1.Radiological evaluation of cortical rim sign of the kidney

The rim sign in renovascular compromise may be defined as a thin nephrographic rim outlinning a kidney with another wise faint nephrogram. Renal infarction with gelform and blood clots was induced experimentally in 5 dogsand high dose IVP and CT were performed to evaluate renal cortical rim sign after renal infarction. One expireddue to anesthetic technical failure, and one was embolized partially. So we could be obtain the result on 3 dogs.The reults were as follows; 1. The high dose IVP films show cortical rim sign on 24 hrs and 72 hrs films in twodogs. The CT films show rim sign in 3 dogs. 2. The rim sign may be visualized, if the renal artery is occuludedenough to drop perfusion presssure below the authoregulatory range. 3. The high dose IVP and early nephrogramphase are necessary for the rim sign.

2.Magnetic Resonance Imaging Fails to Show Evidence of Endolymphatic Hydrops in a Case with Tumarkin's Otolithic Crisis.

Tumarkin's otolithic crisis refers to drop attacks that are an unusual manifestation of Meniere's disease. It has been proposed that these drop attacks result from mechanical deformation of the otolithic organs related to the endolymphatic hydrops that occurs in Meniere's disease, as revealed by MRI with intratympanic gadolinium enhancement. Here we report a patient with Tumarkin's otolithic crisis in whom inner-ear MRI with intratympanic gadolinium administration during the acute attack did not reveal the presence of endolymphatic hydrops.

3.Nitric Oxide Synthase Expression and Apoptosis in Psoriasis.

BACKGROUND: Nitric oxide(NO) is generated from L-arginine by NO synthase(NOS). Three types of NOS are currently known:inducible(iNOS), neuronal(nNOS) and endothelial(eNOS). NO has been found to be important in a number of different physiological processes. Of particular relevance to the skin are the roles of NO in vasodilatation, inflammation, immunomodulation and in oxidative damage to cells and tissues. NO exhibits contradictory effects in the regulation of apoptosis. The proapoptotic effects seem to be linked to pathophysiological conditions, where high amounts of NO are produced by iNOS. In contrast, the continuous release of eNOS inhibits apoptosis. Psoriasis is a common chronic skin disease, but the cause of psoriasis is not definitely known until now. OBJECTIVE: To investigate the role of NO in psoriasis pathogenesis, such as inflammatory infiltration, dermal vessels dilatation, apoptosis, we performed this study. METHODS: Ten cases of psoriasis and 5 cases of normal skin for immunohistochemical with antibodies to iNOS, PCNA, bcl-2, p53 and TUNEL stainings, and 5 cases of psoriasis and 2 cases of normal skin for western blot with antibody to iNOS were investigated. RESULTS: The results were summarized as follows.1. Immunohistochemical staining with iNOS showed positive reactions in 9 cases(90 %) of psoriasis, 4 cases among them were strong positive staining, but all cases of normal skin were negative.2. Labelling index of PCNA staining was 24.4+2.5%, 2.4+0.7% in psoriasis, normal skin, respectively.3. All cases of psoriasis and normal skin were negative in p53 staining, but squamous cell carcinoma as positive control was positive.4. Bcl-2 staining showed focal positivity in 5(50%) cases of psoriasis, but diffuse positivity in epidermal basal layer of normal skin.5. TUNEL staining showed positivity in 7(70%) cases of psoriasis, but all of normal skin were negative.6. Western blot with anti-iNOS showed positive 130 kDa band in psoriatic, but not in normal skin tissues. CONCLUSION: The results suggested that NO was considered to play a role in psoriasis pathogenesis, including apoptosis, dermal vessels dilatation and inflammatory infiltration.

5.Two Cases of Simvastatin-induced Acute Myopathy.

Among the side effects of simvastatin, elevated liver enzyme or creatine kinase activity has been occasionally report-ed, but overt myopathy is rare. We report two cases of simvastatin-induced myopathy with usual dosages of simvas-tatin. Two patients presented with general myalgia, markedly elevated creatine kinase activity, and mild proximal weak-ness. One patient had been treated with concomitant use of cyclosporine and simvastatin after renal transplantation, and another patient had diabetic nephropathy. We recommend careful monitoring when myalgia appears in a patient receiv-ing simvastatin. (J Korean Neurol Assoc 19(4):435~437, 2001)

8.The Effect of Diet, Exercise and Growth Hormone Treatment for Obese Non-Insulin Dependent Diabetes Mellitus(NIDDM).

OBJECTIVE: To investigate whether there is a significant effect of growth hormone(GH) treatment with diet and exercise over the diet and exercise alone in obese non-insulin dependent diabetes mellitus(NIDDM). METHOD: Twenty obese NIDDM adults were studied. We measured the body weight, body composition and exercise capacity before and after 12 weeks of treatment program. The subjects were assigned in a double-blind manner either to the diet, aerobic exercise with placebo treatment group(group A) or to the diet, aerobic exercise with GH treatment group(group B) for twenty-week period. Two groups were compared for the demographic data. RESULTS: After 12-weeks of treatment program, each group showed a significant weight loss (group A: 8.54+/-2.29 kg vs group B: 7.14+/-2.99 kg) than before the treatment, however there was no significant weight loss between two groups. After 12-weeks, the fat fraction of body weight loss was significantly higher in group B than group A(0.80+/-0.40%kg versus 0.55+/-0.30%kg). After 12-weeks, the maximal oxygen consumption was similarly increased in both groups(23.75% in the group A versus 29.2% in the group B). After 12-weeks, the peak torque was similarly increased in both groups(9.7% in the group A versus 17.3% in the group B). After 12-weeks, the endurance was similarly increased in both groups(10.1% in the group A versus 8.1% in the group B). CONCLUSION: Both group A and B showed a significant weight loss and resulted in a comparable gain in the muscle strength, endurance, and maximal oxygen consumption. The addition of GH in a low dose to a the calorie-restricted diet and aerobic exercise resulted in a significant fat loss especially around the visceral area.

9.Regional Distribution of Hepatitis C Virus Infection in the Republic of Korea, 2007-2011.

BACKGROUND/AIMS: In Korea, hepatitis C is included as an infectious disease in a sentinel surveillance system. Recently, a large variation in hepatitis C incidence between different regions in Korea has been noticed. The current study verified the nationwide distribution of hepatitis C infection for effective prevention and management. METHODS: We counted the number of hepatitis C patients who visited a hospital per county using the National Health Insurance database from 2007 to 2011. The age-adjusted prevalence ratio was used, and the age adjustment method was used as an indirect standardization method. Disease mapping and spatial analysis were conducted using a geographic information system. RESULTS: The annual prevalence of diagnosed hepatitis C was approximately 0.12% to 0.13% in Korea. The age-adjusted prevalence ratios in Busan, Jeonnam, and Gyeongnam were high (1.75, 1.4, and 1.3, respectively). The three regions in the southern coastal area of the Korean Peninsula were identified as a high-prevalence cluster (Moran's index, 0.3636). CONCLUSIONS: The present study showed that hepatitis C infection has very large regional variation, and there are several high-risk areas. Preventive measures focusing on these areas should be applied to block the transmission of hepatitis C and reduce the disease burden.

10.Echocardiographic Evaluation of Axial Spondyloarthritis in Korea: Data From the Catholic Axial Spondyloarthritis Cohort

OBJECTIVE:
Axial spondyloarthritis (axSpA) is often accompanied by cardiac manifestations, such as valvular heart disease. In this prospective cohort study, we evaluated the incidence of cardiac abnormalities in Korean axSpA patients by echocardiography.
METHODS:
AxSpA patients were prospectively recruited from a single tertiary hospital. Baseline demographic, clinical, radiographic, and echocardiographic data were collected at the time of enrollment. Echocardiography evaluations were performed with a focus on valvular heart disease and systolic and diastolic function. Logistic regression analyses were used to identify factors associated with diastolic dysfunction in axSpA.
RESULTS:
A total of 357 axSpA patients were included in the analyses, of whom 78 (21.8%) exhibited diastolic dysfunction, with no reports of systolic dysfunction. Thirteen patients (3.6%) had valvular heart disease, and aortic valve regurgitation (n=5) and mitral valve regurgitation (n=6) were most common. Multivariable logistic regression analyses indicated that older age and higher body mass index (BMI) were positively associated with diastolic dysfunction, whereas human leukocyte antigen (HLA)-B27 positivity was negatively associated with diastolic dysfunction.
CONCLUSION
Valvular heart disease is infrequent in Korean axSpA patients. However, diastolic dysfunction is common in axSpA patients, and is significantly associated with older age, higher BMI, and HLA-B27.
