1.A Case of Nerve Sheath Myxoma with Electron Microscopic Study.

We report a case of an uncommon cutaneous neoplasm of nerve sheath myxoma in a 25-year-old woman who had a papule on the left cheek of her face. The histologic findings of the lesion showed that lobulated tumors were composed of spindle shaped and stellate tumor cells in an abundant matrix of mucosubstances. The electron microscopic findings showed numerous cytoplasmic myelin figures, and interdigitating long cytoplasmic processes. 5-100 protein was not discovered and alcian blue and PAS stain showed positive. After surgical excision of tumor, the patient did not have any evidence of disease for one year.

2.The Predictors of Effectiveness on Urokinase Instillation Therapy into Loculated Pleural Effusion.

BACKGROUND: As the pleural inflammation progresses, exudative pleural fluid becomes loculated rapidly with pleural thickening. Complete drainage is important 13 prevent pleural fibrosis, entrapment and depression of lung function Intrapleural urokinase instillation therapy has been advocated as a method to facilitate drainage of gelatinous pleural fluid and to allow enzymatic debriment of pleural surface. This study was designed to investigate the Predictors of effeotiveness of intrapleural urokinase in treatment of loculated pleural effusion METHOD: Thirty-five patients received a single radiographically guided pig-tail catheter ranging in size from 10 to 12 French Twenty-two patients had tuberculous pleural effusions, and 13 had non-tuberculous postpneumonic empyemas. A total of 240,000 units of urokinase was dissolved in 240 ml of normal saline and the aliquots of 80mL was instilled into the pleura1 cavity via pig-tail catheter per every 8hr. Effectiveness of intrapleural urokinase instillation therapy was assessed by biochemical markers, ultrasonography, and technical details. A greater than 50% improvement on follow-up chest radiographs was defined as success group. RESULT: Twenty-seven of 35 (77.1%) patients had successful outcome to urokinase instillation therapy. Duration of symptoms before admission was shorter in sucess group (11.8α6.9day) than in failure group (26.62α16.5day) (P<0.05). Amount of drained fluid during urokinsse therapy was larger in success group (917.1α392.7ml) than in failure group (613.8α259.7ml) (P<0.05). Pleural fluid glucose was higher in success group (89.7 α35.9mg/dl) than in failure group (41.2α47.1mg/dl) (P<0.05). Pleural fluid LDH was lower in success group (878.4α654.31U/L) than in failure group (2711.1α973.1IU/L) (P<0.05). Honeycomb septated pattern on chest ultrasonography was observed in six of eight failure group, but none of success group(P<0.05). CONCLUSION: Longer duration of symptoms before admission smaller amount of drained fluid during urokinase therapy, lower glucose value, higher LDH value in pleural fluid examination and honeycomb septation pattern on chest ultrasonography were predictors for failure group of intrapleural urokinase instillation therapy.

3.Surgical Clipping of Intracranial Aneurysm Regrown after Endovascular Coiling.

Operative clipping after previous endovascular coiling in an aneurysm is a different problem from primary clipping procedure for neurosurgeons. With the increasing use of coil embolization, neurosurgeons will more and more face the similar situation. We report surgical clipping cases of intracranial aneurysm regrown after endovascular coiling. Three patients with a history of subarachnoid hemorrhage due to ruptured aneurysm underwent endovascular treatment (EVT) with detachable coils. The aneurysms were in the posterior communicating artery, the middle cerebral artery, and distal anterior cerebral artery (DACA). Two near-total occlusions and one partial occlusion were achieved by EVT. After several months, angiographic follow-up revealed regrowth of the aneurysm requiring surgical clipping. Here, we report three cases in which surgical clipping was more difficult than a usual clipping procedure performed several months after EVT, because of adhesion and coil bulging into the aneurysmal neck. The difficulty of the treatment of the residual aneurysm after coiling is discussed, as are the surgical complications and limitations of clipping.

5.Diagnostic Ability of Stratus OCT Using Korean Normative Database for Early Detection of Normal-Tension Glaucoma.

PURPOSE: To determine if korean normative database improve diagnostic ability of Stratus OCT for detection of glaucoma. METHODS: Peripapillary retinal nerve fiber layer (RNFL) and optic nerve head regions were measured using a Stratus OCT. The normative data were collected from 129 normal individuals. We obtained values at the 5% and 1% levels according to the disc area stratified into equal thirds to minimize the error by the individual variation of optic disc size and these levels were considered abnormal. One eye of each 94 normal-tension glaucoma patients with early visual field defects (mean deviaton = -3.84+/-1.40dB) and 87 another normal subjects were enrolled. Glaucoma was defined by visual field defects. RESULTS: The use of Korean normative database had higher sensitivity and no significant difference of specificity than that of a Stratus OCT except for a few parameters. The criteria that show the highest diagnostic ability were 1 > or = quadrants RNFL thickness abnormal at the < 5% when using a Korean normative database (sensitivity = 81.9%, specificity = 81.6%) and 1 > or = clock hours abnormal at the < 5% when using a Stratus OCT normative database (sensitivity = 71.3%, specificity = 87.4%) (p=0.004, p=0.180, respectively, McNemar test). CONCLUSIONS: The criterion using a Korean normative database had higher sensitivity and no difference of specificity compared with criterion using a Stratus OCT normative database. The best criterion using a Korean normative database may be helpful for early detection of normal-tension glaucoma.

7.Evaluation of Steroid Therapy in Tuberculous Pleurisy: A Prospective, randomized study.

BACKGROUND: Tuberculous pleurisy has been treated with a combined regimen of corticosteroids-antimicrobial therapy. But whether such combination therapy add to benefits is unknown. We evaluate the effects of corticosteroid and its routine application in relief of clinical symptoms, absorption of pleural effusion, and pleural adhesions. METHODS: A prospective, randomized study of the role of corticosteroid in the treatment of tuberculous pleurisy was performed in 83 patients(nonsteroid group:50 patients, steroid group: 33 patients) from June, 1991 to September, 1994. RESULTS: 1) The mean duration from symptoms(fever, chest pain, dyspnea) to relief was 3.8 days in the steroid group, and 7.4 days in the nonsteroid group(P<0.05). Clinical symptoms including fever, chest pain, sputum and weight loss were relieved more rapidly in the steroid group than other symptoms(weakness, night sweating and dyspnea). 2) Pleural effusion was taken an averge of 88 days in the steroid group and 101 days in the nonsteroid group to be absorbed completely(p>0.05). 3) The incidence of pleural adhesions was 17/33(51.5%) in the steroid group and 32/50(64%) in the nonsteroid group(p>0.05). 4) Side effects of corticosteroids were observed in only one patient causing epigastric pain and discontinuation of drug. CONCLUSION: Corticosteroid exert benefitial role in the more rapid relief of clinical symptoms to patients with tuberculous pleurisy, but absortion of pleural effusion and occurrence of pleural adhesions was not influenced significantly. Therefore, its routine application should be reevaluated.

8.Analysis of lysosomal membrane proteins exposed to melanin in HeLa cells.

OBJECTIVES: There have been developed to use targeting ability for antimicrobial, anticancerous, gene therapy and cosmetics through analysis of various membrane proteins isolated from cell organelles. METHODS: It was examined about the lysosomal membrane protein extracted from lysosome isolated from HeLa cell treated by 100 ppm melanin for 24 hours in order to find associated with targeting ability to melanin using by 2-dimensional electrophoresis. RESULTS: The result showed 14 up-regulated (1.5-fold) and 13 down-regulated (2.0-fold) spots in relation to melanin exposure. CONCLUSIONS: It has been found that lysosomal membrane proteins are associated with melanin to decolorize and quantity through cellular activation of lysosome.

9.Primary lymphoma of genitourinary tract.

BACKGROUND: The primary lymphoma of genitourinary tract is very rare and reported sporadically in Korea. Therefore the aim of this study is to evaluate the clinical characteristics with therapeutic outcome in 15 cases of primary lymphoma of genitourinary tract. METHODS: Reviewed retrospectively medical records of 15 cases with primary lymphoma of genitourinary tract who had been diagnosed from January, 1990 to May, 2002, we analysed the clinical characteristics, stage, disease free survival and overall survival. RESULTS: There were 8 cases of lymphoma of testis, 3 cases of uterine cervix and body, 1 case of ovary, 1 case of vagina, and 2 cases of kidney. The histologic types were all diffuse large B cell, except 5 cases (3 cases T/NK cell, 1 case low grade MALT lymphoma, and 1 case precursor B cell lymphoblastic lymphoma). The median survival duration of all cases is 8.0 months, showing the similarity to that of testis lymphoma (7.0 months). CONCLUSION: In comparison with the prognosis of the lymphomas in uterine body and cervix, it was worse for the testicular and renal lymphoma, even though 3 cases of T/NK cell lymphoma impacted on the prognosis of testicular lymphoma. Therefore, with simultaneous elucidation of the prognostic factors systemically, therapeutic strategy must be continuously sought.

10.Toxic detection in mine water based on proteomic analysis of lysosomal enzymes in Saccharomyces cerevisiae.

OBJECTIVES: Lysosome is the cell-organelle which is commonly used as biomonitoring tool in environmental pollution. In this study, the lysosomal proteomic of the yeast Saccharomyces cerevisiae was analyzed for utilization in the detection of toxic substances in mine water samples. METHODS: This work informs the expression of lysosomal proteomic in yeast in response with toxic chemicals, such as sodium meta-arsenite and tetracycline, for screening specific biomarkers. After that, a recombinant yeast contained this biomarker were constructed for toxic detection in pure toxic chemicals and mine water samples. RESULTS: Each chemical had an optimal dose at which the fluorescent protein intensity reached the peak. In the case of water samples, the yeast showed the response with sample 1, 3, 4, and 5; whereas there is no response with sample 2, 6, and 7. CONCLUSIONS: The recombinant yeast showed a high ability of toxic detection in response with several chemicals such as heavy metals and pharmaceuticals. In the case of mine water samples, the response varied depending on the sample content.
