1.A Clinical and Anatomical Study on the Mandible for Inferior Alveolar Nerve Conductive Anesthesia in Korean.

This study was to investigate the various parts of the mandible related to inferior alveolar nerve conductive anesthesia in 228 dry skulls of Korean adults. Five morphological variations were observed and four measurements were undertaken. The following results were obtained. 1. The Position of coronoid notch in the anterior border of the ramus of mandible is classified into three types and percentage of each type was as follows : Type I (Coronoid notch in the middle part of the anterior border of the ramus of mandible) : 83.3% Type II (Coronoid notch in the inferior part of the anterior border of the ramus of mandible) : 12.7% Type III (No coronoid notch in the anterior border of the ramps of mandible) : 4.0% 2. The position of the lingula of mandible to the imaginary line of the molar occlusal plane is classified into three types and percentage of each type was as follows : Type I (Above the imaginary line of the mandibular molar occlusal plane) : 87.3% Type II (Coincide with the imaginary line of the mandibular molar occlusal plane) : 8.6% Type III (Below the imaginary of mandibular molar occlusal plane) : 4.3% 3. The position of the lingula of the mandible in the internal oblique line-posterior border of the ramus of mandible dimension is classified into three types and percentage of each type was as follows : Type I (Anterior part in the internal oblique line-posterior border of the ramus of mandible dimension) : 82.9% Type II (Middle part in the internal oblique line-posterior border of the ramus of mandible dimension) : 11. 4% Type III (Posterior part in the internal oblique line-posterior border of the ramus of mandible dimension) : 5.7% 4. The position of the apex of the lingula of mandible to the deepest point of the coronoid notch in the anterior border of the ramus of mandible is classified into three types and percentage of each type was as follows : Type I (Coincide with the deepest point of the coronoid notch) : 82.0% Type II (Above the deepest point of the coronoid notch) : 13.2% Type III (Below the deepest point of the coronoid notch) : 4.8% 5. The position of the apex of the lingula of mandible to the imaginary line of the mandibular molar occlusal plane is classified into three types and percentage of each type was as follows : Type I (Above the 8mm) : 65.7% Type II (Above the 5mm) : 18.6% Type III (Above the 10mm) : 15.7% 6. Angle between the line connecting the apex of the lingula of mandible-1-2 premolars in the contralateral side and median line of the mandible is classified into three types and percentage of each type was as follows : Type I (50°) : 67.2% Type II (45°) 21.4% Type III (55°) : 11.4% 7. The averages of the measurement of the various bony landmarks of the mandible related to inferior alveolar nerve conductive anesthesia were as follows : ① Distance from deepest point of the coronoid notch to internal oblique line : 9.2mm ② Distance from internal oblique line to the apex of the lingula of mandible : 11.6mm ③ Distance from deepest point of the coronoid notch to apex of the lingula of mandible : 19.7mm ④ Height of the lingula of mandible : 9.8mm

2.Study on the Anti-Smooth Muscle Antibody and Anti-Nuclear Antibody of Chronic Active Hepatitis, Chronic Persistant Heratitis and Liver Cirrhosis in Korea.

In Korea chronic liver diseases are the important medical issues because of their high incidence, poor prognosis and no available therapeutic resume. It is believed that the etiologic agent of most of chronic liver diseases in Korea is hepatitis B virus. To find out the incidence of positivity and to understand the autoantibodies in some of the chronic liver diseases in Korea, anti-smooth muscle antibody and anti-nuclear antibody tests were performed on 43 patient serum with chronic active hepatitis, 13 patient serum with chronic persistant hepatitis and 20 patient serum with liver cirrhosis, who diagnosed by liver biopsies. The results of the study are summarized as follows: 1) The positivity of anti-smooth muscle antibody in chronic active hepatitis was 93.0%, that in chronic persistant hepatitis 60.0% and that in liver cirrhosis 80.0%. 2) The positivity of anti-nuclear antibody in chronic active hepatitis was 18.6%, that in chronic persistant hepatitis 15.4%, that in liver cirrhosis 5.0%. 3) The test efficiency of anti-smooth muscle antibody to differentiate chronic active hepatitis from chronic persistant hepatitis and liver cirrhosis was 48.7%. The above results suggest that the antismooth muscle antibody may be related to the "necrosis of liver cells", and further studies are needed to fine out any relationship between the autoantibody and the disturbance of immune regulatory function if there is, as this study suggested.

3.The Use Contrast-Enhanced Color Doppler Ultrasonography in the Detection of Hepatocellular Carcinoma-Related Vessels.

PURPOSE: To assess the value of contrast-enhanced color Doppler ultrasonography(US) in the detection of ves-sels related to hepatocellular carcinoma(HCC). MATERIALS AND METHODS: Between July 1997 and April 2000, 76 HCCs in 70 patients (50 men and 20 women; mean age, 57.8 years) were confirmed histologically or clinically. Tumor site and size at gray scale US, and afferent, intratumoral and efferent color signals at precontrast and postcontrast color Doppler US were deter-mined. Afferent signals were classified as basket or penetrating type, and intratumoral signals as spotty, linear or mixed. Efferent signals were categorized as signal to portal vein or signal to hepatic vein, and postcontrast color signal changes as focal, general or marginal spotty type. We also measured the color percentage of intra-tumoral signals as seen during precontrast and postcontrast study. RESULTS: The detection rate changed from 41(53.9%) to 60(78.9%) in cases with afferent signals, from 50(65.8%) to 64(84.2%) in those with intratumoral signals, and from 6(7.9%) to 9(11.8%) in those with efferent signals. Overall, 74(97.4%) cases showed positive findings at postcontrast color Doppler US. The most common enhancing pattern was general, occurring in 33(43.4%) cases. The color percentage of intratumoral signals increased from an average of 8.2% to 34.9%. The detection rate of intratumoral signals from tumors less than 3 cm in diameter increased from 56.8% to 100%, and that of deeply-located tumor-related signals (17 cas-es) increased from 47.1% to 94.1%. CONCLUSION: The use of contrast enhanced color Doppler US increased the detection rate of afferent, intratumoral, and efferent signals, especially that of intratumoral signals from tumors less than 3 cm in diameter and signals from deeply located tumors. In addition, the modality can aid the diagnosis of HCC by evaluating tumor dynamics.

5.An Immunohistochemical and Immunoelectron Microscopic Study of Distribution of Neuropeptide Y in the Cat Spinal Trigeminal Subnucleus Caudalis after Pulpectomy.

The purpose of this study was to investigate the distribution of neuropeptide Y (NPY) in the cat spinal trigeminal subnucleus caudalis following pulpectomy of mandibular premolars and molar by means of an immunohistochemical and immunoelectron microscopic study. The animals were divided into normal and experimental group which were sacrificed at 14 days after pulpectomy. The results were as follows; 1. On the light microscopic observation of the spinal trigeminal subnucleus caudalis in normal group, NPY-immunoreactivity (IR) was weak within lamina I and lamina II outer. In pulpectomy group, NPY-IR was strong and appeared to extend into lamina I and lamina II inner at 14 days. 2. On the immunoelectron microscopic observation of the spinal trigeminal subnucleus caudalis in normal group, NPY-IR was revealed in axon terminals, dendrites, myelinated axons and unmyelinated axons. NPY-IR was associated with membrane structures within microtubules, synaptic vesicles, outer membrane of mitochondria and inner surface of the axolemma. In NPY-immunoreactive structure, there was a small amount of DAB precipita-tions. 3. On the immunoelectron microscopic observation of the spinal trigeminal subnucleus caudalis at 14 days in pulpectomy group, the number of NPY-immunoreactive axon terminals, dendrites, myelinated axons and unmyelinated axons was increased than normal group. DAB precipitations in NPY-immunoreactive structure was increased than normal group. Some NPY-immunoreactive axon terminal formed synaptic glomerulus and axoaxonic synapse. 4. The results indicate that NPY-IR was increased in the spinal trigeminal subnucleus caudalis after pulpectomy, and it is speculated that the increased NPY by injury of peripheral nerve may participate in the processing of nociception.

6.A New Interpretation of Ptosis-like Eyes through the Results of Small-Incision Double-Eyelid Operation.

PURPOSE: Perceiving the experience that MRD1 is increased after small-incision double-eyelid operation, it has been statistically verified the improvement of MRD1 retrospectively. Therefore, we suggest new interpretation for Korean-specific small eye and more definite classification for ptosis. METHODS: In operation, we made 3 small incisions according to the line designed on upper eyelid and removed some soft tissues including connective tissues. Then, figure-of-8 continuous buried ligation was performed through the incision windows. This study is a retrospective study, measures MRD1 changes and excursion distance change of eyelid margine and eyebrow using pre- and post-operative photos of 14 patients. RESULTS: Postoperative photos were taken on the 4th day and all of 14 patients had an improvement in MRD1 statistically. The excursion distance of upper eyelid was improved in half of patients, though not in the range of statistical verification. It was statistically confirmed that the movement of eyebrow was reduced after operation. CONCLUSION: It is thought that MRD1 improvement without any operation on levator complex means that soft tissue including connective tissue eliminated in operation restricts the movement of upper eyelid. It is defined such factors as burden factors and such phenomenon as burdened eyelid. It is thougt that Korean's small eye is mainly burdened eyelid and cosmetic improvement may be obtaind even by removing such burden factors.

7.The Development of a Korean Modification of the Scale to Measure Subjective Well-Being under Neuroleptic Treatment (KmSWN).

OBJECTIVES: The purpose of this study was to develop the Korean modification of SWN (KmSWN), based on the original 38-item SWN (a self-rating scale to measure subjective well-being under neuroleptic treatment) by Naber et. al. in 1994, but containing the reduced number of items and the modified content. METHODS: The KmSWN with 22 items and General Health Questionnaire/Quality of Life-12 (GHQ /QL-12) were administered to 204 out-patients with schizophrenia. Also, their sociodemographic and clinical characteristics were investigated. Data analyses were conducted based on 176 patients whose responses to the scale, as well as whose related informations, were reliable. First, the reliability and validity of KmSWN were assessed comprehensively. Second, the relationships between the total score of KmSWN and the sociodemographic and clinical characteristics were examined. Finally, in order to identify the predictable variables on the total score of KmSWN, the stepwise multiple regression analysis was conducted, where the independent variables were already shown significant correlations with the total score of KmSWN. RESULTS: Three of 22 items of KmSWN were excluded from the following analysis due to their poor internal consistency. KmSWN comprised with 19 items (KmSWN-19) showed high levels of spilthalf reliability (.91), mean of interitem correlations (.38) and internal consistency (Cronbach alpha .92). Factor analysis of the KmSWN-19 revealed that a three-factor solution accounted for 100% of total explained variance. The three factors were interpreted as 'dissatisfaction', 'physical and mental functioning', 'emotional regulation and self-control'. The concurrent validity of KmSWN-19 with GHQ/QL-12 was very high (-.83). Of the sociodemographic and clinical characteristics, age, disposable money, socioeconomic level, number of previous hospitalizations, compliance with medication and satisfaction with medication were positively correlated with the total score of KmSWN-19, while severity of illness, severity of parkinsonism, severity of dyskinesia, administration frequency of medication and omission number of medication were negatively correlated. However, the total score of KmSWN-19 was not correlated with the class and the dose of neuroleptics. In stepwise multiple regression analysis, the satisfaction with medication, the omission number of medication and the disposable money were significantly contributed to the total score of KmSWN-19, and they explained 24% of the variance. CONCLUSIONS: The above findings indicated that KmSWN-19 was a reliable, valid and practical self-rating scale to measure subjective well-being in out-patients with schizophrenia. Moreover, KmSWN-19 seemed to be a general scale applicable to all the schizophrenic or other psychotic patients if they were able to complete it, rather than a specific scale only for the patients undergoing the neuroleptic treatment. Based on the results of the predictable variables on the total score of KmSWN-19, one may argue that intraindividual characteristics such as value, belief and personality traits, although they were not evaluated in this study, should be considered in assessing subjective well-being or the quality of life.

8.The Use of Contrast-Enhanced Color Doppler Ultrasound in the Differentiation of Retinal Detachment from Vitreous Membrane.

OBJECTIVE: To compare the clinical utility of contrast-enhanced color Doppler US in the differentiation of retinal detachment (RD) from vitreous membrane (VM) with that of various conventional US modalities, and to analyze the enhancement patterns in cases showing an enhancement effect. MATERIALS AND METHODS: In 32 eyes examined over a recent two-year period, RD (n=14) and VM (n=18) were confirmed by surgery (n=28) or clinical follow-up (n=4). In all cases, gray-scale, color Doppler, and power Doppler US were performed prior to contrast injection, and after the intravenous injection of Levovist (Schering, Berlin) by hand for 30 seconds at a dose of 2.5 g and a concentration of 300 mg/mL via an antecubital vein, contrast-enhanced color Doppler US was performed. At Doppler US, the diagnostic criterion for RD and VM was whether or not color signals were visualized in membranous structures. RESULTS: Diagnostic accuracy was 78% at gray-scale US, 81% at color Doppler US, 59% at power Doppler US, and 97% at contrast-enhanced color Doppler US. The sensitivity of color Doppler US to color signals in RD increased from 57% to 93% after contrast enhancement. The enhancement patterns observed were signal accentuation (n=3), signal extension (n=2), signal addition (n=3), and new signal visualization (n=5). CONCLUSION: Contrast-enhanced color Doppler US was the most accurate US modality for differentiating RD from VM, showing a significantly increased signal detection rate in RD.

9.Usefulness of BRAF VE1 immunohistochemistry in non–small cell lung cancers: a multi-institutional study by 15 pathologists in Korea

Background:
Next-generation sequencing (NGS) is an approved test to select patients for BRAF V600E targeted therapy in Korea. However, the high cost, long turnaround times, and the need for sophisticated equipment and skilled personnel limit the use of NGS in daily practice. Immunohistochemistry (IHC) is a rapid and relatively inexpensive assay available in most laboratories. Therefore, in this study, we evaluate the usefulness of BRAF VE1 IHC in terms of predictive value and interobserver agreement in non–small cell lung cancers (NSCLCs).
Methods:
A total of 30 cases with known BRAF mutation status were selected, including 20 cases of lung adenocarcinomas, six cases of colorectal adenocarcinomas, and four cases of papillary thyroid carcinomas. IHC for BRAF V600E was carried out using the VE1 antibody. Fifteen pathologists independently scored both the staining intensity and the percentage of tumor cell staining on whole slide images.
Results:
In the lung adenocarcinoma subset, interobserver agreement for the percentage of tumor cell staining and staining intensity was good (percentage of tumor cell staining, intraclass correlation coefficient = 0.869; staining intensity, kappa = 0.849). The interobserver agreement for the interpretation using the cutoff of 40% was almost perfect in the entire study group and the lung adenocarcinoma subset (kappa = 0.815). Sensitivity, specificity, positive predictive value, and negative predictive value of BRAF VE1 IHC were 80.0%, 90.0%, 88.9%, and 81.8%, respectively.
Conclusions
BRAF VE1 IHC could be a screening test for the detection of BRAF V600E mutation in NSCLC. However, further studies are needed to optimize the protocol and to establish and validate interpretation criteria for BRAF VE1 IHC.
10.Clinical Outcomes of Halo-Vest Immobilization and Surgical Fusion of Odontoid Fractures.

OBJECTIVE: In the present study, authors retrospectively reviewed the clinical outcomes of halo-vest immobilization (HVI) versus surgical fixation in patients with odontoid fracture after either non-surgical treatment (HVI) or with surgical fixation. METHODS: From April 1997 to December 2008, we treated a total of 60 patients with upper cervical spine injuries. This study included 31 (51.7%) patients (22 men, 9 women; mean age, 39.3 years) with types II and III odontoid process fractures. The average follow-up was 25.1 months. We reviewed digital radiographs and analyzed images according to type of injury and treatment outcomes, following conservative treatment with HVI and surgical management with screw fixation. RESULTS: There were a total of 31 cases of types II and III odontoid process fractures (21 odontoid type II fractures, 10 type III fractures). Fifteen patients underwent HVI (10 type II fractures, 5 type III fractures). Nine (60%) out of 15 patients who underwent HVI experienced successful healing of odontoid fractures. The mean period for bone healing was 20.2 weeks. Sixteen patients underwent surgery including anterior screw fixation (6 cases), posterior C1-2 screw fixation (8), and transarticular screw fixation (2) for healing the odontoid fractures (11 type II fractures, 5 type III fractures). Fifteen (93.8%) out of 16 patients who underwent surgery achieved healing of cervical fractures. The average bone healing time was 17.6 weeks. CONCLUSION: The overall healing rate was 60% after HVI and 93.8% with surgical management. Patients treated with surgery showed a higher fusion rate and shorter bony healing time than patients who received HVI. However, prospective studies are needed in the future to define better optimal treatment and cost-effective perspective for the treatment of odontoid fractures.
