1.Clinical Features and Histopathological Characteristics of Nevus Depigmentosus.

BACKGROUND: Nevus depigmentosus was first reported in 1884 by Lesser. It is defined as a congenital non-progressive hypopigmented macule or patch that is stable in its relative size and distribution throughout the life of the individual. The etiopathogenesis and histopathological characteristics of nevus depigmentosus are not fully established. OBJECT: The purpose of this study is to investigate the clinical and histopathological characteristics and pathogenesis of nevus depigmentosus. METHODS: Clinieal survey was carried out on forty-nine patients with nevus depigmentosus and two skin biopsies were taken from eighteen patients; from the central part of the depigmented lesion and the border of the lesion including the perilesional normal skin. The sections were stained with hematoxylin-eosin, Fontana-Masson and S-100 protein. The ultrastructural evaluation were also done to detect alternation of melanocytes. RESULTS: The results are as follows ; 1. The lesions were mostly (91.8%) present before the age of three, but some lesions appeared in childhood (8.2%). 2. The lesions were most frequently found on the trunk (42.9%), followed by the face and scalp (20.4%). 3. There were 33 patients (67.3%) with the isolated type, 15 patients (30.6%) with the dermatomal type and one patient with the whorled type. 4. Histopathological studies have shown that the stainability of Fontana-Masson in the lesions of nevus depigmentosus was decreased compared with perilesional nomal skin, but there were no changes in the number of melanocytes. 5. There was a great reduction in the number of melanosomes in melanocytes and keratinocytes of nevus depigmentosus. In keratinocytes, there was some aggregations of melanosomes and some of them showed membrane bound architecture. CONCLUSION: The results of this study support the fact that nevus depigmentosus is caused by functional defects of melanocytes and morphological abnonnalities of melanosomes.

3.Local tumor recurrence at vaginal stump after hysterectomy: comparison of CT and MR imaging.

To assess the value of CT and MR in the detection of local tumor recurrence after hysterectomy, we evaluate CT and MRI findings of the vaginal stump in fifteen patients who underwent hysterectomy for uterine cervical carcinoma (N=14) or endometrial carcinoma (N=1). The presence of stump recurrence was confirmed by needle biopsy in 7 patients and by reoperation in 1 patient. The absence of stump recurrence was diagnosed by needle biopsy in 3 patitients and by clinical follow-up in 4 patients. In eight patients with vaginal stump recurrence, six patients were correctly diagnosed by CT and seven patients by MRI. In remaining seven patients who had no evidence of tumor recurrence, four patients were correctly diagnosed with CT and all the rester patients with MRI. ON the bases of our results, MRI seems to be superior to CT in the evaluation of local tumor recurrence after hysterectomy.

8.A Case of Churg-Strauss Syndrome with Bilateral Pleural Effusions.

A 26-year-old man with a one-year history of asthma and sinusitis presented with bilateral pleural effusions, patch basilar infiltrates on a chest x-ray and a pericardial effusion on an echocardiogram. The peripheral blood showed marked eosinophilia. An obstructive pattern was also observed during the pulmonary fuction test, which was responsive to bronchodilator inhalation. Nerve conduction studies showed right sural neuropathy. Thoracentesis yielded an acidotic exudative effusion with low glucose, low C3 and eosinophilia. An open lung biopsy revealed an eosinophilic interstitial pneumonitis associated with a necrotizing eosinophilic vasculitis, and granulomatous inflammation foci. In the literature, pleural effusions were reported in 29 percent of Churg-Strauss patients, but the number of effusions was low and their characteristics have not been well described. This report describes the characteristic findings of pleural fluid and its histologic features in a case of classical Churg-Strauss syndrome.

9.Clinical Application of Digital Subtraction Angiography.

Digital subtraction angiography(DSA) was made in 72 patients at department of radiology, Seoul National University hospital from February 1982 to January 1983 with our system, SRM-II, developed by cooperation of department of medical engineering, Seoul National University Hospital. The results were as follows; 1. In the total 72 cases, vascular disease was 28 cases in which aortic disease including Takayasu's aortitis was 18 cases. Others were vascular tumors in 32 cases and various conditions such as renal donor and artificial arteriovenous shunt. 2. Extent of the lesion was well delineated especially in aortic disease in cluding 8 cases of Takayasu's aortitis, 6 cases of aortic aneurysm and 2 cases of Behcet's syndrome. 3. DSA can be done to outpatient without admission because of its simplicity and non-invasiveness. DSA is useful especially in the evaluation and follow-up of vascular disease and the diagnostic value is expected to grow continuously according to the renewal of system.

10.Spontaneous Resolution of Infantile Esotropia.

Infantile esotropia is an esodeviation with onset before six months of age, characterized by a large stable angle and initial alternation with cross fixation. The natural history of infantile esotropia has not yet been verified. A 39-month-old girl presented with a history of esotropia. She was firstnoted to constantly cross her eyes at three months of age. At the age of five months, she was diagnosed by one of the authors[SHH]to have 25 prism-diopters of esotropia with a low hyperopic refractive error of 0.50D sphere. At that time, she fixed and followed with either eye. Ductions and versions were full. On three more examinations in her first year, she consistently showed 25~30 prism-diopters of esotropia. Against medical recommendation, she did not undergo any surgery. At the age of 39 months, she was found to be orthophoric at distance and at near by another author[JMH]. She could not identify flies or animals in random dot stereograms or Titmus stereograms. At the age of four years, she was still orthophoric at distance and at near. All the ophthalmologic findings remained the same. At the age of five years, she developed exophoria and DVD OS. She still could not identify flies. In conclusion, in rare cases infantile esotropia with a relatively small angle may spontaneously resolve without any treatment.
