2.A Clinical Study of Herniated Lumbar Intervertebral Disc in Teenagers

Sixteen cases under the age of 19 were underwent surgical treatment for hernlated lumbar intervertebral disc over 10 years of period in the Department of Orthopaedic Surgery,Catholic Medical College. The results were obtained as follows: 1. The incidence was 5.2% of all herniated lumbar intervertebral disc patients who treated surgically, but no sexual predilection. 2. Nine cases (56.3%) had a definite previous history of injuries. 3. Average intervals between the onset of clinical symptoms and the operation were 9.2 months. 4. Neurologic deficits are less severe and frequent (8 cases, 50%) but the clinical symptoms do not differ from than that of the adult. 5. 7 cases (43.8%) revealed definite reptured lumbar intervertebral discs and 5 cases (31.3%) are diffuse buldging of annular fibers in operative findings. 6. According to histopathological study, 8 cases show degenerative fibrocartilagenous changes in surgical specimen. 7. The radiological abnormalities of lumbar spine are less common than that of the adult. 8. Excellent or good results were found in 12 cases (75%) but two cases required a further surgical procedure. 9. In generally, hernlated Iumbar intervertebral disc in teenagers show better results and more rapid recovery of clinical symptoms by surgical treatment, and less residual symptoms than that of the adult.

3.AgNOR Counts in S-phase Human Cells.

The nucleolus of human cell is a morphologically well recognizable nuclear organelle and the argyrophilic NORs (AgNORs) are nucleic acid-argyrophilic nonhistone protein complex in the nucleoli and the silver staining allows their identification and enumeration at the light microscopic level. The AgNOR counts are in parallel with mitotic activity and vary in different phase of cell cycle. It has been reported that human cells have one AgNOR during interphase and S-phase. However, the correlation between the number of AgNORs and S-phase markers is still controversial and they have never been studied simultaneously. In this study, AgNOR and PCNA were stained simultaneously to find out the relationship of AgNOR counts with cell cycle (S-phase) in human palatine tonsil, gastric carcinoma, liver and brain tissues. S-phase cells (PCNA-positive) were found predominantly in lymphoid follicles in palatine tonsil but gastric carcinoma showed diffuse immunoreactivity for PCNA. The AgNOR counts varied according to the type and locus of tissue. More than one AgNOR were identified in S-phase cells and some of hepatocytes and neurons in the brain which were not in S-phase contained two or more AgNORs. The above results suggest that the number of AgNOR is a characteristic feature of each type of cells and can be more than one even in S-phase.

4.Histopathological Analysis of Posterior Fossa Tumor.

The posterior fossa, containing roughly 6ne fourth of the intracranial contents, is the site of about 30-35% of the intracranial tumors. The incidence of primary tumors in the posterior fossa is quite different from that of the cerebivm. We analysed 124 cases of posterior fossa tumor, over a 10 year period, to understand the status of posterior fossa tumor and its histologic characteristics. Medulloblastoma was most common(37cases, 29.8%), followed by astrocytoma, hemangiobla-stoma, ependymoma, meningioma, metastatic tumor, arteriovenous malformation and choroid plexus papilloma in descending order of frequency. Tumors were found most frequently between the ages of two and ten years(28.2%) and sixty seven(54.0%) cases were diagnosed before the age of fifteen. The ratio of male to female was 60 : 64. Astrocytoma revealed a characteristic juvenile pilocytic type and a microcystic change. Hemangioblastoma showed higher frequency(17.7%) than previous reports and the origin of tumor cells is still equivocal. Other tumors revealed the same histologic features as other intracranial tumors.

6.A Study of Relation Between Symptoms and Low Blood pressure.

To establish whether an association exists between low blood pressure and common symptoms such as fatigue, dizziness, headache, and palpitation, we analysed the data of health center in Yeungnam medical college from January 1993 to June 1993. Total 1,133 subjects were taken and results were analysed by combined stratification and logistic regression. The results were as followed : 1. True confounders were sex, age, and body mass index. 2. No association was found between low blood pressure and all symptoms. 3. Positive associations were found between high blood pressure and self reported palpitation and headache, which persisted after adjusted for confounders. The results suggest that low blood pressure and all neurasthenic symptoms such as fatigue, palpitation, headache and dizziness have no association, but the validity is limited.

8.Effect of Cortisone & Vitamin C on Experimental Fracuters in Rabbits

Since cortison was administered to various clinical conditions, a large number of reports reviewed the influence of its application on the repair process of connective tissue. The effect of cortisone was described as consisting of delayed formation of fibroblast, rnucopolysac-charide and collagen, depressed activity of vitamin D in the human body, disturbed calciurn metabolism and consequently the delayed healing of the fracture. And it was therefore decided to investigate the effect of cortisone on fracture healing process. Twenty male rabbits weighing between 1500 and 2000 gms were used for this experiment. Their ribs were fratured and they were divided in 4 groups as follows: 1st group: control group 2nd group: vitamin. C 15mg/Kg. daily I. M. 3rd group: cortione acetate 20mg/Kg. daily I. M. 4th group: cortisone acetate 20mg/Kg. with vitamin. C 15mg/Kg. daily I, M. Gross and histological changes of healing process of ribs were observed, and results confirmed the delaying effect of cortisone administration in the fracture healing, on the contrary, the vitamin. C administration seemed to accelerate.

10.Colles' Fracture Treated with Radial Slab or U-cast

Sixty six Colles' fracture were treated with radial slab or U-cast (sugar tong splint), from January 1982 through May 1984 at the Department of Orthopaedic Surgery of St. Mary Hospital. A prospective study was made and was evaluated under the subjective criteria of Gartland and Werley, and the objective criteria of Scheck. The results of this study were as follows: 1. All of 66 cases was closed fracture and the incidence was high in females who aged over 6thdecades. 2. In 43 cases of stable fracture, 18 (94.7 %) out of 19 cases treated with radial slab and 21 (87.5%) out of 24 cases treated with U-cast showed satisfactory result. 3. In 23 cases of unstable fracture, 6 (54.6%) out of 11 cases treated with radial slab and 9 (75%) out of 12 cases treated with U-cast showed satisfatory result. 4. As a result, we found that the result of the treatment with radial slab was superior to the U-cast for the stable Colles fracture, however, all of two methods were not staisfatory for the unstable Colles fracture although the U-cast group showed better result.
