1.Two Cases of Cutaneous Larva Migrans.

Cutaneous larva migrans(CLM) is a ubiquitous self-limiting skin eruption which has intensely pruritic, serpiginous, raised, sharply demarcated, red lesions. It is most frequently caused by larvae of dog and cat hookworms. We report two typical cases of cutaneous larva migrans that appeared as pruritic exanthems in two people after they had travelled in Southeast Asia. It is suggested one should consider cutaneous larva migrans in the differential diagnoses of erythematous pruritic lesions.

3.Subtrochanteric Fracture Treated with Bent Self

Subtrochanteric fracture is well known for its difficulty in management, though various devices of rigid fixation have been developed. Each internal fixation device requires a lot of surgical instruments and the surgeon must be skillful in using them. Osteo self-compression plate which was preoperatively bent to fit the contour of the lateral surface of the subtrochanteric region was proved to be technically easy to fix these fractures without causing major complication. Present authors recommend to use this prebent Osteo self-compression device to treat this fracture when the surgical instruments are not fully equipped, and suitable fixation devices are not available, and/or the surgeon has no experience in handling the newly developed surgical instruments for the subtrochanteric fracture.

4.Conjunctival Bacterial Flora at Birth: Comparisons between Cesarean Section and Vaginal Delivery.

Conjunctival bacterial cultures for 113 neonates who were delivered by cesarean section or vaginal delivery were performed within 30 minutes after birth. These neonates were in the condition of no eye drops applied. The 43 newborns were delivered by cesarean section(CS group) and the 70 newborns were delivered vaginally(VD group). The 10 newborns(P1) (14 eyes(P2)) in the CS group and the 29 newborns(41 eyes) in the VD group showed culture positivity. These differences between two groups were statistically significant(p1, p2<0.05). The CS group had significantly fewer species and colony forming units cultured per subject (0.395 +/- 0.821(p3), 5.116 +/- 27.095(p4) than the VD group(0.971 +/- 1.393, 25.843 +/- 62.285) (p3,p4<0.05). Coagulase negative staphylococci were predominantly isolated in the CS group and coagulase negative staphylococci, gram positive bacilli, E. coli, nonfermenting bacilli, and S. aureus in the VD group. Therefore it was found that the conjunctiva of the newborns delivered by cesarean section bore significantly fewer bacteria and need less prophylactic measures than that of newborns delivered vaginally.

6.Effects of Intraoperative Exposure and Postoperative Subconjunctival Injections of 5-fluorouracil in Reducing Adhesions after Strabismus Surgery in Rabbits.

Recently, antimetabolotes such as 5-fluorouracil and mitomycin have been used successfully to reduce postoperative adhesions in animal models after strabismus surgery.Nine rabbits, 18 eyes, 36 areas were divided into 4 groups. The first group was a control one. The second group was treated with 5-fluorouracil intraoperatively. The third group was treated in the same manner as the second group, followed by subconjunctival injections of 5-fluorouracil from postoperative 7th day. The fourth group was treated with mitomycin C intraoperatively.The thied group(booster group) and the fourth group(mitomycin group) were superior to the others in preventing the postoperative adhesions. There was no statistically significant diference in scar length and adhesion grade between booster group and mitomycin group.This study reveals that the subconjunctival injections of 5-fluorouracil after intraoperative contact may reduce postoperative adhesions in secondary strabismus surgery and may prevent the possible serious complications of mitomycin C, giving the similar effect to potent mitomycin C.

7.The Novel Low Frequency Oscillation in Pulmonary Artery Pressure.

BACKGROUND: It is well known that systemic blood pressure oscillates with low(0.04~0.1Hz), mid(0.1~0.15Hz), and high(respiratory) frequency range. But there has been no study about oscillation of pulmonary artery pressure(PAP). METHOD: We measured PAP for 5 minutes in 32 patients of ventricular septal defect and stored them to computer files. Power spectral density curve was obtained. Low, mid, respiratory frequency power were measured by integrating the area within each frequency range below the power density curve. RESULT: The incidence of significant low frequency power(more than 5% of total power) were higher in patients of high PAP and hign Rp/Rs than those of low PAP and Rp/Rs(p<0.01 and p<0.005 respectively). The low frequency power positively correlates with PAP and Rp/Rs(r=0.62, p<0.0005 and r=0.61, p=0.0005 respectively). CONCLUSION: It can be said conclusively that as PAP and pulmonary vascular resistance elevates, the PAP tends to definitively oscillate in low frequency range.

8.The Effectiveness for Vagal Stimulation by Ice Water Application to Face during Controlled Respiration and Mechanism of Individual Variability of Responsiveness to Vagal Stimulation (Power Spectral Analysis of Heart Rate Variability).

BACKGROUND: The effectiveness for vagal stimulation by ice water aplication to face during metronome-controlled respiration of 15 breaths/minute was examed. The importance of basal vagal tone and sympathovagal interaction in the individual variability of responsiveness to the vagal stimulation was investigated. METHOD: Fifty three 12~13 year old healthy volunteers were included. Vagal tone and sympathovagal interaction before and after application of ice water to the face were assessed by power spectral analysis of RR interval(heart rate) variability. RESULT: Basal heart rate, high frequency power, and low to high frequency power ratio were 81+/-13(58~110 beats/min), 791+/-1061(56~4161 msec2) and 1.08+/-1.22(0.04~4.85) during controlled respiration. After ice water application twenty three children developed 5 more nodal escape beats due to severe bradycardia. Minimum heart rate, high frequency power and low to high power ratio changed to 42+/-12(19~72/min), 1890+/-1882(221~7258msec2) and 0.64+/-0.43(0.12~1.46). The increased ratio of high frequency power, maximum heart rate decrement and its percent after stimulation were 5.44+/-5.62(0.63~24.26), 39+/-14(10~81/min) and 47+/-15(16~81%) respectively. The increased ratio of high frequency power was correlated with basal logarithmic high frequency power(r=-0.60, p=0.0004). Maximum heart rate decrement was correlated with basal logarithmic high frequency power(r=-0.41, p=0.0018) and low to high frequency power ratio(r=0.27, p=0.04). CONCLUSION: Application of ice water to the face during controlled respiration produces powerful vagal stimulation and bradycardia, however, there is a wide individual variability of responsiveness to it. The absolute basal vagal tone contribute to this individual variability.

9.A Study on Dietary Intake Behavior of Behçet's Disease Patients.

BACKGROUND: The etiology of Behçet's disease(BD) is not yet certain, and various hypotheses are suggested. In order to determine the role of dietary intake behavior in the pathogenesis of BD, we analyzed BD patients by questionnaire regarding their personal, social, and economic characteristics, and dietary intake behavior. METHODS: A series. of 79 patients who were registered at the Behçet's Disease Specialty Clinic at Severance Hospital, Yonsei University College of Medicine, Seoul, Korea were studied. The patients were classified and the data from the patients were analyzed according to the revised Japanese criteria. RESULTS: All patients except for suspected type patients took an average amount of nutrition close to the Korean Recommended Dietary Allowance(RDA). However, on the whole, the amount of consumption of Vitamin B2 and C was low and in the case of females, the consumption of Ca and Fe was makedly insufficient in accordance with the Korean RDA. In case of the level of dietary and serum zinc, there was no significant difference in patients or among groups. CONCLUSION: Since the consumption of some vitamins and minerals of patients were low, the behavior of dietary intake of BD patients which is considered to have a possible role in development of disease should be studied further.
