. years for males and 27.6 years for females. 2. The occupational distribution among males included office workers, 31.5%; drivers, 10.6% and among females was office workers, 6.7%; teachers, 3. 7 %; unemployed, 81.7%. 3. Among the presumed aggrevating factors affecting oral cavity, recurrent tonsillitis(20.8%) was most common, and premenstrual exacervation(12.8%) was most common. 4. The most common initial major manifestation was oral ulcer(80.6%), and the longest duration of major manifestations was oral ulcer(average 6.4 years), and the most frequent relapse of major manifestations was oral ulcer(average 14.6 times/year). 5. The average duration between the appearance of the first and second major manifestations was 5.2 years for complete type, 6.3 years for incomplete type, and 6.6 years for suspected type. 6. Combinations of major manifestations most commonly included oral ulcer, genital ulcers and skin lesions(65.3%) for incomplete type, and oral ulcer, skin lesion(54.8%) for suspected type.

10.Mid Ureteral Stones: 5 Cases Treated with ESWL.

Use of ESWL has made many changes in the treatment of urinary trat stones. However, the treatment of the stone below the iliac crest was not possible owing to the arrangement of the apparatus and the location of the stone within the bony pelvis. For solving the problem, we selected 5 patients having mid ureteral stone. They were placed in prone position and administered shockwave from 500 times to 2600 times. The result was 3 patients had no residual stone and 2 patients showed decreased size of stones in follow-up examinations. All the patients showed no specific complication. The ESWL is able to disintegrate mid ureteral stone by using the change of patient`s position and avoiding the shield of bone around the stone.
