2.Serum Interleukin-6 Concentration in Carcinoma of the Cervix.

Interleukin-6(IL-6) is a multifunctional cytokine, and deregulated IL-6 gene expression has been implicated as being involved in the pathogenesis of a number of diseaaes. So we measured the serum level of IL-6 to find a relationahip between uterine cervical cancer and serum IL-6 level. The serum levels of IL-6 were meesured m 54 patients adrnitted to the Severance hospital, from August, 1992 to February, 1993, of whom 27 patients were diagnosed as invasive cancer of cervix, 17 as CIN, and also utilizing 10 patiente diagnosed as uterine myoma as a control group for matched age and parity. The average serum level of IL-6 in invasive cancer, CIN, and uterine myoma were respectively 30.33pg/ml, 37.27pg/ml, and 12.45pg Jml. In 22% af cervical cancer patients (6 of 27), and 27% of CIN patients (4 of 17) studied blood levels of II-6 were detected whereas only 10% of uterine myoma (1 of 10) shorved the same result. However, the difference does not seem to be statistically significant, There was no statistically significant difference among any of the groups, and age and stage did not show any significant cmrelation, and the results need furt,ber investigations.

3.Femoral Neck Fractures in Young Adults

A fracture of the femoral neck in a young adult differs from the same fracture in an older patient in the following respects; 1. A relatively uncommon injury. 2. A significant difference in the severity of trauma. 3. A less satisfactory result. At the point of view, we studied the femoral neck fcartures in 15, 20- to 50-year-old patients. The aims were to analyze why these fractures occur in young adults, and what results can be expected after internal fixation. The results were as follows; 1. Common in men (3:1). 2. 80% (12/15 cases) of the fractures were caused by severe trauma. 3. The 3 cases (20%) were associated with the other fractures. 4. The factors infuencing the clinical results and complications in our study; a. Severity of trauma. b. Degree of displacement. c. Existence of posterior comminution. d. Time interval between accident and operation. 5. The multiple pinning got the more rigid fixation and diminished the rate of fixation failure. 6. All cases were achieved good to acceptable reduction. 7. Satisfactory functional results were achieved in 93.3%. 8. The rate of avascular necrosis was 15.4% (2/13 cases). It is concluded that the most of femoral neck fractures in younger patients occur because of the significant trauma. In our study, the incidence of avascular necrosis was not greater than in order patients. Therefore, to get as good a result as possible it seems important to perform the anatomical tion and rigid internal fixation with or without bone graft.

4.Surgical Treatment in Rheumatoid Wrist

The authors have analysed 39 rheumatoid wrists in 25 rheumatoid patients, who underwent vsrious surgeries including arthroscopic and open synovectomy, partial and total arthrodesis and total wrist arthroplasty at the department of orthopaedic surgery, St. Mary's Hospital from November 1985 to April 1988, and the effect of eaeh surgery on the wrist function and proper time of operation are discussed. The results were as follows; 1. The patients who were classified as a classical rheumatoid arthritis according to the ARA criteria were four males and twenty-one females, with a mean age of 43 yesrs, and the functional capacity of the patients before operation was graded in III for twenty-threeand in IV for two patients. 2. Out of 25 patients (39 wrists), 14 patients are in bilateral and II patients in unilateral involvement. The synovectomy in 13 wrists, arthrodesis in 24 wrists and total joint replacement in 2 wrists were performed and followed them for average 17 months after operation. 3. The synovectomy were done for 13 wrists in grade II and III according to the classification of rheumstoid arthritis by radiology(3). There were two cases of recurrence in the cases of synovectomy, but not in replacement and arthrodesis. 4. The arthrodesis of wrist was performed for 24 wrists-bilateral arthrodesis in 3 patient (6 wrists) and unilateral fusion in 18 patients. For 4 cases out of 24 cases of wrist arthrodesis, the second and third carpo-metacsrpal joints were fused simultaneously. 5. Protek total wrist arthroplasty was done in 2 cases snd followed them for more than9 months with relatively good clinical results. Finally, it is our beliefs for rheumatoid wrists that surgery is often indicated to control the various wrist problems, early synovectomy is strongly indicated, and partial wrist fusion has proven to be effective in patients with moderate destruction, deformity and pain, but the total wrist arthrodesis and arthroplasty is still in controversy. The proper surgery for rheumatoid wrist should be determined by the clinical severity of disease and radiological changes of wrist.

5.Clinical Observation on Effect of Amlodipine in Angina Pectoris.

We evaluated the clinical effects of amlodipine in 10 patients(7 male and 3 female) with angina pectoris in terms of the effect on the anginal pain, hemodynamic changes and side effects. The results obtained were as follows; 1) The clinical improvement was obsebed in 8(80.8%) of 10 and 9(88.9%) of 9 patients at 2 and 10 weeks after oral amlodipine. 2) The systolic and diastolic blood pressure was decreased significantly(136.0+/-16.5mmHg vs 117.0+/-10.6mmHg, p<0.01 and 85.0+/-9.7mmHg vs 75.0+/-5.3mmHg, p<0.01 respectively) but the heart rate was indepentent of amlodipine administration. 3) The adverse effects of amlodipine were as headache in 3, facial flushing in 3, palpitation, dizziness, urinary difficulty in 1 respectively and one of them discontinued amlodipine due to severe palpitation and facial flushing.

7.Papillary and Solid Epithelial Neoplasm of the Pancreas with Multiple Metastases.

Papillary and solid epithelial neoplasm is a rare pancreatic tumor of low-grade malignancy. We report a case of a 23 year old female having solid and papillary neoplasm of the pancreatic tail with mutiple omental and peritoneal metastases. Microscopically, the main tumor showed typical histologic findings including solid and papillary areas with cystic change. But the metastasizing nodules were largely solid and the tumor cells demonstrated increased nuclear pleomorphism, hyperchromasia and an increased mitotic rate. The tumor cells contained considerable amount of intracellular and extracellular eosinophilic inclusions which were ultrastructually zymogen-like granules. These inclusions were more frequently found in the metastatic nodules. By flow cytometric study, the tumor was hyperdiploid. The DNA index was not significant.

8.The Effect of Posterior Colporrhaphy Performed Concurrently with Midurethral Sling Surgery on the Sexual Function of Women with Stress Urinary Incontinence.

PURPOSE: Some women choose to undergo posterior colporrhaphy (PC) concurrently with procedures for SUI to regain sexual confidence and improve sexual function. We determine the effect of PC on sexual function in women requiring a midurethral sling for SUI. MATERIALS AND METHODS: A total of 119 women were retrospectively reviewed 81 women had the midurethral sling alone (Group A), and 38 women voluntarily had PC concurrently with the midurethral sling (Group B). Sexual function was evaluated using the FSFI before and after surgery. RESULTS: The postoperative composite scores were significantly increased in both groups (Group A: p=0.02; Group B: p=0.04), and significant increases were observed in 'desire,' 'arousal' and 'satisfaction'. However, there were no significant differences in the composite score or six domainscores between the two groups. CONCLUSION: PC performed concurrently with midurethral sling for treatment of SUI does not provide any additional benefits towards improving the sexual function.
