1.The Study of the Serum Electrolytes in the Normal Person.

We studied the serum electrolytes in the 727 normal persons who visited Busan University hosptial, Benedict Hospital and Maryknoll Hospital from 1970 to 1976. The results were as follows: 1. Mean value of the serum Na+ being measured in 729 normal persons was 138.7+/-6.2mEq/L and 140+/-6.5mEq/L in 450 male, while 136.8+/-6.6mEq/L in 279 female. 2. Mean value of serum Cl- being measured in 701 persons was 99.6+/-5.9mEq/L, and 99.7+/-6.0mEq/L in 437 male while 100+/-5.7mEq/L in 264 female. 3. Mean value of serum K+ being measured in 707 normal persons was 4.4+/-0.9mEq/L in 434 male, while 4.2+/-0.8mEq/L in 273 female. 4. Mean value of serum Ca+ being measured in 557 normal persons was 4.5+/-0.6mEq/L and 4.7+/-0.7mEq/l in 355 male, while 4.3+/-0.5mEq/L in 202 female. 5. Mean value of serum phosphorus being measured in 94 normol persons was 4.2+/-0.6mg%, and 4.1+/-1.0mg% in 65 male, while 4.5+/-1.5mg% in 29 female.

2.Influence of Gamma linoleic acid (Epogam) on the Skin Surface Conditions of Atopic Dermatitis.

BACKGROUND: Gamma linoleic acid (GLA, Epogam) is considered a safe and effective modality in the treatment of atopic dermatitis (AD) in which impaired function of the enzyme, delta-6-desaturase, has been reported to result in reduced levels of GLA, desaturated fatty acids. OBJECTIVE: We performed this study to observe the changes of skin surface conditions measured objectively by bioengineering methods in relation to clinical improvement after treatment with GLA (Epogam®) in children with AD. METHODS: Thirty-four children with AD were treated with GLA (Epogam®) and evaluated with clinical parameters.The changes of skin surface conditions were monitored by non-invasive experimental instruments. RESULTS: There was a significant decrease of transepidermal water loss (TEWL) and gradual improvements in clinical severity after 12 weeks of GLA (Epogam®) treatment. The change of skin surface pH was statistically significant on the antecubital fossa and abdomen except the popliteal fossa. The other parameters including skin surface hydration and skin surface lipid did not show consistent changes. CONCLUSION: Clinical improvement of AD with GLA (Epogam) seemed to be achieved by the reduction of TEWL.

3.Evaluation of Medium Preparations for Temporomandibular Joint Disc Chondrocytes.

Recent advances in cell culture and tissue engineering permit the successful treatment of deep articular defects with autologous chondrocyte transplantation. While chondrocyte culturing from articular cartilage is commonplace, in vitro growth of chondrocytes from temporomandibular joint fibrocartilage has not been defined. Likewise, the concept of autologous chondrocyte transplantation has not been applied to the temporomandibular joint. Since fibrocartilage differs structurally and functionally from articular cartilage, an effective culturing method for this tissue is essential as well. The ultimate goal of this project is to enable the use of autologous temporomandibular joint chondrocyte transplantation for the regeneration of damaged or missing components of the joint. The aim of this pilot study is to develop an effective cell culturing technique for chondrocytes harvested from the temporomandibular joint disc of dogs. Cartilage of the temporomandibular joint disc of drug-free mongrel dogs was dissected, dissociated, and centrifuged for chondrocyte collection. Blood specimen was collected from the same animal to produce the autologous serum. Chondrocytes were then dispersed in 24-well plates and incubated in one of four media, DMEM (Dulbecco's modified Eagle's medium), Ham's F-12, DMEM/F-12, or Iscove's MDM(modified Dulbecco's medium). Each medium was also mixed with either 10% fetal bovine serum or 10% autologous serum. Temporomandibular joint disc chondrocytic proliferation was tested in all the culture media with sera on the fifth day. The initial plating count was held constant throughout at 1 x 10(4) cells/well and eight samples were evaluated in each culture. The results demonstrated that fetal bovine serum worked much better than autologous serum in all evaluated media and that DMEM/F-12 mixed with 10% fetal bovine serum was the most optimal culture condition for expansion of temporomandibular joint disc chondrocytes.

4.A study on type 1 allergy to house dust mite in patients with atopic dermatitis.

The authors investigated the radioallergosorbent test (RAST) for the specific IgE and the prick test with hose dust mite in 129 patients with atopic dermatitis(AD) wit4 four or more family numbers. RAST and prick test results were described according to family background of respiratory atopy(RA) and severity of AD. The results are summarized as follows : l. A comparison between the prick test results and RAST reactions gave a concordance of 88%. The prick test was more sensitive than the RAST. 2. The level of specific IgE in patients with personal history(PH) or family history(FH) of RA was significantly increased compared to those in patients with only AD. 3. In patients without PH and FH of RA, the level of specific IgE in patients with FH of AD was significantly increased compared to those in patients without FH of AD. 4. The level of specific IgE did not reflect the severity of AD. From the above results, it could be concluded that type I allergy to house dust mite is not directly related to AD and it occurs predominantly in those patients with AD wha have a predisposition to RA.

5.AN EXPERIMENTAL STUDY ON PATENCY RATE OF MICROVASCULAR ANASTOMOSIS USING FIBRIN GLUE.

Microvascular anastomosis has become an essential technique in reconstructive surgery. A lot of techniques have been developed to improve patency rate and reduce operation time. But interrupted suture technique is still most widely used because of limited indication and inconvenience of other methods. A comparative study was undertaken to evaluate new microvasular anastomosis technique using fibrin glue. In this report 40 femoral arteries of Sprague-Dawley rats were anastomosed by utilizing four stay sutures which were placed 90 degrees apart and the intervals covered with fibrin glue, and by conventional eight suture method served as control. The patency rate(immediate postoperative, postoperative two weeks), time needed for vascular anastomosis, and microscopic evaluation were compared to those of conventional microvascular suture technique. Postoperative patency rate was 100% and 85% by fibrin glue technique compared to 100% and 90% by conventional technique at immediate postoperative and postoperative two weeks. Less time consumed with fibrin glue technique by 16 minutes compared to conventional technique by 21 minutes. Microscopically reendothelization was complete with smooth and less injured inner lining and also less inflammatory response by fibrin glue technique compared to conventional technique.

6.The Study of Serum Free Fatty Acid, Triglycerides and Glycerol in Congestive Heart Failure, Essential Hypertension and Liver Cirrhosis.

Just as hyperbetalipoproteinemia is the most common kind of familiar hyperlipoproteinemia, hyperprebetalipoproteinemia or increased very low density lipoprotein (VLDL) and its associated hypertriglyceridemia is an abnormality frequently found in association with several metabolic and nutritional disorders. This VLDL abnormality is demonstrable in high percentage of insulin dependent diabetic children before they receive treatment. It is also observed in many older, overweight, insulin resistant diabetic, with poor control of diabetes. An elevation of VLDL was also observed after ethanol intake. A mild to moderate degree of VLDL elevation is the most frequently observed from of clinical hyperlipidemia. Many investigators reported that hyperprebetalipoproteinemia (hypertriglyceridemia) is associated with an increasing incidence of coronary artery disease. As opposed to hyperbetalipoproteinemia, hyperprebetalipoproteinemia is prone to influence on the development of atherosclerosis in the middle and older aged persons which indicates the importance of triglyceride determination in these aged groups. There has been relatively rare study reported on the triglyceride metabolism in patients with congestive heart failure, essential hypertension and liver disease in Korea. This study was designed to determine serum triglyceride, free fatty acid and glycerol in the disease mentioned in Busan University Hospital between jan. 1975 and December 1976 and analized the change of lipid profile in each different condition. The results were as follows; 1. The mean of serum FFA, triglyceride and glycerol in the 37 normal were as follows 502.32+/-146.54microEq/L, 111.84+/-40.53mg% and 432.00+/-212.13microM/ml. 2. 23 patients with congestive heart failure showed a significant reduction in serum triglycerides (91.96+/-27.80mg%) and a significant increase in serum free fatty acid (576.77+/-129.67microEqL) and glycerol (432.50+/-212.13microM/ml). 3. In 15 patients with essential hypertension a significant elevation of serum triglycerides (149.38+/-42.28mg%) was noted. 4. In 13 patients with liver cirrhosis, a reduction in serum triglycerides (80.50+/-34.27mg%) was noted.

7.Gross and Microscopic Findings of the Testes Ascended into abdomen During Neonatal and Prepubertal age in Rats.

PURPOSE: Because cryptorchid testes are known to undergo histopathologic changes affecting development, maturation, and tertility, early surgical correction is usually recommended. However there are differing opinions concerning whether retractile testes are affected by similar changes and also whether there is a need for surgical treatment. We aimed to assess the histopathologic changes in retractile testis by studying the changes in testes artificially placed back in the abdomen after they have descended to scrotum in experimental rat models. MATERIALS AND METHODS: Male Sprague-Dawley rats were divided into 3 groups; a control group(controls), prepubertal ascent group (P) in which testis were placed back intraabdominally by bilateral inguinal canal obstruction at 6 weeks, a neonatal group(N) in which intraabdminal testis was induced by bilateral inguinal canal obstruction at birth. The relative weight of the testis, morphology of the seminiferous tubules including Leydig cells, tubular degeneration phase(TDP), spermatogonia per tubules(S/T), and Sertoli cell index (SCI) of these three groups were analyzed and compared. RESULTS: The relative weight of testis was significantly decreased in the group of N(0.0016+/-0.001) and P(0.0015+/-0.0002) compared to controls(0.0037+/-0.0002) (p<0.05). The S/T value was also decreased in P(2.05+/-18.2) and N(73.2+/-32.4) when compared to controls (360.2+/-21.3). Similar changes were observed in SCI of both P(64.5+/-6.4) and N (91.2+/-14.2) when compared to controls (227.9+/-31.1). Only minority of N and P showed higher TDP values. However, although statistically insignificant, TDP was increased in both P and N when compared to controls. The Leydig cells in N and P showed cellular distortion and hypertrophy. CONCLUSIONS: This study demonstrate that prepubertal ascent, similar to that of innate cryptorchid testis, also can induce histopathologic changes such as changes in testicular seminiferous tubule, decrease in the S/T value and decrease in SCI value. Our findings supports that hypothesis that retractile testis may cause histological damage thus surgical correction may also be warranted similar to in case of genuine cryptorchism.

8.Pelvic Lymph Node Evaluation in Uterine Cervical Carcinoma Using Contrast Enhanced MR Imaging.

PURPOSE: To evaluate the usefulness of Gd-DTPA enhanced MR imaging in determining the metastatic lymph nodes in uterine cervical carcinoma. MATERIALS AND METHODS: Sixty nine patients with histologically proven cervical carcinoma underwent Gd-DTPA enhanced MR imaging. One hundred and thirty-eight pelvic regions(69 right,69 left) in 69 patients were analyzed for lymph node metastases. Pelvic lymph nodes were considered to be abnormal if they were greater than lcm in diameter and were enhanced on postcontrast T1 weighted images. RESULTS: Metastatic lymph nodes were found in 22 pelvic regions by surgicopathologic examinations. On MR imaging there were 14 true positives, 106 true negatives, 10 false positives and 8 false negatives. Gd-DTPA enhanced MR image had an accuracy of 86.9%, a sensitivity of 58.3%, a specificity of 91.4%, a positive predictive value of 58.3% and negative predictive value of 92.9% in evaluation of pelvic lymph node metastases. CONCLUSION: MR imaging with contrast enhancement may be useful in the evaluation of pelvic lymph node metastases in patients with uterine cervical carcinoma.
